[Analysis of the antibiotic resistant gene in multidrug-resistant Enterobacter cloacae isolated at Showa University Hospital].

We analyzed the antibiotic resistant genes, metallo-beta-lactamase and extended-spectrum beta-lactamase (ESBL), in highly resistant Enterobacter cloacae isolated at Showa University Hospital from April to June 2008. Thirty eight strains of E. cloacae were isolated. Of these strains, 35 demonstrating resistant to either cefotaxime (Minimum inhibitory concentration: MIC > or = 32 microg/ml), ceftadizime (MIC > or = 16 microg/ml), or aztreonam (MIC > or = 16 microg/ml) were selected for the analysis. The strains were subjected to a Double-disk synergy test (DDST) using sodium mercaptoacetic acid

Prevalence of extended-spectrum beta-lactamases in isolates of the Enterobacter cloacae complex from German hospitals.

In 2002, 119 isolates of the Enterobacter cloacae complex were collected randomly from 11 German laboratories nationwide. Antibiotic susceptibilities were tested by disk-diffusion tests according to CLSI guidelines, and MICs were determined using Etests. PCRs were performed to amplify all TEM and SHV, and most CTX-M and OXA beta-lactamase genes. PCR products were sequenced to identify the precise extended spectrum beta-lactamase (ESBL) types. Isoelectric focusing (IEF) and PM/PML Etests were used to confirm production of the respective ESBLs. According to susceptibility tests and CLSI criteria

Growth of cell-wall-deficient variants of Enterobacter cloacae facilitates beta-lactamase derepressed mutants.

The degree to which cell-wall-deficient bacteria (CWDB) are involved in the generation of beta-lactamase derepressed mutants (DM) was measured using Enterobacter cloacae 3624. The frequency of DM in non-permissive isotonic ticarcillin medium was compared with their frequency in hypertonic ticarcillin medium that supports CWDB growth. DM were resistant to extended spectrum penicillins and cephalosporins and had a basal beta-lactamase activity of >300 units/mg protein. Anaerobic growth of CWDB increased the relative risk of DM 2 x 10(6)-fold. Aerobic incubation produced fewer CWDB colonies but t

Extended-spectrum beta-lactamase producing strains of Enterobacter cloacae transferring resistance to cefotaxime and ceftazidime.

Transferability of ceftazidime resistance and of extended-spectrum beta-lactamase (ESBL) production was recorded in three strains of Enterobacter cloacae from Frankfurt University Clinics. All three strains were resistant to rifampicin, carbenicillin, cephaloridine, cefotaxime, ceftazidime, aztreonam, imipenem and meropenem. They transferred directly determinants of resistance to carbenicillin and cephaloridine to Escherichia coli K-12 No. 185 nal+ recipient strain. All transconjugant colonies also contained determinants of resistance to cefotaxime, ceftazidime and aztreonam. In the second cyc

Antimicrobial susceptibility of oral isolates of Enterobacter cloacae and Klebsiella pneumoniae from a southern Chinese population.

The antibiotic susceptibilities of 59 Enterobacter cloacae and 39 Klebsiella pneumoniae human oral isolates collected from a southern Chinese population in Hong Kong were investigated for their susceptibility to eight antibiotics: ampicillin, cephalothin, cefuroxime, ceftazidime, ciprofloxacin, gentamicin, tetracycline and trimethoprim/sulfamethoxazole using the E-Test method for direct quantification of minimum inhibitory concentrations. Most strains were sensitive to all antibiotics except ampicillin and cephalothin. Ampicillin resistance was exhibited by 82% of K. pneumoniae and 69% of E. c

ERIC-PCR typing profiles of Enterobacter cloacae are stable after development of advanced cephalosporin resistance.

Enterobacterial repetitive intergenic consensus (ERIC) sequence polymerase chain reaction genotype profiles of Enterobacter cloacae were prepared for paired isolates which were obtained before and after the high level expression of the chromosomal cephalosporinase. These profiles were homologous within strain pairs which suggests that the acquired resistance does not lead to major chromosomal alterations. ERIC genotypes are therefore reliable markers in epidemiological investigations where antibiogram modifications may arise as a consequence of antibiotic exposure and beta-lactamase over-produ

In vitro activity of lomefloxacin against multiply resistant Pseudomonas aeruginosa, Enterobacter cloacae, and Staphylococcus epidermidis.

Lomefloxacin is a promising new difluorinated quinolone with good antimicrobial activity and favorable pharmacokinetic properties. The in vitro activity of lomefloxacin was compared to other quinolones, expanded spectrum beta-lactams, and gentamicin. The lomefloxacin MIC90 against multiply resistant strains of P. aeruginosa was 4 mcg/ml, comparable to norfloxacin and ofloxacin. Lomefloxacin was also active against methicillin-resistant S. epidermidis and JK corynebacteria. The role of lomefloxacin in the treatment of infections caused by multiresistant organisms remains to be determined in cli

Discrepancies Between Imaging Reports and Operative Findings in Patients With Cloaca: A Call for Expansion of the Mullerian Anomalies Classification.

BACKGROUND: There is wide variation in the language used to describe Mullerian structures. To standardize terminology, the American Society of Reproductive Medicine (ASRM) created the Mullerian Anomalies Classification (MAC) in 2021. The objective of this study was to evaluate the applicability of the MAC nomenclature to pediatric patients with cloaca. METHODS: A retrospective review of all patients with cloaca was performed at a single institution. Descriptions of Mullerian structures were evaluated and compared to the ASRM MAC categories. Descriptive statistics were used to report findings.

