Influence of Protective Face Coverings on the Speech Recognition of Cochlear Implant Patients.

OBJECTIVES: The objectives were to characterize the effects of wearing face coverings on: 1) acoustic speech cues, and 2) speech recognition of patients with hearing loss who listen with a cochlear implant. METHODS: A prospective cohort study was performed in a tertiary referral center between July and September 2020. A female talker recorded sentences in three conditions: no face covering, N95 mask, and N95 mask plus a face shield. Spectral differences were analyzed between speech produced in each condition. The speech recognition in each condition for twenty-three adult patients with at lea

Speech recognition under conditions of frequency-place compression and expansion.

In normal acoustic hearing the mapping of acoustic frequency information onto the appropriate cochlear place is a natural biological function, but in cochlear implants it is controlled by the speech processor. The cochlear tonotopic range of the implant is determined by the length and insertion depth of the electrode array. Conventional cochlear implant electrode arrays are designed for an insertion of 25 mm inside the round window and the active electrodes occupy 16 mm, which would place the electrodes in a cochlear region corresponding to an acoustic frequency range of 500-6000 Hz. However,

Cognitive and Structural Correlates of Conversational Speech Timing in Mild Cognitive Impairment and Mild-to-Moderate Alzheimer's Disease: Relevance for Early Detection Approaches.

Background: Increasing efforts have focused on the establishment of novel biomarkers for the early detection of Alzheimer's disease (AD) and prediction of Mild Cognitive Impairment (MCI)-to-AD conversion. Behavioral changes over the course of healthy ageing, at disease onset and during disease progression, have been recently put forward as promising markers for the detection of MCI and AD. The present study examines whether the temporal characteristics of speech in a collaborative referencing task are associated with cognitive function and the volumes of brain regions involved in speech produc

Effects of age on concurrent vowel perception in acoustic and simulated electroacoustic hearing.

PURPOSE: In this study, the authors investigated the effects of age on the use of fundamental frequency differences (ΔF(0)) in the perception of competing synthesized vowels in simulations of electroacoustic and cochlear-implant hearing. METHOD: Twelve younger listeners with normal hearing and 13 older listeners with (near) normal hearing were evaluated in their use of ΔF(0) in the perception of competing synthesized vowels for 3 conditions: unprocessed synthesized vowels (UNP), envelope-vocoded synthesized vowels that simulated a cochlear implant (VOC), and synthesized vowels processed to si

Follow-up study of a right- and a left-hemispherectomized child: implications for localization and impairment of language in children.

Two hemispherectomized girls, one operated on the right, the other on the left, were followed from time of surgery until 9 and 10 years of age and compared with respect to course of language acquisition following surgery. At conclusion of follow-up, receptive and expressive language, phoneme perception and production, and sentence processing of the two hemispherectomized children were compared with those of two control groups of similar age, one developing language normally, the other language-impaired. The left-hemispherectomized child's abilities were similar to those of the language-impaire

Pure tone and speech audiometry of presbyacusis in a tertiary hospital in Kaduna, Nigeria: a cross-sectional study.

INTRODUCTION: presbyacusis is age-related hearing loss, manifesting as a bilateral symmetrical sensorineural hearing loss of adult-onset. It has different audiometric configurations and is associated with a decline in speech discrimination sensitivity. The objectives of the study were to determine the degree of hearing loss; types of presbyacusis; and speech discrimination score (SDS) of presbyacusics and to compare the SDS in different types of presbyacusis in patients seen at a tertiary hospital in Kaduna, Nigeria. METHODS: a descriptive cross-sectional study, involving 41 presbyacusics see

Effects of varying unilateral conductive hearing losses on speech-in-noise discrimination: an experimental study with implications for surgical correction.

HYPOTHESIS: Unilateral conductive hearing loss (HL) causes measurable disability in realistic hearing environments. The benefits of improving this loss depend both on the absolute level of final hearing and on hearing asymmetry. BACKGROUND: Surgical success is often measured by the degree of change in the air-bone gap or in the air thresholds. The Glasgow Benefit Plot and the Belfast Rule of Thumb suggest that a given hearing gain will be of greater benefit if the postoperative thresholds in the worse ear are a <30-dB HL or within 15 dB of the contralateral ear. This assertion has important s

Audiological correlates of tumor parameters in acoustic neuromas.

