BACKGROUND: Upper limb amputation is among the most functionally disabling conditions, affecting an estimated 1.6 million people in the United States, with prevalence projected to double by 2050. Despite decades of technological progress, device abandonment rates remain persistently high, up to 45% for body-powered and 35% for myoelectric prostheses in the paediatric population, due to inadequate functionality, poor sensory feedback, discomfort, and social stigma.
OBJECTIVES: This narrative review aims to provide a structured and up-to-date synthesis of upper limb prosthetic technologies, wit

Background/Objectives: Tactile gnosis derives from the interplay between the hand's tactile input and the memory systems of the brain. It is the prerequisite for complex hand functions. Impaired sensation leads to profound disability. Various invasive and non-invasive sensory substitution strategies for providing feedback from prostheses have been unsuccessful when translated to clinical practice, since they fail to match the feeling to genuine sensation of the somatosensory cortex. Methods: Herein, we describe a novel surgical technique for upper-limb-targeted sensory reinnervation (ulTSR) an

Photoreceptor death is an important contributor to irreversible vision loss worldwide. In this review, I outline our work examining the role that purines, such as adenosine triphosphate (ATP), have in normal retinal function and in retinal disease. Our work shows that the actions of ATP, mediated by P2X receptors, are expressed in various retinal layers including photoreceptor terminals, and when stimulated by excessive levels of ATP is associated with rapid death of neurons. Treatment with a compound that blocks the action of P2X and some P2Y receptors reduces photoreceptor death in a mouse m

Consistent, robust performance is critical for the utility and user-acceptance of neurally-controlled powered upper limb prostheses. We preliminarily evaluated the performance consistency of an electromyography (EMG)-driven controller based on a two degree-of-freedom musculoskeletal hand model, whose simplified structure is more practical for real-time prosthesis control than existing, complex models. Parameters of four virtual muscles were computed by numerical optimization from an able-bodied subject's kinematic and EMG data collected during wrist and metacarpophalangeal (MCP) flexion/extens

Recent research and clinical experience with cochlear implants suggest that subjects' speech recognition with monopolar or broad bipolar stimulation might be equal to or better than that obtained with narrow bipolar stimulation or other spatially restricted electrode configurations. Furthermore, subjects often prefer the monopolar configurations. The mechanisms underlying these effects are not clear. Two hypotheses are (a) that broader configurations excite more neurons resulting in a more detailed and robust neural representation of the signal and (b) that broader configurations achieve a bet

BACKGROUND: MUNDUS is an assistive framework for recovering direct interaction capability of severely motor impaired people based on arm reaching and hand functions. It aims at achieving personalization, modularity and maximization of the user's direct involvement in assistive systems. To this, MUNDUS exploits any residual control of the end-user and can be adapted to the level of severity or to the progression of the disease allowing the user to voluntarily interact with the environment. MUNDUS target pathologies are high-level spinal cord injury (SCI) and neurodegenerative and genetic neurom

Nearly all spinal cord injured (SCI) individuals lose bladder control and are prone to kidney complications if intermittent catheterization is not applied. Electrical stimulation of the sacral anterior roots with an implantable neuroprosthesis is one means to restore continence and control micturition. However, only a small percentage of the SCI population benefits from this solution because of its drawbacks, e.g., section of sacral posterior roots with loss of spared sensitivity. There has been renewed interest in spinal cord stimulation in recent years, but most studies have focused on locom

Objective.Implanted electrodes for nerve stimulation and myoelectric recording facilitate bidirectional sensory feedback and control for neuromuscular conditions such as limb loss. While increasing implanted electrode channel count offers potential benefits, it also presents engineering and implementation challenges. This case study examines how increasing implanted electrode channel count affects sensory perception and myoelectric controller performance, thereby supporting the value of these advancements.Approach.One participant with upper extremity transradial limb loss received a percutaneo

Recent advancements in upper-limb prosthetics have enabled users to control multi-degree-of-freedom prostheses using pattern recognition algorithms applied to electromyography (EMG). These methods are built on the extraction of EMG features that treat EMG signals as colored noise, neglecting their biological origin. In contrast, recent works have explored the use of features based on the decomposition of EMG signal into the activity of the motor units. These features hold great potential for control applications as they reflect the actual neural drive to the muscles. Here, we evaluate the feas

