[Vertical Talus: Mid-Term Results of Surgical Therapy].

PURPOSE OF THE STUDY: The aim of the study was to clinically and radiographically evaluate the results of a two-stage surgical technique used in our department to treat congenital vertical talus. MATERIAL AND METHODS: In the years 1990-2010 we treated eight patients (nine feet). We used a two-stage surgical technique, with lengthening of the dorsiflexor muscles of the leg and the tibialis anterior muscle at the first stage. This was followed by cast fixation with the ankle in plantar flexion for 6 weeks. At the second stage, through a Cincinnati approach, the talonavicular and calcaneocuboid

Bilateral talus fracture dislocation: is avascular necrosis inevitable?

Bilateral simultaneous fracture dislocation of the talus is a very rare injury. It occurs usually following high-velocity injuries. We report a 45-year-old man who presented with bilateral talar fracture dislocation following a heavy stone falling on him. On his right side, he had open talus dislocation with lateral process fracture and on his left side he had closed talar neck fracture dislocation. He underwent immediate debridement and external fixator application for his right side and open reduction and internal fixation for his left talus. At the end of 34 months' follow-up, he was able t

Open total talar dislocation with extrusion (missing talus).

Open total dislocation of a talus with extrusion (missing talus) is an extremely rare injury. We present a case of a 63-year-old male who sustained an open total lateral dislocation of the right talus with extrusion and without a concomitant fracture after a fall from a small height. Tibiocalcaneonavicular arthrodesis was performed. Arthrodesis failed, and due to painful and unstable ankle, rearthrodesis was performed two years later, successefully. Because of rarity of this kind of injury, hers importance for foot function, previously described various treatment options and absence of guideli

Traumatic talus extrusion: case reports and literature review.

Total talar dislocation is a rare injury, which is usually open and with associated fractures. We report two cases of open anterolateral talar extrusions following high-energy traffic accidents. Both were treated with wound debridement, joint irrigation, closed reduction and external fixation. At 12 months follow-up, both patients had pain-free motion of the ankle without any signs of avascular necrosis or arthritis.

Autosomal dominant transmission of isolated congenital vertical talus.

We report vertical transmission of isolated congenital vertical talus through three generations of a Honduran family. Five of nine affected family members were examined and the diagnosis was confirmed radiographically. There was incomplete penetrance in one clinically unaffected woman with two affected children. Bilateral and unilateral involvement was seen with a wide range of severity. Based on this family and on cases reviewed from the literature, we propose that isolated congenital vertical talus can be inherited as an autosomal dominant trait with variable expression and incomplete penetr

Surgical management of traumatic talus defects: A comprehensive review.

The talus is a critical bone in the foot, located between the tibia and calcaneus, playing a key role in weight-bearing and force transmission. It forms a stable structure with the tibia and fibula via the ankle joint and helps maintain foot flexibility with the calcaneus and navicular through the subtalar joint. Due to its cartilage-covered surface and lack of direct muscle attachment, its blood supply mainly depends on surrounding soft tissue. As a result, traumatic injuries can disrupt blood flow, leading to ischemic bone necrosis and significant impairment of foot function. Traumatic talus

Arthroscopic Humeral Head Hill-Sachs Grafting With Talus Allograft.

The presence of a Hill-Sachs lesion is a known risk factor for recurrent shoulder instability. Many procedures have been described for the treatment of off-track Hill-Sachs lesions; however, each of these techniques is fraught with potential disadvantages and complications. In this Technical Note and accompanying video, we describe our technique for arthroscopic treatment of a Hill-Sachs lesion with talus osteochondral allograft. This technique recreates native humeral head anatomy with a highly congruent graft without altering the biomechanics of the glenohumeral joint.

Diagnostic yield of exome sequencing in congenital vertical talus.

BACKGROUND: Congenital vertical talus (CVT), also known as "rocker-bottom foot", is a rare foot deformity associated with a dislocation of the talonavicular joint. Although genetic causes of CVT have been described in single isolated and syndromic families, whole-exome sequencing (WES) of large cohorts have not yet been reported. METHODS: In this study, 62 probands with CVT were evaluated for likely causative single nucleotide variants (SNVs) and copy number variants (CNVs) using WES. Segregation of variants within families was determined by Sanger sequencing. RESULTS: In this cohort, CVT oc

Flat-top talus after clubfoot treatment: a literature review.

