Miracles in my time: Reflections of a pediatric respiratory physician.

Miracles, like London buses, just seem to come along. The truth is, there are no miracles, just lots of hard work behind the scenes, minds open to opportunity, serendipity, and possibly a little luck. In my time as a pediatric respiratory physician, I have borne witness to remarkable advances in treatment that have changed patients' fortunes overnight. Examples of these include artificial surfactant replacement for premature newborns, conjugate Haemophilus influenzae type b vaccination, propranolol for infants with subglottic haemangiomas, mandibular distraction for babies with micrognathia, c

From guidelines to reality: how do physicians really manage sarcoidosis?

BACKGROUND: Guidelines exist for the diagnosis, treatment, and follow-up of sarcoidosis; however, considerable variation in clinical practice remains. This study aimed to evaluate the approaches of pulmonologists in Turkey to the management of patients with sarcoidosis through a survey. METHODS: A web-based survey consisting of 26 questions regarding diagnostic and therapeutic approaches and follow-up strategies in patients with sarcoidosis was sent to the email groups of specialists. The study was designed as a multicenter project and targeted only pulmonology physicians. The responses provid

Viewpoint: a multidisciplinary approach to the assessment of patients with systemic sclerosis-associated interstitial lung disease.

Systemic sclerosis (SSc) is a rare and heterogeneous disease affecting the skin and internal organs. SSc-associated ILD (SSc-ILD) is a common and often early manifestation of SSc. This article discusses the rationale for a multidisciplinary approach (MDA) to the early identification and assessment of patients with SSc-ILD. Diagnosis of SSc-ILD is often challenging as patients with early disease can be asymptomatic, and SSc-ILD symptoms, such as exertional dyspnea and cough, are non-specific. High-resolution computed tomography (HRCT) of the lungs is the gold standard for diagnosis of SSc-ILD s

The reality of drug use in COPD: the European perspective.

COPD guidelines provide advice about the appropriate use of various medications in treating patients with this condition. Comparisons of drug therapy as recommended by these guidelines with what is actually prescribed by both primary care physicians and specialist pulmonologists in a number of European countries can be examined in a variety of ways. Nonadherence to guidelines and differences between countries are caused by a number of factors, including varying degrees of misdiagnosis and different national attitudes to various classes of drugs.

[Not Available].

Extrapulmonary tuberculosis (TB) in the gastrointestinal tract is a rare, but yet an essential differential diagnosis to patients with complex fistula disease, since the initiation of immunosuppressive therapy for presumed Crohn's disease can lead to a fulminant course of TB. This case report presents a young Danish woman with a progressive complex fistula disease, where cause and treatment were uncovered by interprofessional collaboration between pulmonologists, gastroenterologists and surgeons including a screening test for TB as well as multiple biopsies from the anal fistula tissue.

[Chronic inflammatory bowel disease and pulmonary involvement].

Pulmonary manifestations associated with chronic inflammatory bowel diseases (IBDs) are a factor of morbidity and mortality. This respiratory involvement can manifest in various ways (airways, interstitial, etc.) and remains rare, but is probably underestimated. Subclinical respiratory involvement is frequently found in IBD patients. Airway involvement with bronchiectasis is the most frequent respiratory manifestation. Management is based on a multidisciplinary approach (pulmonologist and gastroenterologist) and depends on the etiology.

Large Thoracic Lymphadenopathy and Pulmonary Nodules in Young Man.

A 33-year-old man with obesity, systemic arterial hypertension, and psoriasis who had been treated previously with little success by a pulmonologist for chronic unproductive irritant cough came to the outpatient pulmonary department because of profuse cough and short syncope (probably cough-induced). Chest radiography revealed widened mediastinum with lobular, polycyclic contours that was suspected to be a large mediastinal lymphadenopathy or mediastinal mass.

[Updated recommendations for vaccination in adults with respiratory diseases. Argentine Association of Respiratory Medicine, 2023].

