Editor-in-Chief Note:Dr. Djalali is a well-known member of the international disaster medicine community. He is a man always with a smile and sincere in seeking the best for all mankind. His now extremely prolonged imprisonment without due process to allow him to defend himself represents one of the most profound inhumane acts on the globe. His torture and starvation are beyond comprehension for the international health and medicine community as well as all men and women. The pictures that accompany this editorial are published with proper permissions and have been authenticated as untouched f

Comparative studies of how living circumstances influence medication adherence in ≥65 year olds.

BACKGROUND: Resources to help the older aged (≥65 year olds) manage their medicines should probably target those in greatest need. The older-aged have many different types of living circumstances. There are different locations (urban, rural), different types of housing (in the community or in retirement villages), different living arrangements (living alone or with others), and different socioeconomic status (SES) circumstances. However, there has been limited attention to whether these living circumstances affect adherence to medicines in the ≥65 year olds. AIM OF THE REVIEW: The aim was to

Management of severe traumatic brain injury in pediatric patients: an evidence-based approach.

BACKGROUND: Traumatic brain injury (TBI) is a major cause of death and disability worldwide. The decision-making process in the management of severe TBI must be based on the best available evidence to minimize the occurrence of secondary brain injuries. However, healthcare approaches to managing TBI patients exhibit considerable variation. METHODS: Over an 18-month period, a multidisciplinary panel consisting of medical doctors, physiotherapists, nutritional therapists, and nurses performed a comprehensive review on various subtopics concerning TBI. The panel identified primary questions to b

A Sleep Medicine Curriculum for Pulmonary and Pulmonary/Critical Care Fellowship Programs: A Multisociety Expert Panel Report.

BACKGROUND: Pulmonary medicine specialists find themselves responsible for the diagnosis and management of patients with sleep disorders. Despite the increasing prevalence of many of these conditions, many sleep medicine fellowship training slots go unfilled, leading to a growing gap between the volume of patients seeking care for sleep abnormalities and the number of physicians formally trained to manage them. To address this need, we convened a multisociety panel to develop a list of curricular recommendations related to sleep medicine for pulmonary fellowship training programs. METHODS: Su

Research on Tropical Medicine and Germ Theory in Colonial Hong Kong: Focusing on Malaria and Plague Prevention.

At the end of the nineteenth century, Hong Kong was in the midst of a malaria and plague epidemic which caused a fierce dispute within the medical community over disease theories and quarantine practices. However, the Hong Kong colonial authorities and medical community did not immediately accept the theory of etiology based on germ theory. Although germ theory was becoming scientifically established through research on plague and malaria in the 1890s, the Hong Kong colonial authorities and medical community did not immediately accept it. Patrick Manson (1844-1922) began studying tropical medi

Street Stories: Fostering Humanism in Family Medicine.

INTRODUCTION: Physicians frequently encounter social determinants of health such as homelessness, poverty, and addiction, that complicate clinical care and may contribute to emotional fatigue and burnout. While humanism in medicine has been associated with improved patient outcomes and physician well-being, it is often difficult to sustain amid systemic challenges. Medical education lacks sufficient opportunities for learners to engage meaningfully with marginalized populations in ways that reinforce humanistic values and advocacy. This study aimed to enhance humanistic engagement among reside

Translational studies of exosomes in sports medicine - a mini-review.

This review in sports medicine focuses on the critical role of exosomes in managing chronic conditions and enhancing athletic performance. Exosomes, small vesicles produced by various cells, are essential for cellular communication and transporting molecules like proteins and nucleic acids. Originating from the endoplasmic reticulum, they play a vital role in modulating inflammation and tissue repair. Their significance in sports medicine is increasingly recognized, particularly in healing athletic injuries, improving articular cartilage lesions, and osteoarthritic conditions by modulating cel

PrEdiction of Risk and Communication of outcomE followIng major lower limb amputation: a collaboratiVE study (PERCEIVE)-protocol for the PERCEIVE qualitative study.

INTRODUCTION: Deciding whether to proceed with a major lower limb amputation is life-changing and complex, and it is crucial that the right decision is made at the right time. However, medical specialists are known to poorly predict risk when assessing patients for major surgery, and there is little guidance and research regarding decisions about amputation. The process of shared decision-making between doctors and patients during surgical consultations is also little understood. Therefore, the aim of this study is to analyse in depth the communication, consent, risk prediction and decision-ma

AI-Augmented Prevention Science Needs Community-Engaged Prevention Science: a Framework for Greater Accountability.

With the ability to approximate human decision-making and to parse through large and complex data, artificial intelligence (AI) technologies such as machine learning algorithms stand to play important roles in diagnosing, treating, and preventing society's most pressing social, health, and academic challenges. However, some AI applications for these ends have resulted in biased and discriminatory outcomes, demonstrating poor accountability to the communities these technologies impact and serve. The foundational premise of this paper is that AI-augmented prevention science needs community engag

What helps patients access web-based services in primary care? Free-text analysis of patient responses to the Di-Facto questionnaire.

BACKGROUND: The National Health Service (NHS) and general practice are increasingly adopting digital services. These services can impact both positively and negatively upon patient experiences, and access to digital services is not equal amongst all groups. Within a wider project examining digital facilitation (the Di-Facto study) our team conducted a patient survey amongst English primary care practices aiming to investigate patient views of what supports uptake and use of web-based services. This paper reports on the analysis of the free-text responses from the patient survey. METHODS: The

The need for an intersectionality framework in precision medicine research.

Precision medicine research has seen growing efforts to increase participation of communities that have been historically underrepresented in biomedical research. Marginalized racial and ethnic communities have received particular attention, toward the goal of improving the generalizability of scientific knowledge and promoting health equity. Against this backdrop, research has highlighted three key issues that could impede the promise of precision medicine research: issues surrounding (dis)trust and representation, challenges in translational efforts to improve health outcomes, and the need f
