Asthma exacerbation profile of benralizumab for severe eosinophilic asthma (the BenRex study): a multicentre, prospective cohort study.

BACKGROUND: Benralizumab, an interleukin-5 receptor α antagonist, depletes blood eosinophils, reducing exacerbations of severe asthma by approximately 50% versus placebo. In this study, we aimed to characterise mechanisms underlying exacerbations occurring on benralizumab. METHODS: BenRex, a multicentre, prospective cohort study, recruited participants meeting national licensing criteria for benralizumab for asthma. The study was conducted in 15 UK severe asthma centres. After collecting baseline data, open-label benralizumab was administered for 12-18 months. At exacerbation, participants at

Blood Eosinophils and Exhaled Nitric Oxide: Surrogate Biomarkers of Airway Eosinophilia in Stable COPD and Exacerbation.

In recent years, tremendous efforts have been devoted to characterizing the inflammatory processes in chronic obstructive pulmonary disease (COPD) in order to provide more personalized treatment for COPD patients. While it has proved difficult to identify COPD-specific inflammatory pathways, the distinction between eosinophilic and non-eosinophilic airway inflammation has gained clinical relevance. Evidence has shown that sputum eosinophil counts are increased in a subset of COPD patients and that these patients are more responsive to oral or inhaled corticosteroid therapy. Due to feasibility

Is residential ambient air limonene associated with asthma? Findings from the Canadian Health Measures Survey.

Asthma is a common chronic disease. Exposure to limonene is commonplace because it may be used in the manufacture of personal care products and household cleaners. We investigated the influence of limonene on asthma in a population-based sample of approximately 3000 Canadians. During the Canada Health Measures Survey, carried out between 2012 and 2013, participants were asked if they wheezed or had a diagnosis of asthma, and underwent spirometry and measurement of the fraction of exhaled nitric oxide (FeNO). These variables were tested for an association with limonene concentrations measured i

Unlocking the potential of bronchoalveolar lavage: a promising supplement therapy for moderate-to-severe allergic asthma.

BACKGROUND: Bronchoalveolar lavage holds potential therapeutic value for moderate-to-severe asthma that is poorly controlled with standard nebulizer therapy. To evaluate the efficacy and safety of bronchoalveolar lavage (BAL) as an adjunctive therapy in patients with moderate-to-severe allergic asthma inadequately controlled by standard inhaled corticosteroids and long-acting beta₂-agonists therapy. METHODS: In this retrospective study conducted between July 2021 and December 2024, eligible patients were allocated to a control group receiving standard high-pressure nebulizer therapy or a stud

Effect of nebulized beclomethasone on airway inflammation and clinical status of children with allergic asthma and rhinitis: a randomized, double-blind, placebo-controlled study.

We aimed to evaluate the therapeutic effect of nebulized beclomethasone dipropionate (nBDP) on both allergic asthma and rhinitis. In a randomized, double-blind, placebo-controlled study, 40 children (mean age 10.7 ± 2.1 years) with allergic asthma and rhinitis received either nBDP (daily dose of 800 µg, administered twice daily) or placebo for 4 weeks (with a face mask), after a 2-week run-in period of clinical assessment. Nasal and oral fractional exhaled nitric oxide (FeNO) measurements together with pulmonary function tests, nasal and oral exhaled breath condensate (EBC) collection for pH a

What does a single exhaled nitric oxide measurement tell us in asthmatic children?

BACKGROUND: Due to the multiple factors affecting exhaled nitric oxide (NO) value, physicians are often puzzled by the result of a single measurement in asthmatic patients. OBJECTIVE: The aim of this prospective transversal study was to evaluate the relative contributions to exhaled NO fraction (FE(NO)) of the commonly considered major NO determinants, i.e., recent symptoms (upper and lower respiratory tract), atopy (prick skin tests and degree of allergic exposure), and treatment (dose of inhaled corticosteroid [ICS]) to know what information gives a single measure. METHODS: FE(NO) at 50 mL

Long-term omalizumab efficacy in allergic rhinitis.

BACKGROUND: Omalizumab therapy was found to be safe and effective as an add-on therapy for patients with poorly controlled severe asthma. Although several studies over the last decade have demonstrated its efficacy in other Immunoglobulin E related diseases, its use in such conditions is off-label. OBJECTIVE: This study aimed to assess the effectiveness of long-term therapy with Omalizumab in patients with persistent severe allergic rhinitis and inadequately controlled severe asthma. METHODS: Patients with poorly controlled severe asthma and persistent allergic rhinitis were enrolled and tre

Sputum Eosinophils Predict Annual Remission of the Frequent Exacerbator Phenotype in COPD: A Prospective Cohort Study.

