Operation crash and surge: lessons learned from a region-wide exercise.

The Homeland Security Exercise and Evaluation Program (HSEEP) provides base guidance for planning all levels of public safety exercises, from simple seminars to complex full-scale exercises. However, the recent emphasis placed on the Federal Emergency Response Agency Whole Community approach to emergency management involves broadening the spectrum of participants to include multiple community partners. The magnitude and complexity of these exercises necessitates the incorporation of additional planning strategies beyond HSEEP guidance. The objective of this paper is to offer insight on effecti

The earthquake and tsunami--observations by Japanese physicians since the 11 March catastrophe.

Japan was struck by a magnitude 9.0 earthquake and a tsunami on 11 March 2011. Although this catastrophe has caused the most devastating damage to Japan since World War II, we believe that our systematic preparation for disasters somewhat alleviated the damage. Learning lessons from the magnitude 7.3 Great Hanshin earthquake in 1995, the government organized approximately 700 medical teams specialized in disaster management. In this earthquake of 2011, hundreds of medical teams were successfully deployed and started operations within the first 72 hours. Furthermore, the internet, which was not

Responding to international disasters.

Large-scale disasters attract broad coverage by the media and images of death and destruction circulate worldwide in minutes. This prompts the authorities in disaster-stricken areas to request external medical teams, and well-intentioned countries and aid organisations dispatch them to offer care for the victims (World Health Organization/Pan American Health Organization (WHO/PAHO) 2004a). This article provides information on disaster management for those with an interest in responding to international disasters such as the Indian Ocean tsunami in December 2004.

Wind disasters: A comprehensive review of current management strategies.

Wind disasters are responsible for tremendous physical destruction, injury, loss of life and economic damage. In this review, we discuss disaster preparedness and effective medical response to wind disasters. The epidemiology of disease and injury patterns observed in the early and late phases of wind disasters are reviewed. The authors highlight the importance of advance planning and adequate preparation as well as prompt and well-organized response to potential damage involving healthcare infrastructure and the associated consequences to the medical response system. Ways to minimize both the

Can a pediatric trauma center improve the response to a mass casualty incident?

UNLABELLED: Recent events including the 2001 terrorist attacks on New York; Hurricane Katrina; the 2010 Haitian and Chilean earthquakes; and the 2011 earthquake, tsunami, and nuclear disaster in Japan have reminded disaster planners and responders of the tremendous scale of mass casualty disasters and their resulting human devastation. Although adult disaster medicine is a well-developed field with roots in wartime medicine, we are increasingly recognizing that children may comprise up to 50% of disaster victims, and response mechanisms are often designed without adequate preparation for the n

The role of health sectors in disaster preparedness. Floods in southeastern China, 1991.

Disasters, whether natural or man-made, usually are unpredictable. Efforts to reduce morbidity and mortality from a disaster should be put forth before it occurs. A brief survey is presented of the worst flood to occur in a hundred years that affected eight provinces in Southeast China. The disaster preparedness and response for Anhui Province, the hardest hit area, is summarized. The disaster preparedness was comprehensive, and cooperation was achieved among various specialties: military forces; firefighters; civil engineers; mechanics; police; provincial governors; the medical sectors; and s

Disaster situations involving mass burn casualties can occur at any time after a fire, a terrorist attack, an industrial explosion or a transport accident. The various burn societies in the world published recommendations on disaster burn-care planning. French burn society (Société Française d'Étude et de Traitement des Brûlures-SFETB) proposes triage of burned victims according to Total Burn Surface Area (TBSA), inhalation injury and associated traumatic injuries. This plan seeks to classify victims and to refer each burned patient to a bed suited to its needs (burn centre, surgical or medica

Nurses' perception of disaster: implications for disaster nursing curriculum.

