Patient-specific biomechanical model of hypoplastic left heart to predict post-operative cardio-circulatory behaviour.

Hypoplastic left heart syndrome is a complex congenital heart disease characterised by the underdevelopment of the left ventricle normally treated with a three-stage surgical repair. In this study, a multiscale closed-loop cardio-circulatory model is created to reproduce the pre-operative condition of a patient suffering from such pathology and virtual surgery is performed. Firstly, cardio-circulatory parameters are estimated using a fully closed-loop cardio-circulatory lumped parameter model. Secondly, a 3D standalone FEA model is build up to obtain active and passive ventricular characterist

A patient-specific computational model of hypoxia-modulated radiation resistance in glioblastoma using 18F-FMISO-PET.

Glioblastoma multiforme (GBM) is a highly invasive primary brain tumour that has poor prognosis despite aggressive treatment. A hallmark of these tumours is diffuse invasion into the surrounding brain, necessitating a multi-modal treatment approach, including surgery, radiation and chemotherapy. We have previously demonstrated the ability of our model to predict radiographic response immediately following radiation therapy in individual GBM patients using a simplified geometry of the brain and theoretical radiation dose. Using only two pre-treatment magnetic resonance imaging scans, we calcula

The impact of fear-avoidance model variables on disability in patients with specific or nonspecific chronic low back pain.

STUDY DESIGN: A prospective cross-sectional design. OBJECTIVE: The objectives were to describe the occurrence and to investigate the association of the fear-avoidance model variables (pain intensity, kinesiophobia, depression, and disability) in patients with specific or nonspecific chronic low back pain (CLBP). SUMMARY OF BACKGROUND DATA: Affective factors, particularly fear, have proven to be central in the explanation and understanding of chronic pain. The fear-avoidance model has shown that fearful patients with CLBP are at risk of becoming trapped in a vicious cycle of pain, fear, disab

Patient-specific zebrafish models reveal a complex phenotypic spectrum in NBAS-associated atypical osteogenesis imperfecta.

Neuroblastoma amplified sequence gene (NBAS) variants are associated with short stature, optic atrophy, and Pelger-Huët anomaly (SOPH) syndrome. We previously identified compound heterozygous variants in NBAS to cause atypical Osteogenesis Imperfecta (OI), with these patients presenting with short stature, developmental delay, and recurrent long-bone fractures. However, skeletal disease progression due to these variants and the disease mechanisms underlying NBAS-associated OI remain poorly understood. Here, we provide a clinical update on previously identified patients and investigate the role

Predicting the patient-specific centre of rotation for total temporomandibular joint replacement using statistical shape modelling.

This study was performed to explore the use of statistical shape modelling (SSM) to predict the centre of rotation (CR) in patients undergoing total temporomandibular joint replacement (TMJ-TJR), in order to facilitate more accurate prosthesis design. In this retrospective study, a SSM of mandible halves was constructed using preoperative segmented three-dimensional models from cone-beam computed tomography scans, split into 388 mandible halves. The model was evaluated through compactness and generalization curves and by examining the principal components (PCs) of shape variations. A dataset o

A Patient-Derived iPSC-Based Model Reveals Neural Lineage-Specific Transcription of Endogenous HHV-6B.

BACKGROUND: Endogenous human herpesvirus 6 (eHHV-6), in which the entire viral genome is integrated into human chromosomes, is present in approximately 1% of the population and has been associated with various clinical conditions, including neurological disorders. However, its biological significance remains unclear due to the lack of appropriate experimental models. METHODS: We established a patient-derived induced pluripotent stem cell (iPSC)-based tissue culture model using lymphoblastoid cell lines from individuals with eHHV-6B. iPSCs retaining the integrated viral genome were generated a

Effect of stenoses and occlusions in the cerebral venous vasculature on intracranial venous hemodynamics - a patient-specific computational model.

BACKGROUND: Intracranial sinus stenoses (ISS) or sinus occlusions (ISO) can lead to elevated intracranial venous blood pressure. Treatment strategies are advancing, but clinical decision making and the prediction of treatment success is hampered by the variable venous anatomy. We aimed to create a patient-specific computational model that allows for simulating pathologies and their influence on venous flow dynamics and pressure. METHODS: The model was created based on a stroke patient's CT-angiography using semi-automatic threshold-based segmentation with subsequent flow modeling through comp

A fully coupled fluid-structure interaction model for patient-specific analysis of bioprosthetic aortic valve haemodynamics.

BACKGROUND: Bioprosthetic aortic valves (BPAV) have been increasingly used for surgical aortic valve replacement (SAVR), but long-term complications associated with structural valve deterioration remain a concern. The structural behaviour of the valve and its surrounding haemodynamics play a key role in the long-term outcome of SAVR, and these can be quantitively analysed by means of fluid-structure interaction (FSI) simulation. The aim of this study was to develop a fully coupled FSI model for patient-specific analysis of BPAV haemodynamics. METHODS: Using the Edwards Magna Ease valve as an

Patient-specific 3D-tissue slices from peritoneal metastases - An in vitro model for individual susceptibility analysis.

