The effects of diaphragmatic breathing learning on chest wall motion, mechanical efficiency of the respiratory muscles, breathing pattern, and dyspnea sensation were studied in seven patients with severe chronic obstructive pulmonary disease (COPD) (FEV1 34 +/- 7% of the predicted value) during loaded and unloaded breathing. Chest wall motion was studied focusing on amplitude and phase relation of rib cage and abdominal motion. Mechanical efficiency was defined as the ratio of added external power output and added oxygen consumption during inspiratory threshold loading (40% maximal inspiratory

In recent years, several techniques of regional anesthesia have been proposed to provide analgesia to the anterior thoracic cage; notably, most of these techniques require a parasternal approach. However, in this context, the potential role of a more common and well-established technique, namely the modified pectoral nerve block (known as PECS II block), has been poorly investigated. Here, we describe a case involving a patient with bilateral anterolateral multiple rib fractures associated with sternum fracture, who was successfully treated using bilateral PECS II blocks. Our experience indica

The aim of the present study is to control the hypothesis that the rib hump deformity can be adequately corrected when applying vertebral derotation. We retrospectively studied patients treated with full pedicle screw systems (group A), hybrid constructs (group B), and Harrington rod instrumentation (group C). No costoplasties were performed in the patients included in our study. Derotation was applied in groups A and B. The rib hump deformity was assessed on lateral radiographic studies by rib index (RI). Of the 72 patients that were finally included in our study, 30 patients (24 females and

Experimental scoliosis with the potential for marked progression was treated by rib resection on the concave side of the curve, and the alterations of the rib cage and vertebrae in the transverse plane were investigated. Twenty-four chickens were divided into four equal groups (groups R, P, PR, and C) and pinealectomy was performed at 3 days of age in groups P and PR. In group R, three unilateral ribs were resected at the age of 4 weeks. In group PR, three ribs on the concave side of scoliosis were resected at 4 weeks of age if scoliosis of > 20 degrees developed before the age of 4 weeks. Gro

OBJECTIVE: To assess thoracoabdominal asynchrony (TAA) and inspiratory paradoxical motion at different positionings in subjects with Duchenne muscular dystrophy (DMD) versus healthy subjects during quiet spontaneous breathing (QB) and cough.
METHODS: This is a case control study with a matched-pair design. We assessed 14 DMD subjects and 12 controls using optoelectronic plethysmography (OEP) during QB and spontaneous cough in 3 positions: supine, supine with headrest raised at 45°, and sitting with back support at 80°. The TAA was assessed using phase angle (θ) between upper (RCp) and lower r

PURPOSE: The spinal penetration index (SPI) quantifies the portion of the rib cage occupied by vertebrae. When measured by computed tomography (CT) or magnetic resonance imaging, SPI can only be determined in the reclining position, which modifies spinal and thoracic morphology. CT results in high radiation exposure. The authors studied rib cage and spinal morphology using low-dose biplanar stereoradiography and their impact on respiratory function in adolescent idiopathic scoliosis (AIS).
METHODS: In eighty thoracic AIS patients, a slot-scanning radiologic device allowing simultaneous acquis

Transferrin is an important growth-promoting serum glycoprotein synthesized chiefly in the liver in adults. The transferrin found in the mouse foetus is thought to be wholly a product of the foetus itself and its synthesis starts at lest as early as the 7th day of gestation. The major sites of synthesis in mouse foetuses are the visceral yolk sac (VYS) and liver (Adamson, 1982). We now report that other murine foetal tissues synthesize readily detectable amounts, namely lung, spleen, spinal cord and rib cage. Very low levels are also synthesized by the brain, muscle and pancreas. We can detect

Management of pectus excavatum Congenital chest wall deformities are considered to be anomalies in chest wall growth related to excessive or insufficient growth (aplasia or dysplasia) of the osseous or cartilaginous structures or the rib cage. These deformities are associated in 95% to overgrowth leading into depression (pectus excavatum) or protuberance of the sternum (pectus carinatum). Pectus excavatum may lead to cardiopulmonary disorder or impact on patient's self-esteem. The management of pectus excavatum has evolved this last decade with minimal invasive repair.

Thoracic spine fractures have unique characteristics, and should be managed with specific criteria that are different from those used for thoracolumbar injuries. Fracture-dislocation injuries require high-energy injury, and should always be suspected in polytrauma patients with rib cage, sternum, cardiac, or pulmonary injuries. Although treatment is individualized, multisegmental posterior fixation sometimes combined with anterior decompression, is most commonly used. The authors review the current literature on this topic, and present their opinion on the management of such injuries.

