Model of functional restriction in chronic obstructive pulmonary disease, transplantation, and lung reduction surgery.

Mechanical interactions between lung and chest wall are important determinants of respiratory function. When chest wall expansion during maximal inhalation generates insufficiently negative pleural pressures, the lungs remain functionally underinflated; this may be termed functional restriction. To explore mechanisms and effects of functional restriction in patients with emphysema, and to predict effects of single lung transplantation and lung volume reduction surgery (LVRS), we used a computational model based on standard physiology and measurements from individual patients. The model's lungs

Inspiratory muscle mechanics of patients with chronic obstructive pulmonary disease during incremental exercise.

Inspiratory muscles are weak and contribute to exercise limitation in chronic obstructive pulmonary disease (COPD). Differential inspiratory pressure contributions from the diaphragm and inspiratory rib cage muscles (RCMs) during exercise in patients with COPD patients are insufficiently described. We measured, in 16 patients with COPD, the global inspiratory muscle pressure (delta Pmus) and transdiaphragmatic pressure (delta Pdi) during an incremental bicycle exercise to exhaustion. The pressures needed to overcome the elastic load were further partitioned into portions for overcoming the PEE

Nasal positive pressure ventilation in the treatment of chronic hypercapnic respiratory failure: a case report.

This paper describes the use of a nasal mask to deliver intermittent positive pressure ventilation to treat chronic respiratory failure in one patient with severe kyphoscoliosis. After two months of overnight nasal ventilation at home the patient achieved normal blood gases, showed improved inspiratory muscle strength, effort tolerance and was able to return to work. Intermittent nasal ventilation is a safe and effective ventilatory support modality for some patients with hypercapnic respiratory failure.

Pulmonary interstitial emphysema in infants less than 1000 g at birth.

Eighty (32%) of 249 infants weighing less than 1000 g at birth developed pulmonary interstitial emphysema (PIE); its incidence was 42% in infants at 500-799 g, 29% at 800-899 g and 20% at 900-999 g. Fifty-five per cent occurred in the first 24 h and 43% were associated with other forms of air leak. The incidence and mortality of PIE did not improve between 1977 and 1984. The mortality rates in infants with or without PIE were not significantly different in the first 4 years of the study period (53% vs 45%). As the mortality improved in infants without PIE during the second 4 years, the differe

Ventilatory response to exercise in simulated obesity by chest loading.

INTRODUCTION & PURPOSE: Obesity-related increases of weight on the chest wall are known to decrease lung volume and chest wall compliance, but the effect of this mass loading, independent of other obesity-related complications on the ventilatory response to exercise is unknown. The purpose of this study was to investigate the effect of chest mass loading on the ventilatory response to exercise. METHODS: External chest loading (CL) was used to simulate the effect of moderate obesity (BMI = 32 kg x m). Eight healthy nonobese subjects performed two incremental exercise tests on a cycle ergometer

3D analysis of the chest wall motion for monitoring late-onset Pompe disease patients.

Late-onset Pompe disease, for which enzyme replacement therapy is available, induces progressive diaphragmatic weakness. Monitoring diaphragmatic function is therefore crucial but is hindered by the need to insert esophageal and gastric probes. Vital capacity (VC), inspiratory capacity, maximal inspiratory pressure, and sniff nasal pressure are noninvasive measurements but reflect only global inspiratory-muscle function. Diaphragmatic function may be assessable noninvasively based on abdominal contribution to breathing and abdominal volume change during the VC maneuver (AVC-VC), obtained by 3-

Mechanisms and consequences of excess exercise ventilation in fibrosing interstitial lung disease.

The causes and consequences of excess exercise ventilation (EEV) in patients with fibrosing interstitial lung disease (f-ILD) were explored. Twenty-eight adults with f-ILD and 13 controls performed an incremental cardiopulmonary exercise test. EEV was defined as ventilation-carbon dioxide output (⩒E-⩒CO2) slope ≥36 L/L. Patients showed lower pulmonary function and exercise capacity compared to controls. Lower DLCO was related to higher ⩒E-⩒CO2 slope in patients (P<0.05). 13/28 patients (46.4%) showed EEV, reporting higher dyspnea scores (P=0.033). Patients with EEV showed a higher dead space (

[Diaphragm electrical activation in COPD patients].

