Functional reconstruction of large mandibular defects, especially in young patients, presents a significant clinical challenge. The ideal approach should not only restore skeletal contour but also address nerve deficits and facilitate final occlusal rehabilitation, all while minimizing morbidity. This report describes a comprehensive, multi-staged strategy for such a case. A 22-year-old male patient was diagnosed with a large cemento-ossifying fibroma (COF) located in the molar region of the left mandible. He underwent a segmental mandibulectomy, resulting in a 40-mm defect. Immediate reconstr

This prospective study aims to compare the clinical outcomes of two different bridging plate systems in complex mandibular reconstruction surgery. A prospective, single-blinded, randomized controlled clinical trial was conducted on 40 patients with complex mandibular bone defects requiring reconstruction using load-bearing osteosynthesis. Patients were randomly assigned to receive either a novel bridging plate method (MODUS 2 Mandible TriLock Bridging Plate, intervention group) or a traditional method (MODUS 2.5 Locking Reconstruction Plate; control group). The outcome of the two systems was e

BACKGROUND: The fixation of osseous free flaps for segmental mandible reconstruction after resection is most commonly performed with patient-specific 3D printed or conventional load-bearing reconstruction plates. The main challenge with conventional plates is the step of manual bending to adjust the plate to the specific mandible of the patient. To date, the influence of this permanent plate deformation on the biomechanical conditions within the healing regions remains unknown. The present study aimed to investigate the effect of plate pre-bending on intersegmental strains, known to influence

BACKGROUND: Computer-aided modeling and design (CAM/CAD) of patient anatomy from computed tomography (CT) imaging and 3D printing technology enable the creation of tangible, patient-specific anatomic models that can be used for surgical guidance. These models have been associated with better patient outcomes; however, a lack of CT imaging guidelines risks the capture of unsuitable imaging for patient-specific modeling. This study aims to investigate how CT image pixel size (X-Y) and slice thickness (Z) impact the accuracy of mandibular models.
METHODS: Six cadaver heads were CT scanned at var

BACKGROUND: Mandibular reconstruction patients often suffer abnormalities in the mandibular kinematics. In silico simulations, such as musculoskeletal modelling, can be used to predict post-operative mandibular kinematics. It is important to validate the mandibular musculoskeletal model and analyse the factors influencing its accuracy.
OBJECTIVES: To investigate the jaw opening-closing movements after mandibular reconstruction, as predicted by the subject-specific musculoskeletal model, and the factors influencing its accuracy.
METHODS: Ten mandibular reconstruction patients were enrolled in

The Mandible can be damaged by pathological factors, tumors, trauma, infection, and needs a surgical operation for reconstruction and restoring function. There are different methods for the reconstruction of mandible. Based on the surgical approach, primary reconstruction of mandible by reconstruction plate after resection is necessary for maintaining mandibular symmetric and esthetic of the lower third of the face. A finite element model of mandible and masticatory muscles was produced from a normal person (male with 35-year-old). The normal model was resected from the left sixth tooth to the

This case report presents a mandible reconstruction with temporomandibular joint (TMJ) prothesis for treatment of osteomyelitis infection following a bimaxillary orthognathic and genioplasty surgeries. The patient, a 41-year-old female, presented with facial pain, difficulty in opening her mouth, and mandibular deviation four months after the initial orthognathic surgery. During the four months, the patient had experienced two more surgerys post-operative for treatment of the complications. Examination revealed mobility between osteotomy segments of the right mandibular osteotomomie between bo

The fibular free flap has become the gold standard for mandibular reconstruction. Despite its vast array of benefits, this procedure has the potential for several complications. In the pediatric setting, the immature skeleton sets the stage for an increased risk of future skeletal abnormalities, both in the jaw, and in the donor site. Herein we describe the case of a toddler who underwent mandibular reconstruction using a fibula free flap and, years later, subsequently developed ankle instability as a result of residual fibula migration. This indicates the need for careful consideration of lon

The aim of this study was to evaluate the use of a pedicled mandibular osteomuscular flap for zygoma reconstruction through a modified Weber-Ferguson incision. The study included 3 male patients and 2 female patients with an average age of 46 years. Zygomatic ameloblastoma was present in 2 cases. Vascular malformation was present in 2 cases, and chondromyxoid fibroma was present in 1 case. In all 5 patients, surgery consisted of a wide excision of the lesions with margins of at least 5 mm to ensure complete removal. The defects that were created measured between 3 x 3 and 4 x 4 cm. The efficacy of distraction osteogenesis in an irradiated area is controversial, although this procedure is now widely used in the field of craniomaxillofacial surgery. We report the clinical results from 4 patients with mandibular defects treated by lengthening of the irradiated mandibles. All patients had a mandibular defect caused by ablation of a malignant tumor. They had undergone radiotherapy at a total dose of 30 to 50 Gy to the surgical site after tumorectomy. Distraction osteogenesis was used as the secondary reconstruction method in 6 sites of the remaining irradiated mandibles and i

