COVID 19-Induced Smell and Taste Impairments: Putative Impact on Physiology.

Smell and taste impairments are recognized as common symptoms in COVID 19 patients even in an asymptomatic phase. Indeed, depending on the country, in up to 85-90% of cases anosmia and dysgeusia are reported. We will review briefly the main mechanisms involved in the physiology of olfaction and taste focusing on receptors and transduction as well as the main neuroanatomical pathways. Then we will examine the current evidences, even if still fragmented and unsystematic, explaining the disturbances and mode of action of the virus at the level of the nasal and oral cavities. We will focus on its

Smell and Taste in Severe CoViD-19: Self-Reported vs. Testing.

One of the most striking reported symptoms in CoViD-19 is loss of smell and taste. The frequency of these impairments and their specificity as a potential central nervous system function biomarker are of great interest as a diagnostic clue for CoViD-19 infection as opposed to other similar symptomatologic diseases and because of their implication in viral pathogenesis. Here severe CoViD-19 was investigated by comparing self-report vs. testing of smell and taste, thus the objective severity of olfactory impairment and their possible correlation with other symptoms. Because a significant discrep

When insects smell like plants: how sensory bias shapes insect pheromone evolution.

Insects produce a vast diversity of sex pheromone compounds despite strong stabilizing selection on species-specific signals. In multiple insect lineages, these pheromones chemically resemble the volatiles produced by plants. Here, I propose that adaptation of insect sensory systems to plant chemistry biases the origin and diversification of insect pheromones. Repeated independent recruitment of plant-like compounds, either through sequestration or de novo biosynthesis, suggests that pheromone evolution can be shaped by ecological interactions. Receptors with dual roles in host detection and p

Taste and Smell Disturbances Associated With Secondary Adrenal Insufficiency: A Case Report.

BACKGROUND: Adrenal insufficiency often presents with nonspecific symptoms, impeding early diagnosis. Taste and smell disturbances are rarely recognized as presenting features. CASE PRESENTATION: We report the case of a 54-year-old man with a 3-month history of persistent taste and smell disturbances accompanied by fatigue, loss of appetite, weight loss, and diffuse body pain, including thigh stiffness. Endocrine evaluation with stimulation tests revealed markedly low levels of cortisol and adrenocorticotropic hormone, leading to a diagnosis of secondary adrenal insufficiency. After glucocort

Exploring smell ability in tracheal intubated patients: An investigation into sniffing proficiency.

BACKGROUND: Mechanically ventilated patients often experience a loss of smell sensation because of the tubes used in the ventilation process. This can have a significant impact on their overall well-being. As a result, there has been a growing interest in exploring ways to reintroduce and stimulate the sense of smell among these patients. AIM: To explore the smell ability among tracheal intubated patients and identify the factors influencing olfactory function in this population. STUDY DESIGN: A prospective cohort study. Data were collected in four adult intensive care units (ICUs) in two ho

Stellate Ganglion Block for Treating Post-COVID Impairment of Taste and Smell.

BACKGROUND: The loss of smell and taste are some of the chronic complications that have impacted and continue to impact patients who suffered a COVID-19 infection. Impairment of smell and taste have significant impact on patients' lives. Current treatment modalities are very limited. CASE REPORT: We present a 40-year-old patient who completely lost smell and taste following a COVID-19 infection. The patient was treated with bilateral stellate ganglion blocks (SGBs) where each side was blocked at separate visits. At the 6-month follow-up, taste and smell both improved by 90%. CONCLUSIONS: SGB

Distinct smell and taste disorder phenotype of post-acute COVID-19 sequelae.

PURPOSE: Olfactory dysfunction (OD) commonly accompanies coronavirus disease 2019 (COVID-19). We investigated the kinetics of OD resolution following SARS-CoV-2 infection (wild-type and alpha variant) and its impact on quality of life, physical and mental health. METHODS: OD prevalence was assessed in an ambulatory COVID-19 survey (n = 906, ≥ 90 days follow-up) and an observational cohort of ambulatory and hospitalized individuals (n = 108, 360 days follow-up). Co-occurrence of OD with other symptoms and effects on quality of life, physical and mental health were analyzed by multi-dimensional

Lactoferrin supplementation for taste and smell abnormalities among patients receiving cancer chemotherapy.

PURPOSE: Taste and smell abnormalities (TSA) are common in patients receiving chemotherapy and may lead to altered nutritional intake, treatment withdrawal, and impaired quality of life. Lipid peroxidation in the oral cavity is one cause of TSA. Lactoferrin (LFN), an iron-binding salivary protein, reduces production of lipid oxidation byproducts and has been shown to reduce perception of unpleasant flavors. To assess the feasibility of LFN as a treatment for TSA, we conducted pilot investigations among patients with cancer who self-reported TSA following onset of chemotherapy. The primary obje

Loss of Smell and Taste Among Healthcare Personnel Screened for Coronavirus 2019.