Flat-top talus (FTT) is a complication well-known to those treating clubfoot. Despite this, it receives little attention in textbooks, so our knowledge and understanding can be limited to anecdote and hearsay. In particular, its association with different treatments, especially the Ponseti method, is not well understood. This review provides some clarity on the possible pathoanatomical, radiological and iatrogenic causes of FTT. It summarises its prevalence following different treatments. It is seen after surgery, 'pre-Ponseti' casting and the Ponseti method, and seems least common following P

Long-Term Outcome After Operative Management of Talus Fractures.

BACKGROUND:: Controversies remain regarding the preferred treatment strategy for talus fractures. The primary goal of this study was to evaluate the long-term outcome after operative management of talus fractures. Secondarily, we identified those factors that affected the outcome and defined strategies to improve the outcome. METHODS:: This is a retrospective outcome study of 84 patients with an average follow-up time of 9.1 years. We assessed the functional results, return to daily activities, and general health status using the Foot Function Index-5pt, a numeric rating scale for pain, and t

Stem cell therapy in early post-traumatic talus osteonecrosis.

PURPOSE: Avascular necrosis of the talus is one of the most notable complications associated with talar neck fractures with frequent evolution of the osteonecrosis into a difficult arthrodesis. We tested whether the injection of bone marrow mesenchymal stem cells (MSCs) could improve the repair process of the osteonecrosis. MATERIAL AND METHODS: Forty-five early (without collapse) post-traumatic talus osteonecroses (group 1; study group) were treated between 1995 and 2012 with percutaneous injection of progenitor cells (autologous bone marrow concentrate from the iliac crest). The number of M

Percutaneous reduction and screw fixation of fracture neck talus.

BACKGROUND: Fracture neck talus is a rare fracture represents about 1% of all fractures and usually due to high energy trauma. These fractures are usually associated with compromised soft tissues, concomitant skeletal fractures, or life threatening injuries. Talus has a tenuous blood supply which is affected by fracture displacement. Urgent fracture reduction±fracture fixation is mandatory. The associated injuries may make the conventional open reduction and internal fixation is impossible to be done in urgent base as it may impacts the already tenuous blood supply of talus increasing the risk

High-grade intramedullary osteosarcoma of the talus: case report.

This is a presentation of a rare malignant tumor that uncommonly affects the bones of the foot. The following case involves a malignancy which usually presents during childhood or adolescence presenting much later in life. Physicians treating the foot and ankle must include osteosarcoma in their list of differential diagnoses to exclude when vague symptoms involving deep pain and edema present. It is critical to prevent misdiagnosis and delayed treatment with this form of tumor.

Congenital vertical talus in arthrogryposis and other contractural syndromes.

No practical classification system exists to identify which patients may have successful outcome following treatment of congenital vertical talus in arthrogryposis. We classified 229 patients into five distinct groups: Group I had amyoplasia or so-called classic arthrogryposis, Group II had distal arthrogryposis, Group III had a specific syndrome as a diagnosis, Group IV had severe systemic or neurologic involvement, and Group V had unclassifiable contracture syndromes. No patient with amyoplasia (Group I) had congenital vertical talus. The congenital vertical talus seen in distal arthrogrypos

[Talus fractures--fractures of the most important tarsal bone].

The talus is the key bone of the foot due to its location between the ankle and the subtalar joints. Through the flexion and extension of the ankle joint, the talus is the "propulsive bone" situated at the root of the first ray and the hallux. Through the subtalar and talo-navicular joints, the talus allows the foot to be "suspended" using a fancy spring mechanism involving ligaments and tendons. The talus thus transmits forces through two important joints without any direct muscular constraint. The talo-calcaneo-navicular joint may be called the "coxa pedis" due to the anatomical and function

Congenital vertical talus. The results of staged operative reduction.

Congenital vertical talus was diagnosed in 15 feet of 10 children, and was treated by operative reduction. Forefoot deformity was corrected first, using anterolateral soft-tissue release on 11 feet, and manipulation alone in four feet. After prolonged immobilisation in plaster the affected feet had posterior release at the ankle and elongation of the calcaneal tendon. Clinical and radiographic examination at follow-up 15 months to 21 years later showed that a satisfactory outcome had been achieved in 12 of the 15 feet.

All Arthroscopic Decompression and Bone Grafting of Cyst in the Talus.

INTRODUCTION: A talar cyst is a fluid-filled sac in the talus bone, often caused by chronic stress fractures or traumatic injuries. A talar cyst if left untreated can result in chondromalacia of talus cartilage and ultimately arthritis. A timely intervention of cyst can have very good prognosis. An open cyst decompression and bone grafting is a standard procedure followed, but it is more invasive and sometimes involves medial malleolus osteotomy. If the same is done by arthroscopy, the morbidity of open procedure can be avoided. CASE REPORT: A 34-year-old male complaining of pain in the ankle