OBJECTIVES: The prognosis of patients with peritoneal metastases (PM) is poor, and these patients have a brief overall survival. Most patients with advanced PM receive palliative therapy to maintain their quality of life. In our current study, we investigated whether patient-specific 3D-tissue slices from patients with PM subjected to pressurized intraperitoneal aerosol chemotherapy could be cultured in vitro. METHODS: Biopsies from gastric cancer patients with PM were characterized for cytokeratin-positive tumor cells and the proliferation marker Ki-67. Biopsies from seven patients were cut

Patient-Specific 3D Models for Open Frontal Sinus Surgery: Enhancing Precision with Facial Mask-Guidance.

In osteoplastic frontal sinus surgery, the precision of initial bony incisions is critically important. Conventional techniques such as 6-ft Caldwell radiographs, burr holes method, transillumination and navigation systems are widely used. Recently, publications on the use of three-dimensional (3D) printed models in surgical navigation have increased. We aimed to enhance the accuracy of 3D model navigation-a cost-effective and accessible method that can be used alone or alongside conventional approaches-by developing a novel facial mask component not previously described. We developed a patien

Simulation of thoracic endovascular aortic repair in a perfused patient-specific model of type B aortic dissection.

PURPOSE: Complicated type B Aortic dissection is a severe aortic pathology that requires treatment through thoracic endovascular aortic repair (TEVAR). During TEVAR a stentgraft is deployed in the aortic lumen in order to restore blood flow. Due to the complicated pathology including an entry, a resulting dissection wall with potentially several re-entries, replicating this structure artificially has proven to be challenging thus far. METHODS: We developed a 3d printed, patient-specific and perfused aortic dissection phantom with a flexible dissection flap and all major branching vessels. The

Comparing Inducibility of Re-Entrant Arrhythmia in Patient-Specific Computational Models to Clinical Atrial Fibrillation Phenotypes.

BACKGROUND: Computational models of fibrosis-mediated, re-entrant left atrial (LA) arrhythmia can identify possible substrate for persistent atrial fibrillation (AF) ablation. Contemporary models use a 1-size-fits-all approach to represent electrophysiological properties, limiting agreement between simulations and patient outcomes. OBJECTIVES: The goal of this study was to test the hypothesis that conduction velocity (ϴ) modulation in persistent AF models can improve simulation agreement with clinical arrhythmias. METHODS: Patients with persistent AF (n = 37) underwent ablation and were foll

A combined clinical and specific genes' model to predict live birth for in vitro fertilization and embryo transfer patients.

BACKGROUND: We aimed to develop an accurate model to predict live birth for patients receiving in vitro fertilization and embryo transfer (IVF-ET) treatment. METHODS: This is a prospective nested case-control study. Women aged between 18 and 38 years, whose body mass index (BMI) were between the range of 18.5-24 kg/m2, who had an endometrium of ≥ 8 mm at the thickest were enrolled from 2018/9 to 2020/8. All patients received IVF-ET treatment and were followed up until Jan. 2022 when they had reproductive outcomes. Endometrial samples during the window of implantation (LH + 6 to 9 days) were s

Structural and Ultrastructural Analysis of the Multiple Myeloma Cell Niche and a Patient-Specific Model of Plasma Cell Dysfunction.

Multiple myeloma (MM) is a deadly, incurable malignancy in which antibody-secreting plasma cells (PCs) become neoplastic. Previous studies have shown that the PC niche plays a role cancer progression. Bone marrow (BM) cores from MM and a premalignant condition known as monoclonal gammopathy of unknown significance (MGUS) patients were analyzed with confocal and transmission electron microscopy. The BM aspirates from these patients were used to generate 3D PC cultures. These in vitro cultures were then assayed for the molecular, cellular, and ultrastructural hallmarks of dysfunctional PC at day

A patient-specific image-based approach to estimate pulmonary artery stiffness based on vessel constitutive model.

An accurate non-invasive evaluation of the mechanical properties of the vessel wall is important for a variety of screening protocols and surgical treatments. In this work, we focused on a section of the Pulmonary Artery (PA), and developed a patient-specific approach to estimate its stiffness in terms of the Young's modulus along the circumferential direction (E). First, we developed a patient-specific semi-automatic approach to estimate its expected value and standard deviation. To this end, pressure-length curves were derived from magnetic resonance images acquired during the cardiac cycle

Patient-specific computational flow modelling for assessing hemodynamic changes following fenestrated endovascular aneurysm repair.

OBJECTIVE: This study aimed to develop an accessible patient-specific computational flow modelling pipeline for evaluating the hemodynamic performance of fenestrated endovascular aneurysm repair (fEVAR), with the hypothesis that computational flow modelling can detect aortic branch hemodynamic changes associated with fEVAR graft implantation. METHODS: Patients who underwent fEVAR for juxtarenal aortic aneurysms with the Cook ZFEN were retrospectively selected. Using open-source SimVascular software, preoperative and postoperative visceral aortic anatomy was manually segmented from computed to

Generation of 10 patient-specific induced pluripotent stem cells (iPSCs) to model Pitt-Hopkins Syndrome.

Autosomal dominant mutations in transcription factor 4 (TCF4) are associated with a rare syndromic form of Autism Spectrum Disorder (ASD) called Pitt-Hopkins Syndrome (PTHS). Here, we report the generation of a collection of induced pluripotent stem cells (iPSCs) from 5 patients diagnosed with PTHS and 5 familial controls. These patient-derived iPSCs contain a variety of mutations within the TCF4 gene, possess a normal karyotype and express all the appropriate pluripotent stem cell markers. These novel patient lines will be a useful resource for the research community to study PTHS and the fun
