Growing skull fractures are a rare complication of linear skull fractures in children. The authors report a case of a growing skull fracture in a 5-month-old patient with a review of the literature. CT and MRI scans revealed a growing skull fracture with complication of leptomeningeal cyst formation. Surgical removal of the cyst, duraplasty and cranial reconstruction were performed. Follow up showed that the patient was stable neurologically and had improving left upper extremity weakness.

A three week old extremely low birth weight (ELBW) infant infected by vancomycin-resistant Leuconostoc spp is presented. Treatment with appropriate antibiotics was successful after the percutaneous inserted central catheter (PICC) was removed. The infection with Leuconostoc spp is rare but should be suspected when vancomycin-resistant organisms resembling streptococci are isolated. Previous pediatric case reports are also summarized and reviewed.

This report describes the use of a single shot, through the needle caudal approach to epidural blood patch (EBP) in a patient with persistent leakage of cerebrospinal fluid following lumbosacral laminectomy. A previous report of caudal EBP in an adult patient with an epidural catheter suggested that the success of the procedure could be comparable without the use of a catheter. Topical anesthesia is routinely employed to facilitate diagnostic and therapeutic procedures that involve the oropharynx. Although this practice is usually safe, there is always the potential that methemoglobinemia might be induced by the topical anesthetic agent. The clinician should consider this possible complication when the patient experiences signs or symptoms of oxygen desaturation in the absence of another explanation. Because methemoglobinemia can be life-threatening, early recognition and treatment are extremely important.

This is a report of a patient with moderate aortic stenosis and twin gestation who presented for elective Cesarean section. These patients are at high risk of anesthesia related mortality from the pathophysiology of the cardiac lesion, exacerbated by the cardiovascular changes of pregnancy and complicated by the hemodynamic aberrations caused by anesthesia. This case shows that a slowly induced spinal-epidural anesthetic, with invasive blood pressure monitoring, is a safe and effective anesthetic.

Urethral diverticula with calculi have a low incidence as reported in the literature. This elderly female patient was first seen with recurrent urinary tract infections (UTIs), dysuria, and hematuria. She had two large stones, one in the bladder and one in a urethral diverticulum. Litholapaxy with a laser was effective in resolving both calculi. Subsequent urethral diverticulectomy was effective in removal of the urethral diverticulum.

Spinal cord infarction is a relatively uncommon condition in young children and adolescents, however, it should be included in the differential diagnosis of acute cord compression. Depending on the vessel involved, the clinical presentation may differ. The degree of recovery in spinal cord infarcts varies from one individual to another. Infection of pacing and cardioverter defibrillator (ICD) implants is associated with high morbidity and mortality. To report a single center experience of extracted leads, we analyzed 42 (11 ICDs and 31 pacemakers) that had been extracted using laser sheath technology at Charleston Area Medical Center between November 2000 and September 2003. The indications for extractions were infection in 48% of the patients (n = 13) and lead malfunction in 52% (n = 14). Necrotizing fasciitis is a rare, often fatal, soft tissue infection involving any of the layers of soft tissue. Although rare, cases continue to be reported among patients with impariments of the immune system, such as in diabetics. Bacillary angiomatosis, a rare and possibly fatal disease, occurs mainly in HIV-infected patients. However, it has been reported in patients with cancer and in recipients of solid organs receiving immunosuppressive drugs such as cyclosporine. This case report describes a 66-year-old man who came to dermatology practice in Clarksburg for an initial visit. He had a longstanding history of psoriasis and psoriatric arthritis treated with methotrexate. A biopsy of a skin lesion on his neck confirmed the diagnosis of bacillary angiomatosis.

Although rare, botulism should be a consideration in the differential diagnosis of hypotonia in children. The pattern of clinical presentation and physical examination should raise the suspicion to make the appropriate diagnosis. Several recent reports have described porphyria cutanea tarda (PCT) occurring in patients with HIV infection. Current evidence suggests that HIV infection may impair the hepatic cytochrome oxidase system, which could lead to an aberration in porphyrin metabolism and subsequently cause porphyria.
