From January 1989 to July 1990 a total of 562 oocytes was fixed in our In Vitro Fertilization Program, while cytogenetic data were collected in the case of 433. In order to evaluate the possibilities for induction of ovulation, the functional competence of the pituitary gland of a woman with Kallmann syndrome was examined by two consecutive dynamic GnRH tests. Details of the problems and successes obtained after various forms of hormonal stimulation in an IVF programme are presented. The endocrine conditions normally existing in the natural menstrual cycle are discussed first, followed by analyses of the types of responses to clomiphene, clomiphene and HMG, HMG alone and LHRH agonists. Elevated levels of CA-125, an antigen expressed by malignant ovarian tissue, have been found in women who developed OHSS in response to treatment with exogenous gonadotropins for IVF. In contrast, CA-125 concentrations in women who were treated with an identical regimen but who did not show signs of OHSS remained in the normal range. We conclude that the expression of CA-125 is not restricted to neoplastic ovarian tissue but can also occur as a consequence of supraphysiologic stimulation of the ovary with exogenous gonadotropins.

Blodgett Memorial Medical Center began developing an IVF program in September of 1982. It took 10 months to establish animal and human IVF laboratories and an andrology laboratory. All personnel were recruited from within the hospital. Report of radiologic and hormonal results of a patient with typical stigmata of the Aarskog-syndrome. The preoperative diagnosis of ectopic ovarian tissue has been reported only once in the literature. In the present case, differential ovarian and adrenal testing was used to diagnose and aberrant ovarian source of persistently elevated plasma estrogen in a patient who had undergone bilateral oophorectomy and adrenalectomy for metastatic breast carcinoma. The ovarian source was diagnosed by stimulation with human chorionic gonadotropin (hCG), and treatment with human menopausal gonadotropin (hMG) prior to surgery was used to facilitate its location at laparotomy.

PURPOSE: To investigate the ability of anti-Mullerian hormone (AMH) to predict the step up of human menopausal gonadotropins (HMG) dose in women with polycystic ovarian syndrome (PCOS) undergoing IVF/ICSI cycles.
METHODS: AMH was drawn before ovulation induction in 976 PCOS women scheduled for IVF/ICSI. After all cycles ended, a receiver operating characteristic (ROC) curve analysis was done to investigate the ability of AMH to predict step up of the HMG.
RESULTS: The area under the curve (AUC) was 0.820 95%CI (0.792-0.848), and a cutoff value of 4.6 ng/ml (sensitivity 74%, specificity 82%)
