Myocardial protection by retrograde cardioplegia in arterial switch operation.

Retrograde coronary sinus perfusion of cold cardioplegic solution was evaluated in infants undergoing an arterial switch operation for transposition of the great arteries. To assess myocardial injury during ischemia, hemodynamic measurements were conducted at weaning from cardiopulmonary bypass and a postoperative assay of creatine kinase isoenzyme MB was performed. In 22 infants with retrograde coronary sinus perfusion, the initial cardioplegic infusion was performed through the aortic root and additional infusion was repeated every 30 minutes by retrograde coronary sinus perfusion. The other

Utility of Doppler velocimetry in predicting outcome in twin reversed-arterial perfusion sequence.

OBJECTIVE: The aim of this study was to describe Doppler velocimetric findings in pregnancies complicated by the twin reversed-arterial perfusion sequence and to determine the association of these findings with pregnancy outcome. STUDY DESIGN: Six twin pregnancies complicated by twin reversed-arterial perfusion sequence had ultrasonographic and Doppler studies performed between 1990 and 1997. Pulsatile vessels in the umbilical cords of the pump and acardiac twins were insonated, and reversal of flow was confirmed in all cases. Resistive index values were calculated, and the difference in resi

Differentiation of cerebral tumors using multi-section echo planar MR perfusion imaging.

OBJECTIVE: We have investigated the performance of magnetic resonance (MR) perfusion imaging to differentiate between astrocytomas grade II, grade III and glioblastomas in a prospective study. MATERIALS AND METHODS: In 33 patients with suspected supratentorial primary cerebral tumors we performed multi-section Echo Planar MR perfusion imaging. Regional cerebral blood volume (rCBV) maps were calculated and the maximum rCBV was determined from the entire lesion. This value was divided by the mean rCBV value from the contralateral side, which provided the rCBV index used in this study. The rCBV

Loss of Haemodynamic Coherence, Diagnosed Using a Continuous Monitoring of Oesophageal Photoplethysmography.

We report the case of a 75-year-old man with appendicular peritonitis in whom we observed oesophageal tissue perfusion, using a new photoplethysmographic probe. As the patient suffered from lactic acidosis, we chose to increase the mean arterial pressure (MAP) using norepinephrine. Thanks to an oesophageal photoplethysmographic signal, we could observe a loss of haemodynamic coherence and a severe alteration of tissue perfusion triggered by the MAP increase. Considering new information regarding the tissue perfusion status, we were able to adjust our MAP targets accordingly and restore the ini

Breathing maneuvers as a coronary vasodilator for myocardial perfusion imaging.

PURPOSE: A combined breathing maneuver of hyperventilation, followed by a long voluntary breathhold leads to coronary vasodilation. We investigated the impact of breathing maneuvers on MR first-pass cardiac perfusion imaging and its potential clinical utility. MATERIALS AND METHODS: We studied 24 healthy volunteers (37 ± 12 years; 62.5% men) on a clinical 3 Tesla MRI system and performed first-pass perfusion MR at rest, during a short breathhold (S-HVBH) following 60 s of hyperventilation, and at the end of a long breathhold (L-HVBH) following the hyperventilation, performed in random order.

Asymmetry in cerebral perfusion from circle of Willis arterial variations in normal population.

BACKGROUND: Angiographic and cadaveric studies have evidenced variations in the circle of Willis (CoW). Age-related changes in cerebral hemodynamics may be attributable to vascular variations. OBJECTIVES: The objective is to assess interdependence of completeness of CoW with age using non-invasive MRA and cerebral perfusion using arterial spin labeling (ASL). METHODS: This single-center, prospective study segregated 189 subjects into three groups: ≤5, 5 to 18, and >18 years. Angiographic (complete CoW and vascular asymmetry index) using TOF and contrast-enhanced- (CE-) MRA, and perfusion (pe

Uterine Transplant Optimization From a Preclinical Donor Model With Controlled Cardiocirculatory Arrest.