Among 283 symptomatic healthcare personnel (HCP) tested for SARS-CoV-2, 51 (18%) were positive. Among those 51 HCP, self reported loss of smell and taste were present in 51% and 52.9%, respectively, with either present in 60.8%. These symptoms had high specificity (93% each, 96% for either) for a positive SARS-CoV-2 test.

Effects of COVID-19 on Sense of Smell: Human Factors/Ergonomics Considerations.

OBJECTIVE: We review the effects of COVID-19 on the human sense of smell (olfaction) and discuss implications for human-system interactions. We emphasize how critical smell is and how the widespread loss of smell due to COVID-19 will impact human-system interaction. BACKGROUND: COVID-19 reduces the sense of smell in people who contract the disease. Thus far, olfaction has received relatively little attention from human factors/ergonomics professionals. While smell is not a primary means of human-system communication, humans rely on smell in many important ways related to both quality of life

Smell and taste dysfunction during the COVID-19 outbreak: a preliminary report.

In late December 2019, in Wuhan (China), health authorities reported several clusters of pneumo- nia of unknown cause, subsequently attributed to a novel coronavirus, identified as Severe Acute Respiratory Syndrome-Coronavirus 2. Anosmia and dysgeusia have been reported as particular symptoms.4,5  Notably, these sensory symptoms seem to have a peculiar trend, such as usually precede the onset of respiratory symp- toms. So, they have been defined as "sentinel" symptoms. We presented a series of COVID-19 patients. Anos- mia and dysgeusia frequently preceded respiratory complaints. Anosmia and dy

Association Between Orthonasal Olfaction and Chemosensory Perception in Patients With Smell Loss.

OBJECTIVES: Self-ratings seem to be the most effortless strategy for assessment of patients' chemical senses. Notably, although flavor perception strongly relies on olfaction, the relationship between self-reported flavor perception and orthonasal olfactory tests have hitherto not been considered. The aim of this study was to investigate the relationship between self-perceived olfactory function (SO), taste (ST), and flavor perception (SF) and smell test results in patients with olfactory dysfunction (OD). METHODS: We included 203 patients with quantitative OD. Group comparison, bivariate cor

Tracking Smell Loss to Identify Healthcare Workers with SARS-CoV-2 Infection.

BACKGROUND: Healthcare workers (HCW) treating COVID-19 patients are at high risk for infection and may also spread infection through their contact with vulnerable patients. Smell loss has been associated with SARS-CoV-2 infection, but it is unknown whether monitoring for smell loss can be used to identify asymptomatic infection among high risk individuals, like HCW. METHODS: We performed a prospective cohort study, tracking 473 HCW across three months to determine if smell loss could predict SARS-CoV-2 infection in this high-risk group. HCW subjects completed a longitudinal, novel behavioral

Smell preservation following endoscopic unilateral resection of esthesioneuroblastoma: a multi-institutional experience.

BACKGROUND: The gold standard of treatment for esthesioneuroblastoma consists of en bloc craniofacial resection with postoperative therapy dictated by histology and tumor extent. Numerous studies have shown fully endoscopic approaches to provide comparable survival and recurrence rates with decreased patient morbidity. Here we report the first multi-institutional series assessing smell outcomes of patients who underwent unilateral endoscopic resection of esthesioneuroblastoma with preservation of the contralateral olfactory bulb. METHODS: A multi-institutional retrospective review was perform

Recovery of olfactory function induces neuroplasticity effects in patients with smell loss.

The plasticity of brain function, especially reorganization after stroke or sensory loss, has been investigated extensively. Based upon its special characteristics, the olfactory system allows the investigation of functional networks in patients with smell loss, as it holds the unique ability to be activated by the sensorimotor act of sniffing, without the presentation of an odor. In the present study, subjects with chronic peripheral smell loss and healthy controls were investigated using functional magnetic resonance imaging (fMRI) to compare functional networks in one of the major olfactory

Olfactory event-related potentials in normal subjects and patients with smell disorders.

We developed a device to record olfactory event-related potentials (OERP) from the human scalp. Methyl-cyclopentenolone was used as the odorant element. A total of 50 stimuli were delivered. Each stimulus lasted 0.5 sec and was delivered once every three inspirations. Normal OERPs were obtained with this device. The positive peak latencies were approximately 350 msec (P1) and 700 msec (P2), respectively. OERPs were also recorded in 40 patients with smell disorders. A positive response at about 300-400 msec was recorded in 7 patients (all females, 15-59 years old). The other 33 patients showed
