Ultrasound is an effective tool to detect and characterize lesions of the uterosacral ligament, parametrium, and paracervix. They may be the site of diseases such as endometriosis and the later stages of cervical cancer. Endometriosis and advanced stages of cervical cancer may infiltrate the parametrium and may also involve the ureter, resulting in a more complex surgery. New functional, surgical anatomy requires the complete diagnostic description of retroperitoneal spaces and tissues that contain vessels and nerves. Most endometriosis lesions and cervical cancer spread involve the cervical s

BACKGROUND: We report about our experiences using a single supraclavicular incision at the base of the neck for Thoracic Outlet Syndrome (TOS) surgery.
METHODS: 10 patients aged between 12 and 59 years (mean 31 years) underwent 12 times a TOS procedure. Patients suffered from compression of their brachial plexus with main affection of the ulnar nerve (9 out of 12 cases). Electroneurography was positive for TOS 4 times in 3 patients, in other 3 patients additionally a distal nerve compression syndrome was evident. In 7 cases (5 patients) a cervical rib was present on X-ray. In 10 cases (8 pati

OBJECTIVE: To present the unique case of a collegiate wrestler with C7 neurologic symptoms due to T1-T2 disc herniation.
BACKGROUND: A 23-year-old male collegiate wrestler injured his neck in a wrestling tournament match and experienced pain, weakness, and numbness in his left upper extremity. He completed that match and 1 additional match that day with mild symptoms. Evaluation by a certified athletic trainer 6 days postinjury showed radiculopathy in the C7 distribution of his left upper extremity. He was evaluated further by the team physician, a primary care physician, and a neurosurgeon.

A case of adult cervical trauma that resulted in a clinical picture of Brown-Séquard syndrome and unilateral brachial plexus injury is presented. Rotational hyperflexion of the cervical spine is thought to have resulted in subluxation of C5-6 with root avulsions, leading to this unusual combination of findings.

To determine the prevalence of cervical ribs on cervical spine MRI and clinical relevance, we reviewed 2500 studies for cervical ribs and compression of neurovascular structures and compared to CT, when available. Brachial plexus or subclavian artery contact by cervical rib was identified on MRI and/or CT in 12 cases with diagnosis of thoracic outlet syndrome in one. Cervical ribs were identified on 1.2% (25/2083) of examinations, lower than on CT (2%), but MRI may offer equivalent anatomic explanation for patient symptoms.

Blockade of the superior hypogastric nerve plexus was performed for relief of chronic cancer related pelvic pain. The targeted sympathetic nerves lie anterior to the sacral promontory. Twenty-eight patients with neoplastic involvement of pelvic viscera secondary to cervical, prostate, and testicular cancer or radiation injury were treated with neurolytic superior hypogastric plexus block. Sympathetically mediated pain was significantly reduced or eliminated in all cases and no serious complications occurred. Superior hypogastric plexus block is recommended for diagnostic/prognostic and therape

The great auricular nerve that originates from the cervical plexus and supplies sensation to the lower part of the auricle and the skin overlying the angle of the mandible has no motor component. During an elective neck dissection for a squamous cell carcinoma of the tongue, we found that the anterior division of the great auricular nerve divided, with a long branch that passed into the submandibular triangle anterior and superficial to the facial vein, and was joined on its deep surface by the marginal mandibular division of the facial nerve. Although anatomical variants of other branches of

An improvement is achieved by palliative X-ray irradiation in about 70% out of 47 patients with scapulohumeral periarthritis and local cervical syndrome and/or cervicobrachial syndrome which was hitherto resistant to medicomechanical treatment. Higher rates of success (84%) can be achieved if the patients present an identical symptomatology, but less severe alterations of the vertebral column, even if the vertebral column is not irradiated. In case of severe alterations of the vertebral column, the success of palliative irradiation is diminished, even if the cervical vertebral column is additi

The development of the caudal pharyngeal arch arteries was studied in perfused rat embryos in the interval of embryonic day 12 1/2 to embryonic day 13 3/4 using semithin serial sections and computer-aided 3-dimensional reconstructions. The fourth pharyngeal arch artery was already developed at embryonic day 12 1/2. The sixth developed from cystic enlarged vascular sprouts at embryonic day 13 1/4. A real fifth pharyngeal arch artery was not found between the fourth and sixth artery. Instead a vascular plexus was present at embryonic day 12 1/2 which was interposed between the epithelial sheets

