[Efficacy of selective neck dissection: a review of 123 cases of elective and therapeutic treatment of the neck in squamous cell carcinoma of head and neck].

OBJECTIVE: To evaluate the efficacy of selective neck dissection (SND) in elective and therapeutic treatment of the neck in squamous cell carcinoma of head and neck. METHODS: A retrospective review was undertaken of 123 patients undergoing 157 SNDs as a part of initial therapy for squamous cell carcinoma of head and neck from January 1997 to September 2001 at this institute. The primary site included larynx(n = 77), oral cavity(n = 29), oropharynx(n = 2), and hypopharynx(n = 15). Lymph nodes were pathologically negative (pN0) in 99 and positive (pN+) in 24 patients. Peri-operative radiotherap

Impact of contralateral occult neck metastasis in HPV-associated tonsil cancer: Is elective contralateral neck dissection required?

BACKGROUND: Contralateral elective neck dissection in human papillomavirus (HPV)- associated tonsil cancer is a matter of debate. OBJECTIVES: The aim of this study was to analyze rates of contralateral lymph node (LN) metastasis and their prognostic effects on HPV-associated tonsil cancer. We also assessed the necessity of elective contralateral neck dissection. METHODS: To investigate the pathologic incidence of and risk factors for contralateral nodal disease in HPV-associated tonsil cancer treated with upfront primary surgery and bilateral neck dissection, the records of 68 patients were

Factors influencing outcomes in selective neck dissection in 661 patients with head and neck squamous cell carcinoma.

BACKGROUND: Selective neck dissection (SND) is the surgical treatment of choice in squamous cell carcinoma of the head and neck (HNSCC) with suspected or manifest metastases in the cervical lymph nodes. For SND to be successful, treated lymph node levels should be selected according to anatomic considerations and the extent of the disease. Aim of this study was to identify neck dissection levels that had an impact on individual prognosis. METHODS: We conducted a retrospective review of SND as part of primary treatment of HNSCC. Overall survival (OS) and regional control rates (RCR) were calcu

Oncological Feasibility of Limited Neck Dissection in cN0 Supraglottic Laryngeal Cancer.

BACKGROUND: Supraglottic squamous cell carcinoma (SCC) is a significant portion of head and neck cancers, with the management of clinically negative necks (cN0) through selective neck dissection (SND) being debated due to potential morbidities and low metastasis rates in levels IIb and IV. METHODS: This study is a retrospective, multicenter examination of the potential feasibility of limited neck dissection (LND), including only levels IIa and III in cN0 supraglottic SCC patients. It analyzed occult metastasis rates and explored relapse occurrences alongside potential predictors of lymph node

Neck dissection in pT1 maxillary squamous cell carcinoma: Necessity or overtreatment?

PURPOSE: Squamous cell carcinoma (SCC) is the most common malignancy of the oral cavity. Despite cN0 staging, elective neck dissection (END) is often performed to detect occult cervical lymph node metastases (cLNM). Maxillary SCC (MSCC) generally exhibits similar metastases rates to intraoral SCC in other locations. However, due to the rarity of T1 MSCC, the current guidelines of the German Society for Oral and Maxillofacial Surgery do not provide a clear recommendation for neck dissection (ND) in these cases, as existing data are limited. METHODS: A retrospective analysis was conducted on al

Variant of internal jugular vein reconstruction in bilateral radical neck dissection.

BACKGROUND: The internal jugular vein (IJV) plays a major role in collecting venous blood from the cranium, face, and neck. Preserving or reconstructing at least one IJV during bilateral radical neck dissection (RND) allows preventing severe complications. The aim of this report was to present a variant of IJV reconstruction in bilateral radical neck dissection. CASE SUMMARY: A 55-year-old male complained for a gingival mass for about 2 months, which was approximately 4 × 2 cm in size with a surface ulceration, located in the anterior mandibular area. There were bilateral cervical adenopathy.

Prophylactic Central Neck Dissection for Clinically Node-Negative Papillary Thyroid Carcinoma.

OBJECTIVE: We performed a systematic review and meta-analysis of randomized controlled trials (RCTs) that scrutinized the oncological benefits and postsurgical complications of total thyroidectomy (TT) plus prophylactic central neck dissection (pCND) versus TT alone among clinically node-negative (cN0) papillary thyroid cancer (PTC) patients. METHODS: We screened five databases from inception to September 4, 2021 and evaluated the risk of bias of the eligible studies. We pooled dichotomous outcomes using the risk ratio (RR) with 95% confidence interval (CI). RESULTS: Overall, we included 5 R

Retro-auricular endo assisted selective neck dissection (how I do it).