Several studies have demonstrated relatively moderate increase in Pdi during exhaustive exercise in COPD patients. The present study was designed to determine whether the small increase in Pdi is due to a central inhibition of respiratory drive, or an inability of the diaphragm to develop pressure. Diaphragm electrical activation (Edi) and transdiaphragmatic (Pdi) pressure were measured in 19 patients with severe COPD using an esophageal catheter. End-expiratory lung volume (EELV) was assessed by inspiratory capacity (IC) maneuvers. Maximal voluntary Edi (Edimax) was obtained during these mane

Comparison of inspiratory and expiratory airway volumes and luminal areas among standing, sitting, and supine positions using upright and conventional CT.

Upright computed tomography (CT) provides physiologically relevant images of daily life postures (sitting and standing). The volume of the human airway in sitting or standing positions remains unclear, and no clinical study to date has compared the inspiratory and expiratory airway volumes and luminal areas among standing, sitting, and supine positions. In this prospective study, 100 asymptomatic volunteers underwent both upright (sitting and standing positions) and conventional (supine position) CT during inspiration and expiration breath-holds and the pulmonary function test (PFT) within 2 h

Chest mechanics after endoscopic lung volume reduction: Comparison between responders and non-responders based on dynamic hyperinflation.

BACKGROUND: Endobronchial lung volume reduction (ELVR) improves lung function, exercise capacity, and quality of life in case of severe emphysema. Nevertheless, not all patients are improved by ELVR including those with substantially decreased lung volume. We tested the hypothesis that unilateral volume loss could impact differently both responders and non-responders. METHODS: A retrospective analysis was performed on 20 patients, classified as 13 responders and 7 non-responders based on inspiratory capacity increase ≥200 ml at isotime. We compared changes in lung volume, diaphragm shape and

Phrenic and diaphragm function after coronary artery bypass grafting.

We studied respiratory mechanics and phrenic nerve and diaphragm function in 12 patients on the day before and eight to 13 days after coronary artery bypass grafting. The average vital capacity, functional residual capacity, and total lung capacity decreased by 20.5%, 9.5%, and 14.7% respectively after operation. Eleven patients showed less negative maximum inspiratory mouth pressures at any given lung volume after surgery and the magnitude of the change correlated with the reduction in total lung capacity. In 11 of the 12 patients the conduction times of the right and left phrenic nerves did

Comparing two types of loading during inspiratory muscle training in patients with weaning difficulties: An exploratory study.

BACKGROUND: Inspiratory muscle training improves respiratory muscle function and may improve weaning outcomes in patients with weaning difficulties. Compared to the commonly used pressure threshold loading, tapered flow resistive loading better accommodates pressure-volume relationships of the respiratory muscles, which might help to facilitate application of external loads and optimise training responses. OBJECTIVE: The objective of this study was to compare acute breathing pattern responses and perceived symptoms during an inspiratory muscle training session performed against identical exte

Loss of compensation afforded by accessory muscles of breathing leads to respiratory system compromise in the mdx mouse model of Duchenne muscular dystrophy.

Despite profound diaphragm weakness, peak inspiratory pressure-generating capacity is preserved in young mdx mice revealing adequate compensation by extra-diaphragmatic muscles of breathing in early dystrophic disease. We hypothesised that loss of compensation gives rise to respiratory system compromise in advanced dystrophic disease. Studies were performed in male wild-type (n = 196) and dystrophin-deficient mdx mice (n = 188) at 1, 4, 8, 12 and 16 months of age. In anaesthetised mice, inspiratory pressure and obligatory and accessory respiratory EMG activities were recorded during baseline a

Inspiratory muscle training does not improve clinical outcomes in 3-week COPD rehabilitation: results from a randomised controlled trial.

The value of inspiratory muscle training (IMT) in pulmonary rehabilitation in chronic obstructive pulmonary disease (COPD) is unclear. The RIMTCORE (Routine Inspiratory Muscle Training within COPD Rehabilitation) randomised controlled trial examined the effectiveness of IMT added to pulmonary rehabilitation. In total, 611 COPD patients (Global Initiative for Chronic Obstructive Lung Disease stage II-IV) received a 3-week inpatient pulmonary rehabilitation, of which 602 patients were included in the intention-to-treat analyses. The intervention group (n=300) received highly intensive IMT and the
