Mediastinal liposarcoma: Case report with radiology review.

De-differentiated primary mediastinal liposarcomas account for less than 1% of all liposarcoma pathology. We report the case of an 82-year-old male who was suffering from progressive dysphagia, shortness of breath, and dysphonia for a period of 2 months. A CT scan of the chest with contrast revealed a large heterogeneously enhancing posterior mediastinal mass extending into the posterior soft tissues of the neck, abutting bilateral carotid arteries, and displacing the trachea and esophagus. Treatment chosen for our patient was surgical resection followed by adjuvant radiation therapy which res

Thymic carcinoma: case reports and review.

Thymic carcinoma is a rare malignant neoplasm of the thymic epithelium, distinguished from invasive thymoma by the presence of malignant cytology and a greater incidence of local invasion and embolic metastases. Some common features of thymic carcinomas include (a) large and highly aggressive anterior mediastinal mass; (b) areas of necrosis, hemorrhage, calcification, or cyst formation; (c) gross invasion of contiguous mediastinal structures and wide spread to involve distant intrathoracic sites; (d) high incidence of extrathoracic metastases; (e) broad age range, with an average of 46 years;

Mediastinal teratoma and precocious puberty in a boy with mosaic Klinefelter syndrome.

We describe a boy who developed precocious puberty resulting from chorionic gonadotropin produced by a mediastinal germ cell tumor. Following tumor removal he began spontaneous precocious sexual development which was treated and then arrested spontaneously. Investigation of this arrested puberty established that he had Klinefelter syndrome (KS) mosaicism. He represents the first instance of KS mosaicism reported with a mediastinal germ cell tumor, a neoplasm commonly reported in males with a 47,XXY karyotype. We recommend that all males with KS and early sexual development or with "normal" tes

Thymolipoma of the anterior mediastinum: videothoracoscopic removal using a bilateral approach.

Thymolipoma, a rare benign neoplasm of the anterior mediastinum, is often asymptomatic and as a result it can become quite large before it is diagnosed. CT is the most accurate diagnostic technique to identify the adipose tissue, but it often cannot make a differential diagnosis differentiating it from other anterior mediastinal masses; transthoracic biopsy also reveals the presence of fatty tissue, but a definitive diagnosis can only be achieved by means of surgical excision, which is also curative. We describe the case of a young woman who presented with a fatty neoplasm of the anterior medi

A Case Report of Primary Mediastinal Liposarcoma: A Rare Mediastinal Tumor.

Liposarcomas account for about 20% of all sarcomas among mesenchymal neoplasms. Myxomatous liposarcoma is a rare mediastinal tumor that seems the same as other lung disorders. The most common presenting symptoms are chest pain, dyspnea, and dysphagia. Most of the diagnostic findings are provided by radiological or postoperative histopathological tests. Surgery and chemotherapy, in some cases, are the basis of treatment. People with this condition have a higher probability of a favorable outcome if they receive an early diagnosis and treatment. We present a case of a 48-year-old male with prima

Case Report: Multidisciplinary Approach for a Rare Case of Thymic Vascular Malformation.

We report the rare case of a 2.5 cm in size mass diagnostic for residual thymus associated with venous vascular malformation (ISSVA classification, 2008) in a 58 years old man. Diagnosis was obtained only after surgical removal that was complicated by a sudden massive bleeding (about 1,500 cc) requiring emergency conversion to median sternotomy. Difficulty in preoperative diagnosis, rarity of histologic pattern, and surgical challenges make this case very interesting for surgeons, pathologists and radiologist. Our message, dealing with mediastinal masses, is: (a) differential diagnosis between

Thymic Carcinoma Associated with Cerebellar Degeneration.

UNLABELLED: We present the case of a 57-year-old man with ataxia and clinical and radiological features of cerebellar degeneration. Computed tomography showed a mediastinal mass and the patient was diagnosed with thymic carcinoma. Paraneoplastic cerebellar degeneration is aninfrequent disorder and its association with thymic carcinoma very rare. LEARNING POINTS: Unexplained subacute neurological symptoms in an adult patient should beconsidered in the possibility of a paraneoplastic syndrome. In patients over 50 years of age, acute or subacute cerebellar degeneration is paraneoplastic in 50% o

Sonographic characteristics of thymoma compared with mediastinal lymphoma.

Thymomas are uncommon neoplasms that are typically located in the cranioventral mediastinum in domestic species, which generally affects older animals. Medical records and sonograms of 50 animals diagnosed with either mediastinal thymoma (n = 35) or lymphoma (n = 15) between January 2005 and February 2012 were reviewed. Cases with definitive cytological and/or histological diagnoses of either thymoma or lymphoma were included in this study. Masses were characterized by echogenicity, presence or absence of cysts, shape, margination, vascularity, and presence of effusion. Many thymomas (57.1%) w

Metaplastic thymoma: report of 4 cases.

Metaplastic thymoma (MT), accepted in the World Health Organization 2004 scheme, is a circumscribed tumor of the thymus exhibiting biphasic morphology. We herein describe the clinicopathologic features of four MTs and the differential diagnoses of this unusual tumor. There were three women and one man with mean age of 49.5 years. The patients were found to have mediastinal masses, and underwent surgical excision. One exhibited symptoms of myasthenia gravis, and the serum titer for anti-acetylcholine receptor antibody was positive. Grossly, the tumors were encapsulated, and showed vaguely multi

Posterior mediastinal liposarcoma simulating a lung mass: an unusual case report.

