Rapid synovial fluid-induced aggregation of Staphylococcus aureus is currently being investigated as an important factor in the establishment of periprosthetic joint infections (PJIs). Pathogenic advantages of aggregate formation have been well documented in vitro, including recalcitrance to antibiotics and protection from host immune defenses. The objective of the present work was to determine the strain dependency of synovial fluid-induced aggregation by measuring the degree of aggregation of 21 clinical S. aureus isolates cultured from either PJI or bloodstream infections using imaging and

BACKGROUND: Owing to the difficulty isolating microorganisms in periprosthetic joint infection (PJI), current guidelines recommend that 3-5 intraoperative samples be cultured and maintained for 3-14 days. We investigated (1) the optimal number of culture samples and growth duration to diagnose PJI and (2) the microbiology profile at our institution.
METHODS: A retrospective review of 711 patients (329 hips, 382 knees) with PJI that met Musculoskeletal Infection Society criteria from 2000 to 2014 was performed. Two thousand two hundred ninety aerobic and anaerobic cultures were analyzed. A man

ObjectiveThe infection of the endograft after thoracic endovascular aortic repair (TEVAR) is a rare but potentially life-threatening complication. Unfortunately, clinical reports, large case-series or randomized studies are scarce or not available yet, and the optimal management of this complication is under debate. The aim of this review is collecting and analysing data on current practice and outcomes for the treatment of infected endografts after TEVAR.MethodsA search on PubMed/MEDLINE and EMBASE for studies reporting endograft infections after TEVAR was conducted up to January 2023. Data o

INTRODUCTION: There is a dearth of data for artificial urinary sphincter (AUS) erosions after the introduction of InhibiZone-coated devices in 2008. We aim to identify common organisms associated with noninfected and infected AUS device erosions.
METHODS: We reviewed our prospectively collected, institutionally approved quality improvement database of AUS implantations, removals, or revisions performed at our facility. Devices implanted after 2008 that subsequently eroded were included for review. Erosion was confirmed cystoscopically in all cases. Urine cultures and intraoperative cultures (

A 75-year-old man with a history of aortic valve replacement was referred to our department with a diagnosis of prosthetic valve endocarditis. Although antibiotics treatment was initiated, infection progressed rapidly to develop aortic root abscess and pseudoaneurysm. The patient underwent urgent surgery which included extensive debridement of near-circumferential infected annulus. Aortic annulus reconstruction with a bovine pericardial patch was performed followed by aortic valve replacement. The postoperative course was uneventful and the patient was discharged from the hospital after six we

OBJECTIVE: The management of periprosthetic joint infection (PJI) after total ankle replacement (TAR) may be challenging. Our aim was to define the eradication rate and limits of some of the surgical procedures commonly performed to treat acute and chronic PJI after TAR.
METHODS: For this PRISMA-compliant systematic review multiple databases were searched including clinical studies in which acute or chronic PJI after TAR was diagnosed and treated. Data were harvested regarding the cohort, the study design, the diagnostic criteria adopted and the surgical treatment for PJI. The methodological

Periprosthetic joint infection (PJI) remains one of the most challenging complications following total joint arthroplasty, contributing to major morbidities, increased health care costs, and high failure rates of conventional treatments. The gold standard for chronic PJI treatment, the two-stage revision, is associated with prolonged hospitalizations, high attrition rates, and variable success rates, necessitating novel approaches to improve patient outcomes. Despite advancements in surgical techniques, infection control methods, and antibiotic therapies, the incidence and success rates of PJI

PURPOSE OF REVIEW: Stress urinary incontinence is a growing issue in ageing men, often following treatment for prostate cancer or bladder outflow obstruction. While implantable urological devices offer relief, infections are a significant concern. These infections can lead to device removal, negating the benefits and impacting patient outcomes. This review explores the risks and factors contributing to these infections and existing strategies to minimize them. These strategies encompass a multifaceted approach that considers patient-specific issues, environmental issues, device design and surg

