Continuous nasal positive airway pressure with a mouth leak: effect on nasal mucosal blood flux and nasal geometry.

BACKGROUND: Obstructive sleep apnoea is a common condition. Treatment with nasal continuous positive airway pressure (CPAP), while effective and safe, causes nasal congestion and stuffiness in some patients. The hypothesis that this study aimed to test was that nasal CPAP with a mouth leak and subsequent unidirectional airflow across the nasal mucosa causes an increase in nasal mucosal blood flux and a fall in both nasal volume and minimal cross sectional area. A secondary aim was to study if this could be prevented by humidifying the air inspired with nasal CPAP. METHODS: Nasal CPAP was appl

Continuous in vitro cultivation of Babesia gibsoni.

Two stocks of the protozoan parasite Babesia gibsoni, one of the causative agents of canine piroplasmosis, were propagated continuously in dog erythrocytes in microaerophilous stationary-phase culture. Cultures of both stocks were initiated in a humidified 5% CO2, 2% O2, 93% N2 atmosphere at 37 degrees C at a time when very few parasites (<0.01%) were detected in a thin blood smear. Cultures of one stock were also initiated in a humidified atmosphere of 5% CO2 in air at 37 degrees C during a patent parasitaemia (2.6%) in the donor animal. The culture medium was a modified HL-1 medium supplemen

Interaction of sulfur dioxide and dry cold air in causing bronchoconstriction in asthmatic subjects.

To determine whether sulfur dioxide and airway cooling and drying interact in causing bronchoconstriction in persons who have asthma, we measured specific airway resistance in seven asthmatic subjects before and after they performed voluntary eucapnic hyperpnea for 3 min breathing four different gas mixtures. The mixtures, which the subjects breathed through a mouthpiece in random order on 4 different days, were 1) humidified room-temperature air, 2) humidified room-temperature air containing 0.5 ppm SO2, 3) cold dry air, and 4) cold dry air containing 0.5 ppm SO2. Each subject breathed at a r

In vitro comparison between inspiration synchronized and continuous vibrating mesh nebulizer during trans-nasal aerosol delivery.

BACKGROUND: Compared to continuous vibrating mesh nebulizer (VMN), inspiration synchronized VMN has shown increased inhaled dose during noninvasive ventilation; however, its use during aerosol delivery via high-flow nasal cannula (HFNC) is still unknown. METHODS: An adult manikin was connected to a dual-chamber model lung, which was driven by a critical care ventilator to simulate spontaneous breathing. A HFNC system was utilized with temperature at 37 ° C while gas flow at 5, 10, 20, 40, and 60 L/min. Inspiration synchronized and continuous aerosol generation were compared at different posit

Management of Body Temperature via the Respiratory Tract.

Hypothermia, defined as a core body temperature ≤35°C, significantly increases morbidity and mortality in mechanically ventilated patients across numerous care settings. Physiologically, the upper airway conditions inspired gases to body temperature and humidity, minimizing heat energy loss and preventing mucosal damage. Instrumentation, such as endotracheal intubation, bypasses this natural mechanism, leading to considerable heat and moisture loss, potentially exacerbating hypothermia risks in critically ill patients. Active humidifiers and heat and moisture exchangers represent common strate

Home paediatric CPAP/NIV: the basics.

The use of home continuous positive airway pressure (CPAP) and noninvasive ventilation (NIV) is constantly growing in children. CPAP comprises constant CPAP and auto CPAP (APAP), while NIV comprises several modes of ventilation, among which barometric modes are nowadays the most used. Initiation criteria, indications, initiation location and strategies, and CPAP/NIV settings will be reviewed for both CPAP and NIV. The equipment, comprising the devices, interfaces, circuits and humidifiers, has to be mastered, particularly in paediatrics, with the knowledge of their advantages and limits. A clo

High-Flow Nasal Oxygen Therapy in Acute Hypoxemic Respiratory Failure: Concise Review on Technology and Initial Methodology.

High-flow nasal cannula oxygen therapy (HFNCOT) system consists of an air/oxygen supply system capable of delivering up to 100% humidified and heated oxygen at a flow rate of up to 80 L/min. The system includes a blender, active humidifier, single heated tube, and nasal cannula. HFNCOT has many physiological advantages compared with other standard oxygen therapies, such as anatomical dead space washout, more constant fraction of inspired oxygen, positive end-expiratory (PEEP) effect, supplement of adequate humidification and maintenance of muco-ciliary function. HFNCOT is mostly used for hypox

Decrease the flow setting to improve trans-nasal pulmonary aerosol delivery via "high-flow nasal cannula" to infants and toddlers.

OBJECTIVES: Trans-nasal pulmonary aerosol delivery for infants and toddlers has recently gained popularity, however, the reported lung deposition is low. We aimed to investigate the influential factors to improve the delivery. METHODS: Anatomic airway manikins simulating infant (5 kg) and toddler (15 kg) with collecting filter connected the trachea and breath simulator, were set to represent quiet and distressed breathing. Nasal cannula flow was set at 0.125, 0.25, 0.5, 1, and 2 L/kg/min. A mesh nebulizer (Aerogen) was placed at the inlet of humidifier (Fisher & Paykel) and proximal to patien

Symptomatic bronchoconstriction after short-term inhalation of sulfur dioxide.

