BACKGROUND: Over the past 2 decades, the surgical treatment of lymphedema has advanced considerably with lymphovenous bypass and vascularized lymph node transfer. Despite these advances, some patients can only be treated with a radical debulking procedure, commonly known as Charles procedure. However, the Charles procedure is not a commonly performed procedure and can have significant risks such as fluid shifts, blood loss and wound infections. In this article, we present our experience with Charles procedure and share pearls to perform this procedure expediently while avoiding potential pitfa

INTRODUCTION: Altered copper metabolism has been reported in Alzheimer's disease (AD), with post mortem brain tissue showing depletion of 65Cu compared with 63Cu. Whether such isotopic shifts can be detected in biofluids remains unknown.
METHODS: We analyzed copper isotope composition in human cerebrospinal fluid (CSF) and serum from AD patients and controls using multi-collector inductively coupled plasma mass spectrometry (MC-ICP-MS) and single quadrupole inductively coupled plasma mass spectrometry (SQ-ICP-MS). Samples included 22 CSFs (10 AD, 12 controls), 54 sera (28 AD, 26 controls).

Central sleep apnea and Cheyne-Stokes respiration are commonly observed breathing patterns during sleep in patients with congestive heart failure. Common risk factors are male gender, older age, presence of atrial fibrillation, and daytime hypocapnia. Proposed mechanisms include augmented peripheral and central chemoreceptor sensitivity, which increase ventilator instability during both wakefulness and sleep; diminished cerebrovascular reactivity and increased circulation time, which impair the normal buffering of Paco2 and hydrogen ions and delay the detection of changes in Paco2 during sleep

The phase behavior of fluids under nanoconfinement in shale reservoirs holds significant theoretical implications for development and utilization of underground energy, yet remains elusive due to the complex interplay of fluid-rock interactions at the nanoscale. Molecular simulation has emerged as an indispensable tool to decipher these microscopic processes. While the existing literature has explored various aspects of confined fluid phase behavior by molecular simulation, a systematic review is still lacking that focuses specifically on advances and challenges in thermodynamic shift and phas

PURPOSE: Undiagnosed dehydration compromises health outcomes across many populations. Existing dehydration diagnostics require invasive bodily fluid sampling or are easily confounded by fluid and electrolyte intake, environment, and physical activity limiting widespread adoption. We present a portable MR sensor designed to measure intramuscular fluid shifts to identify volume depletion.
METHODS: Fluid loss is induced via a mouse model of thermal dehydration (37°C; 15-20% relative humidity). We demonstrate quantification of fluid loss induced by hyperosmotic dehydration with multicomponent T2

BACKGROUND: Fluid bolus administration is a standard treatment for hypotension. However, the effectiveness of the traditional prophylactic bolus in parturients undergoing spinal anesthesia for cesarean delivery has been questioned. One potential mechanism for the failure of a prophylactic fluid bolus to prevent hypotension is hypervolemia-induced destruction of the endothelial glycocalyx, a structure that plays a vital role in regulating intravascular fluid shifts.
METHODS: Thirty healthy parturients undergoing elective cesarean delivery under spinal anesthesia were recruited. Known endotheli

SIGNIFICANCE: Late postoperative capsular block syndrome is a treatable complication presenting months to years after cataract surgery using continuous curvilinear anterior capsulorhexis and a posterior chamber intraocular lens implantation. Patients may present to optometric practices because of symptom onset significantly after the immediate post-operative period.
PURPOSE: The purpose of this study was to describe the defining clinical features, proposed pathophysiology, and treatment of late postoperative capsular block syndrome. Case 1 was a 68-year-old man who presented with a sudden ons

Loop diuretics are a cornerstone in the management of fluid overload in critically ill patients; however, their use is often complicated by variable pharmacokinetics and the phenomenon of diuretic resistance. This narrative review comprehensively examines the pharmacokinetic properties of loop diuretics and discusses the implications of altered pharmacokinetics due to factors such as organ dysfunction, fluid shifts, and concomitant medications, highlighting the challenges in achieving therapeutic targets. Furthermore, we explore the adverse events associated with loop diuretic administration,

We present a case involving a 60-year-old man with subacute delirium characterized by challenging attention shifts and obstinate behavior, contrasting with his usual mild-mannered personality. The patient developed pneumonia and a urinary tract infection following the onset of subacute delirium. Despite exhaustive investigations, the cause remained elusive until cerebrospinal fluid analysis revealed Tau positivity. Our overview suggests neurodegenerative diseases as the primary cause, rather than infectious or autoimmune factors. The case underscores a significant association between Tau and d

