Accurate dried blood spots collection in the community using non-medically trained personnel could support scaling up routine viral load testing in resource limited settings.

Regular plasma HIV-RNA testing for persons living with HIV on antiretroviral therapy (ART) is now the global standard, but as many as 60% of persons in Africa today on ART do not have access to standard laboratory HIV-RNA assays. As a result, patients in Zambia often receive treatment without any means of determining true virologic failure, which poses a risk of premature switch of ART regimens and widespread HIV drug resistance. Dry blood spots (DBS) on the other hand require unskilled personnel and less complex storage supply chain so are ideal to capture viral-load results from HIV patients

Effectiveness of a community-based nutrition education intervention on metabolic control and diet and lifestyle behaviors among adults with type 2 diabetes in a low-resource primary care setting in Honduras.

BACKGROUND: Evidence on scalable, culturally adapted nutrition education for type 2 diabetes (T2D) in low-resource settings remains limited. METHODS: We conducted a 12-week quasi-experimental study among 50 adults with T2D attending a primary care clinic (intervention n = 25; control n = 25). The intervention included weekly group sessions on healthy eating, portion control, food label reading, and age-appropriate physical activity, plus individualized diet plans. Sociodemographic, anthropometric, biochemical, diet, and behavioral measures were assessed at baseline and after 12 weeks. Comparis

Risk and protective factors for sudden infant death syndrome (SIDS) in low-resource communities in Kolkata India: a mixed methods exploratory study of semi-structured interviews and survey data.

Sudden Unexpected Infant Death (SUID), Sudden Infant Death Syndrome (SIDS) and infant sleep practices are widely documented and studied in high-resource countries. Knowledge of SUID/SIDS occurrence, risk factors and protective factors in low or middle-resource countries such as India is lacking. This was an exploratory study using a mixed methods (qualitative and quantitative) approach to better understand infant sleep practices and the various factors that may influence them amongst caregivers in 5 low-income communities in Kolkata, India. Twenty-eight and 22 caregivers of infants <12 months

Patterns of health care resource utilization after macrolide treatment failure: results from a large, population-based cohort with acute sinusitis, acute bronchitis, and community-acquired pneumonia.

BACKGROUND: Macrolide antibiotics are used as first-line therapy for the treatment of respiratory tract infections. The recent emergence of macrolide-resistant pathogens is a major concern. OBJECTIVE: This study quantifies the frequency of macrolide treatment failure in respiratory infections and examines its impact on health care use. METHODS: Patients with respiratory infections treated with macrolides in outpatient clinics from January to December 2002 were identified from a health insurance claims database. Macrolide treatment failure was defined as the receipt of a second antibiotic, di

High endorsement of resource conservation in both samples alongside divergent perceived waste-sorting contexts: an exploratory comparison of two university-community study areas in Hong Kong and mainland China.

INTRODUCTION: Household waste sorting is a situated pro-environmental choice shaped not only by environmental evaluations but also by perceived norms, sorting knowledge, motivational cues, and practical opportunities or barriers. METHODS: We compared item-level responses in 102 complete questionnaire records collected from two university-community study areas, one in Hong Kong SAR, China, and the other in Chuzhou, Anhui Province, China (51 records per area), using an 18-item, five-point questionnaire adapted from published studies. No archived analytic record was excluded. We report raw respo

Expanded adverse childhood experiences and postpartum mental health among an under-resourced sample of Spanish- and English-speaking mothers referred for community-based mental health services during pregnancy.

BACKGROUND: Adverse childhood experiences (ACEs) historically include 10 types of adversities (the "Conventional 10"), developed in the context of the landmark CDC-Kaiser ACEs study. These 10 ACEs are associated with an increased risk of perinatal depression and anxiety, but they may also overlook traumas that harm disadvantaged and minoritized groups. This study investigated an Expanded set of ACEs (the Conventional 10 plus five additional) among a sample of Spanish- and English-speaking postpartum mothers who had previously received a referral for a prenatal mental health concern from a comm

Mitochondrial Disease Sequence Data Resource (MSeqDR): a global grass-roots consortium to facilitate deposition, curation, annotation, and integrated analysis of genomic data for the mitochondrial disease clinical and research communities.

Success rates for genomic analyses of highly heterogeneous disorders can be greatly improved if a large cohort of patient data is assembled to enhance collective capabilities for accurate sequence variant annotation, analysis, and interpretation. Indeed, molecular diagnostics requires the establishment of robust data resources to enable data sharing that informs accurate understanding of genes, variants, and phenotypes. The "Mitochondrial Disease Sequence Data Resource (MSeqDR) Consortium" is a grass-roots effort facilitated by the United Mitochondrial Disease Foundation to identify and priori

Trial Design for a Diagnostic Accuracy Study of a Point-of-Care Test for the Detection of Taenia solium Taeniosis and (Neuro)Cysticercosis in Community Settings of Highly Endemic, Resource-Poor Areas in Zambia: Challenges and Rationale.