Liposarcomas constitute approximately 15% of all sarcomas among the mesenchymal neoplasms. They usually arise in the lower extremities or in the retroperitoneum but have been reported in sites such as abdomen, vulva and buttocks. Primary mediastinal liposarcomas are rare with less than 100 cases reported worldwide so far. We present an interesting case of primary myxoid LPS of the posterior mediastinum with presenting complaints, imaging diagnosis and management with a review of the current literature.

Primary neuroendocrine carcinomas of the thymus.

Primary neuroendocrine carcinomas of the thymus are rare and comprise a wide spectrum of lesions ranging from well-differentiated to poorly-differentiated neoplasms. The classification of such tumors in the thymus is still controversial. By convention, the better-differentiated examples have been traditionally designated as thymic carcinoids and thought to represent the mediastinal counterpart of carcinoid tumors in other foregut locations. However, recent studies have shown that such neoplasms, when arising in the thymus, exhibit a much more aggressive behavior than those originating at other

[Upper inflow congestion as initial manifestation of idiopathic mediastinal fibrosis].

In about 90% of superior vena caval obstructions the cause is a neoplasm, generally a carcinoma of the right upper lobe. Fewer than 5 percent are due to inflammatory disease of the mediastinum. A case of idiopathic, fibrosing mediastinitis with occlusion of the superior vena cava is described. The clinical symptoms, radiographic finding, diagnostic and therapeutic aspects are discussed.

[Non-neoplastic lesions of the mediastinum].

The mediastinum is a complex body region of limited space but containing numerous organs of different embryonic origins. A variety of lesions that are difficult to distinguish from each other can occur here. Non-neoplastic lesions of the mediastinum represent important differential diagnostic pitfalls to mediastinal tumors, clinically, radiologically and histopathologically. It is important to bear these lesions in mind and to adequately verify or exclude them before starting further differential diagnostic considerations on mediastinal neoplasms. The most common non-neoplastic lesions in this

Evaluation of mediastinal lymphadenopathy with endoscopic US-guided fine-needle aspiration biopsy.

PURPOSE: To evaluate the safety and accuracy of endoscopic ultrasonography (US)-guided fine-needle aspiration biopsy (FNAB) of lymph nodes in the paratracheal, aortopulmonic, subcarinal, and posterior mediastinal regions. MATERIALS AND METHODS: Eighty-six consecutive patients with mediastinal lymphadenopathy who did not have a primary gastrointestinal neoplasm were examined. In 29 patients, endoscopic US-guided FNAB of mediastinal lymphadenopathy was performed as a component of staging non-small cell lung cancer (NSCLC); in the remaining 57 patients, it was performed to obtain a primary diagn

Carcinoid tumour of the thymus with systemic manifestations: a radiological and pathological study.

Lowenthal, R. M., Gumpel, J. M., Kreel, L., McLaughlin, J. E., and Skeggs. D. B. L. (1974).Thorax, 29, 553-558. Carcinoid tumour of the thymus with systemic manifestations: a radiological and pathological study. Following recent reports of an unusual mediastinal tumour described as `mediastinal endocrine neoplasm of probable thymic origin, related to carcinoid tumour' (Rosai and Higa, 1972), a further case, in a 72-year-old man, has been studied. Polyarthropathy was the presenting feature, and the patient also had clubbing of the fingers and clinical evidence of a probable proximal myopathy an

Survival difference among adult and pediatric mediastinal yolk sac tumors cases: A meta-analysis of case reports.

BACKGROUND: Mediastinal Yolk sac tumors (YST) are rare and highly malignant extragonadal germ cell tumors with rapid growth and early metastases. We sought to conduct a meta-analysis of published case reports/case series to compare differences in survival, demographics, and treatment modalities between adult and pediatric patients with YST. METHODS: Ovid Embase, Cochrane, and Ovid Medline databases were searched for primary mediastinal pure YST cases. The primary outcome was overall survival (OS). Log-rank and Cox regression were used. This study is registered on PROSPERO (CRD42022367586). R

Flow cytometry in the diagnosis of mediastinal tumors with emphasis on differentiating thymocytes from precursor T-lymphoblastic lymphoma/leukemia.

Flow cytometry (FC) has become the routine technique in the evaluation of hematopoietic neoplasms. Since the anterior mediastinum is a frequent site of involvement by both primary and secondary lymphoma/leukemia, flow cytometry plays an important role in the evaluation of mediastinal masses. The present study reviews 100 flow cytometry cases from patients presenting with mediastinal lesions. In 5 cases (5%) flow cytometry was not diagnostic due to either insufficient cell yield or low viability. In the remaining cases (95/100) cell suspensions were adequate for flow cytometry evaluation. Middle mediastinal thymoma.

Thymoma is a common primary neoplasm of the anterior mediastinum. However, it can also arise in other locations, including the neck, the posterior mediastinum, the lung, base of the skull and the pleural cavity. There are only three previous case reports of thymoma located in the middle mediastinum. This report describes a patient together with a review of the literature on middle mediastinal thymoma.
