Comparison of sedation scores and propofol induction doses in dogs after intramuscular administration of dexmedetomidine alone or in combination with methadone, midazolam, or methadone plus midazolam.

The objectives of this study were to determine: (1) the sedative effects of dexmedetomidine in combination with methadone, midazolam, or both, and (2) the propofol dose required to achieve endotracheal intubation in healthy dogs. Seven healthy Beagle dogs were included in a prospective experimental, crossover, randomised and masked design. All dogs received four treatments IM, with at least 1 week between sessions, as follows: dexmedetomidine 5 µg/kg (D) alone, or combined with methadone 0.3 mg/kg (DMe), midazolam 0.3 mg/kg (DMi), or both (DMeMi). The degree of sedation was evaluated using a n

The Safety and Complication Rate of Deep Plane Facelift Under Local Anesthesia.

BACKGROUND: The deep plane facelift is traditionally performed under general anesthesia or intravenous (IV) sedation. Because of demand increases among older patients and those with comorbidities, interest has grown in performing comprehensive facelift surgery under local anesthesia; however, safety data remain limited. OBJECTIVES: The objective of this study is to evaluate the complication rate associated with deep plane facelift surgery performed under local anesthesia and to compare these outcomes with established complication rates reported in the literature. METHODS: A retrospective rev

The effect of melatonin on sedation of children undergoing magnetic resonance imaging.

BACKGROUND: Melatonin may induce a natural sleepiness and improve predictability of sedation drugs. We have investigated its clinical value in children sedated for magnetic resonance imaging. METHODS: In a stratified randomized double-blind study, 98 children received either melatonin or placebo 10 min before they were sedated with a standard oral regimen. Children >5 and <15 kg received chloral hydrate and those >or=15 and <40 kg had a combination of temazepam with droperidol (T&D). The doses of melatonin were 3 and 6 mg, respectively. One observer recorded the time taken to reach criteria f

Relationship between coughing and bispectral index-guided sedation during pulsed field ablation for atrial fibrillation and its impact on mid-term outcomes.

BACKGROUND: Coughing can occur during pulsed field ablation (PFA) for pulmonary vein (PV) isolation (PVI) of atrial fibrillation (AF) under deep sedation, and the bispectral index (BIS) is commonly used to monitor sedation depth. OBJECTIVE: We aimed to investigate the relationship between coughing and BIS-guided sedation depth during PFA-based PVI and to evaluate the impact of coughing on clinical outcomes after AF catheter ablation. METHODS: We retrospectively studied 198 consecutive patients undergoing an initial AF catheter ablation procedure using a 31-mm pentaspline PFA catheter under d

Should we include monitors to improve assessment of awareness and pain in unconscious palliatively sedated patients? A case report.

BACKGROUND: Awareness and pain during palliative sedation is typically assessed by observational scales, but the use of such scales has been put into question. CASE PRESENTATION: A woman in her mid-80s was admitted to a palliative care unit, presenting with chronic lymphatic leukemia, depression, and a cerebrovascular accident, with right-sided hemiplegia and aphasia. The patient was unable to eat and was suffering from nausea and vomiting. Before admission, the patient had expressed her desire to discontinue treatment on several occasions. CASE MANAGEMENT: The decision was made to initiate

Influence of Propofol-Induced Sedation on White Matter Functional Connectivity.

BACKGROUND: Propofol is a commonly used anesthetic, and its impact on brain function has been a significant focus of neuroscience research. However, previous studies have primarily focused on the effects of propofol on gray matter function. White matter in the brain is a pathway for transmitting information between different brain regions. Recently, blood oxygen level-dependent signals in white matter have been shown to have physiological significance. However, the effects of propofol on white matter function remain unclear. The purpose of this study is to investigate changes in white matter f

Sedation and shivering management after cardiac arrest.

Management of sedation and shivering during targeted temperature management (TTM) after cardiac arrest is limited by a dearth of high-quality evidence to guide clinicians. Data from general intensive care unit (ICU) populations can likely be extrapolated to post-cardiac arrest patients, but clinicians should be mindful of key differences that exist between these populations. Most importantly, the goals of sedation after cardiac arrest are distinct from other ICU patients and may also involve suppression of shivering during TTM. Drug metabolism and clearance are altered considerably during TTM

Sedation and Analgesia for Cardiac Catheterisation and Coronary Intervention.

BACKGROUND: While cardiac catheterisation is typically well tolerated, discomfort and anxiety are commonplace. Sedation using anxiolytic and analgesic medications has the potential to ameliorate such symptoms, however, is variably employed, with lack of standardised regimens and limited evidence. METHODS: We performed a review of the role of sedation for cardiac catheterisation, including current practices and summarising available evidence relevant to diagnostic and interventional coronary procedures in the cardiac catheterisation laboratory. RESULTS: Use of sedation and the medication regi

Post-injection delirium/sedation syndrome in patients treated with olanzapine pamoate: mechanism, incidence, and management.

Second-generation antipsychotics (SGAs) are a mainstay in the treatment of patients with schizophrenia. However, continuity in intake of the prescribed medication has been one of the greatest challenges in these patients. One option to improve medication adherence is to prescribe depot or long-acting injectable formulations (LAIs) of antipsychotics. Following risperidone, several other SGAs have been introduced as LAIs. Olanzapine pamoate, paliperidone palmitate, and aripiprazole are the new-generation LAIs, which are available for 2- to 4-week intervals of application in many countries. The l

Choice of Anesthesia in Microelectrode Recording-guided Deep Brain Stimulation Surgery for Parkinson's Disease (CHAMPION): A Noninferiority Randomized Controlled Trial.

