Midwifery practice in a Ghanaian referral hospital: experiences using magnesium sulphate for women at risk of eclampsia.

OBJECTIVE: Pre-eclampsia is a leading cause of maternal mortality globally. Magnesium Sulphate is the recommended treatment to prevent seizures, yet its use remains inconsistent in settings like Ghana. This study aimed to explore midwives' experiences and to gain insights into the use of magnesium sulphate in the management of pre-eclampsia and eclampsia. METHODS: Descriptive qualitative research was utilised as part of a larger mixed-methods study investigating the multi-level factors influencing midwives' care of women with pre-eclampsia in Ghana. The findings presented are part of phase 2,

Evaluating Virtual Reality as a feasible and acceptable learning strategy in midwifery and obstetrics using a birthing simulation.

PROBLEM: Perineal tears remain an intractable problem in maternity services and can have a significant negative effect on women's recovery in the postnatal period. BACKGROUND: An evidence-based Perineal Protection Bundle© (the 'Bundle') was developed to guide clinicians towards standardised best practise. The study setting saw static severe perineal tear rates post the Bundle implementation, instigating the creation of a Virtual Reality (VR) simulation to address best practice in perineal care. METHODS: A pre-post study assessed whether a VR simulation training package improved knowledge of

Regulating the Midwife: Power, Public Health, and the Decline of Traditional Midwifery in West Virginia, 1881-1929.

This historical policy analysis centers on how early maternal and child health (MCH) efforts affected the practice and decline of traditional midwifery in West Virginia. From the emergence of official government action in MCH with the establishment of the West Virginia Board of Health in 1881, through the federal Sheppard-Towner Maternity and Infancy Act of the 1920s-the nation's first large-scale attempt at making the provision and improvement of MCH issues a national priority-West Virginia transformed childbirth practices by increasingly regulating, professionalizing, and ultimately marginal

Releasing time to deliver care: a mixed methods evaluation of the implementation of enhanced midwifery continuity of carer.

OBJECTIVES: The enhanced midwifery continuity of carer (eMCoC) pilot programme provided additional resource (funding) to midwifery teams operating in the 10% most deprived areas in England. The eMCoC programme aims to provide additional support to those at greatest risk of poor maternal health outcomes. We conducted a rapid formative evaluation aiming to explore the implementation of the pilot programme to (1) generate timely insights to inform ongoing service delivery; (2) generate a logical framework of the eMCoC service and; (3) inform the design of a longer-term summative evaluation. Factors promoting eRegister and data use for evidence-based midwifery practice in Lesotho: a qualitative study.

OBJECTIVES: Electronic health register's (eRegisters) use have recently gained popularity in Africa. eRegisters are used to capture real-time patient information on several encounters with a healthcare provider. Given poor maternal and child health outcomes in Lesotho, eRegisters provide a promising innovative means of enhancing health outcomes, especially those related to midwifery. eRegisters capture maternal and newborn care services provided at healthcare facilities. Such data are important for informing evidence-based midwifery practice. Lesotho, a landlocked, sub-Saharan African country,

Exploring mathematics anxiety among nursing and midwifery students and the impacts on pharmacological knowledge: A qualitative study.

BACKGROUND: Mathematics anxiety is a well-documented phenomenon that can significantly impact learning, particularly in disciplines requiring numerical proficiency. For nursing and midwifery students, mathematics anxiety poses a critical challenge, potentially compromising pharmacological knowledge and safe medication administration. Despite its importance, limited research has explored the experiences of nursing and midwifery students regarding mathematics anxiety and its direct impact on pharmacological calculations. This study aimed to address this gap and to explore mathematics anxiety for

Economic evaluation and budget impact analysis of midwifery-led care for low-risk pregnancies in Portugal.

BACKGROUND: Midwifery-led models of care for low-risk pregnancies are associated with improved outcomes for mothers and babies, without additional adverse effects. These models are also considered more cost-effective than doctor-led or shared-care approaches. PROBLEM: In Portugal, midwifery-led antenatal care is not widely implemented, and its economic impact remains unexplored. AIM: To estimate the cost implications of implementing a midwifery-led antenatal care model for low-risk pregnancies in Portugal, compared to standard doctor-led care, from the perspective of the Portuguese National

Challenges and opportunities of English as the medium of instruction in diploma midwifery programs in Bangladesh: a mixed-methods study.

BACKGROUND: English is generally recognized as the international language of science and most research on evidence-based medicine is produced in English. While Bangla is the dominant language in Bangladesh, public midwifery degree programs use English as the medium of instruction (EMI). This enables faculty and student access to the latest evidence-based midwifery content, which is essential for provision of quality care later. Yet, it also poses a barrier, as limited English mastery among students and faculty limits both teaching and learning. METHODS: This mixed-methods study investigates t

Implementation of aspirin use during pregnancy in community midwifery-led care in the Netherlands: A pilot survey.

