Protective ileostomy creation after anterior resection of the rectum: Shared decision-making or still subjective?

AIM: The choice of whether to perform protective ileostomy (PI) after anterior resection (AR) is mainly guided by risk factors (RFs) responsible for the development of anastomotic leakage (AL). However, clear guidelines about PI creation are still lacking in the literature and this is often decided according to the surgeon's preferences, experiences or feelings. This qualitative study aims to investigate, by an open-ended question survey, the individual surgeon's decision-making process regarding PI creation after elective AR. METHOD: Fifty four colorectal surgeons took part in an electronic

Evidence-based pelvic floor disorder care pathways optimize shared decision making between patients and surgeons.

INTRODUCTION AND HYPOTHESIS: Evidence-based care pathways improve care standardization and patient outcomes. We created pelvic organ prolapse (POP) and stress urinary incontinence (SUI) care pathways as decision aids for our multidisciplinary team to use when counseling patients. METHODS: Using a modified Delphi process, an expert team reviewed existing guidelines and literature to reach consensus on pathway definitions and components. RESULTS: Entry to the care pathways occurs via an advanced practice provider visit. Symptom and quality-of-life questionnaires as well as open-ended patient g

Improving Patient-Centered Care for Veterans With Treatment-Resistant Depression Using Shared Decision-Making Tools.

INTRODUCTION: Treatment-resistant depression (TRD) exists when patient depression continues without remission or reduction despite treatment. There are no standardized guidelines for identifying TRD, but one failed antidepressant treatment at an adequate dose and duration can constitute TRD, especially in cases of severe depression or suicidality. TRD rates for depressed patients average approximately 50% to 60% of the general population. These numbers are higher in the military population and are often complicated by comorbidities. AIM: Chart audits revealed 68% of psychiatric clinic outpati

Boxed Warnings and Off-Label Use of Allergy Medications: Risks, Benefits, and Shared Decision Making.

The Food and Drug Administration is tasked with evaluating the efficacy and safety of a drug. Despite having a regimented appraisal process in place, safety evidence can emerge during clinical trials as well as from observations and studies conducted after the drug has been on the market, which might require a boxed warning. The boxed warning is the most severe warning that the Food and Drug Administration can give to an approved drug. It is commonly referred to as a Black Box Warning because it is outlined in the package insert by a thick black box to garner the attention of prescribers and p

Shared decision-making in gout treatment: a national study of rheumatology provider opinion and practice.

To assess rheumatologists' views and practices related to shared decision-making (SDM) in gout treatment. We performed a cross-sectional electronic survey of rheumatologists at U.S. Veterans Affairs (VA) medical centers, assessing views and practices related to SDM in gout. Of the 154 VA rheumatology providers eligible, 90 responded (response rate, 58%). Fifty-eight percent were female, the mean age was 51 years (standard deviation, 9.6), 42% had > 20 years of experience in medical practice. Rheumatologists reported routinely offering a choice to their patients for (1) starting urate-lowering

[Shared decision making even for complex systemic autoimmune diseases such as Systemic Lupus Erythematosus (SLE)?].

OBJECTIVE: The treatment of systemic autoimmune diseases, such as systemic lupus erythematosus (SLE) is an enormous challenge for all involved due to the complexity. This study investigated whether shared decision making (SDM) can advantageously be implemented in such a situation. METHOD: To answer this question qualitative group surveys were carried out with rheumatologists according to the standardized method of K&P Psychodrama®. A total of 41 German rheumatologists who had been invited by GlaxoSmithKline (GSK) Germany participated in 2 meetings over a complete day in 2012 and 2013 as well

Patient-centered imaging: shared decision making for cardiac imaging procedures with exposure to ionizing radiation.

The current paper details the recommendations arising from an NIH-NHLBI/NCI-sponsored symposium held in November 2012, aiming to identify key components of a radiation accountability framework fostering patient-centered imaging and shared decision-making in cardiac imaging. Symposium participants, working in 3 tracks, identified key components of a framework to target critical radiation safety issues for the patient, the laboratory, and the larger population of patients with known or suspected cardiovascular disease. The use of ionizing radiation during an imaging procedure should be disclosed

Barriers and facilitators to implement shared decision making in multidisciplinary sciatica care: a qualitative study.

