Progressive Loss of Sensitivity to Electrical Stimulation After Cochlear Implantation in X-Linked Incomplete Partition Type III Deafness.

OBJECTIVES: Patients with X-linked incomplete partition type III (IP3) deafness treated with cochlear implants exhibit higher "Most Comfortable Loudness" (MCL) levels of stimulation and more electrode deactivation than patients with normal morphology. We endeavored to analyze the progression of the MCL levels and electrode deactivation over time and assess those factors that could have led to deactivation. Furthermore, we aimed to assess whether speech perception was affected by a progressive loss of neural contact. DESIGN: All 13 patients with the IP3 malformation in our clinical database we

Binaural benefits for adults who use hearing aids and cochlear implants in opposite ears.

OBJECTIVE: This study aimed to investigate 1) how a hearing aid needs to be adjusted for an adult who uses a cochlear implant in the contralateral ear; 2) whether the use of a hearing aid with a cochlear implant leads to interference; and 3) whether adults derive binaural benefits from using a hearing aid with a cochlear implant for speech perception, localization, and functional performance in everyday life. DESIGN: Twenty-one adults (11 female and 10 male) who used either a Nucleus CI-22 (N = 3) or a Nucleus CI-24 (N = 18) cochlear implant system in one ear participated in this study. Twelv

Measurement of phase-locked neural responses to cochlear-implant stimulation from multiple stages of the auditory system.

Studies investigating objective measures of auditory perception when using cochlear implants (CIs) differ in the stimuli used and in the neural response measured. The present study aims to disentangle the effects of response type and of stimulus type on neural response properties. We report three measures corresponding with separate neural latencies, all collected from the same 19 CI recipients and with the same stimulus, consisting of a continuous low-rate electrical pulse train of about 40 pulses per second (pps). These measures comprise the electrically evoked compound action potential (eCA

The Spectral Extent of Phasic Suppression of Loudness and Distortion-Product Otoacoustic Emissions by Infrasound and Low-Frequency Tones.

We investigated the effect of a biasing tone close to 5, 15, or 30 Hz on the response to higher-frequency probe tones, behaviorally, and by measuring distortion-product otoacoustic emissions (DPOAEs). The amplitude of the biasing tone was adjusted for criterion suppression of cubic DPOAE elicited by probe tones presented between 0.7 and 8 kHz, or criterion loudness suppression of a train of tone-pip probes in the range 0.125-8 kHz. For DPOAEs, the biasing-tone level for criterion suppression increased with probe-tone frequency by 8-9 dB/octave, consistent with an apex-to-base gradient of biasi

Vocal Markers in the Gender Perception of Trans Women and Trans Men.

OBJECTIVE: To identify vocal gender markers in trans people, considering the relationship between gender perception and the acoustic and auditory-perceptual parameters of the voice. METHODS: Observational, cross-sectional study, approved by the Research Ethics Committee (n. 5.353.501). The judges completed auditory-perceptual judgment (APJ) and acoustic analysis of 30 transgender women and 23 transgender men, aged between 18 and 43 years, based on the production of the sustained vowel /a/ and connected speech (number counting and days of the week). The APJ was made in consensus by two judges;

COMT and the neurogenetic architecture of hearing loss induced tinnitus.

As the COMT polymorphism is especially prominent in the prefrontal cortex and has been associated with auditory gating, we hypothesize that tinnitus patients with this polymorphism have altered activity in the ventromedial prefrontal/anterior cingulate areas that modulates the tinnitus percept. To test this, we recruited a total of 40 tinnitus subjects and 20 healthy controls for an EEG study. A comparison between tinnitus subjects and healthy controls and their frequency of being Val/Val genotype or Met carriers (including Val/Met and Met/Met genotype) shows no significant effect, suggesting

Bilateral cochlear implants controlled by a single speech processor.