BACKGROUND: Prosthetic valve endocarditis (PVE) carries high mortality and morbidity as compared to native valve endocarditis (NVE). Contemporary data on PVE are lacking, we aimed to study contemporary trends, outcomes, and burden of PVE using nationally representative data.
METHODS: We used the National Inpatient Sample from 2000 to 2017 to identify patients admitted with PVE using ICD-9-CM and ICD-10 codes. Risk-adjusted rates were calculated using an Analysis of Covariance (ANCOVA) with the Generalized Linear Model (GLM). Trends were assessed with linear regression and Pearson's Chi-square

PURPOSE OF REVIEW: The purpose of this review is to critically analyze and summarize recent studies in the area of penile prosthesis surgery outcomes with a focus on infection prevention in high-risk patients.
RECENT FINDINGS: Reduction of surgical time in complex prosthesis surgery may reduce infection risk. Concomitant implant surgery is not associated with increased infection risk. Certain immunocompromised patients may be more likely to have penile implant infections, but these may not include patients with well-controlled HIV, well-controlled diabetes, or transplant recipients. OBJECTIVE: To detail our early experience and technique of a modified two-stage reimplantation protocol using antibiotic-loaded articulating cement spacers (ALACSs) for treatment of late periprosthetic infection after total knee arthroplasty (TKA).
METHODS: From January 2006 to February 2009, a series of 21 patients (21 knees) with late infected TKAs were treated by radical debridement and removal of all components and cement, and then articulating spacers were implanted using antibiotic-impregnated bone cement. For this purpose, 4 g vancomycin powder was mixed with per 40 g cement. Graduated

We report 3 cases of Brucella melitensis infection of prosthetic hips and knees, and we summarize data about 4 cases reported in the literature. Six of the 7 affected patients were men. The median duration from prosthesis implantation to the onset of symptoms was 38.7 months. Five patients had only local symptoms. Preoperative joint aspirates yielded negative culture results for 3 patients, and blood culture results were negative for 6 patients. Excisional arthroplasty was the initial intervention for 3 patients. Three others responded well to medical therapy alone. One patient had relapse whi

BACKGROUND AND OBJECTIVES: Antibiotic-impregnated shunts seem to be beneficial in preventing bacterial infections and decreasing mortality by effectively inhibiting microbial growth in the shunt system and reducing the risk of shunt-associated infections. This study aimed to evaluate the efficacy of antibiotic-impregnated shunt catheters (AISC) in reducing the incidence of bacterial shunt infection in patients with hydrocephalus.
METHODS: The protocol was registered on PROSPERO. A meta-analysis was conducted by searching 3 databases (PubMed, Scopus, CENTRAL) for relevant randomized controlled

Non-typhoid Salmonellae (NTS) commonly cause gastroenteritis but are rarely found pathogens in prosthetic heart valve endocarditis. The details of two patients from the authors' institution and 15 published cases are reviewed in terms of their risk factors, clinical findings and outcomes. Only two of eight patients with paravalvular leakage or abscess--the most serious local complications--survived, both with surgery. It appears that NTS bacteremia in patients with prosthetic valves and concomitant risk factors should be treated early with high-dose antimicrobials for up to six weeks in order

Recurrent anophthalmic socket infections resulted in spontaneous extrusion of a spherical glass implant filled with sequestered fluid. Orbital imaging had revealed a fluid collection within the implant. While rarely used in developed countries, these implants are commonly used around the world due to their low cost and success in anophthalmic socket rehabilitation. Infection within an anophthalmic socket should alert the practitioner to the possibility of sequestered infectious fluid within these types of implant through a fractured breach.

It has been postulated that bacteria attached to the surface of prosthetic hip joints can cause localised inflammation, resulting in failure of the replacement joint. However, diagnosis of infection is difficult with traditional microbiological culture methods, and evidence exists that highly fastidious or non-cultivable organisms have a role in implant infections. The purpose of this study was to use culture and culture-independent methods to detect the bacteria present on the surface of prosthetic hip joints removed during revision arthroplasties. Ten consecutive revisions were performed by
