[New developments in inner ear imaging for Meniere's disease].

BACKGROUND: The current Bárány Society diagnostic criteria for Meniere's disease (MD) do not include radiological aspects. However, computed tomography (CT), native magnetic resonance imaging (MRI), and time-delayed contrast-enhanced inner ear MRI (iMRI) for visualization of endolymphatic hydrops (ELH) have been used in research for years. OBJECTIVE: The aim of this narrative review is to present the current state of research and provide an outlook on future developments. Furthermore, existing possibilities for quantifying inner ear imaging data are shown. MATERIALS AND METHODS: PubMed, MEDL

Autoimmune Inner Ear Disease from a Rheumatologic Perspective.

Background/Objectives: Autoimmune inner ear disease (AIED) causes sensorineural hearing loss that classically presents as fluctuating, asymmetric loss of hearing. Associated vestibular and other ear symptoms can be present in many patients. First-line treatment of AIED is high-dose corticosteroids. AIED can present either as a primary condition limited to ear involvement or secondary, as part of an underlying systemic autoimmune rheumatic disease, the most common of which include vasculitis and relapsing polychondritis. We described our cohort of primary AIED, including demographics, treatment

Inner Ear Dysfunction in Thyroid Disease: A Scoping Review.

Background and Objectives: Sensorineural hearing loss (SNHL) is influenced by various causes, including thyroid diseases. For example, hypothyroidism and thyroid autoimmunity can damage the inner ear through hormonal, immune, and vascular mechanisms. Vestibular disorders like Ménière's disease (MD) and benign paroxysmal positional vertigo (BPPV) also show possible associations with thyroid dysfunction. Materials and Methods: A review following PRISMA guidelines searched PubMed, Scopus, and Google Scholar for studies linking thyroid disorders with inner ear dysfunction. Results: Out of 985 scre

Evidence for a left ear bias in incidence of Meniere's disease.

OBJECTIVE: To explore a side of lesion differences in Meniere's disease (MD). DESIGN: A retrospective review (2019-2021) was conducted of patients with definite MD, as defined by 2015 Bárány Society diagnostic criteria. Testing information included pure tone audiometry, tympanometry, and extra-tympanic electrocochleography (ECochG). Normative ECochG data from healthy subjects determined the 95% cut-off value for clinical abnormality. STUDY SAMPLE: 107 patients with definite MD were included in the study and 40 healthy controls. RESULTS: The review identified 75 patients with unilateral MD a

Magnetic resonance imaging of the inner ear in Meniere's disease.

Recent magnetic resonance imaging (MRI) techniques have made it possible to examine the compartments of the cochlea using gadolidium-chelate (GdC) as a contrast agent. As GdC loads into the perilymph space without entering the endolymph in healthy inner ears, the technique provides possibilities to visualize the different cochlear compartments and evaluate the integrity of the inner ear barriers. This critical review presents the recent advancements in the inner ear MRI technology, contrast agent application and the correlated ototoxicity study, and the uptake dynamics of GdC in the inner ear.

Middle-ear disease and schizophrenia: case-control study.

BACKGROUND: One hundred years ago psychiatrists thought that ear disease could cause insanity by irritation of the brain. Current understanding of the role of the temporal lobes in schizophrenia and their proximity to the middle ear supports this hypothesis. AIMS: To establish the rate of middle-ear disease pre-dating the onset of schizophrenia. METHOD: Eighty-four patients with schizophrenia were each matched to four non-psychiatric controls by age, gender and season of birth. History of ear disease was obtained from general practice records. Additional information on symptoms was collected

Validation of tele-otology to diagnose ear disease in children.

OBJECTIVE: To determine if digitised still eardrum images, with a clinical history, and audiometry and tympanometry data provide sufficient information to an ear specialist to make an assessment of a patient. METHODS: 66 children (9 months to 16 years) from remote communities were assessed by an ear specialist by standard otoscopy, using a clinical history, audiometry and tympanometry. Up to five images of each ear were digitised. At a later date, the ear specialist made observations, diagnoses and recommendations for management from the images and clinical data. RESULTS: There was a signifi

[Middle ear overpressure treatment in severe Menière's disease].

To evaluate the therapeutic effect of middle ear pressure pulse on endolymphatic hydrops in humans, Meniett20k was used for 12 months in two severe elderly patients with definite Meniere's disease. The average number of vertigo occurrences in the 6 months before treatment and in the 12 months after treatment started was compared. Numeric values (NV) among patients were calculated and categorized into an improved group (NV = 1-40). For hearing function, the pure-tone average (PTA) before and 12 months after treatment started was compared. PTA was calculated based on the following equation: PTA

Clinical and experimental studies of autoimmune inner ear disease.

Otolaryngologists have long sought to identify causes of sensorineural hearing loss that could be reversed by medical treatment. An increasing amount of clinical and experimental evidence indicates that this postulated entity is related to autoimmune inner ear disease. Because of the lack of well-defined detection methods for identifying autoimmune processes within the human inner ear and the fact that it is one of the few organs of the body not amenable to diagnostic biopsy there has been great interest in developing animal models that mimic clinical entities. Different models will be present

Current trends in managing chronic middle ear disease.

Major advances in reconstruction continue to be made in the treatment of chronic middle ear disease, while little prophylaxis is available and indications for intervention remain in many cases controversial. This reflects our lack of knowledge of the true causes of these diseases.

The prevalence of middle ear disease in the adult British population.

The UK National Study of Hearing set out to ascertain the prevalence of hearing impairments of various magnitudes, the prevalence of ear disease and the associated risk factors, and to estimate the percentage of individuals requiring some form of management. A stratified sample of 2708 British adults, aged 18-80 years, was chosen from a sample of 48,313 adults, randomly selected from the electoral roll, for a full otological and audiological assessment. This paper deals primarily with the middle ear results. Otoscopically, 2.6% of British adults had inactive and 1.5% had active chronic otitis

Assessing middle ear disease: beyond visual otoscopy.

Visual otoscopy, the first step in the diagnosis of middle ear conditions, permits an assessment based on the color and appearance of the tympanic membrane. With pneumatic otoscopy, the mobility of the tympanic membrane can be evaluated. Several other techniques can be used to augment visual and pneumatic otoscopy. Impedance tympanometry provides an objective record of tympanic membrane movement. The acoustic otoscope evaluates middle ear contents by the reflectivity of sound from the tympanic membrane.