Single perineal opening with 'H-type' cecovesical fistula and blind-ending foreshortened distal colon: a new variant of persistent cloaca.

Persistent cloaca is a rare and severe variety of anorectal malformation, which is more common in females and includes a spectrum of abnormalities. The urinary tract, genital tract and rectum open into a common channel, which exteriorises as a single perineal opening. We are reporting a patient with a novel variation in the classical anatomy of the cloaca. The child has a short blind-ending colon with a cecovesical fistula associated with mullerian agenesis and lipomyelomeningocoele. The child is being managed in a stepwise approach and she has completed the anal reconstruction. Here, we discu

Treatment guidelines for persistent cloaca, cloacal exstrophy, and Mayer-Rokitansky-Küster-Häuser syndrome for the appropriate transitional care of patients.

We developed treatment guidelines (TGs) for appropriate transitional care of the genitourinary system in patients with persistent cloaca (PC), cloacal exstrophy (CE), or Mayer-Rokitansky-Küster-Häuser syndrome (MRKH). These TGs are in accordance with the Medical Information Network Distribution Service (Minds), published in 2014 in Japan. Clinical questions (CQs) concerning treatment outcomes of the genitourinary system, pregnancy and delivery, and quality of life in adulthood were prepared as six themes for PC and CE and five themes for MRKH. We were able to publish statements on chronic rena

Clean intermittent catheterization as an initial management strategy provides for adequate preservation of renal function in newborns with persistent cloaca.

INTRODUCTION: Persistent cloaca is a rare, congenital anomaly involving the genital, urinary, and rectal organ systems. While prompt bowel diversion is the standard of care, the optimal method of genitourinary decompression is unclear. Bladder outlet obstruction and hydrometrocolpos are common complications that can lead to obstructive uropathy, abdominal distention, infection, perforation, and acidosis. Proposed management strategies include early surgical diversion (vesicostomy, vaginostomy, ureterostomy, nephrostomy) or clean intermittent catheterization (CIC) of the common channel. We hypo

Genomic epidemiology of carbapenemase-producing Enterobacter cloacae complex in Argentina: a retrospective analysis (2016-2022).

Enterobacter spp., after Klebsiella pneumoniae, is among the most common hospital-acquired enterobacteria associated with antimicrobial resistance (AMR) worldwide, yet it remains underexplored. We determined the taxonomic and epidemiological distribution of 131 representative carbapenemase-producing Enterobacter spp. recovered from infections across Argentina between 2016 and 2022. The sample included one isolate per patient from 17 jurisdictions and 68 hospitals. Outbreak-related and surveillance isolates were excluded. Sequencing was performed using Illumina, and bioinformatic analyses were

Genomic and functional characterization of novel therapeutic lytic bacteriophages targeting multidrug-resistant Enterobacter cloacae.

The alarming rates at which extensively drug-resistant (XDR) and pandrug-resistant (PDR) Enterobacter cloacae in hospitals are increasing has begun to severely limit treatment options, and thus the urgency for alternative interventions, including bacteriophage therapy. The purpose of the study was to isolate and molecularly characterize phages that can infect E. cloacae, and, furthermore, to assess the antimicrobial efficacy of the four novel lytic bacteriophages (MMRP1, MMRP2, MMRP3, and MMRP4) against antimicrobial-resistant E. cloacae isolates and to evaluate their potential as alternative

Molecular and pharmacodynamic insights into β-lactam therapy for high-inoculum Enterobacter cloacae complex infections.

High-inoculum Enterobacter cloacae complex (ECC) infections challenge β-lactam therapy due to β-lactamase-mediated resistance and the inoculum effect, where high bacterial densities amplify resistance and worsen outcomes. We evaluated the pharmacodynamic efficacy of cefepime, meropenem, ertapenem, meropenem-vaborbactam, and ceftazidime-avibactam against three high-inoculum (10⁹ colony-forming unit [CFU]/g) ECC isolates (0008, 0032, and 0060) harboring diverse β-lactamases using a 96 h simulated endocardial vegetation (SEV) model with humanized exposures. Bactericidal activity, resistance emerg

Deciphering the role of Enterobacter cloacae as a green whole-cell biocatalyst for production of Calophyllum inophyllum biodiesel.

Marine bacteria are renowned for their resilience to extreme conditions such as variable temperatures and high salinity. Producing biodiesel through whole-cell biocatalysts derived from marine bacteria is an innovative and eco-friendly approach to biofuel production, leveraging the unique enzymatic abilities of microorganisms adapted to extreme marine environments. Their manufacturing costs and environmental footprint are significantly lowered as they involve fewer purification stages, function under gentler conditions, and generate less waste. Calophyllum inophyllum has been selected for this

New sequence type of an Enterobacter cloacae complex strain with the potential to become a high-risk clone.

OBJECTIVES: Enterobacter cloacae complex (ECC) has awakened interest recently because of its increasing resistance to carbapenems codified by several genes all over the globe. Even though there are some sequence types (STs) which represent high-risk clones, there is substantial clonal diversity in the ECC. This work aimed to perform whole-genome sequencing (WGS), genomic analysis, and phylogenetic studies of a Klebsiella pneumoniae carbapenemase (KPC) -producing multidrug-resistant (MDR) ECC isolate from Argentina. METHODS: We analysed the genome of an MDR KPC-producing ECC strain isolated fr