OBJECTIVE: This study aimed to assess, in one profoundly hearing impaired subject, potential benefits and limitations in placing bilaterally implanted scala tympani electrode arrays under control of a single speech processor. STUDY DESIGN: All available stimulation sites in both ears were compared in studies of pitch discrimination and pitch ranking, identifying three bilateral pairs capable of supporting interaural comparisons with no perceptible difference in pitch. Using those pairs, the subject's ability to lateralize sound was studied as a function of interaural time delay and interaural

[In situ sound pressure measurement in a professional violinist with bilateral tinnitus].

BACKGROUND: A case of a professional violinist suffering from a bilateral tinnitus is presented. The musician reported the tinnitus to be louder and more straining when playing his Vuillaume violin (France 1840) as compared with his Carcassi violin (Italy 1763). CASE REPORT: In the 42-year-old musician, audiometry revealed a normal hearing threshold in the right ear and a slight hearing loss in the left ear of up to 20 dB between 2 kHz and 8 kHz. Transitory evoked otoacoustic emissions could only be measured in the right ear. The tinnitus could be masked (distance type); residual inhibition w

Selecting different amplification for different listening conditions.

Ten subjects with diverse hearing impairment trialled a multiple-memory hearing aid in everyday life and in a paired-comparison laboratory test. Five subjects evaluated systematic variations of the compression characteristic in two bands, around an individually fitted response, using the Resound PHS-ED instrument. Another five subjects evaluated systematic variations of the frequency response characteristic, around an individually fitted NAL response, using the Widex Quattro B hearing device. In all, five subjects showed benefit from multiple amplification schemes. Both variations in compressi

Altered intensity coding in the salicylate-overdose animal model of tinnitus.

Tinnitus is one of the leading disorders of hearing with no effective cure as its pathophysiological mechanisms remain unclear. While the sensitivity to sound is well-known to be affected, exactly how intensity coding per se is altered remains unclear. To address this issue, we used a salicylate-overdose animal model of tinnitus to measure auditory cortical evoked potentials at various stimulus levels, and analyzed on single-trial basis the response strength and its variance for the computation of the lower bound of Fisher information. Based on Fisher information profiles, we compared the prec

Ability to achieve gain/frequency response and SSPL-90 under three prescription formulas with in-the-ear hearing aids.

Custom in-the-ear (ITE) hearing aids (standard linear amplifiers with single-pole-filter low-frequency tone control and a class A amplifier output stage) were fit to 90 ears using the revised National Acoustics Laboratories' formula (NAL-R), and to 20 ears each using Prescription of Gain/Output II (POGO II) and Memphis State University (MSU) formulas. Both real-ear insertion gain and 2-cc coupler gain were evaluated. Examination of differences between prescribed gain and that actually achieved in the fittings revealed that too much gain was often given in the low- and mid-frequency range and i

The Relevance of Interoception in Chronic Tinnitus: Analyzing Interoceptive Sensibility and Accuracy.

In order to better understand tinnitus and distress associated with tinnitus, psychological variables such as emotional and cognitive processing are a central element in theoretical models of this debilitating condition. Interoception, that is, the perception of internal processes, may be such a psychological factor relevant to tinnitus. Against this background, 20 participants suffering from chronic tinnitus and 20 matched healthy controls were tested with questionnaires, assessing interoceptive sensibility, and participated in two tasks, assessing interoceptive accuracy: the Schandry task, a

Speech onset enhancement improves intelligibility in adverse listening conditions for cochlear implant users.

Speech perception by cochlear implant (CI) users can be very good in quiet but their speech intelligibility (SI) performance decreases in noisy environments. Because recent studies have shown that transient parts of the speech envelope are most important for SI in normal-hearing (NH) listeners, the enhanced envelope (EE) strategy was developed to emphasize onset cues of the speech envelope in the CI signal processing chain. The influence of enhancement of the onsets of the speech envelope on SI was investigated with CI users for speech in stationary speech-shaped noise (SSN) and with an interf

In situ Hearing Tests for the Purpose of a Self-Fit Hearing Aid.

