Tissue cytokine patterns distinguish variants of rheumatoid synovitis.

Rheumatoid arthritis (RA) is a chronic inflammatory disease with primary manifestations in the synovial membrane. Tissue infiltrates are composed of T cells, B cells, and macrophages, but histopathological appearances vary widely and are rarely pathognomonic. Mechanisms underlying the phenotypic heterogeneity of rheumatoid synovitis are not known. To explore whether a correlation exists between the microscopic patterns of rheumatoid synovitis and in situ production of cytokines, tissue samples from 21 consecutive patients with clinically active RA were examined. Based upon the organization of

Plant thorn synovitis diagnosed by magnetic resonance imaging.

Plant thorn synovitis is an overlooked cause of monoarthritis, with difficulties in diagnosis and treatment (1). We report on a case of thorn induced synovitis, not confirmed by histopathology, but strongly suspected by magnetic resonance imaging (MRI), that visualizes the foreign body (first case reported).

A toddler with transient synovitis and COVID-19 infection.

Transient synovitis of the hip is a common medical emergency in children. Although reactive arthritis in adults may occur 1-4 weeks after COVID-19 infection or post-vaccination, few reports of transient synovitis in children associated with acute COVID-19 infection have been made. Transient synovitis of the hip occurs most frequently in children 3-8 years of age with a recent upper respiratory infection, bacterial infection, or trauma. This case report presents a unique case of an otherwise healthy 15-month-old girl with right hip pain and the refusal to ambulate associated with COVID-19 infec

Arthroscopic Treatment of Pigmented Villonodular Synovitis of the Elbow.

Pigmented villonodular synovitis (PVNS) is a benign proliferative disorder of synovium that surrounds the joints, tendon sheaths, and bursae. The elbow is rarely affected, making it difficult to establish treatment guidelines. This article relates on a case of a male patient who presented with elbow pain and decreased range of motion. Diagnosis was established with magnetic resonance imaging (MRI) and biopsy, followed by arthroscopic removal and synovectomy. The patient was pain free shortly after surgery and gained free range of motion after six weeks. At the most recent follow-up after six m

Diagnosis of Metacarpophalangeal Synovitis with Musculoskeletal Ultrasound Images.

Rheumatoid arthritis (RA) is a chronic autoimmune disease that can result in considerable disability and pain. The metacarpophalangeal (MCP) joint is the most common diseased joint in RA. In clinical practice, MCP synovitis is commonly diagnosed on the basis of musculoskeletal ultrasound (MSUS) images. However, because of the vague criteria, the consistency in grading MCP synovitis based on MSUS images fluctuates between ultrasound imaging practitioners. Therefore, a new method for diagnosis of MCP synovitis is needed. Deep learning has developed rapidly in the medical area, which often requir

Recurrent pigmented villonodular synovitis of the temporomandibular joint.

Pigmented villonodular synovitis is a benign but locally aggressive extra-articular tumor arising from the synovial membrane of tendons and bursae occurring near a joint space. Rarely, pigmented villonodular synovitis can involve the temporomandibular joint, which is emphasized in this paper. Diffuse and localized types have been described in the literature. The diffuse type involves the entire synovial membrane and infiltrates adjacent structures, which tend to be more aggressive and associated with a higher rate of recurrence when compared with the localized type.

The Value of an Automated Ultrasound System in the Detection of Synovitis.

BACKGROUND: The detection of joint swelling caused by synovitis is important for the diagnosis of inflammatory arthritis. Ultrasound (US) and MRI have proven to be more sensitive and reliable than physical examination, but they are time-consuming and expensive. The automated breast volume scanner was developed to acquire serial B-mode pictures of the female breast and these can be analyzed in all three dimensions. OBJECTIVES: To analyze the value of automated B-mode ultrasound employing the ABVS system in detecting synovitis of the finger joints compared to manual ultrasound (mUS) and physica

Pigmented villonodular synovitis occurring in the temporomandibular joint.