Upper limb robotic (myoelectric) prostheses are technologically advanced, but challenging to use. In response, substantial research is being done to develop person-specific prosthesis controllers that can predict a user's intended movements. Most studies that test and compare new controllers rely on simple assessment measures such as task scores (e.g., number of objects moved across a barrier) or duration-based measures (e.g., overall task completion time). These assessment measures, however, fail to capture valuable details about: the quality of device arm movements; whether these movements m

Trunk instability is a major problem for individuals with thoracic and cervical spinal cord injury. Functional electrical stimulation (FES) neuroprosthesis, a technology that uses small electrical currents to artificially contract muscles, has previously been utilized to improve trunk stability during quasi-static and dynamic sitting. The aim of this paper was to develop the first powered wheelchair-based neuroprosthesis and to test its feasibility for improving trunk stability. Eleven male, able-bodied individuals participated in the feasibility study. While participants were seated, the whee

This study investigated whether vagal nerve stimulation (VNS) leads to improvements in ischemic heart failure via heart rate modulation. At 7 ± 1 days post left anterior descending artery (LAD) ligation, 63 rats with myocardial infarctions (MI) were implanted with ECG transmitters and VNS devices (MI + VNS, N = 44) or just ECG transmitters (MI, N = 17). VNS stimulation was active from 14 ± 1 days to 8 ± 1 weeks post MI. The average left ventricular (LV) end diastolic volumes at 8 ± 1 weeks were MI = 672.40 μl and MI + VNS = 519.35 μl, p = 0.03. The average heart weights, normalized to body wei

Myoelectric prostheses are generally unable to accurately control the position and force simultaneously, prohibiting natural and intuitive human-machine interaction. This issue is attributed to the limitations of myoelectric interfaces in effectively decoding multi-degree-of-freedom (multi-DoF) kinematic and kinetic information. We thus propose a novel multi-task, spatial-temporal model driven by graphical high-density electromyography (HD-EMG) for simultaneous and proportional control of wrist angle and grasp force. Twelve subjects were recruited to perform three multi-DoF movements, includin

Conventionally, the practice of neurosurgery has been characterized by the removal of pathology, congenital or acquired. The emerging complement to the removal of pathology is surgery for the specific purpose of restoration of function. Advents in neuroscience, technology, and the understanding of neural circuitry are creating opportunities to intervene in disease processes in a reparative manner, thereby advancing toward the long-sought-after concept of neurorestoration. Approaching the issue of neurorestoration from a biomedical engineering perspective is the rapidly growing arena of implant

BACKGROUND: Neuroprosthetic devices controlled by persons with standard limb amputation often lack the dexterity of the physiological limb due to limitations of both the user's ability to output accurate control signals and the control system's ability to formulate dynamic trajectories from those signals. To restore full limb functionality to persons with amputation, it is necessary to first deduce and quantify the motor performance of the missing limbs, then meet these performance requirements through direct, volitional control of neuroprosthetic devices.
METHODS: We develop a neuromuscular

OBJECTIVE: Electrical stimulation via microelectrodes implanted in cortex has been suggested as a potential treatment for a wide range of neurological disorders. Despite some success however, the effectiveness of conventional electrodes remains limited, in part due to an inability to create specific patterns of neural activity around each electrode and in part due to challenges with maintaining a stable interface. The use of implantable micro-coils to magnetically stimulate the cortex has the potential to overcome these limitations because the asymmetric fields from coils can be harnessed to s

A hybrid neuroprosthesis that combines human muscle power, elicited through functional electrical stimulation (FES), with a powered orthosis may be advantageous over a sole FES or a powered exoskeleton-based rehabilitation system. The hybrid system can conceivably overcome torque reduction due to FES-induced muscle fatigue by complementarily using torque from the powered exoskeleton. The second advantage of the hybrid system is that the use of human muscle power can supplement the powered exoskeleton's power (motor torque) requirements; thus, potentially reducing the size and weight of a walki