INTRODUCTION: Aspirin nowadays is widely used in pregnancy, but implementation among gynecologists took nearly four decades. For a complete insight in the implementation of aspirin, community midwives are to be involved. Community midwives do not have authority to prescribe aspirin and have to refer to a general practitioner or consultant obstetrician for a prescription. METHODS: The study was an online, national pilot survey about the implementation of aspirin use during pregnancy among independently practicing community midwives consisting of 29 items with five categories: background, advis

Investing in midwifery for sustainable development goals in low- and middle-income countries: a cost-benefit analysis.

BACKGROUND: Maternal and neonatal mortality in low- and middle-income countries is frequently caused by inadequate management of obstetric and neonatal complications and a shortage of skilled health workers. The availability of these workers is essential for effective and high-quality healthcare. To meet the needs of sexual, reproductive, maternal, new-born, child, and adolescent health by 2030, more than one million health workers, including 900 000 midwives, are required globally. Despite this, uncertainty persists regarding the return on investment in the health workforce. METHODS: The obj

Cross-Cultural Adaptation and Validation of the Perceptions of Empowerment in Midwifery Scale in the Spanish Context (PEMS-e).

Midwifery empowerment is an important topic. The most widely used instrument to measure the perceived empowerment of midwives is the Perceptions of Empowerment in Midwifery Scale (PEMS), which has not been validated in Spain. The aim of this study was to translate and adapt the PEMS to the Spanish context. This research was carried out in two phases; Phase 1: Methodological study; translation, backtranslation and cross-cultural adaptation of the PEMS and pilot study on the target population (10 midwives) for evaluation of face validity. Phase 2: Cross-sectional observational study to obtain a

Midwifery management of second-degree perineal tears in New Zealand: A cross-sectional survey of practice.

BACKGROUND: Second-degree tears are the most common form of perineal trauma occurring after vaginal birth managed by New Zealand midwives, although little is known about midwives' perineal practice. AIM: The aim of this study was to identify how midwives managed the last second-degree perineal tear they treated and the level to which their practice reflects National Institute for Health and Care Excellence guidelines. METHODS: An (anonymous) online survey was conducted over a six-week period in 2013. New Zealand midwives who self-identified as currently practising perineal management and cou

Northern Ireland and 'The Troubles', outlining an innovative approach to nursing/midwifery student teaching and module evaluation.

This discussion paper outlines an approach to developing and evaluating an educative programme primarily delivered by lay `citizen trainers' in educating student nurses, and student midwives to the impact of and experience of extended and extensive civil unrest within their communities (`the Troubles' ). This is drawn from experience within the Northern Ireland `Troubles' and all of the citizen trainers were directly affected physically/psychologically. The programme was intended to both educate but primarily to help facilitate student nurses and student midwives to better understanding to exp

Outcomes for Labor Induction Compared With Expectant Management Among Women Receiving Hospital-Based, Midwifery-Led Care.

INTRODUCTION: Use of labor induction has increased rapidly for medical and nonmedical reasons, raising questions about its impact on cesarean birth rates. With growing incidence of elective induction of labor (IOL) at 39 weeks, uncertainty remains about how care practices associated with IOL influence outcomes. Observational studies have reported varied cesarean birth rates following IOL. This issue is particularly relevant for midwives, who prioritize physiologic labor and recognize benefits of spontaneous labor in achieving vaginal birth. This study evaluated cesarean birth rates after labor

Development of a midwifery regulatory environment index using data from the Global Midwives' Associations map survey.

BACKGROUND: Global policymakers have proposed strengthening midwifery regulation to improve access to and quality of care provided by midwives, thereby enhancing maternal healthcare delivery and outcomes. However, quantifying 'midwifery regulatory environments' as a construct across countries has been difficult, limiting our ability to evaluate relationships between regulatory environments and key outcomes and hindering actionable steps toward improvement. The Global Midwives' Associations map survey includes data across five domains of regulation (overarching regulatory policy and legislation

Midwifery Care of Pregnant Individuals Experiencing Opioid use Disorder: Changing Regulations, Complexities, and Call to Action.

Opioid use disorder (OUD), overdose, and death have exploded in the United States in the past 2 decades. The number of pregnant and birthing people reporting opioid use and misuse is also rising. Co-occurring mental illness, multisubstance use, and associated medical comorbidities often complicate care for pregnant individuals with OUD. Neonates who are exposed to opioids in utero are at risk for neonatal opioid withdrawal syndrome and other short- and long-term sequelae. Recent changes to the Department of Health and Human Services Practice Guidelines for the Administration of Buprenorphine f
