A clinical evaluation of alternative fixation techniques for medial malleolus fractures.

BACKGROUND: Medial malleolus fractures have traditionally been managed using partially threaded screws and/or Kirschner wire fixation. Using these conventional techniques, a non-union rate of as high as 20% has been reported. In addition too many patients complaining of prominent hardware as a source of pain post-fixation. This study was designed to assess the outcomes of medial malleolar fixation using a headless compression screw in terms of union rate, the need for hardware removal, and pain over the hardware site. SETTING: Saint Louis University and Mercy Medical Center, Level 1 Trauma Ce

A new method to predict refracture risk after locking compression plate removal of clavicle shaft.

BACKGROUND: The incidence rate of subsequent refracture after removal of the implant in mid-shaft clavicle fracture patients is relatively high. This can lead to additional medical costs and cause doctor-patient dispute. This study tries to introduce a new method to predict the refracture risk of the clavicle after hardware removal. METHODS: A retrospectively review of 244 patients who had undergone ORIF with LCP for mid-shaft clavicle fractures, and had hardware removal surgery after bony union from January 2013 to September 2022 at our hospital was performed. We evaluated basic demographic

Risk Factors for Removal of Calcaneus Screws: A Retrospective Study.

Although techniques using calcaneus screws have shown high union rates, posterior heel pain due to prominent hardware at the posterior and plantar aspect of the calcaneal tuberosity seems to be a significant complaint that often leads to hardware removal. The purpose of the present study was to identify the clinical and radiologic risk factors associated with calcaneus screw removal. A retrospective study of adult patients who required calcaneus screw fixation from January 2008 to December 2016 was conducted. We reviewed the medical records and radiographs to evaluate the risk factors for scre

Sural Nerve Entrapment in Postoperative Scar Tissue: A Case of Successful Management With Hydrodissection After Calcaneal Fracture Repair.

Sural nerve entrapment is a recognized yet frequently overlooked etiology of persistent lateral ankle and heel pain following lower extremity trauma or surgery. This report describes a case of refractory sural nerve entrapment within scar tissue that persisted after calcaneal fracture repair and subsequent hardware removal, which was successfully managed with ultrasound-guided hydrodissection. A 31-year-old male presented with refractory right heel pain following open reduction and internal fixation (ORIF) for a calcaneal fracture and subsequent elective hardware removal. Physical examination

Complications of Volar Locked Plating of Distal Radius Fractures: A Prospective Investigation of Modern Techniques.

Although volar locked plating (VLP) of distal radius fractures is common, complications remain a considerable concern for upper extremity specialists using modern techniques. Complications following VLP of DR fractures were recorded prospectively from January 2005 to January 2017. Fractures were characterized using the AO classification, and complications were described by severity. Severe complications required operative treatment and/or resulted in permanent impairment, moderate complications required nonoperative treatment, and mild complications resolved without intervention. The available

Ultrasound-Assisted Minimally Invasive Clavicle Plate Removal.

We present the feasibility of point-of-care ultrasound (POCUS) to aid minimally invasive surgery (MIS) for clavicular plate removal. We present a case of a 29-year-old male who sustained a displaced midshaft clavicle fracture after a bike accident. On day four, post-injury, he underwent an open reduction and internal fixation with an anatomic 3.5mm locking plate, complicated by numbness of the anterior chest extending to the medial arm. Four years post-operatively, he presented for hardware irritation, which was palpable on physical exam. After unsuccessful conservative treatments, the patient

IOFix™ with plate construct is similar to locking plate construct for Lapidus bunionectomy procedure outcomes.

BACKGROUND: The Lapidus bunionectomy has evolved with advanced fixation constructs since its first description. These new techniques have aimed to reduce complications such as non-union or hardware related symptoms. The IOFix™ system, an intramedullary device that has been designed to enhance stability and compression, may offer outcomes that are comparable to traditional medial locking plate with lag screw constructs. PURPOSE: To compare outcomes of the Lapidus bunionectomy using the IOFix™ and plate construct versus a traditional medial locking plate with a plantar lag screw. METHODS: This

Do Lisfranc ORIFs fail? Low long-term conversion to arthrodesis in the largest ORIF-specific cohort to date.

BACKGROUND: Lisfranc injuries are uncommon but potentially debilitating midfoot injuries associated with chronic pain, arthritis, and gait dysfunction. Open reduction and internal fixation (ORIF) is commonly performed for unstable injuries, yet concerns remain regarding fixation failure and subsequent need for arthrodesis. However, large-scale data defining mid- and long-term conversion rates to arthrodesis after primary ORIF are limited. METHODS: A retrospective cohort study was performed using the TriNetX Research Network to evaluate adults undergoing ORIF of Lisfranc injuries between 2005

Third Molar Eruption and Hardware Fungal Infection Following Treatment of Mandibular Angle Fracture.

Appropriate management of lower 3rd molars is crucial to successful treatment of mandibular angle fractures. A unique case of a patient who presented for initial management of a left mandibular angle fracture with an unerupted lower 3rd molar was discussed in this study. After treatment with open reduction and internal fixation of the fracture, the lower 3rd molar erupted and the patient subsequently developed fungal osteomyelitis and hardware failure. This was successfully treated with hardware removal, maxillomandibular fixation, and combined use of an intravenous antibiotic and oral antifun

Low-profile 2.7 mm VA-LCP plates vs. 3.5 mm LCP plates for midshaft clavicle osteosynthesis: a retrospective comparison.

PURPOSE: Implant related irritation and subsequent removal are one of the main disadvantages of clavicle plate osteosynthesis. 2.7 mm VA-LCP clavicle plates with a lower profile were introduced in recent years, with the aim to improve fit and reduce plate prominence. This study aims to investigate whether this leads to a lower implant removal rate compared to the older 3.5 mm LCP clavicle plates. METHODS: All patients that underwent single-plate osteosynthesis for midshaft clavicle fractures between 2019 and 2024 in a Swiss level I trauma-center were included. Patients were categorized into tw

A Persistent Problem: Mycobacterium abscessus Infection of an Orthopedic Device after Trauma.

Surgical site infection of orthopedic trauma patients is common and tends to be treated with optimal therapy after an organism is identified from intraoperative cultures. However, delayed presentation and multidrug resistance can lead to complications. In our case, we investigate a delayed presentation of Mycobacterium abscessus orthopedic hardware infection in a patient with a forearm fracture secondary to a motor vehicle accident. Six weeks after stabilization of the fracture, purulence from an iliac crest bone graft donor site was noted. Further investigation led to the diagnosis of M. absc

Peri-implant soft tissue colonization after osteosynthesis in asymptomatic patients: a prospective study.

Fracture-related infection (FRI) remains a serious complication in orthopedic trauma surgery, with microbiological confirmation based on positive cultures from deep tissue samples. However, the clinical interpretation of positive microbiological findings in asymptomatic patients remains unclear. This prospective study aimed to evaluate the incidence and characteristics of peri-implant soft tissue colonization in patients undergoing routine removal of osteosynthetic material without clinical or radiological signs of FRI. Indications for implant removal surgery included routine removal, prominen
