[Anesthesia in obstetric patient with sickle cell anemia and thalassemic trait after plasmapheresis: case report].

BACKGROUND AND OBJECTIVES: Plasmapheresis is the technique of choice for severe hemolytic anemia patients. A consequence is plasma cholinesterase depletion, which interferes with metabolism of some neuromuscular blockers currently used in anesthesiology. CASE REPORT: Pregnant patient, 26 years old, physical status ASA IV, 30 weeks and 3 days gestational age, with sickle cell anemia, thalassemic trait and allo-immunization for high frequency antigens. Patient presented sickling crisis being transfused with incompatible blood. Patient evolved with massive hemolysis being admitted with 3 g/dL he

Anesthetic Management of Delayed Primary Arterial Switch Operation in a Two-Month-Old Infant With Dextro-Transposition of the Great Arteries (d-TGA), Preserved Left Ventricular Mass, and Patent Ductus Arteriosus: A Case Report.

For dextro-transposition of the great arteries (d-TGA) with an intact ventricular septum, the arterial switch operation (ASO) is usually performed early to avoid left ventricular (LV) deconditioning. When infants present late, the risk of postoperative LV failure rises substantially. In this case report, we describe a two‑month‑old infant (3.98 kg) with d‑TGA, severe hypoxemia (peripheral oxygen saturation (SpO2) 49%), and failure to thrive. Despite late presentation, LV mass and its circular geometry were maintained (ejection fraction 85.1%) in association with a large, nonrestrictive 6.1 mm

Maternal, fetal and neonatal effects of beta-adrenergic stimulation in connection with cesarean section.

To investigate the combined metabolic effects of beta-mimetic therapy and general anesthesia on maternal and fetal/neonatal metabolism, 14 patients were treated with isotonic saline, 11 with intravenous fenoterol (3 microgram/min) and 9 with intravenous isoxsuprine (150 microgram/min) for 30 minutes prior to cesarean section. The maternal heart rate and blood pressure as well as the maternal and fetal acid-base balance were followed. The neonatal glucose level was monitored for 36 hours after delivery. The heart rate and the diastolic and systolic blood pressure values increased during the ope

Combined Regional Analgesia for Complex Unilateral Clavicular and Thoracic Trauma Undergoing Clavicle Fixation: A Case Report.

Patients with simultaneous clavicular and ipsilateral thoracic trauma, including multiple rib fractures and traumatic pneumothorax, present a complex perioperative analgesic challenge. Effective pain control must be balanced against the preservation of respiratory function while minimizing opioid-related adverse effects. Individual regional anesthesia techniques for clavicle surgery and rib fracture analgesia are well established; however, selecting an appropriate multimodal analgesic strategy for patients with combined injuries requires careful clinical judgment. We report the case of a 51-ye

Esketamine opioid-free intravenous anesthesia versus opioid intravenous anesthesia in spontaneous ventilation video-assisted thoracic surgery: a randomized controlled trial.

BACKGROUND: Opioid-free anesthesia (OFA) provides adequate analgesia and can reduce postoperative opioid consumption, but its efficacy in spontaneous ventilation video-assisted thoracic surgery (SV-VATS) has not been demonstrated. We aimed to investigate the hypothesis that OFA could provide the same perioperative pain control as opioid anesthesia (OA), maintain safe and stable respiration and hemodynamics during surgery, and improve postoperative recovery. METHODS: Sixty eligible patients (OFA group: n=30; OA group: n=30) treated between September 15, 2022, and December 15, 2022, at The Firs

Clinical evaluation of non-intubated spontaneous ventilation video-assisted thoracoscopic surgery in patients with non-small cell lung cancer and compromised pulmonary function: a propensity score-matched analysis.

