Haemodynamic changes during modified ultrafiltration immediately following the first stage of the Norwood reconstruction.

BACKGROUND: Modified ultrafiltration has been shown to reverse haemodilution and improve ventricular function following cardiopulmonary bypass. There has been concern, however, about the safety and efficacy of modified ultrafiltration after the first stage of Norwood reconstruction for palliation of neonates with hypoplasia of the left heart and its variants. METHODS: We reviewed the intraoperative course of all patients undergoing the first stage of Norwood reconstruction between September 1, 2000, and August 31, 2002. RESULTS: The first stage of reconstruction was performed in 99 neonates,

Modified ultrafiltration may improve postoperative pulmonary function in children with a ventricular septal defect.

To evaluate the effectiveness of modified ultrafiltration (MUF) on ventricular septal defect (VSD) repair in children, we retrospectively examined 10 patients who underwent VSD repair with MUF at the Kinki University School of Medicine hospital between June 1998, and December 1998 (MUF group). These patients were compared with 14 patients who underwent the same procedure without MUF (control group) between January 1997 and June 1998. Systolic blood pressure and hematocrit values increased significantly during MUF. By the time of postoperative transfer to the intensive care unit, PaO2 was highe

Mechanistic insights into ultrafiltration fouling mitigation by coagulation pretreatment in livestock biogas slurry treatment.

The treatment and utilization of livestock biogas slurry (BS) is significantly hindered by severe membrane fouling associated with high turbidity and complex dissolved organic matter (DOM). This study established an integrated coagulation-ultrafiltration (UF) pretreatment system using aluminum sulfate or ferric chloride as coagulants to provide stable influent for subsequent membrane concentration. Results demonstrated that while raw BS caused a rapid increase in transmembrane pressure (TMP) during UF, optimized coagulation at 3 g/L maintained TMP below 10 kPa over 6 h. Specifically, reversibl

Phosphoric Acid-Modified TiO2 Nanoparticles as Functional Additives for High-Performance Polyethersulfone Ultrafiltration Membranes.

Polyethersulfone (PES) ultrafiltration membranes were modified with phosphoric acid-functionalized TiO2 nanoparticles (0-1 wt%) via phase inversion to enhance permeability, antifouling performance, and dye removal for wastewater treatment. Structural and morphological analyses confirmed the successful modification and uniform incorporation of TiO2 within the PES matrix. The membrane containing 0.5 wt% TiO2 exhibited optimal performance, achieving the lowest contact angle (~39°), highest porosity (~65.5%), and maximum pure water flux (~292 L·m-2·h-1), compared to 204 L·m-2·h-1 for pristine PES.

Melatonin decreases GSDME mediated mesothelial cell pyroptosis and prevents peritoneal fibrosis and ultrafiltration failure.

Peritoneal fibrosis together with increased capillaries is the primary cause of peritoneal dialysis failure. Mesothelial cell loss is an initiating event for peritoneal fibrosis. We find that the elevated glucose concentrations in peritoneal dialysate drive mesothelial cell pyroptosis in a manner dependent on caspase-3 and Gasdermin E, driving downstream inflammatory responses, including the activation of macrophages. Moreover, pyroptosis is associated with elevated vascular endothelial growth factor A and C, two key factors in vascular angiogenesis and lymphatic vessel formation. GSDME defici

Zero balance ultrafiltration to correct metabolic alkalosis occurring during veno-arterial extracorporeal membrane oxygenation.

INTRODUCTION: We describe a case report of metabolic alkalosis that occurred during veno-arterial extracorporeal membrane oxygenation (VA ECMO) that was corrected during cardiopulmonary bypass (CPB) using zero balance ultrafiltration with normal saline (NS) 0.9%. CASE REPORT: A 67-year-old male received a heart transplant 6 days after being placed on VA ECMO. During VA ECMO the patient developed metabolic alkalosis. During CPB, zero balance ultrafiltration with NS 0.9% was used to correct metabolic alkalosis. DISCUSSION: Metabolic alkalosis during CPB is rare. To our knowledge, this is the f

Bioimpedance Spectroscopy-Guided Ultrafiltration Normalizes Hydration and Reduces Intradialytic Adverse Events in Hemodialysis Patients.

Chronic fluid overload as well as excessive fluid removal are associated with increased morbidity and mortality in hemodialysis (HD) patients. The clinical method to probe the dry weight is often inaccurate and the bioimpedance spectroscopy (BIS) is shown to improve the accuracy. We compared the impact of BIS and clinical methods to guide ultrafiltration (UF) in a randomized controlled study on the intradialytic complications and blood pressure control in prevalent HD patients. Fifty patients on maintenance HD were randomized to BIS method (BIS-group) and clinical method (CL-group) to guide UF

Fluorescent natural organic matter responsible for ultrafiltration membrane fouling: Fate, contributions and fouling mechanisms.

