Cost implications of self-management education intervention programmes in arthritis.

UNLABELLED: The purpose of this review is to examine cost implications, including cost-effectiveness analyses, cost-savings calculated from health-care utilisation and intervention delivery costs of arthritis-related self-management education (SME) interventions. METHODS: Literature searches, covering 1980-March 2012, using arthritis, self-management and cost-related terms, identified 487 articles; abstracts were reviewed to identify those with cost information. RESULTS: Three formal cost-effectiveness analyses emerged; results were equivocal but analyses done from the societal perspective,

Value Based Purchasing to Increase Tobacco Cessation Counseling and Medications in Behavioral Health Homes.

Tobacco use among individuals with behavioral health conditions is higher than in the general population and a leading cause for morbidity and early mortality. This study examines a value-based payment (VBP) model to incentivize provision of tobacco cessation counseling (TCC) and pharmacological treatment with varenicline among 38 behavioral health homes (a person-centered approach to coordinating comprehensive healthcare in behavioral health service settings for individuals with chronic behavioral and physical health conditions) within a non-profit Medicaid behavioral health managed care netw

Solutions to reduce healthcare costs and improve outcomes in diagnostics: safety nets, optimized navigation, and decision support.

OBJECTIVES: Diagnostic error is common, harmful, and costly, yet most health systems lack active interventions to improve diagnostic safety. This study aimed to identify scalable care models that advance diagnostic excellence while reducing costs for healthcare systems across diverse delivery and reimbursement environments. METHODS: We employed a multi-method care model development framework that integrated: (1) a literature review of 1,632 sources, (2) 19 semi-structured expert interviews, (3) in-depth analysis of four exemplar programs, and (4) four iterative refinement cycles with a 12-mem

Novel oral iron therapy for iron deficiency anaemia: How to value safety in a new drug?

AIMS: Novel oral iron supplements may be associated with a reduced incidence of adverse drug reactions compared to standard treatments of iron deficiency anaemia. The aim was to establish their value-based price under conditions of uncertainty surrounding their tolerability. METHODS: A discrete-time Markov model was developed to assess the value-based price of oral iron preparations based on their incremental cost per quality-adjusted life year (QALY) gained from the perspective of the NHS in the UK. Primary and secondary care resource use and health state occupancy probabilities were estimat

Health Care Quality Measures: Methodological Considerations and the Role of the Consensus-Based Entity (Battelle Partnership for Quality Measurement).

This article examines the methodological foundations and evolving frameworks of health care quality measurement, with a focus on cardiothoracic surgery. It highlights the importance of rigorous measure development, including appropriate population selection, endpoint definition, and data sourcing. Central to provider performance evaluation is risk adjustment to account for case-mix variability. The article critiques inconsistencies in existing quality metrics and emphasizes the need for reliability and validity in measurement models. It also describes the role of the Consensus-Based Entity (Ba

Beyond cost-effectiveness: Reframing the value of essential nursing care in healthcare priority-setting.

Cost-effectiveness analysis (CEA) has become a dominant framework for healthcare priority-setting, offering policymakers a standardized way to compare costs with expected health gains under conditions of scarcity. Yet its dominance raises ethical and conceptual problems when applied to forms of care that are relational, continuous, preventive, and foundational to human wellbeing. Essential nursing care is often difficult to represent through the outcome metrics that CEA privileges, particularly in critical care, chronic illness management, palliative care, gerontological care, and end-of-life

Health care utilization patterns of patients enrolled in an interdisciplinary program for back pain.

BACKGROUND: Chronic low back pain is a costly condition and the leading cause of disability worldwide. A recent call to action identified the overuse of low-value health care as ineffective and potentially harmful in the context of back pain. Interdisciplinary pain programs (IPPs) incorporate physical therapy and behavioral pain treatments with the goal of empowering patients to actively use skills to self-manage pain, decreasing stress, disability, and reliance on low-value care. While IPPs are deemed "cost-effective," few studies elaborate upon the patterns of healthcare utilizations among p

Are hospital process quality indicators influenced by socio-demographic health determinants.

BACKGROUND: This population-level health service study aimed to address whether hospitals assure the same quality of care to people in equal need, i.e. to see if any associations exist between social determinants and adherence to four hospital process indicators clearly identified as being linked to better health outcomes for patients. PARTICIPANTS: This was a retrospective cohort study based on administrative data collected in the Veneto Region (northeast Italy). We included residents of the Veneto Region hospitalized for ST-segment elevation myocardial infarction (STEMI) or acute myocardial

The Economics of Transcatheter Valve Interventions.