BACKGROUND: Conventional video-assisted thoracoscopic surgery (VATS) under intubated general anesthesia poses significant risks in non-small cell lung cancer (NSCLC) patients with compromised pulmonary function, including postoperative pulmonary complications and respiratory failure. Non-intubated spontaneous ventilation video-assisted thoracoscopic surgery (NIVATS), which avoids one-lung ventilation (OLV) and airway instrumentation, may mitigate these risks and accelerate recovery. However, concerns remain regarding intraoperative hypoxemia, hypercapnia, and mediastinal flutter. This study ai

Easy-to-implement educational interventions to bring climate-smart actions to daily anesthesiologic practice: a cross-sectional before and after study.

BACKGROUND: Anesthesia contributes significantly to a hospital's carbon footprint. Climate-smart actions have the potential to reduce greenhouse gas emissions. Prerequisites for sustainable behavior of providers are knowledge and awareness. We aimed to assess the change in anesthesiologists' climate-friendly behavior before and after educational interventions in three areas that every anesthesiologist can address in their daily clinical routine: 1) energy use; 2) recycling opportunities; 3) consumption of volatile anesthetics. METHODS: We performed a cross-sectional before-and-after single ce

Anesthetic technique and the cytokine and matrix metalloproteinase response to primary breast cancer surgery.

BACKGROUND: Breast cancer is the most common malignancy in women. Surgery remains the most effective treatment. Several perioperative factors, including the surgical stress response, many anesthetics and opioids, adversely affect immune function. Regional anesthesia-analgesia attenuates perioperative immunosuppression. We tested the hypothesis that patients who receive combined propofol/paravertebral anesthesia-analgesia (propofol/paravertebral) exhibited reduced levels of protumorigenic cytokines and matrix metalloproteinases (MMPs) and elevated levels of antitumorigenic cytokines compared wi

Intracranial Vascular Procedures.

Anesthesia for intracranial vascular procedures is complex because it requires a balance of several competing interests and potentially can result in significant morbidity and mortality. Frequently, periods of ischemia, where perfusion must be maintained, are combined with situations that are high risk for hemorrhage. This review discusses the basic surgical approach to several common pathologies (intracranial aneurysms, arteriovenous malformations, and moyamoya disease) along with the goals for anesthetic management and specific high-yield recommendations.

The impact of tubeless anesthesia versus intubated anesthesia on cerebral oxygen saturation and postoperative cognitive function in patients undergoing video-assisted thoracoscopic surgery: a randomized trial.

BACKGROUND: In video-assisted thoracoscopic surgery (VATS), intubated anesthesia may affect cerebral oxygen balance and postoperative cognitive dysfunction (POCD). To avoid complications associated with intubated anesthesia, tubeless strategies have been proposed in recent years, but its effect on cerebral oxygen balance and POCD is still unclear. This prospective study compared the cerebral oxygen saturation and the incidence of POCD in patients undergoing VATS anesthetized with tubeless anesthesia vs. intubated anesthesia. METHODS: A total of 60 patients with American Society of Anesthesiol

Opioid-free anesthesia in oncologic surgery: the rules of the game.

BACKGROUND: Opioids are frequently used in the postoperative period due to their analgesic properties. While these drugs reduce nociceptive somatic, visceral, and neuropathic pain, they may also lead to undesirable effects such as respiratory depression, urinary retention, nausea and vomiting, constipation, itching, opioid-induced hyperalgesia, tolerance, addiction, and immune system disorders. Anesthesiologists are in the critical position of finding balance between using opioids when they are necessary and implementing opioid-sparing strategies to avoid the known harmful effects. Comparison of the impact of the method of anesthesia on intraoperative nociceptive response during spine surgeries: Prospective randomized control trial.

BACKGROUND: Surgical intervention triggers nociception, whereas the main aim of anesthesia is to suppress nociceptive signal processing. The impact of different methods of anesthesia on the nociceptive response and their correlation with post-operative outcomes remains unclear. Regional anesthesia affects transduction, transmission, and modulation by interrupting the conduction of pain impulses by local anesthetics. Moreover, nerve blocks influence pain perception by reducing opioid consumption (OC), which helps prevent the development of pain sensitization and opioid tolerance. Owing to this