BACKGROUND: Currently, the standard treatment for multiple rib fractures is still under debate. This study explores the efficacy of three treatments (thoracoscopic internal fixation, open surgical internal fixation and conservative treatment) for multiple rib fractures and their influence on thoracic cavity volume and postoperative rehabilitation.
METHODS: Patients with multiple rib fractures admitted to Weifang People's Hospital of Shandong Province from March 2019 to March 2021 were enrolled in this study. The thoracic volume changes before and after treatment were measured by three-dimensi

A 50-year-old woman presented with mid-thoracic back pain that radiated to the left rib cage in a radicular manner. Magnetic resonance imaging revealed an extradural mass compressing the left T9 nerve root. The mass was adjacent to a degenerated T9-10 facet joint with an enhancing low signal intensity rim. Recognition of the characteristic imaging findings of an intraspinal synovial cyst resulted in the correct preoperative diagnosis despite the unusual location of this lesion at T9-10.

Jarcho-Levin syndrome (JLS) is a rare, congenital disorder, inherited in an autosomal recessive pattern, that represents a spectrum of clinical and radiographic abnormalities of the spine and chest. The present case report discusses two siblings, an 11 year old girl and a 6 year old boy, diagnosed as cases of JLS based on family history and clinical-radiological findings. The main features of the syndrome are shortness of stature with a spinal abnormality, multiple abnormal vertebral defects, and a small malformed "fan-like" or "crab-like" rib cage due to posterior fusion and anterior flaring

We have previously shown that flail chest in the dog causes an inspiratory inward displacement of the ribs and an increased inspiratory activity in the external intercostal muscles, and we have speculated that this increased activity is due to an increased spindle afferent activity. The present studies were designed to test this hypothesis. Twenty-nine supine anesthetized dogs were studied, and flail was produced surgically by fracturing ventrally and dorsally two to four contiguous ribs on the right side of the chest. Although flail elicited an increased inspiratory activity in the external i

The double S shape of the vertebral column is one of the most important evolutionary adaptations to human bipedal locomotion, providing an optimal compromise between stability and mobility. It is commonly believed that a six element long lumbar spine facilitated the critical adoption of lumbar lordosis in early hominins, which contrasts with five lumbars in modern humans and four in chimpanzees and gorillas. This is mainly based on the juvenile Homo erectus skeleton KNM-WT 15000 from Nariokotome, Kenya. Yet, the biomechanical advantage of a long lumbar spine is speculative. Here we present new

Flail chest is a life-threatening condition that occurs when 3 or more consecutive ribs are segmentally fractured due to severe trauma and become detached from the rest of the thoracic cage. Flail chest is usually associated with other intrathoracic injuries, including pulmonary contusion, which can result in respiratory failure. We present a case of a 44-year-old man who was hit by a truck and a forklift resulting in multiple rib fractures bilaterally, and bilateral hemopneumothorax along with left chest wall depression and severe flail chest. The Nuss procedure was performed for both stabili

This review summarizes the anatomic and biomechanical features of the thoracic spine, which are different from those of the more mobile segments of the spine, and emphasizes their role in trauma. The distinguishing characteristics of the thoracic spine are the presence of the ribs and their articulations. The rib cage restricts motion and adds stiffness to the spine. During trauma, it provides the thoracic spine with additional strength and energy-absorbing capacity. Above the T10 level, most injuries produce a basic pattern consisting of an anterior fracture-dislocation involving two contiguo

We examined 10 fetuses with nuchal cysts and compared the findings with 32 cases from the literature. Based on their characteristics, we propose that nuchal cysts are signs of four causally and pathogenetically different entities: cystic nuchal blebs present in otherwise normal fetuses as a postmortem change; 45,X fetuses who have a particular appearance and multiple congenital malformations; an apparently autosomal recessive syndrome of multiple cysts (that extend into deep muscular planes), generalized edema, cleft palate, peculiar skeletal characteristics, acutely angulated ribs (producing

The present report aims to describe the case of a duodenal obstruction ileus in a dairy cow that was caused by a cage magnet. The 4.7-year-old German Fleckvieh cow was hospitalized because of symptoms of intestinal obstruction such as anorexia, noticeable drop in milk yield, reduced defecation, dehydration and positive percussion and swinging auscultation in a circumscribed area cranial of the right flank over the last 2 ribs. Six months as well as 3 days prior to hospitalization the cow had already been treated for signs of hardware disease, which included administration of a cage magnet.Afte

A 37-year-old woman, who presented with low grade fever and productive cough, was admitted for evaluation of an abnormal shadow on chest X-ray film. On physical examination, she had bilateral hallux valgus and funnel chest, the center of which was at the fifth rib on the right edge of the sternum. Since chest CT scan, selective bronchography, pulmonary arteriography and aortography revealed that she had pulmonary sequestration in the right cardiophrenic region associated with localized cystic bronchiectasis in the right S7b, she received right lower lobectomy. BACKGROUND: Thoracoabdominal hernias (TH), encompassing abdominal wall hernias such as flank hernias and diaphragmatic hernias, are rare conditions with poorly defined epidemiology and no standardized surgical strategy. Repair is technically challenging because it requires reconstruction across distinct anatomical structures, including the diaphragm, ribs, and abdominal wall. Optimal repair methods remain controversial.
CASE PRESENTATION: A 70-year-old man with a history of descending aortic aneurysm repair presented with a progressive left flank bulge. Computed tomography revealed a large fl