BACKGROUND: The Dutch multidisciplinary sciatica guideline recommends that the team of professionals involved in sciatica care and the patient together decide on surgical or prolonged conservative treatment (shared decision making [SDM]). Despite this recommendation, SDM is not yet integrated in sciatica care. Existing literature concerning barriers and facilitators to SDM implementation mainly focuses on one discipline only, whereas multidisciplinary care may involve other barriers and facilitators, or make these more complex for both professionals and patients. Therefore, this qualitative st

Effect of a patient decision aid on shared decision making in patients with differentiated thyroid cancer: a randomized controlled trial.

BACKGROUND: : Over the last decades optimal treatment for patients with differentiated thyroid cancer (DTC) is debated. Two treatment decisions for patients who could benefit from more individualized approaches and shared decision making (SDM), are the extent of surgery decision in patients with low-risk DTC, and the decision to start or delay the treatment with Tyrosine Kinase Inhibitors (TKIs) in patients with advanced DTC. Our aim is to examine the effect of a Patient Decision Aid (PtDA) on observed SDM, while physicians were trained in SDM. METHODS: In this multicenter RCT (2020-2024), al

Lessons Learned From Shared Decision-Making With Oral Anticoagulants: Viewpoint on Suggestions for the Development of Oral Chemotherapy Decision Aids.

Oral chemotherapy is commonly prescribed, and by using decision aids (DAs), clinicians can facilitate shared decision-making (SDM) to align treatment choices with patient goals and values. Although products exist commercially, little evidence informs the development of DAs targeting the unique challenges of oral chemotherapy. To address this gap in the literature, our objective was to review DAs developed for oral anticoagulation, DA use in oncology, and patient preference surveys to guide the development of DAs for oral chemotherapy. We focused on reviewing SDM, patient preferences, and speci

Developing a Two-Sided Decision Box to Facilitate Shared Decision-Making for Switching From Conventional to Pharmacokinetic-Tailored Prophylaxis in Haemophilia.

INTRODUCTION: Decision-making in haemophilia is challenging due to the small evidence base, disease heterogeneity, and inter-patient variability. Shared decision-making (SDM) supports patient-clinician decisions. AIM: Creation of a two-sided decision box facilitating SDM for haemophilia patients switching from conventional (weight-based) to pharmacokinetic driven individualized prophylaxis. METHODS: We developed an SDM tool as suggested by Giguere et al. A stakeholder discussion with haemophilia treaters and patients identified goals, burden, values and preferences. Shared Decision-Making for Prostate Cancer Screening in Racial and Ethnic Minority Populations: A Systematic Review.

OBJECTIVES: Prostate cancer screening (PCS) remains controversial in its efficacy, making shared decision-making (SDM) essential to ensure informed choices. However, racial and ethnic minority men in the United States experience persistent disparities in PCS participation and outcomes. Despite these disparities, how SDM interventions have been designed, implemented, and evaluated for these populations remains unclear. To address this gap, this systematic review examined SDM interventions for PCS among minority men, with a specific focus on the extent to which interventions incorporated key SDM

Shared decision-making, treatment priorities and communication gaps in chronic lymphocytic leukemia: an Israeli national survey.

A national cross-sectional survey of 267 Israeli patients with chronic lymphocytic leukemia (CLL) examined patient-hematologist communication, shared decision-making (SDM), and treatment priorities. Perceived adequacy of information was moderate at diagnosis (mean 2.9/5) and higher during treatment selection (mean 3.5/5). Physician-led decision-making predominated (58%), while SDM was reported by only 29.5% of patients; venetoclax-exposed patients were significantly more likely to participate in SDM (OR = 4.93, p = 0.008). Treatment priorities varied, with complete response (36.5%) and quality

Assessing the Implementation of Shared Decision-Making in Primary Care: A Study With Unannounced Standardized Patients.

RATIONALE, AIMS, AND OBJECTIVES: Shared decision making (SDM) is widely endorsed in clinical guidelines, yet clinicians may not use it consistently across different clinical problems. This study explores how SDM is influenced by the clinical topic under discussion. METHODS: Using unannounced standardized patients (USPs), we observed how resident physicians engaged in SDM during new patient visits involving both gastroesophageal reflux disease (GERD) and smoking cessation. Audio recordings were evaluated using the validated Observer OPTION-5 tool. RESULTS: SDM scores were significantly higher
