[Utility of sigmoid intramucosal pH in the early diagnosis of ischemic colitis after aorta surgery].

A 62-year-old man with grade III ischemia of the legs and occlusion of an aortofemoral shunt underwent axillofemoral bypass and bilateral profundoplasty. During surgery, an aneurysm in the aortic origin of the right common iliac artery ruptured, requiring ligation of the inferior vena cava, the iliac veins and the right common iliac artery. Upon transfer of the patient to the recovery unit, the sigmoid intramucosal pH (pHi) was 6.83 (arterial pH 7.35), the regional CO2 pressure (PrCO2) was 100 mmHg (arterial PCO2 35.2 mmHg), and the lactic acid concentration was 3.6 mmol/L. Ischemic colitis wa

Ischemic colitis and acquired resistance to activated protein C in a woman using oral contraceptives.

We present the case history of a 22-yr-old woman diagnosed with ischemic colitis associated with the use of oral contraceptives (OC). At the time of her presenting symptoms activated protein C (APC) resistance, a risk factor for thrombosis, was demonstrated. There was no laboratory evidence of inherited thrombophilia; that is, antithrombin III, protein C and protein S levels were normal and the factor V Leiden mutation was not present. The OC were discontinued and the patient's symptoms improved. Subsequent evaluation revealed that the activated protein C resistance had resolved. This case ill

Ischemic colitis arising in watershed areas of the colonic blood supply: a report of two cases.

There are several weak points in the colonic blood supply, known as watershed areas, which result from incomplete anastomoses of the marginal arteries. These watershed areas are more vulnerable to ischemic injury than other parts of the colon. We report herein the cases of two patients who developed ischemic colitis well localized in the cecum, and in the rectosigmoid region at Sudeck's point, respectively. This report and our review of the literature suggest that watershed areas, including the splenic flexure, or Griffith's point, Sudeck's point, and the ileocecal region, are high-risk region

Quantitative evaluation of experimental ischemic colitis correlated with the degree of artificial bowel obstruction in rats.

Experimental ischemic colitis proximal to artificial bowel obstruction was produced under poor regional circulatory conditions in rats. Only the vessels of the left hemi-colon were ligated to obtain poor circulation (ischemia). Two types of aluminum rings were used to obstruct the bowel at the most distal portion of the ischemic colon. One, with a 4 mm internal diameter, was used for the model of partial colon obstruction with ischemia (partial obstruction, n = 25) and the other, with a 2 mm internal diameter, was used for complete colon obstruction with ischemia (complete obstruction, n = 25)

Gut Trouble: A Rare Case of Amoxicillin-Clavulanate-Induced Ischemic Colitis in a Young Adult.

Ischemic colitis is a potentially life-threatening complication resulting from diminished blood flow to the colon. While the classic presentation is acute, crampy abdominal pain preceding hematochezia in older patients with cardiovascular comorbidities, it can present atypically in different demographics. The diagnosis can be established through appropriate clinical suspicion, along with endoscopic and histological confirmation. We report a case of amoxicillin-clavulanate-induced ischemic colitis in a young male without comorbidities. Following cessation of the medication, he demonstrated full

Low Diastolic Blood Pressure and Risk of Ischemic Colitis in the Women's Health Initiative Cohort.

INTRODUCTION: To identify risk and protective factors in older women for incidence of ischemic colitis (IC) and 30-day mortality. METHODS: We conducted a prospective study of 100,825 women in the Women's Health Initiative, with an average of 13.1-year follow-up and extensive phenotypic and outcomes data in diverse race and ethnic groups. Lasso Cox regression selected variables related to incidence of IC from multiple domains (demographic, comorbidities, risk factors, biomarkers, psychosocial factors, and dietary factors). Cox regressions modelled the selected variables to obtain adjusted haza

A Rare Case of Hughes-Stovin Syndrome Presenting with Ischemic Colitis Caused by an Arteriovenous Malformation.

Hughes-Stovin syndrome (HSS) is an exceedingly rare disease characterized by thrombotic phlebitis and pulmonary or bronchial artery aneurysms. A 68-year-old man presented with ischemic colitis, and angiography revealed occlusion of the inferior mesenteric vein and arteriovenous malformation in the inferior mesenteric artery region. Medical treatment was unsuccessful, and a left hemicolectomy was therefore performed following coil embolization. Subsequently, a pulmonary artery aneurysm was incidentally detected, and coil embolization was performed. However, the patient developed deep vein throm

[A Case of Ischemic Colitis on the Anal Side of the Anastomosis Following Surgery for Descending Colon Cancer].

A 76-year-old man underwent laparoscopic partial resection of the descending colon for descending colon cancer. The common trunk of the left colic artery and the first branch of the sigmoid artery was ligated, while the inferior mesenteric artery(IMA)was preserved and the inferior mesenteric vein(IMV)was divided. The pathological diagnosis was pT3pN0M0, pStage ⅡA, and no adjuvant chemotherapy was administered. At 6 months postoperatively, CT showed inflammatory changes extending from the anastomosis to the rectum, but observation was continued as the patient was asymptomatic. At 8 months posto

Risk factors and surgical outcomes in pediatric patients with congenital heart disease and ischemic colitis.