Elective neck dissection (END) is preferred in all treatment naïve patients with invasive oral squamous cell carcinoma (OSCC), including the early stage, node negative cases (T1/T2 N0). Usually the conventional horizontal neck crease incision leads to a faintly visible scar in the neck. However sometimes, the neck scar is hypertrophic and is highly unaesthetic and psychologically distressing to the patient. Retro-auricular hairline approach has been popularized in the Robot assisted neck dissections (RoAND), to avoid easily visible scar in neck crease. We have been using the retro-auricular in

Neck dissection with harmonic instruments and electrocautery: a prospective comparative study.

BACKGROUND: Harmonic instruments are becoming popular in head and neck surgeries. In this prospective, randomized study, the efficacy of the harmonic instruments and electrosurgical technique is compared. MATERIALS AND METHODS: A total of 48 patients undergoing unilateral neck dissection were divided into two groups. In one group, surgery was performed using conventional hemostatic instruments while in the other, only harmonic instruments were used. The two techniques were then compared with regard to intra- and post-operative blood loss, complications in operating time, drain, tracheotomy an

Management of the marginal mandibular nerve during and after neck dissection.

PURPOSE OF REVIEW: Marginal mandibular nerve palsy (MMNP) is often an understated complication after neck dissection. This article reviews literature regarding anatomic landmarks that help define marginal mandibular nerve (MMN) during neck dissection, oncologic safety of surgical maneuvers, implications of MMNP, and reconstructive options. RECENT FINDINGS: A thorough knowledge of anatomy of the nerve can aid in its preservation. Course, branching pattern and communications of MMN are extremely variable. The Hayes Martin method classically described to preserve the nerve may not be oncological

Common carotid artery dissection: a rare cause of acute neck swelling.

Spontaneous carotid artery dissection is a rare condition with potentially devastating consequences. Internal carotid artery and vertebral artery dissections have been implicated as the cause of 20% of strokes occurring in patients younger than 45 years. We describe a very rare case of a nontraumatic common carotid artery dissection in a 45-year-old man that was initially misdiagnosed as a sternocleidomastoid hematoma. This case highlights the need for vigilance for this often-missed diagnosis, as well as the indication for noninvasive imaging in unidentified neck swellings.

Electrophysiologic analysis of injury to cranial nerve XI during neck dissection.

BACKGROUND: Despite preservation of the accessory nerve, a considerable number of patients report partial nerve damage after modified radical neck dissection (MRND) and selective neck dissection. METHODS: Accessory nerve branches for the trapezius muscle were stimulated during neck dissection, and the M wave amplitude was measured during distinct surgical phases. RESULTS: The accessory nerve was mapped in 20 patients. The M wave recordings indicated that major nerve damage occurred during dissection at levels IIa and IIb in the most proximal segment of the nerve. The M waves evoked from this

Utility of internal jugular vein reconstruction in modified radical neck dissection.

The benefits to modified radical neck dissection (MRND) are established but the procedure involves substantial neck dissection with occasional resection of the internal jugular vein (IJV). Loss of the IJV is associated with morbidity including increased cerebral edema, stroke, laryngeal edema, blindness, facial fullness, and dural thrombosis. This paper discusses the morbidity associated with MRND, especially regarding venous outflow concerns and technical approaches to IJV reconstruction. Patients who have previously undergone MRND may benefit from immediate reconstruction and/or reanastomosi

Robotically assisted selective neck dissection in parotid gland cancer: preliminary report.

OBJECTIVES/HYPOTHESIS: Robot-assisted neck dissection (ND) in head and neck cancer patients has been demonstrated to be feasible. We hypothesized that surgery for parotid cancer could be achieved via a gasless modified facelift approach using the robotic system. We intended to evaluate the feasibility of robot-assisted selective ND (SND) via a modified facelift (MFL) approach in parotid gland cancer. STUDY DESIGN: Case series. METHODS: Primary tumor resection with robotic SND I-III via MFL was performed in five patients with parotid gland cancer. RESULTS: All cases of robot-assisted SND I-I

Transoral CO2 laser management for selected supraglottic tumors and neck dissection.

This study aimed to evaluate transoral laser resection as a method of choice for conservation surgery for supraglottic laryngeal carcinoma in carefully selected patients. Between 1987 and 2006, 55 patients with early supraglottic carcinoma were selected for transoral laser surgery. The outcome of the endoscopic CO(2) laser resection and larynx-sparing functional results without tracheotomy was evaluated. Fifty-five patients with T1, T2 supraglottic carcinomas underwent transoral CO(2) laser resection and seven patients with manifest neck metastasis required a neck dissection at one session wit