Membrane fouling has been a main obstacle to the success of ultrafiltration (UF) technology. Recently, fluorescent natural organic matter (FNOM), including humic-like substances (HS) and protein-like substances, has been recognized as substances responsible for membrane fouling. In this study, the matrix of FNOM in natural river water was substantially modified by combined coagulation and powdered activated carbon adsorption to enhance the diversity of the FNOM matrix. Fluorescence excitation emission matrix spectroscopy was employed to characterize FNOM components during the UF process. The c

Reactive iron sulfide (FeS)-supported ultrafiltration for removal of mercury (Hg(II)) from water.

This study investigated removal of Hg(II) from water using FeS(s) with batch and continuous contact filtration systems. For the batch system, kinetic experiments showed that removal of Hg(II) by FeS(s) was rapid at lower concentration (500 μM), but at higher concentration (1000 and 1250 μM), more time was required to achieve greater than 99% removal. The concentration of iron released to the solution remained relatively low, typically below 3 μM. This would theoretically present less than 1% of the Hg(II) removed. Thus, a simple exchange of Hg(II) for Fe(II) in the solid (FeS(s)) does not expl

Relationship between malnutrition-inflammation syndrome and ultrafiltration volume in continuous ambulatory peritoneal dialysis patients.

OBJECTIVE: Malnutrition inflammation syndrome may contribute to a change of peritoneum, leading to high peritoneal membrane transport, peritoneal albumin loss, and increased glucose uptake into systemic circulation and decreased ultrafiltration (UF) volume. Fluid overload is a common problem among CAPD patients which has an effect on morbidity and mortality in these patients. The present study was designed as a pilot to find out a correlation between malnutrition and UF volume in CAPD patients. MATERIAL AND METHOD: A cross-sectional study was comducted in 42 stable CAPD cases at CAPD clinic,

Ultrafiltration of natural organic matter and black carbon: factors influencing aggregation and membrane fouling.

There are concerns about black carbon (BC), due to its potential for sorption of toxic pollutants and inevitably entering drinking water sources. This study aimed to evaluate factors affecting BC aggregation and membrane fouling in the ultrafiltration of river water. Hydrophilic carbon black (CB) was selected as a surrogate of submicron BC in natural waters. Calcium, pH, and natural organic matter (NOM) were found to influence CB aggregation. Calcium induced interparticle interactions in a pH range of 4.3-7.7. In river water at 0.3mM Ca2+, CB remained as fine aggregates (<300 nm) that caused s

Aquaporin-1 plays an essential role in water permeability and ultrafiltration during peritoneal dialysis.

The water channel aquaporin-1 (AQP1) is considered as the molecular counterpart of the ultrasmall pore predicted by the three-pore model of fluid transport across the peritoneal membrane. However, the definitive proof of the implication of AQP1 in solute-free water transport, sodium sieving, and ultrafiltration (UF) during peritoneal dialysis (PD) is lacking, and the effects of its deletion on the structure of the membrane are unknown. Using real-time reverse transcriptase-polymerase chain reaction and immunogold electron microscopy, we showed that AQP1 is the most abundant member of the AQP g

Does modified ultrafiltration reduce the systemic inflammatory response to cardiac surgery with cardiopulmonary bypass?

Cardiopulmonary bypass (CPB) is associated with an accumulation of total body water and a systemic inflammatory response syndrome (SIRS), which, in turn, is associated with organ dysfunction and postoperative morbidity. It has been suggested that modified ultrafiltration (MUF) may be capable of reducing SIRS and improving clinical outcome by filtering out the inflammatory mediators generated during CPB. This paper reviews the data regarding the use of MUF in paediatric and adult settings. Specifically, three issues will be considered: 1) Does MUF improve clinical outcome? 2) Does MUF reduce th

Circulatory response to fluid overload removal by extracorporeal ultrafiltration in refractory congestive heart failure.

OBJECTIVES: The goal of this study was to investigate the hemodynamic and circulatory adjustments to extracorporeal ultrafiltration (UF) in refractory congestive heart failure (rCHF). BACKGROUND: In rCHF, UF allows clinical improvement and restores diuretic efficacy. However, in the course of a UF session, patients are exposed to rapid variations of body fluid composition so that, as fluid is withdrawn from the intravascular compartment, hypotension or even shock could occur. METHODS: In 24 patients with rCHF undergoing UF, we measured, after every liter of plasma water removed, hemodynamics