BACKGROUND: Uterus transplantation from deceased donors offers a promising solution to the organ shortage, but optimal preservation methods are crucial for successful outcomes. Our primary objective is to conduct an initial assessment of the contribution of oxygenated hypothermic perfusion in uterine transplantation. METHODS: We performed a preclinical study on a porcine model of controlled donation after circulatory death (60 min warm ischemia). Ten uterus grafts were preserved for 12 h using static cold storage or hypothermic machine perfusion (VitaSmart device, perfusion pressure at 15 mm

Adenosine triphosphate (ATP) and adenosine cause similar vasodilator effect in patients undergoing stress perfusion cardiac magnetic resonance imaging.

To evaluate the vasodilator effect of adenosine triphosphate (ATP) compared with adenosine in stress perfusion cardiac magnetic resonance (CMR) examinations. A total of thirty-three patients underwent clinically indicated stress/rest perfusion CMR examination following intravenous injection of a total dose of 0.2 mmol/kg of gadobutrol. Individuals were randomly assigned to ATP (160 mcg/kg/min) or adenosine (140 mcg/kg/min). The vasodilator effect of both drugs was analyzed by comparing differences in heart rate, symptoms during stress, and semiquantitative myocardial and splenic perfusion para

Lack of agreement between different observers and methods in the measurement of capillary refill time in healthy volunteers: an observational study.

OBJECTIVE: Peripheral perfusion abnormalities are relevant manifestations of shock. Capillary refill time is commonly used for their evaluation. However, the reproducibility of capillary refill time measurements and their correlation with other variables of peripheral perfusion, have not been comprehensively evaluated. Our goal was to determine, in healthy volunteers, the agreement between different methods of capillary refill time quantification and different observers, as well as their correlation with other markers of peripheral perfusion. Methods: We studied 63 healthy volunteers. Two obse

Texture Analysis Using Indocyanine Green Fluorescence Measurement During Porcine Hypothermic Oxygenated Machine Perfusion.

BACKGROUND: Development of novel organ viability assessment methods during MP under hypothermic conditions is important to expand the donor pool with DCD donors for transplantation. This study proposed a novel quantitative method using Indocyanine Green (ICG) fluorescence image for assessing organ viability during hypothermic oxygenated machine perfusion (HOPE), specifically focusing on homogeneous flow distribution analyzed with the gray-scale level co-occurrence matrix (GLCM) method, which is one of the most effective texture analysis method. METHOD: The proposed method quantifies ICG image

Intrarenal Doppler approaches in hemodynamics: A major application in critical care.

The treatment of severe cases usually requires multimodality hemodynamic monitoring approaches, particularly for tissue and organ perfusion tracking. Currently, only a few studies have investigated renal perfusion status at the bedside. Ultrasound has become increasingly utilized to guide the hemodynamic management of severe patients. Similarly, intrarenal Doppler (IRD) is widely used to assess renal perfusion from both the intrarenal artery and vein perspectives. The renal resistive index (RRI), which reflects the renal arterial blood flow profile, is often applied to predict the reversibilit

Assessment of foot perfusion in patients with a diabetic foot ulcer.

Assessment of foot perfusion is a vital step in the management of patients with diabetic foot ulceration, in order to understand the risk of amputation and likelihood of wound healing. Underlying peripheral artery disease is a common finding in patients with foot ulceration and is associated with poor outcomes. Assessment of foot perfusion should therefore focus on identifying the presence of peripheral artery disease and to subsequently estimate the effect this may have on wound healing. Assessment of perfusion can be difficult because of the often complex, diffuse and distal nature of periph

Disparities between peripheral perfusion and sublingual and nailfold microcirculation in patients with septic shock and healthy volunteers.

BACKGROUND: In shock states, microcirculation has been proposed as a potential resuscitation endpoint. However, despite the availability of videomicroscopy, its clinical use remains limited. In contrast, skin perfusion monitoring is simple, widely available and has increasingly been proposed as a surrogate of tissue perfusion. Nevertheless, the relationship between sublingual microcirculation and skin perfusion in shock remains complex. This study aimed to characterize the relationship between peripheral perfusion and sublingual and nailfold microcirculation, in septic shock patients and in he
