Hemodynamic effects of atrial septal pacing in cardiac resynchronization therapy patients.

INTRODUCTION: Spontaneous or pacing-induced interatrial conduction delay may affect the outcome of heart failure patients treated with cardiac resynchronization therapy (CRT). The objective of this study was to evaluate the impact of the atrial pacing site (right atrial appendage, RAA; and low interatrial septum, LIS) during biventricular (BV) pacing on the left ventricular (LV) systolic function in candidates for CRT. METHODS AND RESULTS: Fifteen heart failure patients with left bundle branch block and LV ejection fraction < or =35% were enrolled. Electrodes were placed at the RAA, LIS, righ

Left ventricular dyssynchrony predicts response and prognosis after cardiac resynchronization therapy.

OBJECTIVES: This study was designed to predict the response and prognosis after cardiac resynchronization therapy (CRT) in patients with end-stage heart failure (HF). BACKGROUND: Cardiac resynchronization therapy improves HF symptoms, exercise capacity, and left ventricular (LV) function. Because not all patients respond, preimplantation identification of responders is needed. In the present study, response to CRT was predicted by the presence of LV dyssynchrony assessed by tissue Doppler imaging. Moreover, the prognostic value of LV dyssynchrony in patients undergoing CRT was assessed. METH

Sustained sympathoinhibitory effects of cardiac resynchronization therapy in severe heart failure.

Evidence is available that in heart failure, cardiac resynchronization therapy by biventricular pacing improves myocardial function and exercise capacity. Whether this is accompanied by a sustained inhibition of heart failure-dependent sympathoexcitation is uncertain. In 11 heart failure patients (mean+/-SEM age, 68.4+/-1.5 years) in New York Heart Association (NYHA) class III and IV under medical treatment with an intraventricular conduction delay (QRS duration > or =130 ms), with a markedly depressed left ventricular ejection fraction, and undergoing implantation of a biventricular pacemaker

Response to cardiac resynchronization therapy in non-ischemic cardiomyopathy is unrelated to medical therapy.

INTRODUCTION: Current guidelines recommend at least 3 months of guideline-directed medical therapy (GDMT) for patients with a new onset of non-ischemic cardiomyopathy (NICM) and left bundle branch block (LBBB) prior to cardiac resynchronization therapy (CRT). For patients who do not receive optimal GDMT, response to CRT is unknown. METHODS: Patients with NICM and LBBB with QRS ≥ 120 ms were identified among all patients who underwent CRT. Patients who received GDMT for ≥ 3 months before CRT were compared to those who did not. Among 38 patients who met inclusion criteria, 24 received optimal G

Conduction System Pacing-Optimized Cardiac Resynchronization Therapy for Congenitally Corrected Transposition.

Patients with congenitally corrected transposition of the great arteries (CCTGA) are predisposed to atrioventricular block, pacing-induced cardiomyopathy, and right ventricular (RV) failure. Although biventricular cardiac resynchronization therapy (CRT) and conduction system pacing (CSP) have been described, RV failure may persist. Three cases of CCTGA with intrinsic RV electrical delay are presented where conventional CSP targeting the His bundle or proximal left bundle branches resulted in incomplete electrical resynchronization. With CSP-optimized CRT, there was greater electrical synchrony

Single-Port Robotic-Assisted Epicardial Lead Placement for Cardiac Resynchronization Therapy.

We present a case demonstrating the novel use of the da Vinci SP (single-port) robotic platform (Intuitive Surgical) in cardiac surgery. A 69-year-old man with ischemic cardiomyopathy and failed endovascular cardiac resynchronization therapy defibrillator lead placement underwent successful robotically assisted left ventricular epicardial lead implantation through a 4-cm minithoracotomy. The da Vinci SP system enhanced visualization and dexterity, thus allowing for precise lead placement with minimal invasiveness. The patient recovered uneventfully and remained stable at follow-up. This case h

Cardiac resynchronization therapy in inotrope-dependent heart failure: a meta-analysis.

AIMS: The viability of cardiac resynchronization therapy (CRT) in inotrope-dependent heart failure (HF) has been a matter of debate. METHODS AND RESULTS: We searched Medline, EMBASE, Scopus, and the Cochrane Library until 31 December 2022. Studies were included if (i) HF patients required inotropic support at CRT implantation; (ii) patients were ≥18 years old; and (iii) they provided a clear definition of 'inotrope dependence' or 'inability to wean'. A meta-analysis was performed in R (Version 3.5.1). Nineteen studies comprising 386 inotrope-dependent HF patients who received CRT (mean age 64

Successful Catheter Ablation of Ventricular Tachycardia Precipitated by Cardiac Resynchronization Therapy.

A patient with ischemic cardiomyopathy and an implantable cardioverter-defibrillator underwent an upgrade with an epicardial left ventricular lead, which precipitated recurrent ventricular tachycardia (VT). An electrophysiological study with electroanatomic mapping showed the site of the left ventricular lead to be part of the re-entrant circuit, and substrate modification of an endocardial channel led to the resolution of VT and an improvement in symptoms.

Cardiac Resynchronization Therapy: Current Guidelines and Recent Advances Beyond Drug Treatment.

