[Studies on therapeutic strategies for congenital myasthenic syndromes.].

Congenital myasthenic syndromes(CMS)are caused by germline mutations in genes encoding molecules expressed at the neuromuscular junction. Mutations have been identified in 25 genes. Clinical phenotypes and rational therapies widely vary depending on the defective genes. Drugs that are used for CMS include cholinesterase inhibitors, 3,4-diaminopyridine, effedrine, albuterol(salbutamol), acetazolamide, quinidine, fluoxetine, and atracurium. Some of these drugs have opposing effects. Specific drug is thus required for a specific disease mechanism. In addition, we are developing a novel therapeuti

Mechanism of action of newer anticonvulsants.

Bipolar disorder, like epilepsy, is episodic in nature. It should not be surprising, then, that anticonvulsants such as carbamazepine and valproate have proven efficacy as mood stabilizers. The newer anticonvulsants-agents like lamotrigine, gabapentin, topiramate, oxcarbazepine, and zonisamide-may also be effective treatments for bipolar disorder. Identifying an anticonvulsant for use in bipolar disorder should take into account not only the pathophysiology of bipolar disorder but also the mechanism of action of the anticonvulsant. This article will explore the mechanisms of action of the newe

Inhibition of mammalian carbonic anhydrase isoforms I, II and VI with thiamine and thiamine-like molecules.

Here we determined the in vitro inhibitory effects of 5-(2-hydroxyethyl)-3,4-dimethylthiazolium iodide (1), 3-Benzyl-5-(2-hydroxyethyl)-4-methylthiazolium chloride (2) and thiamine (3) on human erythrocyte carbonic anhydrase I, II isozymes (hCA I and hCA II) and secreted isoenzyme CA VI. K(I) values ranged from 0.38 to 2.27 µM for hCA I, 0.085 to 0.784 µM for hCA II and 0.062 to 0.593 µM for hCA VI, respectively. The compounds displayed relatively strong actions on hCA II, in the same range as the clinically used sulfonamidesethoxzolamide, zonisamide and acetazolamide.

How accurately do adult patients report their absence seizures?

We depend upon self-reporting to determine seizure frequency for epilepsy management decisions, but people often misreport their seizures. Here, we determined misreporting rates in adults with absence seizures, undergoing inpatient video-EEG telemetry (VET) or outpatient ambulatory electroencephalography (aEEG). Under-reporting rates were based on VET data, where behavior could be assessed, whilst over-reporting was assessed using both VET and aEEG. Forty-two patients (31 female and 11 males, median age 28.5 years) and 759 reported absence seizures were included in this study. Overall, only 24

Essential tremor: treatment options.

The treatment of essential tremor depends on the tremor severity, location, and risk benefit ratios. Mild to moderate tremor usually will respond to oral agents such as noncardiac selective beta-blockers or primidone. Other agents including ethanol, topiramate, benzodiazepines, gabapentin, levetiracetam, and zonisamide may be effective. There are very little data comparing different oral agents, but there is support for polypharmacy in some cases. Botulinum toxin injections are effective in some tremor patterns, especially wrist flexion/extension and head tremor. For severe tremor, surgical le

Drug-Induced Liver Injury.

Drug-induced liver injury (DILI) is an underrecognized cause of hepatic disease in dogs and cats. Successful identification of cases requires an initial suspicion by the practitioner, a thorough drug exposure history, and knowledge of the toxic potential for common veterinary drugs. This article reviews the pathogenesis, classification, and diagnosis of DILI in small animals. It also discusses the clinical presentation, prevalence, and outcomes of DILI for several drugs important in veterinary medicine including azathioprine, azole antifungals, carprofen, diazepam, doxycycline, lomustine, meth

Influence of medications on thyroid function in dogs: An update.

Erroneous thyroid function test results can occur because of drugs that alter thyroid hormone physiology in one or more aspects, including synthesis, secretion, distribution, and metabolism. Research since publication of the last review in the Journal of Veterinary Internal Medicine (JVIM) 20 years ago has evaluated the effects of amiodarone, zonisamide, inhalant anesthetics, clomipramine, trilostane, and toceranib on thyroid function tests in the dog. In addition, recent work on the effects of glucocorticoids, sulfonamides, phenobarbital, and nonsteroidal anti-inflammatory drugs will be revie

[Dramatic seizure reduction with levetiracetam in adult Dravet syndrome: a case report].

