Repair of peripheral nerve defect by direct gradual lengthening of the distal nerve stump in rats: cellular reaction.

We investigated the effects of direct gradual lengthening of the distal stump of a peripheral nerve and subsequent nerve regeneration in rats. A segment 10 mm long was resected from rat sciatic nerve. The distal nerve stump was fixed to a ring and pulled directly at a rate of 1 mm/day using an original external nerve distraction device. After distraction for 10, 15, and 20 days, the lengthened nerves were evaluated macroscopically and immunocytochemically. At day 20, the mean (SD) distances from the ring to the 3 mm and 6 mm distal part, which were marked with sutures on the epineurium, were 7

Maxillofacial deformities in neurofibromatosis.

Four patients with neurofibromatosis are described: all displayed the feature of an increased depth of the sigmoid notch probably due to an expansive growing neurofibroma of the inferior alveolar nerve. One patient had a local neurofibroma with an invasive growth pattern and only this patient shows hyperplasia of the bone. The suggestion is made, contrary to what is found in the literature, that only the invasive growth type gives rise to hyperplasia and this type is always concomitant with a localized form of neurofibromatosis.

Denervation of vagal cardiopulmonary receptors by injection of kainic acid into the nodose ganglia in dogs.

We determined if kainic acid, a neuroexcitotoxin, could be used to denervate the cell bodies of cardiopulmonary vagal sensory neurons. Kainic acid (5 microg) was injected into the nodose ganglion of five dogs. Ten to fourteen days following this procedure, these kainic acid-injected dogs were anesthetized and tested for the extent of the deafferentation. Five additional dogs were used as the control group. Heart rate and mean arterial pressure were measured, and a Swan-Ganz catheter was advanced into a branch of the pulmonary artery to measure pulmonary capillary wedge pressure. We recorded re

[Epidermoid cyst of the cerebellopontine angle. Report of a case].

Epidermal cysts of the cerebello-pontine angle (CPA) are lesions arising from epidermal remainders having an expansive and slow growth sitting on the PCA space or on the temporal bone. Are diagnosed approximately in the fourth life's decade by its deficient or irritant symptoms related to VIIth and VIIIth cranial nerves. It is important the nuclear magnetic resonance exam because the specific radiologic characteristics that distinguish these cysts from other conditions of CPA as neuromata or meningiomata.

HIV lumbosacral radiculoplexus neuropathy mimicking lymphoma: diffuse infiltrative lymphocytosis syndrome (DILS) restricted to nerve?

Diffuse infiltrative lymphocytosis syndrome (DILS) is a hyperimmune reaction against HIV. It leads to MHC-restricted clonal expansion of CD8 T cells characterized by circulating CD8 hyperlymphocytosis and CD8 T-cell infiltration in organs. Our patient presented with painful lumbosacral radiculoplexus neuropathy and tested positive for HIV. Nerve biopsy showed large collections of CD8 lymphocytes suspicious for lymphoma. Symptoms, signs, and repeat biopsy improved with antiretroviral treatment. The presentation and treatment response suggest that this case is localized DILS.

The surgical management of extra-cranial trigeminal nerve palsies: A retrospective case series.

OBJECTIVES: To assess for functional improvement in outcomes following supermicrosurgical restoration across a cohort of patients with a spectrum of trigeminal nerve disorders. PATIENTS & METHODS: In a retrospective case series over three years (2016-18), twenty patients were included in a cohort of trigeminal nerve palsy patients. Eleven patients with corneal anaesthesia underwent corneal neurotisation procedures and were followed up using Cochet-Bonnet aesthesiometry. Six patients with sensory trigeminal nerve injuries had direct nerve coaptation and neuroma excision. Three patients with tr

Ageing and the auditory nerve: Hearing sensitivity and endbulb synapses.

Age-related hearing loss impairs speech understanding for socialization and music appreciation for enjoyment, both of which compromise quality of life and can lead to cognitive decline. It has previously been shown that while standard audiometric hearing thresholds can remain normal over time, speech understanding in noise is more difficult and there is the emergence of tinnitus. These specific hearing difficulties are not revealed by standard audiograms but we now know that they have been attributed to loss of high threshold auditory nerve fibers caused by the disappearance of terminal ending

Activation of skeletal muscle-resident glial cells upon nerve injury.

Here, we report on the identification of Itga7-expressing muscle-resident glial cells activated by loss of neuromuscular junction (NMJ) integrity. Gene expression analysis at the bulk and single-cell level revealed that these cells are distinct from Itga7-expressing muscle satellite cells. We show that a selective activation and expansion of Itga7+ glial cells occur in response to muscle nerve lesion. Upon activation, muscle glial-derived progenies expressed neurotrophic genes, including nerve growth factor receptor, which enables their isolation by FACS. We show that activated muscle glial ce

[The inguinal paravascular technique of lumbar plexus block--anatomical pretests were unsuccessful].

Using 28 anatomic preparations, the boundaries of dye expansion in tissues adjacent to nervous stems have been experimentally studied during the application of lumbar plexus local anesthesia technique via inguinal paravascular approach according to A. Winnie. It has been established that the above technique is not associated with anatomic conditions for simultaneous contact of a model dye solution with 3 basic derivates of the lumbar plexus both at the level of formation and further on. There is massive dye diffusion from femoral nerve perineural space into the intermuscular connective tissues

Impact of Medicaid Expansion on Rhinologic Cancer Presentation, Treatment, and Outcomes.

