Posttraumatic Compressive Neuropathy of the Deep Motor Branch of the Ulnar Nerve Caused by Heterotopic Ossification.

We report a case of posttraumatic compressive neuropathy of the deep motor branch of the ulnar nerve occurring in the setting of heterotopic ossification following a direct blow to the hypothenar eminence. Over several weeks, the patient developed ring and little finger claw deformities and atrophy of his first dorsal interosseous and adductor pollicis muscles with sparing of sensation. Electromyography and nerve conduction study localized the area of injury, and computed tomography confirmed the presence of heterotopic bone near the deep motor branch of the ulnar nerve. Intraoperatively, the

Long-term outcomes in patients with ulnar neuropathy at the elbow treated according to the presumed aetiology.

OBJECTIVES: Ulnar neuropathy at the elbow (UNE) consists mainly of two conditions: entrapment under the humeroulnar aponeurosis (HUA) and extrinsic compression in the retrocondylar (RTC) groove. These in our opinion need different treatment: surgical HUA release and avoidance of inappropriate arm positioning, respectively. We treated our UNE patients accordingly, and studied their long-term outcomes. METHODS: We invited our cohort of UNE patients to a follow-up examination consisting of history, neurological, electrodiagnostic (EDx) and ultrasonographic (US) examinations performed by four bli

High resolution sonographic examination: a newer technique to study ulnar nerve neuropathy in leprosy.

OBJECTIVES: A prospective case control study was conducted to calculate normal dimensions of ulnar nerve and study the size, echotexture and morphologic alterations in ulnar nerve in patients with leprosy. METHODS: The study group included 48 patients diagnosed with leprosy on basis of clinical, bacteriologic and/or histopathologic evaluation. Sonographic measurements were taken at 3 levels around elbow. The morphology and vascularity were also studied. Control group consisted of 60 clinically normal ulnar nerves, on which ultrasound was performed to calculate normal parameters. 96 Ulnar nerv

MRI shows thickening and altered diffusion in the median and ulnar nerves in multifocal motor neuropathy.

OBJECTIVES: To study disease mechanisms in multifocal motor neuropathy (MMN) with magnetic resonance imaging (MRI) and diffusion tensor imaging (DTI) of the median and ulnar nerves. METHODS: We enrolled ten MMN patients, ten patients with amyotrophic lateral sclerosis (ALS) and ten healthy controls (HCs). Patients underwent MRI (in a prone position) and nerve conduction studies. DTI and fat-suppressed T2-weighted scans of the forearms were performed on a 3.0T MRI scanner. Fibre tractography of the median and ulnar nerves was performed to extract diffusion parameters: fractional anisotropy (FA

The Ulnar Nerve After Surgical Transposition: Can Sonography Define the Reason of Persisting Neuropathy?

PURPOSE: Ulnar nerve neuropathy is mainly caused by compression at the level of the cubital tunnel. Two main approaches are currently known for the surgical treatment of this condition: decompression of the nerve in its usual position or transposition to the ulnar flexor side. This study was performed to define the usefulness of ultrasound in patients with persisting symptoms after ulnar nerve transposition. MATERIALS AND METHODS: We present the data of 8 subjects with persisting symptoms after nerve transposition due to compressive neuropathy. The cross-section areas (CSA) and texture change

Ulnar neuropathy caused by a widespread synovial cyst of the elbow joint in a patient with rheumatoid arthritis.

A 53-year-old man with rheumatoid arthritis presented with radiating pain, numbness, and diminished motor strength in the right hand according to the ulnar nerve functions. Magnetic resonance imaging and peripheral nerve ultrasound revealed a widespread cystic lesion descending from the elbow joint along the ulnar nerve over a length of 8 cm. After relapse under a therapeutic attempt with antirheumatic drugs, neurosurgical exploration was done using peripheral nerve ultrasound. A synovial cyst of the elbow was extirpated as a whole with subsequent anterior synovectomy. The postoperative course

Subluxation-related ulnar neuropathy (SUN) syndrome related to distal radioulnar joint instability.

Ulnar neuropathy coexistent with distal radioulnar joint (DRUJ) instability has previously been observed in our practice. The aim of this study was to define this phenomenon and investigate the hypothesis that the cause of this intermittent, positional ulnar neuropathy is related to kinking of the ulnar nerve about the DRUJ. Ulna neuropathy was present in 10/51 (19.6%) of a historical cohort of patients who presented with DRUJ instability. Nine subsequent patients with DRUJ instability and coexistent ulnar neuropathy underwent 3-T magnetic resonance imaging to better understand the mechanism o

[Evaluation of digital vibration threshold for asymptomatic ulnar neuropathy in patients with carpal tunnel syndrome].

