Three dimensional Fourier transformation constructive interference in steady state (3DFT-CISS) magnetic resonance imaging (MRI) allows a detailed visualization of the membranous labyrinth of the inner ear. In this study the endolymphatic duct and sac is investigated in patients with Menière's disease using 3DFT-CISS MRI. In addition, the distance between the vertical part of the posterior semicircular canal and the fossa posterior is quantified.

Thirty-seven patients with Ménière's disease were treated with ultrasonic irradiation to the lateral and superior semicircular canals. In a longterm follow-up study, lasting alleviation of the vertiginous attacks was found in 24 out of 33 patients (72%). In most of the cases, hearing remained unchanged. It is suggested that the therapeutic effect is associated with a change in the endolymphatic drainage dur to histological alterations in the labyrinth produced by ultrasound.

The integrity of the blood-labyrinth barrier (BLB) is essential for inner ear homeostasis, regulating the ionic composition of endolymph and perilymph and preventing harmful substance entry. Endothelial hyperpermeability, central in inflammatory and immune responses, is managed through complex intercellular communication and molecular signaling pathways. Recent studies link BLB permeability dysregulation to auditory pathologies like acoustic trauma, autoimmune inner ear diseases, and presbycusis. Polymorphonuclear granulocytes (PMNs), or neutrophils, significantly modulate vascular permeabilit

Labyrinthitis most commonly results from an infectious and less commonly from an inflammatory process of the inner ear, but it can be associated with temporal bone trauma, hemorrhage, or tumor. This inflammation (regardless of the etiology) disrupts the transmission of sensory information from the ear to the brain. Labyrinthitis ossificans is the pathological ossification of the membranous labyrinthine spaces in response to an insult to the inner ear involving membranous labyrinth or the endosteum of the otic capsule. Herein, we present a case of a 67-year-old female with progressive bilateral

Refractory inner ear disease (RIED) encompasses a group of diseases in which pathologic lesions involve both the cochlea and vestibule, disrupting auditory-vestibular conduction pathways and their central projections. Clinically, RIED typically manifests as varying combinations of hearing loss, tinnitus, and vertigo. Despite adherence to standardized treatment protocols recommended by current clinical guidelines, a subset of inner ear diseases remain resistant to therapy and fail to achieve satisfactory outcomes. Recent epidemiological studies from both domestic and international sources indic

The administration of a dehydrating agent like glycerol may improve the audiometric thresholds and decrease the summating potential in the electrocochleographic recordings of some Méière's diseases. There are many discussions concerning the endolymphatic sac's rôle in this cases. To clarify this subject we have reviewed the labyrinth's liquids physiopathology, the structural observations in cases of Ménière's diseases, the animals models date. We conclude that the presence of an endolymphatic sac is not necessary for obtaining a positive glycerol test. The clinical meaning of this test is disc

Surgical labyrinthectomy in patients with Menière's disease produces unsatisfactory results in 10-70% of the cases. A pathological process in the vestibular nerve comprising an increase in connective tissue formation and the development of bony material within the nerve is possibly the reason for the pathological function of the vestibular system. This process may arise after a mechanical trauma to the labyrinth. Labyrinthectomy should be performed only for the treatment of Menière's disease if other therapeutical regimens or neurectomy under general anaesthesia cannot be tolerated by the pati

HYPOTHESIS: Prosthetic electrical stimulation can evoke compensatory eye and head movement despite vestibular implant electrode insertion occurring years after prior labyrinthectomy.
BACKGROUND: Vestibular implants sense head rotation and directly stimulate the vestibular nerve, bypassing damaged end organs. Animal research and current clinical trials have demonstrated the efficacy of this approach. However, candidacy criteria for vestibular implants currently require presence of a patent labyrinth in the candidate ear and at least aidable hearing in the opposite ear, thus excluding patients