AIMS AND OBJECTIVES: The aims of the study were to identify nurses' perception of disaster, whether they considered some of the events that have occurred in Hong Kong to be disasters and the types of disastrous events that they considered likely in Hong Kong. BACKGROUND: The frequent occurrence of disasters has caused concern internationally. When disaster strikes, the demands on nursing staff are much higher than those on other healthcare professionals. There is little understanding of the concept of disaster among nurses in Hong Kong. DESIGN: This was a descriptive study. A questionnaire w

Climate-fuelled disasters and perinatal health: the catch 22 when undertaking research within an under-resourced health sector.

BACKGROUND: Climate-fuelled disasters are increasing in frequency and duration, with impacts known to disproportionately affect vulnerable populations, such as perinatal women and young families. Local healthcare workforce engagement into planning responses is required to ensure improved healthcare for these populations. One important component of perinatal care in Australia occurs through the maternal and child health nurses (MCHN). Attempted research engagement with the maternal child health nurse sector regarding the impact of disasters on the care of perinatal women during times of disaste

Chronic diseases and natural hazards: impact of disasters on diabetic, renal, and cardiac patients.

BACKGROUND: Inadequately controlled chronic diseases may present a threat to life and well-being during the emergency response phase of disasters. Chronic disease exacerbations (CDE) account for one of the largest patient populations during disasters, and patients are at increased risk for adverse outcomes. OBJECTIVE: The objective of this study was to assess the burden of chronic renal failure, diabetes, and cardiovascular disease during disasters due to natural hazards, identify impediments to care, and propose solutions to improve the disaster preparation and management of CDE. Reducing health inequalities in disasters: A cross-sectional study of the viability of 'vulnerability' terminology and of priority lists in the UK.

BACKGROUND: In disasters, people with certain characteristics repeatedly experience health inequalities. In the UK, people predicted to experience poorer health outcomes are often described as 'vulnerable'. Various services compile lists of 'vulnerable' people eligible for interventions in disasters to reduce health disparities. STUDY AIM: To explore the viability of current approaches to reducing health inequalities in disasters, we tested whether people typically described as 'vulnerable' by public health and emergency planners self-identify as 'vulnerable' in a disaster, and whether they a

Climate resiliency: A unique multi-hazard mitigation approach.

Baltimore's unique combination of shocks and stresses cuts across social, economic and environmental factors. Like many other post-industrial cities, over the past several decades, Baltimore has experienced a decline in its population -- resulting in a lower tax base. These trends have had deleterious effects on the city's ability to attend to much needed infrastructure improvements and human and social services. In addition to considerable social and economic issues, the city has begun to experience negative impacts due to climate change. The compounding nature of these trends has put Baltimo

Issues in the support and disaster preparedness of severely disabled children in affected areas.

Relative to their numbers, more than twice the number of disabled children fell victim to the Great East Japan Earthquake than did normal people. It was important to find out needs and provide support, as the needs of disabled children vulnerable to the disaster, such as a shortage of diapers of the right size for disabled children in the affected areas, were not given priority. In addition, the role of coordinators to spread word of who needed what and where, and linking this to specific support, was important. Regions and authorities need to determine how disabled children are to be evacuate

A typology for the classification of disasters: implications for intervention.

A model that can be used to plan for disaster intervention services is presented. The model, which is based upon an ability to classify disasters, presents four dichotomous categories for intervention: (1) psychotherapeutic vs. educational, (2) individual vs. systems, (3) prevention vs. treatment, and (4) direct vs. indirect. The article presents a rationale for determining which intervention, or combination, would be appropriate for any given disaster, based on its classification.

Addressing gender-based violence in disaster contexts: institutional, professional, and social responses to the 2021 La Palma (Spain) volcanic eruption.

This study explores the institutional, professional, and social responses to the gender-based violence that occurred in the wake of the 2021 volcanic eruption in La Palma, Spain. Using a qualitative methodology, it analyses the perspectives of 46 stakeholders-political decision-makers, technical staff, and social activists-acquired through in-depth interviews. The findings reveal that gender-based violence was initially overlooked during the crisis as emergency actions prioritised general safety and housing. The absence of gender-sensitive planning increased the vulnerability of women, particu
