BACKGROUND: Papilledema is thought to be the hallmark sign of increased intracranial pressure (ICP). Distension of the subarachnoid space within the optic nerve sheath is also commonly reported in MR studies as an indirect sign of increased ICP.
HYPOTHESIS: General anesthesia and positive pressure ventilation might result in changes in optic sheath diameter (OSD) observed on clinical brain MRI.
STUDY TYPE: Retrospective.
POPULATION: One hundred forty-five patients (154 MRI scans, 7.3 years ± 5.1); 97 studies in the anesthesia group (4.4 years ± 3.4) of which 22 had papilledema, and 57 in t

BACKGROUND: Propofol use is contraindicated in patients on ketogenic diet (KD) due to higher risk of propofol infusion syndrome (PIS). This study is intended to provide a descriptive analysis of our experience with propofol bolus and short infusions for anesthetic care in patients on the KD and to evaluate if any signs of PIS were observed.
METHODS: All patients on the KD who underwent anesthesia with propofol between 2012 and 2022 were reviewed. Anesthetic encounters and charts were studied for type of surgical procedure; signs of PIS, including new cardiac arrhythmias, acidosis, or rhabdomy

BACKGROUND: Medication errors pose a significant risk perioperatively. In the perioperative environment, common medication administration processes are not typically performed. Given the common administration of potentially harmful medications and the potential for medication errors, patient risk is substantial. Specifically, intravenous acetaminophen has been used more frequently in the perioperative period which yielded an increase in medication administration errors reported.
AIMS: This project was initiated by Cincinnati Children's Hospital after an increase in perioperative acetaminophen

We aimed to investigate the anesthetic management of percutaneous closure of atrial and ventricular septal defects (ASD/VSD) in pediatric patients. A retrospective review of the anesthetic data of 351 patients who underwent transcatheter closure of ASD/VSD was conducted. The mean age was 8.42 ± 5.71 years (4 months-18 years). VSD closure was performed in 52 patients and the remaining 299 had a procedure for ASD closure. All patients were premedicated with midazolam. All procedures were performed under general anesthesia in the catheterization laboratory. After anesthesia induction with sevoflu

BACKGROUND: Following adverse reactions to anesthesia, tests are carried out to determine the mechanism of the reaction and to identify the agent responsible. No specific data are available in France concerning such skin tests in children.
METHODS: Between 1989 and 2001, we assessed hypersensitivity reactions to general anesthesia in 68 children. Thirty underwent more than one operation, for congenital malformations. Immunoglobulin (Ig)E-mediated anaphylaxis was diagnosed on skin tests combined with the clinical history.
RESULTS: Grade I, II and III reactions were observed in 20, 27 and 21 c

Serial cardiac arrests occurred during the induction of a 3-year-old boy for elective 1-sided orchiopexy surgery and evaluation under anesthesia of previously placed ear tympanoplasty tubes. The child's history included Williams syndrome along with hypercalcemia and mild supravalvular aortic stenosis. The initial arrests included significant ST wave changes along with profound brodycardia, hypotension, and pulseless electrical activity requiring full resuscitation twice. The patient was transferred on an emergency basis to the intensive care unit (the surgery was cancelled), and a heart cathet

We report a case of severe hyperthermia in a 6-month-old boy with a single ventricle, dextrocardia, asplenia, and transposition of the great arteries, during induction of anesthesia on three separate occasions. To our knowledge, this is the first case reported of repeated intraoperative hyperthermia not related to malignant hyperthermia, infection, neuroendocrine tumor, or iatrogenic causes (e.g., anticholinergic blockade or warming devices). The severe hyperthermia may be secondary to the medications given before and during induction and/or the stress of the induction. Among the induction med

Background: Effective pain control is fundamental in pediatric dentistry. Supraperiosteal infiltration (SPA) and inferior alveolar nerve block (IANB) are the most used local anesthesia (LA) techniques. This review evaluated the available evidence on intraligamentary anesthesia (ILA) to assess its efficacy, safety, and viability as an alternative to conventional techniques. Methods: The review protocol was registered in PROSPERO (CRD420261284494) and conducted in accordance with PRISMA guidelines. Three databases were searched for RCTs published in English after 2000 involving children. Studies

Background and Objectives: Spinal anesthesia is increasingly preferred in pediatric surgeries due to its rapid onset, high success rate, and low risk of systemic complications. However, the influence of age and body mass index (BMI) on block characteristics in adolescents remains insufficiently studied. Materials and Methods: This retrospective study evaluated 190 pediatric patients (aged 9-18 years; 154 male, 36 female) undergoing elective surgery with spinal anesthesia. Patients were stratified by age (Group 1: 9-14 years; Group 2: 15-18 years) and BMI (Group A: 16.00-19.65 kg/m2; Group B: 1