Field-applicable, high-quality, and low-cost diagnostic tools are urgently needed for Taenia solium. The aim of this paper is to describe the design, challenges, and rationale for the design of a diagnostic accuracy study in low-resource community settings in Zambia. The trial was designed as a prospective study with a two-stage design to evaluate a new point-of-care test (TS POC) for the detection of taeniosis and (neuro)cysticercosis. Participants within randomly selected households were tested with the TS POC test (index test). Participants who tested TS POC positive for taeniosis and/or cy

Filling the human resource gap through public-private partnership: Can private, community-based skilled birth attendants improve maternal health service utilization and health outcomes in a remote region of Bangladesh?

BACKGROUND: In Sunamganj there are fewer than four skilled providers per 10,000 population and just 27% of births are assisted by a skilled attendant. We evaluate a private community skilled birth attendant (P-CSBA) model, developed through the GSK-CARE Frontline Health Worker Programme, designed to address this gap and report on changes in service utilization and health outcomes from baseline to three years post-baseline. METHODS: This analysis presents the results of a pre-post cross sectional design. A baseline survey (n = 1800) was conducted using a multistage cluster sampling approach. T

An Efficient Multi-Dimensional Resource Allocation Mechanism for Beam-Hopping in LEO Satellite Network.

Low Earth Orbit (LEO) satellite communication networks have become an important means to provide internet access services for areas with limited infrastructure. Compared with the Geostationary Earth Orbit (GEO) satellites, the LEO satellites have limited on-board communication caching and calculating resources. Furthermore, the distribution of traffic requests is dynamically changing and uneven due to the relative movement between the LEO satellites and the ground. Therefore, how to schedule the multi-dimensional resources is an important issue for the LEO satellite communication networks. Bea

Predictors of use of services among dementia caregivers.

Caregivers of persons with dementia do not use community resources until late in the disease process, despite the fact that judicious use of community resources can delay nursing home admission. Data from the National Caregiver Training Project, based on Hall and Buckwalter's (1987) progressively lowered stress threshold (PLST) model, were used to examine variables related to use of community resources. Spouse and adult child caregivers were divided into two groups based on amount of community resources used per week. Within this geographically diverse sample of caregivers, 64% did not use pro

Food Insecurity Screening and Immediate Access to Prepackaged Groceries in the Pediatric Emergency Department: A Pilot Study.

OBJECTIVE: Food insecurity (FI) is prevalent in many urban settings across the United States. Many vulnerable families who lack access to a medical home utilize the pediatric emergency department (PED), making the PED a critical access point to address FI. We aimed to understand caregivers' experiences in receiving prepackaged groceries and community resources within a PED. METHODS: In this pilot initiative, caregivers who screened positive for FI in the PED received prepackaged groceries for their household and a list of community resources. Caregivers were interviewed to understand their ex

Accessibility patterns and community integration among previously homeless adults: a Geographic Information Systems (GIS) approach.

Although a desired rehabilitation goal, research continues to document that community integration significantly lags behind housing stability success rates for people of a variety of ages who used to be homeless. While accessibility to resources is an environmental factor that may promote or impede integration activity, there has been little empirical investigation into the impact of proximity of community features on resource use and integration. Using a Geographic Information Systems (GIS) approach, the current study examines how accessibility or proximity to community features in Boston, Un

Universal Prevention Exposure as a Moderator of the Community Context: Findings from the PROSPER Project.

This study examined how participation in a universal family skills-building program may interact with community risks and resources to produce youth outcomes. Prior research has noted community-level variability in risk and protective factors, but thus far no study has examined the role that participation on a community-wide intervention may play in moderating the effects of community risks or resources. The study included 14 communities (seven in Iowa, seven in Pennsylvania) that implemented a family focused evidence-based program as part of the PROSPER project. Community level variables incl

Getting On the Same Page? Insights from the Public Discourse of Managed Retreat in Atlantic Canada.

Managed retreat is a term used in academia and increasingly in policy for the movement of people and infrastructure away from high-risk areas and natural hazards. Climate change impacts are anticipated to increase the need for managed retreat. Multiple terms for managed retreat are often used in the literature, which creates a diffuse literature and confusion among scholars. But the real challenge comes with engaging the public, for whom the term is largely unknown, and the concept represents a threat. We conducted a longitudinal content analysis of news media to explore the public discourse a

Vocal individuality in drumming in great spotted woodpecker-A biological perspective and implications for conservation.

Animals-including conservation biologists-use acoustic signals to recognise and track individuals. The majority of research on this phenomenon has focused on sounds generated by vocal organs (e.g., larynx or syrinx). However, animals also produce sounds using other parts of the body, such as the wings, tail, legs, or bill. In this study we focused on non-syrinx vocalisation of the great spotted woodpecker, called drumming. Drumming consists of strokes of a bill on a tree in short, repeated series, and is performed by both males and females to attract mates and deter rivals. Here, we considered