OBJECTIVE: The United States Patient Protection and Affordable Care Act allocated funds for states to expand Medicaid coverage. However, several states declined expansion. We aim to determine whether Medicaid expansion is associated with healthcare coverage, cancer stage at diagnosis, treatment, and survival among patients with rhinologic cancer. Rhinologic cancer was defined to include cancer of the nasal cavity, paranasal sinus, nasopharynx, or olfactory nerve. STUDY DESIGN: Cohort study. METHODS: Patients diagnosed with primary rhinologic malignancies between 2007 to 2016 were extracted f

Very late-onset Friedreich ataxia despite large GAA triplet repeat expansions.

BACKGROUND: Most patients with Friedreich ataxia (FRDA) have abnormal GAA triplet repeat expansions in both X25 genes. The size of the GAA expansion in the shorter of the 2 expanded alleles correlates significantly with parameters of clinical severity and is inversely related to the age at onset. OBJECTIVES: To describe the clinical and molecular genetic findings in a patient with very late-onset FRDA and to review the literature. PATIENT AND METHODS: A 58-year-old white woman with mild progressive gait disturbance of 15 years' duration whose examination revealed mild incoordination was anal

Neuropathy of haematopoietic stem cell niche is essential for myeloproliferative neoplasms.

Myeloproliferative neoplasms (MPNs) are diseases caused by mutations in the haematopoietic stem cell (HSC) compartment. Most MPN patients have a common acquired mutation of Janus kinase 2 (JAK2) gene in HSCs that renders this kinase constitutively active, leading to uncontrolled cell expansion. The bone marrow microenvironment might contribute to the clinical outcomes of this common event. We previously showed that bone marrow nestin(+) mesenchymal stem cells (MSCs) innervated by sympathetic nerve fibres regulate normal HSCs. Here we demonstrate that abrogation of this regulatory circuit is es

[Myotonic dystrophies: clinical presentation, pathogenesis, diagnostics and therapy].

The autosomal-dominant myotonic dystrophies dystrophia myotonica type-1 (DM1, Curschmann-Steinert disease) and dystrophia myotonica type-2 (DM2, proximal myotonic myopathy (PROMM)), are, contrary to the non-dystrophic myotonias, progressive multisystem disorders. DM1 and DM2 are the most frequent of the muscular dystrophies. In both diseases the skeletal muscle is the most severely affected organ (weakness, wasting, myotonia, myalgia). Additionally, they manifest in the eye, heart, brain, endocrine glands, gastrointestinal tract, skin, skeleton, and peripheral nerves. Phenotypes of DM1 may be

Topography and correlation of radial peripapillary capillary density network with retinal nerve fibre layer thickness.

PURPOSE: To analyse the expansion of radial peripapillary capillary (RPC) network with optical coherence tomography angiography (OCT-A) in normal human eyes and correlate RPC density with retinal nerve fibre layer thickness (RNFLT) at various distances from the optic nerve head (ONH) edge. METHODS: Fifty eyes of 50 healthy subjects underwent imaging with RTVue XR-100 Avanti OCT. OCT-A scans of Angio disc (6 × 6 mm) and Angio retina (8 × 8 mm) were combined to create a wide-field montage image of the RPC network. RPC density and RNFLT was calculated at different circle diameter around the ONH,

Bilateral Dentigerous Cysts of the Permanent Mandibular First Molars Treated with Marsupialization.

Bilateral dentigerous cysts (DC) associated with unerupted mandibular first molars in a non-syndromic pediatric individual are rare. Secondary infections may lead to complications, such as discomfort due to pain, disfigurement caused by enlargement of the cyst with cortical expansion of the jawbone, displacement of teeth and paraesthesia of the adjacent nerve. This case report describes the occurrence of bilateral DC in an eight-year-old patient. Marsupialization was the treatment of choice to preserve the permanent teeth and other adjacent tissues.

Sequential membrane remodeling by cholesterol distinctly modulates HCN channels in naïve and neuropathic DRG neurons.

Cholesterol, abundantly present in distinct plasma membrane pools, is a critical modulator of ion channel function, including hyperpolarization-activated cyclic nucleotide-gated (HCN) channels that regulate the excitability of dorsal root ganglion (DRG) nociceptor neurons. Depletion of membrane cholesterol potentiated HCN channel opening and accelerated activation kinetics, whereas cholesterol supplementation reduced channel opening and slowed activation kinetics. However, the relative contributions of cholesterol that organizes ordered membrane domains (OMDs) versus freely accessible choleste

A novel case of Horner syndrome as the presenting sign of craniosynostosis.

Craniosynostosis, the premature fusion of cranial sutures, can lead to distortion of skull shape and neurological dysfunction. We present a novel case of Horner syndrome as the presenting sign of craniosynostosis associated with elevated intracranial pressure. A 10-year-old boy presenting for strabismus follow-up was noted to have new-onset anisocoria, greater in the dark, and mild right upper eyelid ptosis. Apraclonidine testing was concerning for Horner syndrome. Neuroimaging demonstrated previously undiagnosed sagittal craniosynostosis with tortuous optic nerves and large cerebrospinal flui

The role of regional analgesia in personalized postoperative pain management.

Pain management plays a fundamental role in enhanced recovery after surgery pathways. The concept of multimodal analgesia in providing a balanced and effective approach to perioperative pain management is widely accepted and practiced, with regional anesthesia playing a pivotal role. Nerve block techniques can be utilized to achieve the goals of enhanced recovery, whether it be the resolution of ileus or time to mobilization. However, the recent expansion in the number and types of nerve block approaches can be daunting for general anesthesiologists. Which is the most appropriate regional tech