OBJECTIVE: Pigmented villonodular synovitis occurring in the region of the temporomandibular joint is a rare disease, requiring a review of the treatment method, follow-up period. METHOD: Refer to the past literature, along with a retrospective search. RESULTS: An excision, including the skull base bone, was performed in all cases; however, recurrence was found in one case on which fractional excision was performed. Past reports have also indicated that en bloc resection was considered desirable. CONCLUSION: It is necessary to perform en bloc resection on patients with pigmented villonodula

Villonodular Synovitis of the Subtalar Joint: A Case Report.

INTRODUCTION: Villonodular synovitis (SVN) is a rare benign pseudotumoral proliferation of the synovial joint, of unknown etiology. In general, it reaches the big joints, especially the knee. Localization at the subtalar level is extremely rare, with only a few cases published in the literature. CASE REPORT: We report the case of an SVN of the subtalar in a 60-year-old patient without particular histories, who consulted for a swelling of the external border of the right foot evolving for 2 years. Biology, as well as the standard radiographs, was without anomalies. The diagnosis was evoked on

Carpal synovitis with capitate bone tuberculosis in a child.

We present a 10-year-old boy with 2-month duration non-traumatic wrist pain and inflammatory signs. Due to elevated inflammatory markers on blood tests, with an increase in radiocarpal and intercarpal joints synovial fluid and no bony lesions, the patient was submitted to wrist arthrocentesis for the suspicion of septic arthritis. The patient did not improve on conventional treatment, however. An MRI showed synovitis around the carpus and a lytic lesion of the capitate bone due to osteomyelitis. A biopsy was able to identify the causative agent as Mycobacterium tuberculosis, and the patient wa

Synovitis in osteoarthritis: current understanding with therapeutic implications.

Modern concepts of osteoarthritis (OA) have been forever changed by modern imaging phenotypes demonstrating complex and multi-tissue pathologies involving cartilage, subchondral bone and (increasingly recognized) inflammation of the synovium. The synovium may show significant changes, even before visible cartilage degeneration has occurred, with infiltration of mononuclear cells, thickening of the synovial lining layer and production of inflammatory cytokines. The combination of sensitive imaging modalities and tissue examination has confirmed a high prevalence of synovial inflammation in all

Localized pigmented villonodular synovitis in a child knee.

Pigmented villonodular synovitis is a benign proliferative tumor of the synovium. It is very rare, and most cases occur in the knee joint. In this article, we report a case of localized pigmented villonodular synovitis in the knee joint of a 14-year-old boy. This condition is rare in the knees of the children. We preferred to remove the tumor with arthrotomy instead of arthroscopy for two reasons: (i) the patient was obese, (ii) we thought that recurrence risk was high after arthrotomy.

Could knee inflammatory synovitis be induced by pembrolizumab?

Pembrolizumab, a selective anti-PD-1 humanized monoclonal antibody, reactivates T cells to fight cancer. Immune-related adverse events such as autoimmune colitis, pneumonitis, hepatitis, nephritis, hypophysitis, and thyroiditis may occur during, or weeks to months after therapy. Pemprolizumab-induced synovitis is rarely reported. With the wide use of immunotherapy to treat cancers, physicians need to be aware of this rare immune-related adverse event and provide immediate treatment to avoid permanent joint damage.

Pigmented Villonodular Synovitis of the Hip: A Systematic Review.

Pigmented villonodular synovitis is a rare proliferative condition of the synovium that affects large joints. The primary treatment options are synovectomy and a combination of synovectomy and arthroplasty. We performed a systematic review of the literature, excluding all nonclinical and review articles with follow-up of less than 2 years. Primary outcomes reported were disease recurrence, symptom progression, and revision surgery. Student t tests were used to compare outcomes after synovectomy with outcomes after synovectomy combined with arthroplasty. Twenty-one studies (82 patients) were in

Musculoskeletal ultrasonography in the diagnosis of acute crystalline synovitis.

Both gout and calcium pyrophosphate deposition disease are common metabolic arthopathies, presenting not only diagnostic but also management challenges. Though histological crystal aspiration is definitive, diagnosis is commonly established through a composite of clinical features and laboratory findings. Musculoskeletal ultrasound has a contributory and growing role not only in routine disease surveillance but also in helping render a timely and specific diagnosis for patients presenting with new-onset oligoarticular arthritis in the emergency setting. In this article, we review the various g