INTRODUCTION: This study investigates risk factors and surgical outcomes in pediatric patients with congenital heart defects (CHD) who develop ischemic colitis (IC). Previous research indicates a higher IC risk in very low birth weight neonates with CHD. METHODS: A retrospective analysis compared an IC-CHD group to a CHD-only group. Key variables included gestational age, birth weight, multiple pregnancies, prematurity, APGAR scores, cardiac and surgical characteristics, Aristotle-Score, and mortality rates. Surgical outcomes such as bowel resection and stoma procedures were also evaluated.

The correlation between heart failure and the risk of ischemic colitis: a systematic review and meta-analysis.

BACKGROUND: Ischemic colitis is a relatively common gastrointestinal disease caused by hypoperfusion of the colon. Recently, studies have suggested an association between heart failure (HF) and ischemic colitis, even though the magnitude of the reported association varied considerably across the studies. This systematic review and meta-analysis were performed to comprehensively explore whether patients with HF are at a higher risk of ischemic colitis compared with individuals without HF by combining the results of all available observational studies. METHODS: Systematic literature review was

Ischemic Colitis Associated with Rhabdomyolysis and Heat Stroke after an Intense Exercise in Young Adult.

Ischemic colitis primarily affects the elderly with underlying disease, but it rarely occurs in young adults with risk factors, such as coagulopathy or vascular disorder. Moreover, it is extremely rare in the very young without risk factors. This paper presents a patient with ischemic colitis associated with heat stroke and rhabdomyolysis after intense exercise under high-temperature conditions. A 20-year-old man presented with mental deterioration after a vigorous soccer game for more than 30 minutes in sweltering weather. He also presented with hematochezia with abdominal pain. Predisposing Factors of Ischemic Colitis: Data from 14 Years of Experience in a Single Center.

BACKGROUND AND AIMS: While several case reports on ischemic colitis (IC) suggest the presence of predisposing causative factors, a few studies have investigated the predisposing factors in IC. This study aimed to identify the characteristics of patients with IC, particularly focusing on the predisposing factors. METHODS: We conducted a single-center, retrospective analysis of 159 patients with IC. Clinical characteristics, laboratory data, endoscopic findings, and medical records were reviewed. Data were compared between groups of patients defined according to the predisposing factors. The pr

Ischemic colitis caused by strict dieting in an 18-year-old female: report of a case.

Ischemic colitis is typically limited to elderly patients who have concomitant disorders such as cardiovascular disease and chronic renal failure, but rarely affects a young person. The patient was an 18-year-old Japanese female who started dieting to obtain a slim figure three months before admission and presented with a two-month history of constipation and a sudden onset of lower abdominal pain and bloody diarrhea. She underwent colonoscopy, which revealed edema, hemorrhages, and several longitudinal shallow ulcers in the descending colon. Stool and blood culture results were negative for p

[Total diverticulosis of the large intestine, complicated by ischemic colitis with development of multiple inflammatory polyps].

One observation of total colon diverticulosis in a male of 62 is described. Clinical signs (abdominal pains, blood in faeces, vomiting) persisted for 7 years and was aggravated by ileus. Instrumental examination revealed multiple colon diverticula, stenosing villous tumour of sigmoid colon. Besides pseudodiverticula, chronic colitis with multiple inflammatory polyps stenosing the lumen of the sigmoid colon and simulating a villous tumour was found. The conclusion is made of the ischemic nature of the colitis complicating the course of diverticulosis.

Ischemic colitis as a cause of massive lower gastrointestinal bleeding and peritonitis. Report of five cases.

Vascular disorders of the colorectum are considered rare in Taiwan, however, recently the authors encountered five cases of ischemic colitis, all in men over 50 years of age. Cases one and five involved stricture, case two was secondary to obstructive colon cancer, and case three was of the gangrenous type presenting with peritonitis due to colonic perforation. Case four was of the transient type with acute massive lower gastrointestinal bleeding secondary to traumatic shock. All five patients were treated successfully by resection.

Vaso-Occlusive Thrombotic Ischemic Colitis Presenting With Large-Volume Hematochezia in Sickle Cell Beta+-Thalassemia.

We present the case of a 60-year-old African American man with sickle cell beta plus thalassemia who developed large-volume hematochezia during hospitalization for a vaso-occlusive crisis. Sickle cell disease is characterized by chronic hemolysis and vaso-occlusion that can affect the gastrointestinal tract, although clinically significant colonic ischemia remains rare. The patient had a history of diverticulosis and hemorrhoids but presented with acute lower gastrointestinal bleeding after dehydration and nonsteroidal anti-inflammatory drug exposure, both of which can impair mucosal perfusion

Deep learning analysis of capillary refill dynamics in ischemic colitis: differentiating reversible vs. gangrenous mucosa.

Ischemic colitis remains a diagnostic challenge, as endoscopic grading of mucosal ischemia is subjective and insensitive for transmural infarction. While the majority of non-gangrenous cases are resolved conservatively, gangrenous mucosa necessitates immediate resection. The current reliance on visual signals, such as color and bleeding, results in misclassification and variability. We propose a novel deep learning approach to quantify micro-capillary refill dynamics from colonoscopy video. The technology produces objective perfusion measures and real-time viability classification by examining
