Nutritional interventions for reducing gastrointestinal toxicity in adults undergoing radical pelvic radiotherapy.

BACKGROUND: Across the developed world, an estimated 150,000 to 300,000 people are treated annually with pelvic radiotherapy and 80% will develop gastrointestinal (GI) symptoms during treatment. Acute GI symptoms are associated with a greater risk of chronic, often debilitating, GI symptoms. Up to one-third of patients are malnourished before pelvic radiotherapy and up to four-fifths of patients lose weight during treatment. Malnutrition is linked to a higher risk of GI toxicity, which can lead to breaks in radiotherapy and early cessation of chemotherapy, thus compromising the efficacy of the

Immunoenhanced enteral nutrition formulas in head and neck cancer surgery: a systematic review.

INTRODUCTION: Significant malnutrition exists in a high percentage of patients with head and neck cancer. Malnutrition is associated with defects in immune function that may impair the host response to malignancy. Malnutrition and immunosupression make patients highly susceptible to postoperative infections and complications. OBJECTIVES: Some studies of patients receiving immuno-nutrition in the perioperative period in head and neck cancer have shown beneficial effects on clinical outcome and inmune status. The authors carried out a systematic review of randomised control trials to determine

Cholesterol-lowering activity of lactic acid bacteria probiotic strains in vivo.

Cholesterol-lowering activity of probiotic strains of lactic acid bacteria genera Lactobacillus and Bifidobacterium in the in vivo experiments on the model of experimental hypercholesterolemia in mice was studied. It is established that the prophylactic scheme of introduction of probiotic cultures is more effective than therapeutic one for the manifestation of cholesteraze activity of probiotic cultures. The most effective were the cultures: L. acidophilus and B. bifidum, as well as the composition B. bifidum + B. longum. Cholesterol-lowering activity of the studied strains and their compositi

[Intervention of coarse cereals on lipid metabolism in rats].

OBJECTIVE: To observe the effect of coarse cereals on improving the disorder of lipid metabolism and the expression of PPARgamma mRNA in white adipose tissue in rats to investigate the mechanism of coarse cereals on lipid metabolism disorder. METHODS: Forty four SPF rats were randomly divided into 4 groups: the negative control group was fed with normal diet and 3 experimental groups were fed with high-fat modeling diet for 6 weeks for model building. The 3 experimental groups, the coarse cereals group,rice-flour group and the hyperlipemia model group, were then fed with coarse cereals high-f

Clinical evaluation of food allergy and food intolerance dietary treatment in children.

Dietary treatment with Humana SL (33) or Prosobee (42) was administered to 75 children at the age from 1 month to 3 years with cow's milk allergy (36), coeliac disease (9), secondary enteropathy (30). The treatment time ranged from a few days (in case of intolerance symptoms) to 12 months. In 13 children intolerance symptoms or unwillingness to take food was observed: in 4 (12%) after the application of Humana SL and in 9 (21%) after Prosobee. Lack of improvement after Prosobee treatment was observed in 4 (10%) children, and in 4 (12%) children with Humana SL. The partial or total relief of sy

Nutrition and growth in patients with chronic liver disease.

Protein energy malnutrition leading to growth failure is an inevitable consequence of chronic liver disease in childhood. Although the precise pathophysiology is not understood considerable progress has been made in understanding the mechanisms of fat malabsorption and protein turnover in liver disease. There are many difficulties with the correct assessment of nutritional parameters in children with liver disease related to their abnormal body composition and energy expenditure and care needs to be taken with the interpretation of results. The effects of malnutrition secondary to chronic live

Chylothorax: report of a case complicating orthotopic heart transplantation.

We describe a case of a chylothorax that occurred in a 62-year-old man after orthotopic heart transplantation. Injury to collateral lymphatics in the anterior mediastinal or thymic areas was the probable cause of the chylous fistula. Excessive chyle drainage causes loss of large amounts of fat, protein, electrolytes, and lymphocytes. These losses further compromise the nutritional and immunologic status of the transplant patient. Conservative therapy with chest tube drainage and a strict medium-chain triglyceride diet allowed the chylous fistula to spontaneously close without the need for surg

Insurance coverage of special foods needed in the treatment of phenylketonuria.

Optimal medical management of phenylketonuria (PKU) requires the use of special low-phenylalanine foods for many years. For women with PKU, elevated maternal blood levels of phenylalanine even at conception can lead to fetal damage. Despite this need, private health insurance, Medicaid, and other public health programs often exclude the cost of these foods from their benefits. The New York State Department of Health conducted a survey of metabolic disorders treatment centers to elucidate the problems PKU patients have obtaining and paying for the special foods essential to their care. Payment

Medical management of severe inflammatory disease of the rectum: nutritional aspects.

It is clear that the nutritional state of patients with inflammatory bowel disease is often impaired and can be improved by the provision of nutritional support. Improvement in nutritional status can be achieved as effectively with enteral as with parenteral nutrition. Nutritional support appears to have no primary therapeutic effect in patients with ulcerative colitis. With regard to nutritional support in Crohn's disease, parenteral nutrition should be restricted to use as supportive rather than primary therapy. Available information now seems to suggest that most of the benefits of parenter

Effect of dietary tin deficiency on growth and mineral status in rats.