Despite the multiple causes of the shock state, all causes possess the common abnormality of oxygen supply not meeting tissue metabolic demands. Compensatory mechanisms may mask the severity of hypoxemia and hypoperfusion, since catecholamines and extracellular fluid shifts initially compensate for the physiologic derangements associated with patients in shock. Despite the achievement of normal physiologic parameters after resuscitation, significant metabolic acidosis may continue to be present in the tissues, as evidenced by increased lactate levels and metabolic acidosis. This review discuss

We report the case of a patient who experienced recurrent elevations of intraocular pressure during haemodialysis. We measured a rise in intraocular pressure of 10-12 mmHg after dialysis. The lens showed intumescence, and a ciliary block was detected by means of Orbscan II. Within 6 h findings had normalized. Changes in serum osmolarity due to haemodialysis may result in fluid shifts between different compartments. Decrease of serum osmolarity during dialysis may lead to lens swelling and thus cause a ciliary block. This may end with regression of the lens swelling.

Upper extremity arterial injuries in preterm infants are usually of iatrogenic origin. Current microsurgical techniques permit extremity revascularization in these patients. The authors report the microsurgical repair of a 0.7-mm brachial artery in a 940-g preterm infant. The preterm infant warrants special consideration due to physiological immaturity. Rapid fluid shifts, a relative polycythemia, and the potential for low cardiac output states increase the risk for vascular thrombosis. Systemic heparinization is contraindicated in this population due to the risk of intraventricular hemorrhage

A new method for surgical treatment of secondary glaucoma (neovascular, postuveal, traumatic, and aphakic) is described. The aims of the new method are as follows: regulation of ophthalmic tone in the postoperative period, pain relief, organ preservation, stabilization of the process, and creation of reliable routes of intraocular fluid discharge with formation of a stable diffuse functional pad. Seven patients (8 eyes) aged 13-70 years were operated on and followed up for up to 6 months. Preliminary results are presented. Positive shifts were observed in all cases.

The approach to fluid resuscitation in burn shock continues to be refined in step with improved knowledge of the complex fluid, electrolyte, and protein shifts that characterize this form of shock. Local burn tissue and generalized nonburn tissue edema occur initially after injury because of the release of histamine, which causes increased microvascular permeability. Subsequent edema formation in burned and nonburned tissue occurs according to distinctly different mechanisms. Burn tissue edema forms because of direct thermal injury to endothelial cells and increased burn tissue osmolarity. Non

Given the paucity of information on local tissue water (LTW) in the upper extremity and trunk of women after breast cancer surgery, the purpose of this study was to compare tissue dielectric constant (TDC) values between the affected and unaffected sides of breast cancer survivors with and without upper extremity lymphoedema (LE). Differences in LTW were assessed using the TDC method for three sites in the upper limbs, three sites in the lateral thorax and two sites on the back. Additional measures included demographic and clinical characteristics, arm circumference and bioimpedance analysis.

The water sodium overload is a factor of morbi-mortality and its treatment is one of the markers of adequacy of the hemodialysis treatment. Its first clinical assessment was improved by tools such as echocardiography and ultrasonography of the inferior vena cava, the per-dialytic curve of plasma volume, measuring BNP or proBNP and by impedancemetry. The combination of the evaluation of these parameters and of the clinical situation allows one to assess the extracellular overload, the state of the blood volume and the potential of plasma refilling. The latter is a key factor of the per-dialytic

BACKGROUND: On the basis of cardiovascular compliance, hemodialysis (HD) patients can be classified as hypotension prone (HP) or hypotension resistant (HR).
METHODS: We compare the hemodynamic behavior and myocardial performances in 6 HP and 6 HR patients before and after an isolated ultrafiltration (IU) session removing 3% of total body water.
RESULTS: HP show higher basal plasma angiotensin II levels during IU (p<0.01), whereas angiotensin II remained unchanged in HR patients (p<0.001 between groups). The percentage changes of plasma volume (PV) was similar in the 2 groups. A significant r

The effect of space flight and microgravity on the chemical senses is reviewed. Skylab-4 and Soyuz 30-31 studies revealed changes in taste thresholds while no effect was found in a Canadian investigation (41-G) and conflicting results were obtained on another Soyuz mission. Two simulated microgravity studies found no effect on taste or smell sensitivity; while 5 other studies found an effect. Microgravity induces physiological changes including an upward shift of body fluids toward the head, which may lead to an attenuation of the olfactory component in the flavor of foods. Chemosensory change

Physiotherapists utilise the effect of gravity in treatment of conditions associated with increased limb volume. In contrast with knowledge about the relationship between body position and lower limb volume, the effect of body position on arm volume is not well understood. To assess the efficacy of a simple intervention proposed for patients to reduce arm volume during sleep, this study investigated the effects on forearm and hand volume of two hours of head-up tilt in supine at three angles of 8, 11 and 14 degrees. Six asymptomatic subjects were measured at each tilt angle over a three-week p
