The effect of vernal keratoconjunctivitis on clinical outcomes of penetrating keratoplasty for keratoconus.

BACKGROUND: Although good visual results and low complication rates are commonly reported following penetrating keratoplasty (PKP) in eyes with keratoconus, the outcome of PKP in eyes with keratoconus and concomitant vernal keratoconjunctivitis (VKC) has not been well documented. We performed a study to compare the outcome and relative risk factors of PKP for keratoconus with and withoutVKC. METHODS: Review of the medical records of all patients who underwent PKP for keratoconus at a university-affiliated hospital in Izmir,Turkey, from Nov. 1, 1991, to Jan. 31, 2002. Only eyes that had been f

Expression of CD23/CD21 and CD40/CD40 ligand in vernal keratoconjunctivitis.

PURPOSE: The overproduction of immunoglobulin E (IgE) antibodies is associated with vernal keratoconjunctivitis (VKC). CD23/CD21 and CD40/CD40 ligand (CD40L) interactions have been proposed to be involved in the regulation of IgE synthesis. The purpose of the present study was to investigate the presence and distribution of CD23, CD21, CD40 and CD40L in the conjunctiva from patients with active VKC. METHODS: Conjunctival biopsy specimens from 8 subjects with active VKC and 6 control subjects were studied. We used immunohistochemical techniques and a panel of monoclonal antibodies (mAbs) direc

Histamine effects on conjunctival fibroblasts from patients with vernal conjunctivitis.

Histamine, an important mast cell mediator in allergic disorders, may affect extracellular matrix production and cell growth in vernal keratoconjunctivitis (VKC). In the present study, the histamine reactivity of conjunctival fibroblasts derived from VKC patients was investigated in vitro. Conjunctival fibroblast cultures were derived from biopses of 8 tarsal VKC patients and 5 normal subjects. These cells were maintained in vitro and stimulated with different concentrations of histamine with and without H1 (clorpheniramine) and H2 (cimetidine) receptor antagonists. Comparisons were made to fi

Vernal keratoconjunctivitis: a model of 5q cytokine gene cluster disease.

Clinical studies of vernal keratoconjunctivitis (VKC) patients show that total IgE serum levels are increased even in the absence of IgE antibodies to common allergens. Activated eosinophils are also a constant feature of VKC at both the circulation (cytofluorimetry) and tissue (tear cytology and conjunctival scrapings) levels. Moreover, allergen challenge induces a prolonged inflammatory reaction with a prevalent participation of eosinophils, lymphocytes and possibly basophils. Immunohistochemical studies of VKC biopsies show a multicellular inflammatory infiltrate with prevalence of activate

Tear histamine levels in vernal conjunctivitis and other ocular inflammations.

Histamine levels were measured in tears from normal volunteers (geometric mean, 5 ng/ml), patients with vernal conjunctivitis (16 ng/ml), and patients with a wide variety of external ocular inflammatory disease (3 ng/ml). Histamine levels were significantly higher in patients with vernal conjunctivitis than in normal subjects (P < 0.01) or patients with other types of ocular inflammation (P < 0.001). We postulate that histamine is an important mediator in the expression of the inflammatory response in vernal conjunctivitis, and that its elevation may be caused by abnormalities in the number of

Clinical trial with 2% sodium cromoglycate (Opticrom) in vernal keratoconjunctivitis.

An open assessment study was carried out during the summer of 1972 in which 2% sodium cromoglycate eyedrops were evaluated in the treatment of patients with vernal keratoconjunctivitis. A highly significant number of patients found the eyedrops effective, while a marked improvement was recorded in mild and moderate cases; in severe cases or in acute exacerbations additional steroid therapy was recommended. Sodium cromoglycate eyedrops were found to be as effective as Decadron and superior to Antistin-Privine. Furthermore SCG eyedrops could replace or reduce local steroid therapy in vernal kera

Enigmatic Role of IL-31 in the Itch and Inflammation of Vernal Keratoconjunctivitis.

OBJECTIVES: Vernal keratoconjunctivitis (VKC) is a recurrent allergic inflammation of the conjunctiva characterized by itchiness. The cytokine IL-31 is involved in itch and inflammation in other conditions, so we investigated whether it plays similar role in VKC. METHODS: We estimated messenger RNA (mRNA) IL-31 expression in giant papillae (GP) with real-time polymerase chain reaction (RT-PCR). We used immunohistochemistry to examine IL-31 receptor (IL-31R) expression in neurons and epithelial cells in GP. We then performed fluorescence-activated cell sorting and immunofluorescence to examine

Inflammatory Mediators Involved in Conjunctival and Corneal Remodeling of Vernal Keratoconjunctivitis.

Vernal keratoconjunctivitis (VKC) is a chronic condition that causes remodeling of the cornea and conjunctiva through recurring episodes of allergic inflammation of the ocular surface. This can lead to corneal scarring, keratoconus, and chronic conjunctival papillae. However, the details of the immunopathological processes behind these remodeling changes are not completely understood. Despite involving IgE-modulated mechanisms, about half of patients with VKC test negative on systemic allergy tests. This calls for the need to understand the common and novel inflammatory mediators involved in t