We studied the relationship between duration and concentration of exposure in SO2-induced bronchoconstriction in 8 asthmatic subjects. On separate days, we administered SO2 in humidified air through a mouthpiece at 2 concentrations (0.5 and 1.0 ppm) for 3 time periods (1, 3, and 5 min) during eucapnic hyperpnea (60 L/min). Humidified air was administered for 5 min as a control. Bronchoconstriction was assessed by measurement of specific airway resistance (SRaw). The magnitude of the bronchoconstrictor response to both concentrations of SO2 increased progressively over the 3 time periods studie

Rapid changes in mucociliary transport in the tracheal epithelium caused by unconditioned room air or nebulized hypertonic saline and mannitol are not determined by frequency of beating cilia.

BACKGROUND: Inspired air is heated and humidified in the nose before it reaches lower airways. This mechanism is bypassed during tracheostomy, directly exposing the airways to colder and drier air from the environment, known to negatively affect mucociliary transport; however, little is known about how quickly mucociliary transport deteriorates. This study determines the short-term effect of flowing room air and nebulized hypertonic saline and mannitol on mucociliary transport in the trachea. In an ovine perfused in vitro tracheal model (N = 9) the epithelium was exposed to 25 L/min of flow, h

A comparison of two interventions for HHHFNC in preterm infants weighing 1,000 to 1,500 g in the recovery period of newborn RDS.

BACKGROUND: Nasal cannula, beside administering low-flow therapy, showed the capability for the administration of continuous positive airway pressure (CPAP) through high-flow nasal cannula (HFNC). Meeting specific physical criteria of 100% relative humidity (RH) and temperature of 37(°)C are the basic interventional requirements to administer oxygen for the newborns through a nasal cannula. Recently, two systems, MR850 and PMH7000, received the Food and Drug Administration (FDA) approval to administer heated, humidified HFNC (HHHFNC). These systems are evaluated in this study based on their hu

Nursing Management of Humidification Levels for the Extremely Low Birth Weight Infant: A Process Improvement Protocol to Reduce Hypernatremia in the First 2 Weeks of Life.

The use of a humidified environment has long been established as a method of maintaining a thermoneutral environment and minimizing evaporative insensible water losses in the extremely low birth weight (ELBW) population. The ELBW infant is at high risk for insensible water losses leading to dehydration, excessive weight loss, temperature instability, and electrolyte abnormalities, thereby increasing the ELBW infant's risks of morbidity and mortality. Literature supports the use of a humidified environment; however, a lack of consensus has led to widespread variation in the implementation of hu

[Operating principles, physiological effects and practical issues of high-flow nasal oxygen therapy].

First-line symptomatic treatment of acute respiratory failure (ARF) usually requires standard oxygen therapy, of which the limits have nonetheless led to the development of heated and humidified high-flow nasal oxygen therapy (HFNO). HFNO enables the delivery, through simple nasal cannula, of up to 100% of well-heated and humidified fraction of inspired oxygen (FiO2), at a maximum flow rate of 50 to 70 L/min of gas according to the devices chosen (specific or ventilator). The technical characteristics and operating principles of HFNO (coverage of the patient's spontaneous inspiratory flow, imp

Prediction of upper airway dryness and optimal continuous positive airway pressure conditions.

Continuous positive airway pressure is the most effective long-term treatment for obstructive sleep apnoea, which is a sleeping disorder characterized by pauses in breathing during sleep. It introduces pressurized atmospheric air into the respiratory system in order to maintain open airways without blockage. Some continuous positive airway pressure devices incorporate a convective heat transfer humidifier to overcome dryness. However, many side effects, including the unacceptable excess of water droplets in the air supply line, have been reported and improvements are essential for better patie

Airway extravasation induced by increasing airway temperature in ovalbumin-sensitized rats.

This study was carried out to determine whether hyperventilation of humidified warm air (HWA) induced airway extravasation in ovalbumin (Ova)-sensitized rats. Our results showed: (1) After isocapnic hyperventilation with HWA for 2 min, tracheal temperature (Ttr) was increased to 40.3°C, and the Evans blue contents in major airways and lung tissue were elevated to 651% and 707%, respectively, of that after hyperventilation with humidified room air in Ova-sensitized rats; this striking effect of HWA was absent in control rats. (2) The HWA-induced increase in Evans blue content in sensitized rats

In vitro cultivation of a Babesia sp. from cattle in South Africa.

A South African Babesia sp. of cattle which is as yet unclassified, was continuously cultivated in micro-aerophilous stationary-phase culture. The parasites were resuscitated from a blood stabilate stored in liquid nitrogen. A modified HL-1 medium supplemented with either horse or bovine serum was used. Cultures were initiated in a humidified atmosphere containing 2% O2, 5% CO2 and 93% N2 at 37 degrees C. Parasites were detected on Giemsa-stained smears after 2 d in culture. On day 4, the cultures were split at a ratio of 1:2 (v/v) and transferred into a humidified atmosphere of 5% CO2 in air.

Hypothermia.

Significant hypothermia is an increasing clinical problem that requires a rapid response with properly trained personnel and techniques. Although the clinical presentation may be such that the victim appears dead, aggressive management may allow successful resuscitation in many instances. Initial management should include CPR if the victim is not breathing or is pulseless. Further core heat loss should be prevented by removing wet garments, insulating the victim, and ventilating with warm humidified air/oxygen to help stabilize core temperature. Core temperature and cardiac rhythm should be mo