BACKGROUND: Mitochondrial encephalomyopathy, lactic acidosis, and stroke-like episodes (MELAS) is a mitochondrial disease. We report here the safe use of remimazolam in a pediatric MELAS patient.
CASE PRESENTATION: A 10-year-old girl (118 cm, 16 kg) was scheduled for an open gastrostomy to improve nutrition and epileptic seizure control. We induced and maintained general anesthesia with remimazolam, remifentanil, fentanyl, and rocuronium. We also performed a bilateral subcostal transversus abdominis plane block before the surgery. The surgery finished uneventfully. After we discontinued remim

Objective Anesthesia may be required to ensure the immobility of the patient during a magnetic resonance imaging (MRI) scan, particularly in pediatric patients. An MRI scanner generates radiofrequency radiation (RFR) to obtain images of parts of the body. During an MRI procedure, an amount of RFR is transformed into heat by the body, thereby leading to increased body temperature. However, patients are at increased risk of hypothermia due to the impairment of thermoregulation by anesthesia and the cold and dry environment of the MRI room. The aim of this study was to investigate the effects of

OBJECTIVES: To review airway management with anesthesia for children with Treacher Collins syndrome (TCS) and determine whether intubation was more difficult with increasing age.
BACKGROUND: Treacher Collins syndrome is a rare disorder of craniofacial development characterized by maxillary, zygomatic, and mandibular dysplasia. TCS is associated with difficult intubation, but reports of airway management are limited to case reports and small cases series. Children with TCS may require multiple general anesthetics, and it has been suggested that intubation becomes more difficult with increasing

This review aimed to comprehensively assess the risk factors, clinical implications, and management strategies related to postoperative respiratory adverse events (RAEs) in pediatric anesthesia, particularly focusing on children recovering from general anesthesia. An extensive literature search was conducted across PubMed, Embase, and the Cochrane Library, prioritizing studies published from 2000 to 2025 that addressed RAEs in pediatric populations. The selected studies included randomized controlled trials, observational studies, and systematic reviews, focusing on various aspects, such as th

IMPORTANCE: Pediatric anesthesia emergence delirium (PAED) is a common complication of general anesthesia and has unknown etiology. Exposure to volatile anesthetics may contribute to PAED, and excessive exposure may occur frequently during routine pediatric anesthesia.
OBJECTIVE: To examine whether use of electroencephalography (EEG) monitoring can reduce PAED by minimizing exposure to sevoflurane while maintaining a state of unconsciousness under anesthesia.
DESIGN, SETTING, AND PARTICIPANTS: A single-center, parallel-group, 2-arm, superiority randomized clinical trial with a 1:1 allocation

Thoracic segmental spinal anesthesia (TSSA) is a developing regional anesthesia approach that involves administering local anesthetics into the thoracic spine to produce a targeted nerve block. This segmental block provides stable hemodynamics and early recovery. Lumbar spinal anesthesia is a common practice in children, and TSSA can be a promising technique due to its advantages. Despite the theoretical advantages of TSSA in pediatric patients, there is a paucity of literature describing its clinical use, safety profile, and outcomes in this population. This case series presents our initial c

BACKGROUND: Pediatric cardiac transplant recipients undergo elective cardiac catheterization and endomyocardial biopsy (CC/EMB) for graft dysfunction surveillance often facilitated by general anesthesia (GA). GA and positive pressure ventilation (PPV) also depress cardiac function confounding the graft's functional assessment. We aimed to evaluate the frequency of cardiac function decline, going from the awake to the anesthetized state, and determine its association with anesthetic and patient-related factors.
METHODS: Electronic medical records of pediatric heart transplant recipients underg

BACKGROUND: Opioid use is common and associated with side effects and risks. Consequently, analgesic strategies to reduce opioid utilization have been developed. Regional anesthesia and multimodal strategies are central tenets of enhanced recovery pathways and facilitate reduced perioperative opioid use. Opioid-free anesthesia (OFA) protocols eliminate all intraoperative opioids, reserving opioids for postoperative rescue treatment. Systematic reviews show variable results for OFA.
METHODS: In a series of Quality Improvement (QI) projects, multidisciplinary teams developed interventions to te

BACKGROUND: Emergence delirium (ED) is a mental disturbance in children during recovery from general anaesthesia. The Pediatric Anesthesia Emergence Delirium (PAED) scale is the only validated scale that assesses ED in paediatric patients undergoing general anaesthesia. The aim of this study was the translation and cross-cultural adaptation of the PAED scale into Spanish (Chile). METHODS: A five-stage translation and cross-cultural adaptation process was carried out. The reliability of the Spanish version of the PAED scale was evaluated in paediatric patients independently by a set of two rat