The objective of this study was undertaken to evaluate a correlation of pure tone audiometry, speech audiometry with tumor size, the presence of intrameatal extension, contralateral pure tone audiometry, and contralateral speech audiometry results at acoustic neuroma (AN) patients. Seventy-six patients with a unilateral-cerebellopontine angel tumor with/without internal acoustic canal (IAC) were included in this study. Data on pure tone audiometry, speech discrimination scores (SDS), speech reception thresholds (SRT), tumour size, and extension to IAC were analyzed retrospectively. As a result

Cochlear implantation in children with Waardenburg syndrome.

Waardenburg syndrome is an autosomal-dominant trait resulting from mutations occurring in different genes. It is often characterized by varying degrees of: congenital hearing loss; dystopia canthorum; synophrys; broad nasal root; depigmentation of hair (white forelock), skin or both; and heterochromic or hypochromic irides. A retrospective case study was done to assess speech perception, speech production, general intelligence and educational setting in six profoundly hearing-impaired children with Waardenburg syndrome (four with type I, one with type II and one with type III) ranging in age f

Temporal envelope perception in dyslexic children.

Speech intelligibility depends heavily on the accurate perception of auditory temporal envelope cues, that is the slower amplitude modulations present in the speech waveform. In a previous study, McAnally and Stein demonstrated that dyslexics may show impaired audibility (i.e. detectability) of these envelope cues. In the present psychophysical study, the ability to process temporal envelope cues was further investigated in dyslexic children by measuring detection thresholds of sinusoidal amplitude-modulation (SAM) and discrimination thresholds of SAM depth and SAM rate. Each threshold was mea

Temporal analysis in normal and impaired hearing.

The ear contains an array of filters that separate the components of a complex signal into "channels" tuned to different center frequencies. Temporal analysis can be considered as two processes: analysis of the time pattern occurring within each channel, and comparison of the time patterns across channels. Within-channel acuity can be characterized by tasks such as gap detection, or by the ability to detect amplitude modulation as a function of modulation rate. The smallest detectable gap duration for a white noise stimulus is 2-3 ms. The results can be modeled by an array of filters, with eac

Fitting prelingually deafened adult cochlear implant users based on electrode discrimination performance.

OBJECTIVE: This study investigated the hypotheses that (1) prelingually deafened CI users do not have perfect electrode discrimination ability and (2) the deactivation of non-discriminable electrodes can improve auditory performance. DESIGN: Electrode discrimination difference limens were determined for all electrodes of the array. The subjects' basic map was subsequently compared to an experimental map, which contained only discriminable electrodes, with respect to speech understanding in quiet and in noise, listening effort, spectral ripple discrimination and subjective appreciation. Active middle ear implant after lateral petrosectomy and radiotherapy for ear cancer.

BACKGROUND: Tumor of the temporal bone is a rare disease with a very poor prognosis. Surgery and postoperative radiotherapy are usually the recommended treatments for squamous cell carcinoma (SCC) of the external and middle ear, which may cause conductive hearing loss. The purpose of this study was to evaluate the audiologic results and compliance of active middle ear implant (AMEI) and establish the feasibility of the procedure in a patient treated for middle ear cancer. METHODS: A 73-year-old patient treated with lateral petrosectomy, neck dissection, reconstruction/obliteration by pedicled

Speech and language development in a population of Swedish hearing-impaired pre-school children, a cross-sectional study.

OBJECTIVE: There is little information on speech and language development in pre-school children with mild, moderate or severe hearing impairment. The primary aim of the study is to establish a reference material for clinical use covering various aspects of speech and language functions and to relate test values to pure tone audiograms and parents' judgement of their children's hearing and language abilities. METHODS: Nine speech and language tests were applied or modified, both classical tests and newly developed tests. Ninety-seven children with normal hearing and 156 with hearing impairmen

Comparison of middle latency responses in presbycusis patients with two different speech recognition scores.

OBJECTIVE: The purpose of this study is to evaluate whether the middle latency responses (MLR) can be used for an objective differentiation of patients with presbycusis having relatively good (Group I) and relatively poor speech recognition scores (Group II). METHODS: All the participants of these groups had high frequency down-sloping hearing loss with an average of 26-60 dB HL. Data were collected from two described study groups and a control group, using pure tone audiometry, monosyllabic phonetically balanced word and synthetic sentence identification, as well as MLR. The role of the striatum in phonological processing: evidence from early stages of Huntington's disease.

The linguistic role of subcortical structures such as the striatum is still controversial. According to the claim that language processing is subdivided into a lexical memory store and a computational rule system (Pinker, 1999) several studies on word morphology (e.g., Ullman et al., 1997) and on syntax (e.g., Teichmann et al., 2005) have suggested that the striatum is specifically dedicated to the latter component. However, little is known about whether the striatum is involved in phonological operations and whether its role in linguistic rule application generalizes to phonological processin
