Quadrivalvular Heart Disease Complicated by Fatal Infective Endocarditis of Pulmonary Valve: A Case Report.

A middle-aged sick male with rheumatic heart disease and prior mitral and aortic prosthesis was admitted for tricuspid valve replacement. Preoperative assessment revealed a hitherto undiagnosed pulmonic condition. This unusual case is reported not only to highlight the quadrivalvular disease with possible coexisting rheumatic and congenital etiology but also the fulminant endocarditis of pulmonary valve.

Modified-Bentall Single-Patch Konno Enlargement Technique for Aortic Stenosis and Prosthesis-Patient Mismatch.

BACKGROUND: Aortic stenosis and prosthesis-patient mismatch complicate surgery for patients with small left ventricular outflow tracts. We present outcomes of a modified-Bentall single-patch Konno enlargement (BeSPoKE) technique for complex left ventricular outflow tract obstruction in adults. METHODS: The BeSPoKE technique facilitates a true outflow tract enlargement through an anterior septoventriculoplasty, using a single pericardial patch, followed by composite aortic valve-graft root replacement. Postoperative outflow tract geometry and valvular physiology were compared against preoperat

Assessment of trans-aortic pressure gradient using a coronary pressure wire in patients with mechanical aortic and mitral valve prostheses.

Accurate evaluation of trans-aortic valvular pressure gradients is challenging in cases where dual mechanical aortic and mitral valve prostheses are present. Non-invasive Doppler echocardiographic imaging has its limitations due to multiple geometric assumptions. Invasive measurement of trans-valvular gradients with cardiac catheterization can provide further information in patients with two mechanical valves, where simultaneous pressure measurements in the left ventricle and ascending aorta must be obtained. Obtaining access to the left ventricle via the mitral valve after a trans-septal punc

[Traumatic aortic regurgitation complicating chronic bronchitis; report of a case].

A 61-year-old man with diabetes mellitus and chronic bronchitis was brought to the hospital after falling from a roof accidentally. He received blunt trauma to the left chest wall including left rib fractures, pneumothorax, hemothorax and a fracture of left scapula. After endotracheal intubation and chest drainage, he was transferred to the intensive care unit. Sixteen days later, congestive heart failure appeared. Echocardiography showed a severe aortic valve regurgitation with a prolapse of non-coronary cusp. Eight months after the injury, his heart failure deteriorated. He underwent aortic

Aortic valve replacement with 17-mm mechanical prostheses: is patient-prosthesis mismatch a relevant phenomenon?

BACKGROUND: We sought to evaluate the long-term performance of a consecutive cohort of patients implanted with a 17-mm bileaflet mechanical prosthesis. METHODS: Between January 1995 and December 2005, 78 patients (74 women, mean age=71±12 years) underwent aortic valve replacement with a 17-mm mechanical bileaflet prosthesis (Sorin Bicarbon-Slim and St. Jude Medical-HP). Preoperative mean body surface area and New York Heart Association class were 1.6±0.2 m2 and 2.6±0.8, respectively. Preoperative mean aortic annulus, indexed aortic valve area, and peak and mean gradients were 18±1.6 mm, 0.42

Intellectual function late after open-heart operation.

Cerebral emboli are not uncommon complications after valve replacements. We investigated the hypothesis that there are emboli that are clinically unnoticeable but that may affect the intellectual function of the brain. One hundred sixty-five patients were studied with a battery of psychometric tests before and after operation, two months postoperatively, and 2 to 8 years after operation. Ten patients sustained late cerebral infarction or hemorrhage. Their late intellectual function was low, indicating an impairment in brain performance. The remaining 155 patients had had no cerebral complicati

The better substitute for tricuspid valve replacement in patients with severe isolated tricuspid regurgitation.

OBJECTIVE: The ideal alternative for tricuspid valve replacement (TVR) in patients with severe isolated tricuspid regurgitation remains unclear. The aim of the present study was to retrospectively investigate the outcomes of using bioprosthetic and mechanical valves at the tricuspid position. METHODS: A total of 98 consecutive patients without left-side cardiac disease or history of heart surgery who underwent first-time TVR between January 2010 and March 2017 at the West China Hospital, China were included in the study. Patient data, including all-cause death and need for tricuspid valve reo

Outcomes of mitral valve replacement in children: a competing-risks analysis.

OBJECTIVE: We sought to define patient characteristics, outcomes, and associated factors after mitral valve replacement in children. METHODS: We included 104 children undergoing at least one mitral valve replacement between 1980 and 2003 and reviewed clinical records. Competing-risks methodology was used to determine time-related prevalence and associated risk factors after initial mitral valve replacement for death and repeat replacement. RESULTS: The underlying mitral valve disease was congenital in 83%, rheumatic in 13%, Marfan syndrome in 3%, and isolated endocarditis in 1%, with 64% hav

Prosthesis-patient mismatch is less frequent and more clinically indolent in patients operated for aortic insufficiency.