A 28 year-old man who had been diagnosed as having Dravet syndrome (DS) since his childhood by a pediatric hospital was referred to our department from the local pediatric clinic. Until then, his seizures were medically intractable, and generalized tonic-clonic convulsions had occurred monthly even when administered enough valproate, zonisamide and clorazepate. After adding levetiracetam (LEV) to his drug regimen at the age of 29, the seizures disappeared for more than one year. LEV was found to be effective in this adult patient as well as in a series of children affected with DS.

Anti-seizure medications and DRESS in paediatric patients: A FAERS disproportionality and time-to-onset analysis.

AIMS: Anti-seizure medications (ASMs) are in the list of the most common drug culprits of drug reaction with eosinophilia and systemic symptoms (DRESS). However, no study has systematically evaluated DRESS associated with ASMs in a large paediatric cohort. This study aimed to investigate association of DRESS with ASMs and to characterize the onset pattern of ASM-related DRESS in paediatrics using FDA Adverse Event Report System (FAERS) database. METHODS: FAERS data from the first quarter of 2004 to the second quarter of 2025 were analysed using R software (version 4.5.1). For signal detection

Carbonic anhydrase inhibitors. Cloning, characterization, and inhibition studies of a new beta-carbonic anhydrase from Mycobacterium tuberculosis.

The Rv3273 gene product of Mycobacterium tuberculosis, a beta-carbonic anhydrase (CA, EC 4.2.1.1), mtCA 3, shows appreciable catalytic activity for CO(2) hydration (k(cat) of 4.3 x 10(5) s(-1), and k(cat)/K(m) of 4.0 x 10(7) M(-1) x s(-1)). A series of sulfonamides/sulfamates was assayed for their interaction with mtCA 3. Sulfanilyl-sulfonamides, acetazolamide, methazolamide, ethoxzolamide, dichlorophenamide, dorzolamide, brinzolamide, benzolamide, and zonisamide, showed effective, submicromolar inhibition (K(I)s of 104-611 nM), the best inhibitor being 2-amino-pyrimidin-4-yl-sulfanilamide (K(

Complex Visual Hallucinations in Post-stroke Epilepsy: A Case Report and Literature Review.

We herein report an 82-year-old man with post-stroke epilepsy (PSE) who presented with persistent complex visual hallucinations (CVH) involving past episodic scenes. Electroencephalography revealed epileptic discharges in the right posterior temporal and parietal regions. The symptoms resolved with levetiracetam and were maintained with zonisamide. A literature review suggested that CVH may occur when epileptic activity spreads to non-dominant temporal or parietal regions. In conclusion, CVH may reflect visual memory and retention and can be accompanied by psychiatric symptoms. Improved recogn

Protein-losing cytomegalovirus gastritis in a patient with Stevens-Johnson syndrome.

We present a case of protein-losing cytomegalovirus gastritis in a previously immunocompetent 14-year-old Japanese girl that occurred during treatment of drug (zonisamide)-induced Stevens-Johnson syndrome with hepatic failure. Her hepatic failure and symptoms of Stevens-Johnson syndrome were successfully treated with intravenous prednisolone and infusion of fresh-frozen plasma or albumin, as the occasion demanded. However, during the course of treatment, she complained of severe epigastralgia together with hypoproteinemia, and cytomegalovirus gastritis was found by endoscopic and histological

[Juvenile myoclonic epilepsy with recurrent myoclonic status: efficacy of valproate].

Juvenile myoclonic epilepsy (JME) is characterized by a specific pharmacological sensitivity. JME is controlled by certain antiepileptic drugs but may be aggravated by others. Valproate is the treatment of choice of this epilepsy. Among the "newer" antiepileptic drugs those that appear to be useful are lamotrigine, topiramate, zonisamide and levetiracetam. We present a patient with JME who experienced myoclonic status epilepticus every months not controlled with lamotrigine monotherapy (200 mg/d), not controlled with levetiracetam monotherapy (1000 mg/d). Addition of valproate (500 mg/d) to le

Reliability of motion phase identification for long-track speed skating using inertial measurement units.

Obesity is a chronic and highly prevalent medical condition associated with increased risk for the development of numerous and sometimes fatal diseases. Despite its severity, there are few anti-obesity agents available on the market. Although psychotropic agents are not approved for the treatment of obesity, they have been used by clinicians as a therapeutic tool in daily clinical practice. The purpose of this article is to review the rationale, as well as the evidence, for the potential use of these agents in obesity treatment. Evidence for the efficacy of psychotropic agents in obesity treat