INTRODUCTION: The frequent-exacerbator (FE) phenotype guides COPD management, yet its year-to-year stability and the role of airway eosinophilia in FE remission are uncertain. METHODS: In a single-center prospective cohort (n = 289), patients were classified as FE (≥ 2 moderate or ≥ 1 severe exacerbation in the prior year; n = 88) or non-FE (n = 201) and followed 12 months. FE patients were labeled FE-persistent or FE-remitted at 1 year. Clinical data, spirometry, CT, fractional exhaled nitric oxide (FeNO), blood counts, and induced-sputum cytology were collected. Diagnostic accuracy of FeNO [fractional exhaled nitric oxide] and asthma symptoms increased when evaluated with a superior reference standard.

OBJECTIVES: The objective of the study is to determine the impact of changing reference standards (RS), namely spirometry vs. whole-body plethysmography (WBP), on estimation of the diagnostic accuracy of fractional exhaled nitric oxide (FeNO) and clinical signs and symptoms (CSS) as index tests regarding asthma diagnosis. STUDY DESIGN AND SETTING: This was a diagnostic study conducted in 393 patients attending a private practice of pneumologists with complaints suspicious of asthma. First, the index tests were compared with the diagnostic results of spirometry in terms of forced expiratory vo

Assessment of health-related quality of life and olfactory loss in relation to nasal polyp recurrence after endoscopic sinus surgery in patients with chronic rhinosinusitis: a real life-study.

BACKGROUND: Chronic rhinosinusitis with nasal polyps (CRSwNP) impacts the health-related quality of life (HrQoL) and sense of smell. The association between recurrent NPs after surgery and patient-reported outcomes has not been extensively studied. OBJECTIVE: To investigate whether patients with recurrent NPs can be distinguished from non-recurrent cases based on pre- and postsurgical evaluations of HrQoL, sense of smell, blood eosinophils and fractional exhaled nitric oxide (FeNO). METHODS: Thirty-five patients with CRSwNP were recruited pre-surgery and followed up to 12 months post-surgery

FeNO levels increase with degree of sensitisation in apprentices at risk of occupational asthma.

BACKGROUND: Atopy has emerged as a major determinant of airway inflammation. OBJECTIVE: To examine whether early markers of occupational asthma increase with degree of sensitisation. METHOD: This study was a prospective follow-up study of apprentices in baking, pastry-cooking and hairdressing during their 2-year apprenticeship. Four visits were conducted to administer a standardised questionnaire, a methacholine challenge test to assess bronchial hyperresponsiveness (BHR) and to measure fractional exhaled nitric oxide (FeNO). Degree of sensitisation was estimated based on the number of posit

Cough variant asthma with coexisting gastroesophageal reflux disease: A case report.

Chronic cough is a common clinical challenge and a leading cause of outpatient visits to respiratory clinics. In primary care settings, limited diagnostic resources and the absence of standardized evaluation protocols often result in misdiagnosis and suboptimal management. The present report presents the case of a 30-year-old woman with a >1-year history of persistent, non-productive cough unresponsive to initial anti-inflammatory and antitussive therapies. Comprehensive evaluations revealed coexisting cough-variant asthma and gastroesophageal reflux disease, supported by a positive bronchial

New onset asthma during pregnancy: two case reports.

Asthma is the most common chronic respiratory disease during pregnancy. However, reports of new onset asthma during pregnancy are lacking. We report two cases of new onset asthma during pregnancy following respiratory tract infection, subsequently one case with M. pneumoniae infection and the other case with a combined infection with respiratory syncytial virus and rhinovirus.   Case presentation:   Both patients presented with the clinical features of an acute asthma exacerbation during pregnancy without a medical history of asthma. During follow up the diagnosis of asthma was supported by sp

Beyond biomarkers: the role of clinical factors associated with biologic therapy response in severe asthma.

BACKGROUND: Severe asthma carries a high burden and often requires biologic therapy targeting type 2 (T2) inflammation. However, treatment response is heterogeneous, and traditional biomarkers, such as blood eosinophils, fractional exhaled nitric oxide (FeNO), and total serum IgE, may not fully explain this variability . OBJECTIVE: To evaluate clinical and inflammatory characteristics associated with response to biologic therapies in a real-world cohort of severe asthma patients. METHODS: A single-center, ambispective observational study was conducted in the Allergology Department of A Coruñ

Allergic Asthma in the Era of Personalized Medicine.

Allergic asthma is the most common asthma phenotype and is characterized by IgE sensitization to airborne allergens and subsequent typical asthmatic symptoms after exposure. A form of type 2 (T2) airway inflammation underlies allergic asthma. It usually arises in childhood and is accompanied by multimorbidity presenting with the occurrence of other atopic diseases, such as atopic dermatitis and allergic rhinitis. Diagnosis of the allergic endotype is based on in vivo (skin prick tests) and/or in vitro (allergen-specific IgE levels, component-resolved diagnosis (CRD)) documentation of allergic