Adult smokers, those with comorbidities, and the elderly, are at greater risk of contracting infections and their complications. Community acquired respiratory infections due to viruses, pneumococcus and other bacteria, affect both healthy and sick adults. There are vaccines that the pulmonologist must know and prescribe. The target strains of the influenza vaccine are defined by the WHO for the Southern hemisphere considering those involved in the previous influenza season in the Northern hemisphere. Its effectiveness depends on virulence, concordance between circulating and vaccine strains,

Initial staging of head and neck squamous cell carcinoma. What is the place of bronchoscopy and upper GI endoscopy?

To determine the place of bronchoscopy and upper GI endoscopy in the initial staging of head and neck squamous cell carcinoma (HNSCC). A 10-year retrospective study was conducted on a series of 838 patients. As part of initial staging of the tumor, all patients were examined by neck and chest CT scan, 487 patients were examined by bronchoscopy and 588 patients were examined by upper GI endoscopy. Esophageal cancer was detected in 4.25 % of cases and lung cancer in 6.35 % of cases. Chest CT scan was statistically superior to bronchoscopy to detect second lung cancers (p < 0.05). On multivariate

Clinical and Serological Characteristics of Idiopathic Inflammatory Myopathies According to the Presence of Interstitial Lung Disease and Initial Evaluating Medical Specialty: A Single-Center Experience.

Background: Idiopathic inflammatory myopathies (IIMs) are a heterogenous group of disorders frequently complicated by interstitial lung disease (ILD). We sought to discern phenotypic and serological differences according to the presence of ILD and initial evaluating medical specialty, i.e., rheumatology vs. pulmonology, with the goal of advancing personalized medicine. Methods: A computer-assisted search was conducted to identify patients with a diagnosis of IIM seen at Attikon University Hospital, from January 2010 to December 2025. Medical records were reviewed for clinical, laboratory and s

Clinician-Perceived Barriers and Facilitators for Initiation and Management of Noninvasive Ventilation Among Patients With Chronic Hypercapnia From COPD: A Qualitative Study.

BACKGROUND: Long-term non-invasive ventilation (NIV) improves survival and re-admission risk for patients with chronic hypercapnia from chronic obstructive pulmonary disease (COPD). However, outpatient NIV prescription remains low. METHODS: We conducted a qualitative study using semi-structured interviews with pulmonary physicians. We identified a convenience sample of community-affiliated outpatient pulmonologists who were recruited via email. Deductive and inductive thematic content analysis were used to identify how physicians screen and manage patients with COPD at risk for chronic hyperc

The rise of artificial intelligence in respiratory primary care and pulmonology: a scoping review.

Artificial intelligence (AI) is rapidly advancing respiratory disease management, from diagnosis to population lung health. This scoping review synthesizes the most promising uses of AI in respiratory medicine, with a particular focus on pulmonologists and family physicians interested in lung health. In diagnostics, deep-learning systems streamline chest-imaging workflows by triaging radiographs, detecting COVID-19 pneumonia, and classifying lung nodules on CT. In pulmonary function testing, algorithms detect technical errors and classify spirometric patterns, some claiming to outperforming pu

Diagnosis in Children With Prolonged or Recurrent Cough: Findings From the Swiss Paediatric Airway Cohort.

INTRODUCTION: Prolonged or recurrent cough is a common reason for referral to pediatric pulmonologists, yet few studies have assessed its causes. We examined records of children visiting respiratory outpatient clinics in Switzerland and assessed how diagnoses vary by age. METHODS: We analyzed data from the multicenter Swiss Paediatric Airway Cohort study. We included 363 children (median age 6 years, range 0-16) referred for prolonged or recurrent cough. From outpatient records, we extracted information on diagnostic investigations, final diagnoses proposed by pediatric pulmonologists, and tr

The impact of interstitial lung disease in patients with acid sphingomyelinase deficiency (ASMD) - A case series.

BACKGROUND: Lysosomal storage diseases (LSD) are inherited diseases caused by mutations affecting genes encoding the function of lysosomal enzymes. Acid sphingomyelinase deficiency (ASMD) is an ultra-rare, progressive, and often fatal LSD with an estimated prevalence of 1-in-50,000-250,000 individuals. The PrevASMD study established the ASMD prevalence in Spain at 35 patients. Almost all present asymptomatic interstitial lung disease (ILD), since >90 % patients with chronic visceral ASMD have radiographic evidence of ILD. OBJECTIVE: This case series aims to emphasize the importance of ASMD in
