The clinical manifestations of contact allergic dermatitis to dental materials are not uniform. This study was performed to detect the frequent allergens in the dental series associated with contact dermatitis and to define the causal relationship between the different allergens and the relevant clinical presentations. Rosacea is a chronic skin disorder of the face. Initially erythema and telangiectasia develop, followed at a later stage by papules and papulopustules. Females between 30 and 50 years of age are most affected. Pathogenesis is not clearly understood. Finally rhinophyma and persistent lymphoedema can develop. Photochemotherapy with psoralen and long-wave ultraviolet light, so-called PUVA-treatment, is currently being evaluated in many dermatologic departments. Side effects such as nausea, pruritus and erythema are well known. Recently the development of acneiform eruptions was reported in a British patient treated with PUVA (3). We found that 4 out of 80 patients treated in our clinic with 8-methoxy-psoralen according to the usual weight schedule (6) and long-wave ultraviolet irradiation developed perioral dermatitis, in 2 cases, together with acneiform eruptions localised to the forehead.

Sixteen episodes of a distinctive, papular rash occurred in eight patients following withdrawal of 6-mercaptopurine (6MP) and methotrexate (MTX) used as maintenance therapy for acute lymphoblastic leukemia (ALL). The rash also developed in one of the eight patients when only 6MP was discontinued. A 43-year-old man had severe circumocular exanthema associated with chronic rejection 10 years after receiving a kidney transplant to treat end-stage renal failure. After the renal allograft was extracted, the exanthema diminished rapidly without any treatment. Donor-reactive immune cells seem to have cross-reacted with unknown pathogens on the skin and contributed to inflammation.

We report a case of a 44-year-old woman with an 8-year history of gnatophyma. Rosacea is a facial dermatosis that may present as flushing, erythema, telangiectases, papules, pustules and phyma. Phyma is considered the final evolution stage of rosacea and is a rare variant. Treatment of phyma with atypical localization may be a challenge for dermatologists in clinical practice.