A subset of patients who require correction of a stenotic or incompetent valve are deemed to be at excessive surgical risk, which precludes surgical repair or replacement. Transcatheter valve interventions are viable alternatives in these patients. However, these technologies are costly, and in the setting of a constrained Canadian health care budget, economic value is an important consideration to allow for fair allocation of scarce resources. Accordingly, we review the economic literature on transcatheter valve interventions, targeting a general audience. Our specific goals are highlighting

Considerations for Payment Bundling in Cleft Care.

Since the introduction of the Bundled Payments for Care Improvement initiative, progress has been made in piloting bundled payment models to improve care coordination and curtail health care expenditures. In light of improvements in patient outcomes and the concomitant reduction in health care spending for certain high-volume and high-cost procedures, such as total joint arthroplasty and breast reconstruction, the authors discuss theoretical considerations for bundling payments for the care of patients with orofacial clefts. The reasons for and against adopting such a payment model to consolid

High and low value care recommended and undertaken prior to knee or hip arthroplasty: a survey study.

INTRODUCTION: National and international clinical practice guidelines have stratified the value of osteoarthritis (OA) interventions. Interventions with strong evidence supporting effectiveness and benefit are 'high value care'. Appointment attendance, audits and practitioner surveys are widely used to determine frequency of recommendations and adherence to high value care. Greater patient reported data is needed in this evidence base. OBJECTIVE: To describe the frequency of high and low value care being recommended and undertaken by individuals awaiting OA-related lower limb arthroplasty. To

Measuring value at the provider level in the management of cleft lip and palate patients.

"Value" has become a buzzword in current health-care discussions. This study demonstrates a provider-led strategy to measuring costs, an understudied component of the value equation, for a complex diagnosis for the purposes of improvement. A retrospective, microcosting methodology was used to measure costs for all hospital and physician services and costs to the patient over 18 months of multidisciplinary care for patients with cleft lip and palate. Short-term outcomes were also recorded. Overall costs to all parts of the system ranged from $35,826 to $56,611 for different subtypes, and insigh

Optimizing diabetes self-care in patients with limited health literacy: a randomized controlled trial (RCT) of SCT-based education with and without an AI-designed photo-novel.

BACKGROUND: Type 2 diabetes mellitus (T2DM) requires continuous self-care to prevent complications. Patients with limited health literacy often struggle to understand health information and adhere to treatment, resulting in poorer outcomes. Educational interventions tailored to improve self-care and health literacy arSCTe therefore essential. This study evaluated the effectiveness of a Social Cognitive Theory-based intervention, with and without health literacy strategies, in enhancing self-care among patients with T2DM and limited health literacy. METHODS: This interventional study included

Health related quality of life, service utilization and costs for patients with Huntington's disease in Norway.

BACKGROUND: Huntington's disease (HD) is a progressive genetic neurodegenerative disease accompanied by mental and neurocognitive disabilities, which requires long-term and comprehensive treatment and care. Information on the health and economic burden of HD is scarce, but essential for conducting health economic analyses, in light of the prospect of new therapies for HD. In this study, we aim to identify values for Health-Related Quality of Life (HRQoL), describe service utilization and costs, and their associations with clinical and socio-demographic variables across all phases of HD. METHO

A Machine Learning Framework for Predicting Functional Status and Estimating Health Care Costs in Pulmonary Arterial Hypertension: Retrospective Cohort Study.

BACKGROUND: Pulmonary arterial hypertension (PAH) is a rare and progressive disease associated with substantial clinical heterogeneity and high health care costs. While machine learning (ML) has been increasingly applied to predict clinical outcomes, approaches that integrate clinical prediction with downstream economic modeling remain limited. OBJECTIVE: This study proposes and evaluates an ML-based framework that combines prediction of future functional status with estimation of associated health care costs in patients with PAH. METHODS: In this retrospective cohort study, data from 181 pa

Foundations and Operational Mechanics of Value-Based Care.

The global escalation of healthcare expenditures alongside fluctuating clinical outcomes has forced a paradigm shift from volume-driven reimbursement frameworks to value-based healthcare (VBHC) models. This review article outlines the fundamental architectural principles, structural mechanisms, and financial configurations underpinning value-based delivery systems, with a particular focus on macro-level healthcare system transformation. VBHC fundamentally realigns institutional and clinical incentives by substituting the legacy fee-for-service methodology with outcome-linked reimbursement prot

Confirmation of family cancer history reported in a population-based survey.

BACKGROUND: Knowledge of family cancer history is essential for estimating an individual's cancer risk and making clinical recommendations regarding screening and referral to a specialty cancer genetics clinic. However, it is not clear if reported family cancer history is sufficiently accurate for this purpose. METHODS: In the population-based 2001 Connecticut Family Health Study, 1019 participants reported on 20 578 first-degree relatives (FDR) and second-degree relatives (SDR). Of those, 2605 relatives were sampled for confirmation of cancer reports on breast, colorectal, prostate, and lung