BACKGROUND: Deep brain stimulation for Parkinson's disease is often performed under conscious sedation or general anesthesia. However, anesthetic agents may influence intraoperative microelectrode recording, and the optimal anesthesia method for microelectrode recording remains unclear. This study compared general anesthesia and conscious sedation in preserving microelectrode recording signal intensity during deep brain stimulation. METHODS: In this prospective, noninferiority randomized controlled trial, patients with Parkinson's disease (United Kingdom Brain Bank criteria) undergoing electi

Risk factors leading to failed procedural sedation in children outside the operating room.

OBJECTIVES: Deep sedation enables effective performance of imaging or procedures in children, but failed sedation still occurs. We desired to determine the factors that were associated with failed sedation in children receiving deep sedation by a dedicated nonanesthesia sedation service and hypothesized that the presence of an upper respiratory infection (URI) and/or other risk factors would increase the probability of failing sedation. METHODS: Patient sedation records from January 2007 to December 2011 were reviewed to identify 83 failed sedations. A convenience sample of 523 patients with

The effect of propofol sedation on the intracranial pressure of patients with an intracranial space-occupying lesion.

The fear of producing CO(2) retention and a secondary increase of intracranial pressure (ICP) sometimes precludes the use of sedation for the spontaneously breathing patient in the presence of an intracranial space-occupying lesion. In this study we assessed the effect of moderately deep propofol sedation on the ICP of patients undergoing stereotactic brain tumor biopsy under regional anesthesia. Thirty patients were randomized into 2 groups to receive propofol titrated to a level of 2 on the Observer's Assessment of Alertness/Sedation Scale or no sedation. ICP was measured via the biopsy need

Pulsed-field versus cryoballoon ablation for atrial fibrillation-Impact of energy source on sedation and analgesia requirement.

INTRODUCTION: Pulsed field ablation (PFA) represents a novel, nonthermal energy modality that can be applied for single-shot pulmonary vein isolation (PVI) in atrial fibrillation (AF). Comparative data with regard to deep sedation to established single-shot modalities such as cryoballoon (CB) ablation are scarce. The aim of this study was to compare a deep sedation protocol in patients receiving PVI with either PFA or CB. METHODS: Prospective, consecutive AF patients undergoing PVI with a pentaspline PFA catheter were compared to a retrospective CB-PVI cohort of the same timeframe. Study endp

Sedation strategies in children with pediatric acute respiratory distress syndrome (PARDS).

In this review, we discuss the changing landscape of sedation in mechanically ventilated children with pediatric acute respiratory distress syndrome (PARDS). While previous approaches advocated for early and deep sedation with benzodiazepines, emerging literature has highlighted the benefits of light sedation and use of non-benzodiazepine sedating agents, such as dexmedetomidine. Recent studies have emphasized the importance of monitoring multiple factors including, but not limited to, sedation depth, analgesia efficacy, opiate withdrawal, and development of delirium. Through this approach, we

An Oxycodone-Based Conscious Analgesia Strategy with Supplemental Low-Dose Propofol Improves Safety in Elderly Obese Patients Undergoing Gastroscopy: A Prospective Randomized Controlled Trial.

BACKGROUND: During diagnostic gastroscopy, intolerance is usually manifested as retching, coughing, and involuntary movement. Conscious analgesia relieves discomfort while patients continue to breathe spontaneously and remain partly responsive. Oxycodone, a semi-synthetic opioid, may reduce the need for deep propofol sedation. We compared an oxycodone-based conscious analgesia strategy with supplemental low-dose propofol with propofol-based deep sedation in elderly obese patients. PATIENTS AND METHODS: In this prospective, single-center, open-label randomized controlled trial, 128 patients ag

Determination of the appropriate propofol infusion rate for outpatient upper gastrointestinal endoscopy-a randomized prospective study.

BACKGROUND: Pain and discomfort related to endoscopy sessions can be alleviated by sedation, which minimizes anxiety and allows safe examination. For outpatient endoscopy, reliable short-term sedation without secondary effects is required. This study aimed to assess the effects of intravenous propofol rates on sedation in outpatients undergoing upper gastrointestinal endoscopy. METHODS: This randomized prospective study evaluated 300 outpatients submitted to upper gastrointestinal endoscopy. Patients received propofol at 500, 1000 or 2000 ml/h. The primary outcome assessed was hypoxemia incid

Comparison of sedative effects of medetomidine-midazolam, acepromazine-butorphanol and midazolam-butorphanol in dogs.

Sedative effects of medetomidine at 20 micrograms/kg and midazolam at 0.3 mg/kg (MM), acepromazine at 0.05 mg/kg and butorphanol at 0.2 mg/kg (AB), and midazolam at 0.1 mg/kg and butorphanol at 0.2 mg/kg (MB) were compared in dogs. All dogs given MM were laterally recumbent within 11 min of the administration of the drugs, and this combination also induced deep sedation accompanied by analgesia and muscle relaxation, and strongly depressed arousal reactions to external stimuli. However, undesirable effects such as bradycardia and decreased respiration were also observed in dogs given MM. Six o
