The intrauterine contraceptive device, "Multiload Cu 250": a regulatory problem.

Reports to the control agencies in Sweden and Australia of fractures of the intrauterine contraceptive device. Multiload Cu 250, led to a review of the safety of this device in these countries. The review revealed that fractures of the device had occurred in 15 other countries but in these countries only 5% of reports had been directed to the relevant national control authorities. The remainder either had been reported to the sponsor or had appeared in the published literature. An additional problem identified was one of thread rupture with the resultant difficulty in removal and the need for

Transcatheter Mitral Valve Interventions for Mitral Regurgitation: A Review of Mitral Annuloplasty, Valve Replacement, and Chordal Repair Devices.

Significant mitral regurgitation (MR) is the most common valvular disease in the United States in patients older than 75. However, many patients with severe MR are at a high risk for surgical repair due to other significant comorbidities. Over the past decade, many transcatheter mitral valve devices have been studied that address the different mechanisms of MR, but only a few have received a Conformité Européene (CE) mark or United States Food and Drug Administration (FDA) approval. The Carillon, Cardioband, and Mitralign all received CE marking for percutaneous mitral annuloplasty, while the

Wearable Ultrasound Devices for Continuous Bladder Monitoring: Emerging Utility and Current Limitations in Neurourology.

Monitoring bladder filling is crucial in patients with neurogenic lower urinary tract dysfunction, particularly for safe and timely clean intermittent catheterization. Wearable ultrasound devices have recently gained attention as potential aids for continuous bladder monitoring, but they differ fundamentally from traditional bladder scanners. While portable scanners directly measure postvoid residual volume in milliliters, first-generation wearable devices using amplitude mode (A-mode) ultrasound primarily monitor bladder fullness trends and provide threshold-based alerts rather than precise q

[Atherectomy with a Novel Device Offering Wider Options].

Catheter-based atherectomy has been discussed for some years, although the conclusions have been controversial. Recent study data did show the feasibility of multifunctional atherectomy devices for complex lesion morphologies, with moderate material use. This is then thus an alternative to bypass surgery, although a direct comparison is lacking. The results of the ByCross approval study were compared to technical and clinical data from various atherectomy systems in terms of range of indication, limitations as defined by the manufacturer, the success and complication rate and the instructions f

Overview of high-risk medical device recalls in obstetrics and gynecology from 2002 through 2016: implications for device safety.

The field of women's health has endured numerous recent controversies involving medical devices such as pelvic meshes, laparoscopic morcellators, and a hysteroscopic sterilization device. With the recent passage of the 21st Century Cures Act, new legislation will change how the Food and Drug Administration regulates medical devices. Given these controversies and new changes, we investigated high-risk, class I recalls in women's health from 2002 through 2016. Class I recalls for medical devices are defined by the Food and Drug Administration as the most serious recall events and are designated

Efficacy and safety of a novel semiautomated vitrification system for human blastocysts: a study protocol for a multicentre randomised controlled device trial with a live-birth extension study.

INTRODUCTION: Cryopreservation, particularly vitrification, is a cornerstone of assisted reproductive technology, preventing ice crystal damage to ensure high survival rates. However, existing manual systems, such as Cryotop, depend on the embryologist's skill, which introduces procedural variability and hinders standardisation. While some semiautomated systems have been developed, issues with laboratory integration and limited throughput have prevented their widespread adoption. To address this, Biorocks (China) has created a novel semiautomated vitrification system. This study consists of a

[Pediatric and congenital arrhythmia treatment : A complex interplay between expertise, re-imbursement, and industry].

Children, adolescents, and patients with congenital heart defects exhibit a unique and broad spectrum of arrhythmias. Diagnosis and treatment of these arrhythmias requires highly specialized medical care structures. In addition to highly qualified medical expertise, this includes appropriate antiarrhythmic drugs and medical devices specifically tailored to this patient group for catheter ablation and device therapy. The increasing economization of the healthcare system and legal requirements-particularly the EU Medical Device Regulation-are jeopardizing existing and urgently needed new care st

Asystole and Laryngospasm After Vagal Nerve Stimulator Generator Replacement: A Case Report.

Vagal nerve stimulator (VNS) generator replacements continue to increase given the various indications of the device since its FDA approval in 1997. VNS generator replacements are usually safe and serious cardiac complications are not typically encountered during VNS generator replacement surgeries. We present a rare case of VNS generator replacement complicated by asystole and laryngospasm while exploring the plausible factors contributing to these events in our patient. The general recommendations to prevent and manage VNS therapy-related complications in the perioperative setting are descri

European regulators' views on a wearable-derived performance measurement of ambulation for Duchenne muscular dystrophy regulatory trials.

Development of novel therapies for Duchenne muscular dystrophy (DMD) are driving the need for more efficient ways of detecting changes in disease- progression in DMD [1]. However, medicines' approval must be based on outcome measures that are acceptable from a regulatory perspective. In this article, European regulators provide an update on the recent regulatory consideration of a new endpoint (Stride Velocity 95th Centile (SV95C)) that could be used in therapeutic DMD trials. This new endpoint aims to quantify a patient's ambulation directly, reliably and continuously in a home environment wi

Transcatheter Closure of Intracardiac Shunts.

Intracardiac shunts, such as atrial septal defects (ASDs), patent foramen ovales (PFOs), and ventricular septal defects (VSDs), are common forms of congenital or acquired heart disease. Traditional treatment has consisted of surgical closure. Transcatheter closure using implantable devices is now an alternative approach to the treatment of these lesions. Transcatheter closure offers advantages over surgical closure: 1) it is less invasive, resulting in shorter recovery times, less hospital time, and no scarring; 2) it avoids the deleterious neurocognitive effects of cardiopulmonary bypass; 3)