To clarify the influence of dietary tin deficiency on growth and mineral status, the following two different synthetic diets were fed to male Wistar rats: group 1--a diet containing 1.99 micrograms tin/g; group 2--a diet containing 17 ng tin/g. The rats in group 2 showed poor growth, lowered response to sound, and alopecia, with decreased food efficiency compared with rats in group 1. The changes of mineral concentrations in tissues observed in group 2, compared with group 1, are summarized as follows: calcium concentration in lung increased; magnesium concentration in lung decreased; iron con

[Mechanism of promoting effects of riboflavin deficiency on carcinogenesis of nitrosamines--effects on rat liver glutathione content].

The effect of riboflavin deficiency and simultaneously, nitrosodimethylamine given by gastric intubation on the glutathione content of rat liver is reported. On different days of the experiments, glutathione content in the riboflavin deficient rat liver decreased to 55-61% of the controls. When nitrosodimethylamine was given by gastric intubation, glutathione content decreased markedly to 39-43% of the controls. The hepatic glutathione content of riboflavin deficient rats recovered to the level of the controls by supplying riboflavin. The alteration of rat hepatic glutathione content during ri

Nutritive utilization of Ca and Mg in Mg-deficient rats: a recovery study.

Recovery from Mg deficiency was studied in adult Wistar rats fed a semisynthetic diet containing 0.04% Mg in the form of magnesium carbonate. Daily administration of a diet containing recommended levels of calcium carbonate to Mg-deficient rats led to the recovery, within the first seven days of treatment, of normal values in most of the parameters studied: gain in weight/day, and Mg retention and content in the Longissimus dorsi muscle and femur. Mg levels in whole blood, however, did not fully recovery until the second week of treatment. Net Ca absorption (ADC) and balance were significantly

Noninvasive outpatient nutritional therapy in inflammatory bowel disease.

Twelve courses of outpatient nutritional therapy using a specific defined formula, Precision High Nitrogen Diet, were given orally to nine malnourished patients with active inflammatory bowel disease. The regimen was continued for a 2-month period on an outpatient basis. Patients had significant increases in weight, height, and nutritional status as evaluated by serum protein and albumin levels and anthropometric measurements of triceps and subscapular skinfold thickness. Patients with no alteration of pharmacologic therapy did not differ significantly from those with increases in pharmacologi

[Biomedical evaluation of new diabetic baked goods].

Medical and biologic assessment was made of new baked goods, the Salomat Uzbek diabetic press cakes, made of Category II wheat flour with addition of rice flour and whey. The diabetic press cakes contain increased levels of protein, some indispensable amino acids (lysine, leucine, valine), cellulose (approximately 3-fold more), and decreased carbohydrate level (1.4-fold), as compared to the traditional press cakes. Clinical studies have confirmed good consumption and therapeutic characteristics of the cakes.

Dietary management of Type I glycogen storage disease.

The most commonly recognized type of glycogen storage disease (von Gierke's disease) results from deficient glucose-6-phosphatase activity. This enzyme is the last step in the release of free glucose from the liver into the circulation. Thus, the most prominent and life-threatening complication in the illness is severe and often prolonged hypoglycemia, which occurs after the dietary glucose is normally removed from the circulation. With an optimal dietary intake spaced at 2 1/2- to 3 1/2-hour intervals, the blood glucose can be maintained in the normal range during the daytime, but hypoglycemi

Peripheral vein complications in patients receiving amino acid/dextrose solutions.

The incidence and severity of peripheral vein phlebitis was evaluated in patients receiving infusions of amino acids alone (1.4 g/kg/day) (group I), amino acids (1.4 g/kg/day) plus 5% dextrose (group II) and 5% dextrose alone (Group III). Ninety-three patients with carcinoma of the bladder were randomized to receive one of three peripheral parenteral nutrition solutions which were infused predominantly via scalp-vein needles. Peripheral parenteral nutrition solutions contained sodium and potassium chloride as necessary to maintain normal serum electrolyte levels. The osmolality of each solutio

Pesticide content of infant formulae and weaning foods available in New Zealand.

A survey of the pesticide content of 25 commercially available infant formulae and 30 weaning foods available in New Zealand was undertaken in 1996. It included a representative mixture of imported and New Zealand manufactured infant foods. Three different pesticide screening techniques were used: a high-sensitivity organochlorine screen was carried out on all infant formulae, while a multiresidue screen (organochlorine and organophosphorus pesticides, synthetic pyrethroids, carbamate pesticides, fungicides and herbicides), and a specific screen for dithiocarbamate fungicides were both carried

The effects of FG 7142 on sensory-specific satiety in rats.

Three experiments used intake, clusters, and licks per cluster to study the effects of the GABA inverse agonist, FG 7142, on sensory-specific satiety in rats. In Experiment 1, rats were offered one of two palatable solutions and 30min later tested with the same or the other solution. Rats that received the same solution consumed less, exhibited fewer clusters, fewer licks per cluster and a more rapid decline in rate of licking than rats tested with the other solution. In Experiments 2 and 3, rats tested with the same solution under FG 7142 showed fewer clusters and fewer licks per cluster than