Vascularized osteocutaneous scapular flaps belong into the most convenient ones in reconstruction of mandibular defects followed by massive loss of adjacent skin and mucous membrane due to its vascular supply, bulkiness, suitability and mobility of cutaneous component of the flap. In seven wounded patients such defects of the mandible and adjacent soft tissues after war wounding have been reconstructed at the Clinic for Maxillofacial Surgery of the Military Medical Academy - Belgrade during the six years period with vascularized osteocutaneous scapular grafts. Here we present localization and

OBJECTIVE: To analyze the long-term effect of mandibular bone as donor material in bone grafting of the alveolar process defect in patients with unilateral cleft lip and palate (UCLP), compared with iliac crest cancellous bone.
METHOD: During a 7-year period, 101 UCLP patients were bone grafted, 57 cases with iliac crest cancellous bone and 44 with mandibular symphyseal bone. The long-term results with an observation time of more than 4 years were analyzed with respect to marginal bone level and dental and gingival condition in the grafted area. Complications were recorded.
RESULTS: The bone

The authors report the first case of mandibular distraction osteogenesis applied to a fibula microvascularized flap used to reconstruct an almost entire mandible. The technique and complications are described. The literature is reviewed. The biological and physiological process of bone elongation by Ilizarov's method is explained according new studies on chondroid tissue. Preoperative planning of vectors of device placement and trajectory is essential for good results.

The authors have performed 13 cases of vascularized cranial bone grafts for reconstruction of maxillofacial defects since 1986. Two types of flaps were used: the parietal osteofascial flap pedicled to the parieto-temporal fascia based on the superficial temporal artery and the temporalis osteomuscular flap pedicled to the temporalis muscle based on the deep temporal artery. Zygomatico-orbital complex, maxilla and mandible were reconstructed and hemifacial microsomia was also treated. The results of vascularized cranial bone grafts pedicled to fascia were as good as those of grafts pedicled to

This paper describes the evolution and refinement, over a four-year period, of a simple technique for shaping and stabilising the bony component of a composite radial forearm free flap to reconstruct the mandible following segmental resection for neoplastic disease. The results of reconstruction in 30 consecutive patients are reported. The current method of adapting and stabilising the radial bony strut using a specially designed 32 hole osteosynthetic miniplate is described.

Comminuted mandibular fractures (CMFs) associated with panfacial trauma are challenging because restoration of skeletal continuity does not necessarily re-establish functional occlusion or facial form. We report a staged multidisciplinary rehabilitation in a 34-year-old man who sustained severe craniofacial injuries in an industrial explosion, including a CMF, multiple midfacial fractures, and extensive anterior maxillary destruction. Initial treatment consisted of hemorrhage control, top-down open reduction and internal fixation, and simultaneous orbital floor reconstruction. Because severe m

Reconstructing facial structures after tumor removal is a complex challenge in maxillofacial surgery. The submental osteocutaneous flap offers a reliable and versatile solution for patients requiring reconstruction of the jaw and surrounding areas. This technique is particularly beneficial for individuals with underlying health conditions or advanced age, who may not tolerate more complex procedures such as free microvascular flaps. In our study, we describe the successful use of this flap in one patient with maxillofacial tumor. Our patient had part of the mandible reconstructed after cancer

This report describes mandibular reconstruction in a 4-year-old male with recurrent trabecular juvenile ossifying fibroma. Following wide segmental mandibulotomy with periosteal preservation, a 6-cm parasymphyseal defect was reconstructed using a cadaveric tibial allograft combined with a cellular bone allograft. Imaging demonstrated progressive remodeling with complete osseous mandibular arch continuity at 22 months, and excellent function and facial harmony were observed on clinical follow-up to 42 months. This report reviews the paucity of current literature on pediatric mandibular reconstr

Patient-specific mandibular reconstruction plates (PSMRPs) have gained prominence for their precise adaptation to mandibular contours and reported enhanced mechanical performance compared to the manual-bent mandibular reconstruction plates (MBMRPs). However, clinical adoption remains cautious due to insufficient biomechanical evidence directly and carefully comparing their performance. Hence, this study investigated the biomechanical behavior between two mandibular reconstruction assemblies with MBMRP and PSMRP, respectively. Mechanical properties of these two reconstruction systems, including