Quantitative assessment of vibration threshold at 125 Hz of index and little finger tips together with measurements of median and ulnar sensory nerve conduction velocity (SNCV) was performed for 25 limbs of 15 patients with carpal tunnel syndrome (CTS), 25 limbs of 13 disease controls with neurological symptoms similar to CTS and the nearly same number of limbs of normal controls. There was no significant difference in ulnar SNCV among 3 groups. However, ulnar SNCV at wrist was significantly reduced in patients with CTS as compared with normal value. In 17 limbs with CTS, vibration threshold o

Subacute High-Grade Ulnar Neuropathy Without Trauma: A Case of Cubital Tunnel Syndrome.

Cubital tunnel syndrome (CuTS) typically presents gradually, and an acute overnight onset of severe ulnar neuropathy is rare. A 46-year-old female recreational pickleball player awoke with a right-hand claw deformity and sensory loss without preceding symptoms or trauma. Examination showed intrinsic weakness and decreased ulnar sensation; electrodiagnostic studies revealed subacute on chronic axonal and demyelinating changes at the elbow consistent with moderate CuTS. She underwent cubital tunnel release one week after onset, with rapid sensory improvement but delayed motor recovery. This case

Incidence and Risk Factors of Ulnar Neuropathy After the Surgical Treatment of Distal Humeral Fractures.

BACKGROUND: One of the major complications associated with open reduction and internal fixation (ORIF) for distal humeral fractures is ulnar neuropathy. However, varying reports on its incidence have led to inconsistent opinions. This study aimed to investigate the incidence of ulnar neuropathy in patients who underwent open reduction and internal fixation using two precontoured anatomical locking plates for distal humeral fractures at our hospital. Additionally, we aimed to examine the symptoms and progression of ulnar neuropathy in these cases. METHODS: Among patients who underwent surgery

Predictors of Disease Severity for Ulnar Neuropathy at the Elbow: Analysis from a Randomized Clinical Trial.

BACKGROUND: Ulnar neuropathy at the elbow (UNE) is the second most common nerve compression in the upper extremity. Painful paresthesias in fingers progress to muscle weakness and atrophy, impairing activities of daily living. Despite the common occurrence and effects on quality of life, risk factors that predispose to UNE and contribute to disease severity have not been thoroughly investigated. METHODS: Patients enrolled in the Clinical Trial for Surgery of the Ulnar Nerve at the Elbow (SUN) were included. Data on patient demographics, distance traveled, disease duration, and comorbidities c

Core outcomes in nerve surgery: development of a core outcome set for ulnar neuropathy at the elbow.

OBJECTIVE: Ulnar neuropathy at the elbow (UNE) is common, affecting 1%-6% of the population. Despite this, there remains a lack of consensus regarding optimal treatment. This is primarily due to the difficulty one encounters when trying to assess the literature. Outcomes are inconsistently reported, which makes comparing studies or developing meta-analyses difficult or even impossible. Thus, there is a need for a core outcome set (COS) for UNE (COS-UNE) to help address this problem. The objective of this study was to utilize a modified Delphi method to develop COS-UNE. METHODS: A 5-stage appr

Determining the utility of minimum F-wave latency alterations in the electrodiagnosis of ulnar neuropathy at the elbow.

BACKGROUND: Ulnar neuropathy at the elbow (UNE) is the second most common entrapment neuropathy. There is little information about the application of F-wave studies for evaluation of UNE. OBJECTIVE: The aim of this study was to evaluate the diagnostic value of minimum F-wave (F-min) latency alterations by comparing this with nerve conduction analyses in UNE-suspected patients. METHODS: Ninety-four UNE-suspected patients were admitted to this study. Sensory and motor nerve conduction and F-wave analyses on the median and ulnar nerves were performed on both upper extremities. RESULTS: A total

Utility of nerve ultrasound in ulnar neuropathy with abnormal non-localizing electrophysiology in diabetes mellitus.

We studied the utility of ultrasound in the diagnostic workup of ulnar neuropathy with abnormal non-localizing electrophysiology (NL-UN) in patients with diabetes. Eighteen ulnar nerves (15 patients) were scanned from wrist to mid-upper arm. Ultrasound showed: (a) focal nerve enlargement at the elbow (8/18 nerves), either alone (6) or superimposed upon diffuse nerve abnormality (2); (b) diffuse nerve enlargement without focal abnormality (8/18); (c) segmental abnormality in upper-arm or forearm without extrinsic nerve compression (2/18). This study shows a pivotal role for ultrasound in the cl
