[Fetal injury and alcohol drinking during pregnancy].

BACKGROUND: There is still an ongoing discussion whether moderate alcohol consumption during pregnancy is harmful to the fetus. In most western countries, however, total abstinence is recommended. MATERIAL AND METHODS: Over the last five years we have seen 64 infants and children with alcohol-related disorders. We present the clinical records of one alcohol-damaged child, and present the latest recommended categorisation of alcohol-related disorders in infants and children. RESULTS: In our experience, some women with heavy alcohol consumption are allowed to continue their abuse without inter

[Pathologic anatomy of hard drinking and alcoholism].

The analysis of 826 autopsies is presented. 654 persons in their life abused alcohol but refused treatment; they were considered as hard drinkers. 172 persons were repeatedly treated for alcoholism and were included into the group of alcoholics. The control group consisted of persons who had not abused alcohol. Morphological and morphometric methods were used. It is shown that chronic alcoholic intoxication involves all internal organs with the same trend in pathological processes in hard drinkers and alcoholics. There was progressing alteration of microcirculatory bed, fat degeneration of par

Trihalomethanes in drinking water and spontaneous abortion.

Trihalomethanes (chloroform, bromoform, bromodichloromethane, and chlorodibromomethane) are common contaminants of chlorinated drinking water. Although animal data indicate that these compounds may be reproductive toxicants, little information exists on their relation to spontaneous abortion in humans. We examined exposure to trihalomethanes and spontaneous abortion in a prospective study of 5,144 pregnant women in a prepaid health plan. Seventy-eight drinking water utilities provided concurrent trihalomethane sampling data. We calculated total trihalomethane levels by averaging all measuremen

Resolution of drinking problems without formal treatment.

TOPIC: Self-resolution of alcohol abuse among individuals with mild to moderate drinking problems. PURPOSE: To examine self-resolution of drinking problems and clinical/research implications. SOURCE: Review of the literature. CONCLUSION: Two pathways toward self-resolution of alcohol problems have been proposed. One appears to be relatively spontaneous, the other involves ongoing cognitive appraisals and a conscious effort to change. Nurses have the potential to play an important role in encouraging self-change by conducting screenings, implementing brief interventions, and promoting the us

Public drinking water contamination and birth outcomes.

The effects of public drinking water contamination on birth outcomes were evaluated in an area of northern New Jersey. After excluding plural births and chromosomal defects, 80,938 live births and 594 fetal deaths that occurred during the period 1985-1988 were studied. Information on birth outcome status and maternal risk factors was obtained from vital records and the New Jersey Birth Defects Registry. Monthly exposures during pregnancy were estimated for all births using tap water sample data. Odds ratios of > or = 1.50 were found for the following: total trihalomethanes with small for gesta

Dispositional traits as risk in problem drinking.

A trait-dispositional paradigm for conceptualizing personality provided the framework for investigating the relationship between personality dispositions and drinking problems. This approach was compared directly with personality research based on the Minnesota Multiphasic Personality Inventory (MMPI). A total of 241 subjects (192 men and 49 women) were tested at a mandatory Driving While Intoxicated (DWI) first-offender education program. Information was gathered from the Michigan Alcoholism Screening Test (MAST), Blood Alcohol Concentration (BAC) at time of arrest, and two personality tests-

Reproductive risks from contaminants in drinking water.

Reproductive toxicity is a complex subject which, besides birth defects or sterility, includes adverse effects which may be less readily observed but more relevant to chronic low-level exposures (e.g., impaired functional development of target organs or systems, secretion of toxic chemicals in maternal milk). Sodium chlorite, when present at high concentration, may be related to impaired reproduction; among water chlorination by-products, further research is required on the developmental toxicity of chloroacetic acid and dichloro- and trichloro acetonitrile. Among the water pollutants which ma

Moderate maternal drinking and outcome of pregnancy.

The adverse effect of light or moderate maternal drinking during pregnancy on the well being of the newborn has been investigated. The study group included 2145 live births in the obstetric units of 11 Italian cities between February 1989 and July 1990. A detailed life style questionnaire was administered to the mothers. Information on the newborn was collected from clinical records as well as from a clinical examination. Both univariate and multivariate analyses were suggestive of a decrease in mean birth weight associated with maternal drinking pregnancy, especially in women who also smoked

What happens to drinking after therapeutic intervention?

Eighty-four men and 51 women who had been recruited to treatment through newspaper advertisements were matched pairwise and randomly assigned to either brief counselling or short-term group therapy. All participants were followed-up after 3, 9, 15 and 21 months by personal interviews and medical examinations. The average reduction in alcohol consumption was greatest to start with in the group attending group therapy, but this group experienced greater set-back during the following period. At 21 months there were no significant differences between the two groups as regards amount of alcohol con

Drinking beliefs and behaviors among Irish adolescents.

Drinking beliefs and behaviors were investigated in a survey of 2,700 Irish postprimary students. Overall, 47% of the students reported drinking within the month prior to the survey. Drinking was most frequent among older students and males. A regression analysis indicated that perceived peer drinking was the primary predictor of current alcohol use. Smoking and involvement with other problem behaviors were also important, and parental disapproval had a small effect. This pattern of results is similar to that found in other countries and provides evidence for the applicability of social learni

Selenium in pregnancy: effect of maternal drinking.

To study the effect of drinking alcohol during pregnancy on serum selenium, selenium concentrations were measured in 41 women (ten abstinent, 31 problem drinkers) throughout pregnancy as well as in 24 nonpregnant abstinent women. In abstinent pregnant women, serum selenium decreased with advancing gestational age, a pattern not affected by moderate maternal drinking (42 to 126 g of ethanol weekly, N = 14). Seventeen women drank heavily (more than 140 g of ethanol weekly), and nine of them (53%) gave birth to infants with fetal alcohol syndrome. The women with infants having fetal alcohol syndr

Maternal drinking behavior and decreased intrauterine growth.

Reduced intrauterine growth has been a widely observed effect of maternal alcohol use during pregnancy in both animals and humans, although results in studies of humans have been inconsistent. The goal of this prospective study was to evaluate the relative importance of volume, frequency, and beverage source of alcohol as determinants of infant birthweight, while adjusting for other known determinants using multivariate techniques. In a sample of 5,093 maternal-infant pairs, 44 potential determinants of intrauterine growth were evaluated. The frequency of beer drinking, controlled for wine dri

Drinking by alcoholic patients following hospital admission.

Ad lib fluid intake of abstinent alcoholics and alcoholics in acute withdrawal exceeded that of psychiatric controls for 3 d following admission to hospital. The urinary output of abstinent alcoholics reflected their high fluid intake, but alcoholics in withdrawal displayed an initial oliguria. Routine blood and urine analysis did not provide any evidence of dehydration among alcoholics in withdrawal. Some possible causal factors contributing to a generalized alcoholic polydipsia are outlined.

Anencephalus, drinking water, geomagnetism and cosmic radiation.

The mortality rates from anencephalus from 1950-1969 in Canadian cities are shown to be strongly correlated with city growth rate and with horizontal geomagnetic flux, which is directly related to the intensity of cosmic radiation. They are also shown to have some association with the magnesium content of drinking water. Prior work with these data which showed associations with magnesium in drinking water, mean income, latitude and longitude was found to be inadequate because it dismissed the observed geographic associations as having little biological meaning, and because the important variab