This study investigated the potential and limitations of a self-fit hearing aid. This can be used in the "developing" world or in countries with large distances between the hearing-impaired subjects and the professional. It contains an on-board tone generator for in situ user-controlled, automated audiometry, and other tests for hearing aid fitting. Twenty subjects with mild hearing losses were involved. In situ audiometry showed a test-retest reliability (SD <3.7 dB) that compared well with the precision of diagnostic audiometry using headphones. There was good correspondence (SD <5.2 dB) wit

The Bing test in the detection of conductive hearing impairment.

The Bing test is based on the principle that occlusion of the external auditory meatus improves the perception of bone-conducted sounds unless there is a conductive hearing impairment. However, its sensitivity and specificity have not been reported. Tuning-fork and audiometric Bing tests were carried out on 126 patients with otological symptoms prior to any otological history or examination. Test results were compared with the pure tone air-bone gaps. The specificity and sensitivity of each test method was estimated. The tuning-fork Bing test was less sensitive than the audiometric Bing test,

Dysarthria treatment for Parkinson's disease: one-year follow-up of SPEAK OUT!® with the LOUD Crowd®.

INTRODUCTION: SPEAK OUT! with The LOUD Crowd is a standardized speech therapy program typically consisting of 12 one-on-one treatments and ongoing weekly group maintenance sessions for patients with dysarthria due to Parkinson's disease (PD). It is based upon the hypothesis that increased attention to speech, which is a goal-directed motor activity, may compensate for the impairment in automatic sequential motor behaviors often demonstrated in patients with PD. We present results on the 1-year response to treatment. METHODS: Forty individuals with idiopathic PD received SPEAK OUT! delivered i

Acoustic CR Neuromodulation Therapy for Subjective Tonal Tinnitus: A Review of Clinical Outcomes in an Independent Audiology Practice Setting.

OBJECTIVE: To describe the quantitative treatment outcomes of patients undergoing acoustic coordinated reset (CR) neuromodulation at a single independent audiology practice over a 22- to 26-week period as part of an open label, non-randomized, non-controlled observational study. METHODS: Sixty-six patients with subjective tonal tinnitus were treated with acoustic CR neuromodulation with a retrospective review of patient records being performed in order to identify changes of visual analog scale (VAS, n = 66) and in the score of the tinnitus handicap questionnaire (THQ, n = 51). Patients had t

[Difficulties encountered by old patients wearing a hearing aid].

Presbycusis is a common cause of deafness in the elderly. The histopathology shows degenerative changes of the hair cells in the organ of Corti, and of the auditory nerve fibers. This implies a loss of sensibility for low intensity stimuli, although the perception of loud sounds remains normal. The dynamic range of the cochlea is therefore reduced. In addition, the discrimination is also altered. Hearing aids do not restore a normal hearing. Manufacturers have reduced their size, making them difficult to handle by old patients. This paper reports some of the difficulties encountered by old pat

A loudness model for time-varying sounds predicts asymmetries in estimates of loudness change for sounds that increase or decrease in level.

Previous research has demonstrated a robust asymmetry in loudness judgements for rising and falling level tone stimuli, showing greater loudness and loudness change for rising sounds. In the literature, this has been interpreted to reflect an adaptive perceptual bias, resulting from an evolutionary advantage that increases the margin of safety when reacting to an approaching sound-producing object. The current experiment tested whether a loudness model was able to predict asymmetries in loudness-change judgments for rising and falling sounds, providing an alternative explanation to the adaptiv

Tinnitus loudness in quiet and noise after resection of vestibular schwannoma.

OBJECTIVES: (1) To use a loudness model to assess the influence of loudness recruitment on estimates of the loudness of tinnitus obtained by loudness matching; (2) To compare the effect of background noise on the loudness of tinnitus for individuals who are unilaterally deaf after resection of vestibular schwannoma (VS) and those with idiopathic tinnitus. BACKGROUND: After translabyrinthine resection of VS, patients experience unilateral deafness and tinnitus in the operated ear. Most complain that their tinnitus is more bothersome in noisy environments, unlike those with idiopathic tinnitus.