OBJECTIVE: To date, no study has focused on the incidence and effects of prosthesis-patient mismatch in patients requiring aortic valve replacement for aortic insufficiency. We hypothesized that the incidence and implications of prosthesis-patient mismatch in patients with aortic insufficiency might be different than for aortic stenosis or mixed disease because the annulus is generally larger in aortic insufficiency and left ventricular remodeling might differ. METHODS: Ninety-eight patients with lone aortic insufficiency (>or=3+ with a preoperative mean gradient <30 mm Hg) were followed over

[Repeat operations on patients with recurrent mitral stenosis].

An analysis of 282 reoperated mitral commissurotomies was made. Under consideration was the dependence of late results of the operation on the morphological state of the valve, regurgitation degree and concomitant defects of other heart valves. Remote results were good in 47 patients (28.2%), mediocre in 102 patients (51.3%) and bad in 43 patients (20.5%). Indications for the repeated closed mitral commissurotomy and for prosthesis of the mitral valve are given for patients with recurrent mitral stenosis.

Transcatheter Mitral Valve Implantation Using the HighLife System.

OBJECTIVES: This study is the first report of 2 cases of HighLife (HighLife, Paris, France) implantation in humans. BACKGROUND: Transcatheter mitral valve implantation represents a promising approach to treating mitral regurgitation in patients at increased risk of perioperative mortality. The HighLife transcatheter mitral valve is a 2-component system. The valve is implanted in the mitral position and is anchored by interacting and then reaching an equilibrium position with a previously positioned subannular implant. METHODS: The procedures were successfully performed in a 69-year-old man a

[Infective endocarditis with para-aortic abscess in mechanical aortic prosthesis].

Infective endocarditis (the infection of heart valves or, in generally, of endocardium) is a serious disease that requires a fast diagnosis, an aggressive and appropriate therapy and, if necessary, a collaboration between internist and surgeon. In this article we report a clinical case of an infective endocarditis in an old female patient, yet treated for aortic valve stenosis with replacement with mechanical prosthesis complicated by paravalvular abscess.

Diagnosis and management of complications of prosthetic heart valves.

Complications after heart valve replacement remain a substantial source of morbidity and mortality despite continuing advances in surgical care and prosthetic design. Infectious endocarditis occurs in about 4 percent of patients and may appear early (within 60 days) or late after operation. Endocarditis of early onset is commonly due to staphylococcal, fungal or gram-negative organisms and is fatal in 70 percent or more of cases. Infection of late onset is more often of streptococcal origin and the mortality rate is lower, about 35 percent. With either type, prompt recognition, vigorous and ap

Case report: Transcatheter pulmonary valve-in-valve implantation in a deteriorated self-expandable valve caused by infective endocarditis.

BACKGROUND: Infective endocarditis is a complication with high mortality in patients with congenital heart disease, particularly for those with bioprosthetic valve. CASE SUMMARY: We report a case of a 54-year-old female with a history of tetralogy of Fallot who had been surgically repaired using a transannular patch due to severe pulmonary insufficiency with right heart enlargement and presented with worsening dyspnea. She had received transcatheter pulmonary valve implantation (TPVI) 5 years ago. Unfortunately, bioprosthesis-associated infective endocarditis occurred due to dental caries. Gi

Immediate and mid-term results of transfemoral aortic valve implantation using either the Edwards Sapien transcatheter heart valve or the Medtronic CoreValve System in high-risk patients with aortic stenosis.

OBJECTIVE: We sought to describe the results of transfemoral aortic valve implantation using either the Sapien prosthesis or the CoreValve System. BACKGROUND: Results of transfemoral aortic valve implantation using both commercially available prostheses have rarely been studied. PATIENTS: Of 236 patients at high-risk or with contraindications to surgery, consecutively referred for transcatheter aortic valve implantation between October 2006 and June 2009, 83 were treated with transfemoral aortic valve implantation. The Sapien was the only prosthesis available until May 2008 and, since then,

Imaging for Predicting and Assessing Prosthesis-Patient Mismatch After Aortic Valve Replacement.

Prosthesis-patient mismatch (PPM) occurs when the effective orifice area (EOA) of the prosthetic valve is too small in relation to a patient's body size, thus resulting in high residual postoperative pressure gradients across the prosthesis. Severe PPM occurs in 2% to 20% of patients undergoing surgical aortic valve replacement (AVR) and is associated with 1.5- to 2.0-fold increase in the risk of mortality and heart failure rehospitalization. The purpose of this article is to present an overview of the role of multimodality imaging in the assessment, prediction, prevention, and management of P

Echocardiography in Transcatheter Aortic Valve Replacement.

Transcatheter aortic valve replacement (TAVR) is a safe and efficient alternative for surgical valve aortic replacement in patients with symptomatic severe aortic stenosis who are inoperable or have a high risk for surgery. Randomised clinical trials have shown that TAVR is not inferior to surgical aortic valve replacement in intermediate-risk patients and ongoing trials will demonstrate the effects of TAVR in asymptomatic severe aortic stenosis patients and in patients with heart failure and moderate aortic stenosis. Continuous developments in procedural and post-procedural management along w