Cardiac resynchronization therapy (CRT) is the therapy of choice for patients with symptomatic systolic heart failure (HF) and left bundle branch block (LBBB), despite optimal medical therapy (OMT). The recently published 2021 European Society of Cardiology (ESC) Guidelines on cardiac pacing and cardiac resynchronization therapy highlight the importance of CRT on top of OMT in HF patients with left ventricular ejection fraction (LVEF) ≤ 35%, sinus rhythm and typical LBBB with QRS duration ≥ 150 ms. In the presence of medically intractable or recurrent after catheter ablation atrial fibrillatio

Evolution of Echocardiography-Derived Hemodynamic Force Parameters After Cardiac Resynchronization Therapy.

Echocardiography-derived hemodynamic forces (HDF) allow calculation of intraventricular pressure gradients from routine transthoracic echocardiographic images. The evolution of HDF after cardiac resynchronization therapy (CRT) has not been investigated in large cohorts. The aim was to assess HDF in patients with heart failure implanted with CRT versus healthy controls. HDF were assessed before and 6 months after CRT. The following HDF parameters were calculated: (1) apical-basal strength, (2) lateral-septal strength, (3) the ratio of lateral-septal to apical-basal strength ratio, and (4) the f

Complications of cardiac resynchronization therapy implantation: De novo implants versus upgrades.

BACKGROUND: Cardiac resynchronization therapy (CRT) is a well-established treatment of patients with advanced heart failure and electrical dyssynchrony. Implantation of those devices is in some cases associated with intervention on a formerly implanted system. The aim of this analysis was to compare the rate and type of complications of de novo implants and upgrades to CRT-D. METHODS: Retrospective data were collected from medical records, including 326 patients treated with CRT-D between 2015 and 2020. The following data were analyzed: procedure data including complications, demographics, co

Determinants of left ventricular function improvement for cardiac resynchronization therapy candidates.

AIMS: A waiting period of more than 3 months is recommended for patients before undergoing cardiac resynchronization therapy (CRT). However, due to an anticipated high mortality rate, early implementation of CRT might be beneficial for some patients. We aimed to evaluate the rate and the probability of left ventricular (LV) function improvement and their predictors in patients with heart failure (HF) with indications for CRT. METHODS AND RESULTS: From March 2011 to February 2014, a total of 5625 hospitalized patients for acute HF were consecutively enrolled in 10 tertiary hospitals. Among the

Sex-Specific Ventricular Arrhythmias and Mortality in Cardiac Resynchronization Therapy Recipients.

OBJECTIVES: The study goal was to examine whether there are sex-related differences in the incidence of ventricular arrhythmias and mortality in CRT-defibrillator (CRT-D) recipients. BACKGROUND: Few studies have evaluated sex-related benefits of cardiac resynchronization therapy (CRT). Moreover, data on sex-related differences in the occurrence of ventricular tachyarrhythmias in this population are limited. METHODS: A multicenter retrospective study was conducted in 460 patients (355 male subjects and 105 female subjects) from the UMBRELLA (Incidence of Arrhythmia in Spanish Population With

Novel electrocardiographic dyssynchrony criteria improve patient selection for cardiac resynchronization therapy.

AIMS: We hypothesized that the greater the intra- or interventricular dyssynchrony (intraD, interD), the more effective cardiac resynchronization therapy (CRT) is. We sought to improve patient selection for CRT by using novel ECG dyssynchrony criteria. METHODS AND RESULTS: Left ventricular (LV) intraD was estimated by the absolute time difference between the intrinsicoid deflections (ID) in leads aVL and aVF divided by the QRS duration (QRSd): [aVLID - aVFID]/QRSd (%). InterD was estimated from the formula: [V5ID - V1ID]/QRSd (%). Their >25% value indicated electrical dyssynchrony present (ED

Clinical outcome of cardiac resynchronization therapy in dilated-phase hypertrophic cardiomyopathy.

BACKGROUNDS: Clinical trials have demonstrated that cardiac resynchronization therapy (CRT) is effective in patients with "non-ischemic cardiomyopathy". However, patients with dilated-phase hypertrophic cardiomyopathy (DHCM) have been generally excluded from such trials. We aimed to compare the clinical outcome of CRT in patients with DHCM, idiopathic dilated cardiomyopathy (IDCM), or ischemic cardiomyopathy (ICM). Prognostic implications of mitral regurgitation in patients after cardiac resynchronization therapy.

AIM: Mitral regurgitation (MR) is a common finding in patients with heart failure with debatable effects on prognosis. Reduction in MR is one of the mechanisms by which cardiac resynchronization therapy (CRT) exerts its beneficial effects. We investigated the prognostic impact of baseline MR and MR persistence after CRT on outcomes of treated patients. METHODS AND RESULTS: We prospectively followed 1122 CRT patients (66.4 ± 10.3 years, 78% male) who were stratified according to baseline MR severity as having MR- (degree 0-1; n = 508, 45%) or MR+ (degrees 2-3-4; n = 614, 55%). Selective versus non-selective his bundle pacing for cardiac resynchronization therapy.

Cardiac resynchronization therapy (CRT) has an established role in the device-based therapy for patients with systolic dysfunction and intraventricular conduction delay, particularly left bundle branch block (LBBB). Recently, His bundle pacing (HBP) has emerged as a viable alternative for resynchronization which can successfully narrow surface QRS and improve mechanical dyssynchrony. The role of selective (i.e., an isoelectric His-paced to QRS interval similar to native HV interval) versus non-selective capture (i.e., engagement of His along with adjacent local myocardial tissue and pseudo-del
