Pharmacokinetics of antimicrobial agents in the elderly.

Many elderly individuals are treated with antibiotics, but knowledge of the pharmacokinetic performance of the compounds in these patients is limited. Advanced age is accompanied by many physiologic alterations that may have secondary effects on absorption, distribution, metabolism, and excretion of drugs. Information on absorption and metabolism in the elderly is scarce and conflicting. Because renal excretion of penicillins and cephalosporins decreases with the physiologic signs of the kidneys, dose reduction is advisable in elderly patients. Monitoring of serum drug levels is recommended fo

Intraoperative Monitoring: Recent Advances in Motor Evoked Potentials.

Advances in electrophysiological monitoring have improved the ability of surgeons to make decisions and minimize the risks of complications during surgery and interventional procedures when the central nervous system (CNS) is at risk. Individual techniques have become important for identifying or mapping the location and pathway of critical neural structures. These techniques are also used to monitor the progress of procedures to augment surgical and physiologic management so as to reduce the risk of CNS injury. Advances in motor evoked potentials have facilitated mapping and monitoring of the

Physiological Signal Entropy in Pediatric Traumatic Brain Injury: Looking Beyond the Obvious: A STARSHIP Study.

OBJECTIVES: Multimodality monitoring based prognostication in pediatric traumatic brain injury (TBI) relies heavily on the evaluation of instantaneous and easily interpretable monitoring values. Entropy quantifies the level of disorder within a system reflecting overall activity of sensitive closed-loop feedback homeostatic mechanisms. Multiscale entropy (MSE) assesses entropy across different time scales to examine various physiologic systems and processes that operate across different time scales. The current understanding of MSE suggests that low entropy reflects increased rigidity of the v

Cobras.

Human encounters with cobras in Africa and Asia are common with substantial mortality and primarily respiratory paralysis in untreated cases. First aid includes maintaining calm, placing a lymphatic retardant on the bitten extremity, and transportation to a medical facility. Medical management is antivenin. Physiologic monitoring and the ability to manage the compromised airway are important.

Anesthetic Management of an Urgent Hip Fracture Repair in a Nonagenarian With Critical Aortic Stenosis.

Severe aortic stenosis presents substantial anesthetic risk because fixed left ventricular outflow obstruction limits the ability to compensate for perioperative hemodynamic changes. These risks are further amplified in elderly patients with additional valvular disease and limited physiologic reserve who require urgent noncardiac surgery. We report the anesthetic management of a 96-year-old woman with critical aortic stenosis and concomitant mitral valve disease who required urgent hip fracture repair. Preoperative echocardiography demonstrated severe valvular calcification with a markedly red

Cerebral physiologic insult burden in acute traumatic neural injury: a Canadian High Resolution-TBI (CAHR-TBI) descriptive analysis.

BACKGROUND: Over the recent decades, continuous multi-modal monitoring of cerebral physiology has gained increasing interest for its potential to help minimize secondary brain injury following moderate-to-severe acute traumatic neural injury (also termed traumatic brain injury; TBI). Despite this heightened interest, there has yet to be a comprehensive evaluation of the effects of derangements in multimodal cerebral physiology on global cerebral physiologic insult burden. In this study, we offer a multi-center descriptive analysis of the associations between deranged cerebral physiology and ce

Pharmacologic management of acute intracranial hypertension.

Acute intracranial hypertension requires aggressive treatment with physiologic and pharmacologic measures guided by intracranial pressure monitoring devices. Therapy involves the use of diuretics, corticosteroids, and barbiturates in combination with hyperventilation, ventricular drainage, and general supportive measures. This review focuses on the pathophysiology of increased intracranial pressure and the pharmacologic agents used in its management.

Chronobioengineering toward a cost-effective quality health care.

The current disease-cure-oriented health care system does too little too late for too few. Engineering for concomitantly resolving the external conditions and the internal schedules, chronoengineering in the broad sense, could change this status quo into a system that recognizes environmental and organismic risk early, blows a whistle as a minimum and acts automatically, as an optimum. The chronobioengineering feature (of the broader chronoengineering for external as well as internal monitoring and decision helping) would gain new individualized information from the resolution of predictable v

Multimodal Intelligent Monitoring of Parkinson Disease: Scoping Review of Progress and Translational Challenges.

BACKGROUND: Parkinson disease (PD) is a progressive neurodegenerative disorder with a rapidly growing global prevalence. Current clinical assessments, such as the Unified Parkinson Disease Rating Scale, are limited by subjectivity and episodic application, creating a need for continuous, objective monitoring solutions. While previous reviews have often focused on single technologies, there is a growing trend toward integrating multiple data sources to provide a more holistic view of PD. OBJECTIVE: This scoping review synthesizes progress in multimodal intelligent monitoring systems for PD, fo

Advances in Multimodality Monitoring in Traumatic Brain Injury.

PURPOSE OF REVIEW: Multimodal monitoring (MMM) has the potential to transform the management of secondary brain injury following traumatic brain injury (TBI) to enable more personalized care. Despite its promise, the optimal combination of monitoring modalities and the interpretation of complex data streams remain insufficiently mature for MMM to be considered standard of care. This review evaluates the current state of MMM technologies, their clinical applications, and their emerging role in improving TBI management and guiding the path toward broader clinical adoption. This review focuses on

Assessment of a 16-Channel Ambulatory Dry Electrode EEG for Remote Monitoring.

Ambulatory EEGs began emerging in the healthcare industry over the years, setting a new norm for long-term monitoring services. The present devices in the market are neither meant for remote monitoring due to their technical complexity nor for meeting clinical setting needs in epilepsy patient monitoring. In this paper, we propose an ambulatory EEG device, OptiEEG, that has low setup complexity, for the remote EEG monitoring of epilepsy patients. OptiEEG's signal quality was compared with a gold standard clinical device, Natus. The experiment between OptiEEG and Natus included three different

Sensitivity of pteropod calcification to multi stressor variability in coastal habitats.

Comprehensive understanding of environmental multiple stressors on calcification in marine calcifiers remains an important topic of study, especially under ocean global change associated with multiple stressors. We explore the impact of multiple stressor on pteropod calcification in the southern Salish Sea (Washington, U.S.), a coastal estuarine system that exhibits a high degree of spatial and temporal variability in multiple environmental parameters across sampling locations. We hypothesized that such variability is associated with differences in pteropod calcification. Shell thickness and s

Monitoring Inputs, Control Architectures, and Failure Modes in Closed-Loop Vasopressor Systems: A Comprehensive Review.

Closed-loop vasopressor systems integrate real-time blood pressure monitoring with automated decision logic to support hemodynamic stability in perioperative and critical care environments. These technologies sit at the intersection of biomedical sensing, signal processing, and clinician-supervised automation: the quality, latency, and failure behavior of the monitoring input can directly shape controller performance, safety margins, and clinical usability. In this comprehensive review, we synthesize the major closed-loop vasopressor architectures reported in the literature, examine how sensor