Peripheral (N9) and cervical (N13) SEPs as recorded at Erb's point and over the fifth cervical spinous process respectively, from the musculocutaneous, median and ulnar nerve stimulation, were examined in 30 cases of unilateral non-penetrating brachial plexus injuries. Among 14 cases (42 nerves examined) who underwent surgical exploration, the SEPs were found to give good prediction of the actual lesions in 9 cases, gave helpful information in 1 and correlated partially in the other 4. In none of the cases did SEPs give totally misleading data. The results indicated that this SEPs testing pack

BACKGROUND: Spontaneous intracranial hypotension (SIH) is a rare condition characterized by positional headache, for which contrast-enhanced magnetic resonance imaging (MRI) is the preferred diagnostic method. Although MRI reveals characteristic findings, head computed tomography (CT) is usually the first diagnostic step, but identifying features of SIH on CT is often difficult. This study was specifically designed to evaluate the utility of head CT in detecting upper cervical epidural venous engorgement as a sign of SIH.
OBSERVATIONS: Of 24 patients with SIH diagnosed between March 2011 and

BACKGROUND: Anesthesia management for patients with severe ankylosing spondylitis scheduled for total hip arthroplasty is challenging due to a potential difficult airway and difficult neuraxial block. We report 4 cases with ankylosing spondylitis successfully managed with a combination of lumbar plexus, sacral plexus and T12 paravertebral block.
CASE PRESENTATION: Four patients were scheduled for total hip arthroplasty. All of them were diagnosed as severe ankylosing spondylitis with rigidity and immobilization of cervical and lumbar spine and hip joints. A combination of T12 paravertebral bl

Brachial plexus block anesthesia is widely utilized for upper limb surgical procedures in clinical settings. The diffusion of local anesthetics within the intermuscular sulcus structure can lead to nerve paralysis. However, we report an exceptionally rare case of concurrent extensive neural blockade, with a spread far beyond expected boundaries, which carries significant clinical cautionary implications. This case report described a previously healthy patient who successfully underwent arthroscopic rotator cuff repair on the right shoulder under general anesthesia combined with a brachial plex

BACKGROUND AND PURPOSE: The objective of this study was to determine the success rate and complications of using the percutaneous approach of the external jugular vein (EJV) for placement of a totally implantable venous-access port (TIVAP) with a preoperative estimate of the detailed anatomical orientation of the cervical venous plexus using computed tomography venography (CT-V).
METHODS: A prospective cohort study of 45 patients in whom placement of a TIVAP was attempted via the right EJV was conducted. The preoperative anatomical estimation of the cervical venous plexus was performed with C

Background: Neurogenic thoracic outlet syndrome typically presents with paresthesia, pain, and impaired strength in the neck, shoulder, and arm, and is typically a diagnosis of exclusion. This condition is caused by compression of the brachial plexus, typically by a bony or soft tissue anomaly present congenitally and influenced by repetitive motion or significant trauma. Treatment typically involves removal of the first rib and anterior scalene to decompress the thoracic outlet and relieve stress to the brachial plexus if the patient has failed conservative treatment with physical therapy and

The neural plexuses are intricate networks of nerve fibers interposed between the spinal cord or anterior primary rami proximally and the most proximal portions of peripheral nerves distally. If the lumbar and sacral plexuses are considered as a single entity, then they constitute the largest peripheral nervous system structure. Each of the plexuses varies substantially from the others in its overall vulnerability to injury, the specific types of trauma or disease that most often affects it, and the ease with which it is assessed by the two laboratory diagnostic procedures in current use for d

OBJECTIVE: High-energy impact to the head, neck, and shoulder can result in cervical spine as well as brachial plexus injuries. Because cervical spine injuries are more common, this tends to be the initial focus for management. We present a case in which the initial magnetic resonance imaging (MRI) was somewhat misleading and a detailed neurological exam lead to the correct diagnosis.
CLINICAL PRESENTATION: A 19-year-old man presented to the hospital following a shoulder injury during football practice. The patient immediately complained of significant pain in his neck, shoulder, and right ar
