Vacuum-assisted closure for complex cranial wounds involving the loss of dura mater.

The aim in this study was to describe the safety and efficacy of vacuum-assisted closure (VAC) in patients with complex cranial wounds with extensive scalp, bone, and dural defects who were not candidates for immediate free tissue transfer. Five patients (4 men and 1 woman) ages 24-73 years with complex cranial wounds were treated with VAC at Wake Forest Baptist Medical Center. Etiologies included trauma, squamous cell carcinoma, and malignant meningioma. Cutaneous wound defects measured as large as 15 cm in diameter. Four of the 5 patients had open skull defects with concomitant dural defects

Emergence of feline sporotrichosis with zoonotic risk in a municipality of the Eastern Amazon, Brazil.

Sporotrichosis is a zoonotic implantation mycosis caused by dimorphic fungi of the genus Sporothrix. Transmission occurs through traumatic inoculation of the fungus and may involve different routes, including sapronotic transmission from contaminated soil, plants, and organic matter, zoonotic transmission mainly associated with infected felines, and, less frequently, contaminated fomites. The disease remained neglected for many years; however, several regions have recently reported outbreaks that are difficult to control. Thus, this study aimed to describe the emergence of sporotrichosis in th

Gut microbiome signatures predict 5-ASA efficacy in ulcerative colitis.

Ulcerative colitis (UC) prevalence is rising globally, yet fewer than 50% of patients achieve mucosal healing (MH) with first-line 5-aminosalicylic acid (5-ASA) therapy. We aimed to identify microbial signatures that could predict the treatment efficacy of 5-ASA. Active UC patients on standardized 5-ASA treatment were prospectively enrolled. Shotgun metagenomic sequencing was performed to identify the taxonomic and functional profiles before and after treatment. Six species were enriched in the effective group and 3 species in the ineffective group at baseline. Faecalibacterium prausnitzii, Bl

Foundation model based prediction of lung cancer survival using temporal changes in dual time point CT scans.

Lung cancer remains a significant cause of mortality, with non-small cell lung cancer (NSCLC) representing most cases. Currently, clinical data based models fall short in predicting survival while more advanced deep learning based image models require vast amounts of data and are often limited to predictions based on single time points. This study uses dual time point CT scans and features derived from a foundation model to predict survival. A dataset containing 102 NSCLC patients treated with radiation therapy was used, with each patient having both pre-treatment and post-treatment CT scans.

Congenital supravalvular aortic stenosis in a kitten.

A three-month-old, male intact Norwegian forest cat without any clinical signs was referred to the cardiology service of the author's teaching hospital for evaluation of a cardiac murmur. The murmur was systolic with an intensity of 4 out of 6 with the point of maximal intensity at the left heart base. Echocardiography revealed a moderate mitral valve regurgitation and a moderate dynamic left ventricular outflow tract obstruction both resulting from systolic anterior motion of the mitral valve (SAM). Moreover, left ventricular concentric hypertrophy was noted. Oral atenolol therapy was initiat

Chronic Morel-Lavallée Lesion Treated With Percutaneous Ultrasonic Debridement.

Morel-Lavallée lesion is defined as an internal degloving injury. A trauma involving shearing forces causes separation of the superficial fascia from the overlying subcutaneous tissue and skin, creating a potential space for hemolymphatic fluid to accumulate. This leads to the development of a self-resolving or potentially chronic collection. Classically, lesions arise in the lateral thigh. Imaging features depend on the duration of the lesion. Acutely, ultrasound and MR imaging will demonstrate a hematoma-like mass that is parallel to the underlying fascia. As the lesion evolves over time, ca

Predictive biomarkers validation of CD3+ cell apheresis yield in CAR-T manufacturing for diffuse large B-cell lymphoma: a machine learning approach.

Chimeric antigen receptor (CAR) T-cell therapy has shown significant success in treating diffuse large B-cell lymphoma (DLBCL). The initial step involves collecting autologous CD3+ lymphocytes through apheresis, in which obtaining an adequate CD3+ cell yield is essential for therapeutic efficacy. Despite prior research, the factors influencing CD3+ cell apheresis remain poorly understood. Traditional statistical analyses offer limited insights, but machine learning (ML) approaches enable precision modeling of clinical predictors owing to their advanced pattern-recognition capabilities. In this

The Impact of Statin Therapy on Postoperative Short- and Long-term Mortality and Morbidity After Non-Cardiac Surgery.

AIMS: Statins' anti-inflammatory and endothelial-stabilising effects suggest perioperative value, but trials of de novo initiation were discredited following research misconduct. Whether established preoperative statin therapy lowers postoperative mortality dose-dependently remains unclear. METHODS: We studied 56,362 patients aged ≥45 years undergoing non-cardiac surgery at two university hospitals (2016-2023). Preoperative statin exposure was assessed as status (any vs none), intensity (high vs low/intermediate), and regimen (high vs low/intermediate vs none). Primary outcomes were 30- and 3

Machine learning approaches to predict early cardiac immune-related adverse events in patients receiving immune checkpoint inhibitors.

PURPOSE: Immune checkpoint inhibitor (ICI)-induced cardiac immune-related adverse events (cardiac irAEs) are rare yet serious complications. Clinical assessment tools to identify at-risk patients would allow for more effective prevention strategies, thus improving clinical outcomes. We constructed various machine learning (ML) models to predict these events among patients receiving ICI therapy. METHODS: A cohort of patients receiving ICI therapy from 2010 to 2023 was identified from the TriNetX database. Cardiac irAEs were defined as the occurrence of relevant diagnosis codes within 90 days o

Intravenous iron therapy for heart failure and iron deficiency: An updated meta-analysis of randomized clinical trials.

Heart failure (HF) patients frequently exhibit iron deficiency, which is associated with a poor prognosis. Although various trials have been conducted, it is uncertain if intravenous (IV) iron replenishment improves clinical outcomes in HF patients with iron deficiency. A comprehensive literature search was conducted using PubMed/MEDLINE, Embase, and the Cochrane Library from inception till 15 September 2023 to retrieve randomized controlled trials (RCTs) that compared IV iron therapy with placebo or standard of care in patients with HF and iron deficiency. Clinical outcomes were assessed by g

Development and evaluation of a predictive algorithm for unsatisfactory response among patients with pulmonary arterial hypertension using health insurance claims data.

OBJECTIVE: This study aimed to develop and validate a predictive algorithm for unsatisfactory response to initial pulmonary arterial hypertension (PAH) therapy using health insurance claims. METHODS: Adult patients with PAH initiated on a first PAH therapy (index date) were identified from Optum's de-identified Clinformatics Data Mart Database (1/1/2010-12/31/2019). A random survival forest algorithm was developed using patient-month data and predicted the "survival function" (i.e. risk of not having unsatisfactory response) over time. For each patient-month observation, risk factors were ass

It's Green Gym, but not as we know it: examining the historical roots of voluntary sector provision of social prescribing focusing on gardening on prescription in the UK.

RESEARCH AIMS: This article investigates the historical roots of green prescriptions, which are a form of social prescribing. It seeks to establish when and why they began to be used in Britain to inform practice in the present. CONTEXT: Social prescriptions are a broad range of interventions for patients for whom standard medical treatments may not be effective. Many social prescription programmes are delivered by charities in partnership with the National Health Service. 'Green prescriptions' are a subset of these interventions, and the term now refers specifically to environmental and natu