Since the declaration of coronavirus disease 2019 (COVID-19) as a pandemic, it remains a widespread infection with a major impact on global resources and health infrastructure. The hallmark of COVID-19 continues to be the well-documented effects it has on the respiratory system. With the passage of time, the involvement of the severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) virus in other systems has become more apparent, with the increased incidence of thromboembolic events, cardiac involvement as well as gastrointestinal and neurological symptoms secondary to the infection. Bilateral loss of vestibular sensation can disable individuals whose vestibular hair cells are injured by ototoxic medications, infection, Ménière's disease or other insults to the labyrinth including surgical trauma during cochlear implantation. Without input to vestibulo-ocular and vestibulo-spinal reflexes that normally stabilize the eyes and body, affected patients suffer blurred vision during head movement, postural instability, and chronic disequilibrium. While individuals with some residual sensation often compensate for their loss through rehabilitation exercises, those who fail to do

Peripheral vestibular pathology disturbs the vestibulospinal reflex in a number of cases. The recording of postural sway furnishes an evaluation of this vestibulospinal influence. Two common peripheral vestibular syndromes were studied: Ménière's disease and peripheral unilateral vestibular hypofunction. It is evident that, by posturography, vestibulospinal disturbance is shown in these peripheral syndromes, whereas its occurrence cannot be predicted by other tests, e.g. caloric and rotation tests. Normalization of the vestibulospinal reflex depends on central adapting mechanisms, which were e

In the eponymous test described by Romberg in 1846, the erect patient is asked to close his eyes. If he falls, the test is positive and indicates the presence of a dorsal column lesion. Slight modifications have been described in the test but in all cases, great care must be taken to do it carefully. The test compares the stability of eye-opening with that of eye-closure. A positive test does not indicate vestibular or cerebellar disease.

Streptomycin perfusion of the labyrinth through the round window membrane in the middle ear is a simple, safe, and effective way to stop the dizzy spells, fullness, and tinnitus of Ménière's disease without making the hearing worse. My results with 24 patients in the last 8 months are presented. Although the long-term effect of this operation remains to be seen, my experience with streptomycin perfusion of the labyrinth through the lateral semicircular canal leads me to believe these good results will be maintained in the future. Although the caloric response is not usually eliminated complete

The report deals with 22 cases of two-phase head-shaking nystagmus. The results can be summarised as follows: 1. The two-phase head-shaking nystagmus is only found in connection with peripheral-vestibular disorders and particularly with unilateral disorders. It occurs only temporarily in the course of the vestibule disorder and may be observable for several months. 2. The head-shaking nystagmus, which follows immediately after the head-shaking (first phase), is to be considered a deficiency nystagmus. After a short period of latency after the disappearance of the first phase, a nystagmus in th

Ischemia, hemorrhages, and other vascular disorders can result in various central or peripheral vestibular syndromes with vertigo, oculomotor/balance disturbances, and nausea. The vascular vertigo syndromes listed in Table 1 can however be brought about by other causes such as demyelitizing focuses in multiple sclerosis or space-occupying lesions, so that not only localization of the damaged structure but also the various etiologies are decisive for the choice of therapy. Occasionally, combined functional disturbances of the peripheral and central vestibular system appear, such as an infarction

A prospective longitudinal study was designed to assess the role of pretreatment proneness to nausea and vomiting (NV) in the development of postchemotherapy NV in a group of Chinese breast cancer patients receiving moderately highly emetogenic chemotherapy. Seventy-one chemotherapy-naive subjects participated in the study. Patients were assessed the day before chemotherapy with measurements of their anxiety level, depression, fatigue and proneness to NV, motion sickness, NV experienced in past pregnancies, history of labyrinthitis, expectation of developing NV and expectation of developing pa

Delayed endolymphatic hydrops, an entity which can be differentiated from Meniere's disease, was first reported simultaneously by Nadol et al. (1975) and Wolfson & Leiberman (1975) and specified by Schuknecht (1978). They diagnosed it from the anamnesis of patients and confirmed the hydropic condition of the labyrinth solely by evidence obtained from surgical procedures such as labyrinthectomy. The aim of the present study was to emphasize the usefulness of the furosemide test for distinguishing this entity before surgery by detecting the hydropic condition in the labyrinth. In these 3 years,
