Global burden and trends of firearm violence in 204 countries/territories from 1990 to 2019.

BACKGROUND: Gaps remained in the updated information of the firearm violence (FV) burden from a global landscape. Understanding the global burden of FV could contribute to decision-making. METHODS: Data on the FV burden, including physical violence by firearm (PVF), self-harm by firearm (SHF), and unintentional firearm injuries (UFI), were extracted from the Global Burden of Disease 2019. The temporal trends of age-standardized rate (ASR) were estimated using estimated annual percentage change (EAPC). RESULTS: In 2019, PVF, SHF, and UFI reported 710.64 × 103, 335.25 × 103, and 2,133.88 × 103

Epidemiology of Upper Extremity Firearm Injuries among Major Trauma Hospitals in the United States.

BACKGROUND: As many as 34 percent of nonfatal firearm injuries involve the upper extremity. Although not lethal, these injuries cause substantial morbidity. The authors conducted an epidemiologic study characterizing upper extremity firearm-related injuries presenting to U.S. trauma centers over a 10-year period. METHODS: The authors used the National Trauma Databank from 2007 to 2017 to identify isolated upper extremity firearm-related injuries. Descriptive statistics were performed to characterize patient demographic data, firearm type, extremity injury patterns, treatments received, hospit

Firearm Ownership and Acquisition Among Parents With Risk Factors for Self-Harm or Other Violence.

OBJECTIVE: Recent policy initiatives aiming to reduce firearm morbidity focus on mental health and illness. However, few studies have simultaneously examined mental health and behavioral predictors within families, or their longitudinal association with newly acquiring a firearm. METHODS: Population-based, longitudinal survey of 4251 parents of fifth-grade students in 3 US metropolitan areas; 2004 to 2011. Multivariate logistic models were used to assess associations between owning or acquiring a firearm and parent mental illness and substance use. RESULTS: Ninety-three percent of parents in

Complications and risk factors for mortality in penetrating abdominal firearm injuries: analysis of 120 cases.

Due to the high kinetic energy, of bullets and explosive gun particles, their paths through the abdomen (permanent cavity effect), and the blast effect (temporary cavity effect), firearm injuries (FAI) can produce damage not only in the organ they enter, but in the surrounding tissues as well. Since they change route after entering the body they may cause organ damage in locations other than those at the path of entry. For example, as a result of the crushing onto bone tissues, bullet particles or broken bone fragments may cause further damage outside of the path of travel, For these reasons i

The demographics of significant firearm injury in Canadian trauma centres and the associated predictors of inhospital mortality.

OBJECTIVE: Our primary objective was to evaluate demographic and causal factors of inhospital mortality for significant firearm-related injuries (i.e., those with an Injury Severity Score [ISS] > 12) in Canadian trauma centres. METHODS: We analyzed data submitted to the Canadian Institute for Health Information (CIHI) in the National Trauma Registry for all firearm-injured patients for fiscal years 1999-2003. Univariate and bivariate adjusting for ISS and multivariate logistic regression were performed. RESULTS: Men accounted for 94% of the 784 injured. In all patients, the percentages of se

Cumulative Incidence and Risk Factors for Pediatric Firearm Reinjury: A Two-Center Cohort Study.

BACKGROUND: Pediatric patients who experience firearm injury may be at risk for subsequent reinjury, yet the timing and predictors of recurrence remain poorly defined. Characterizing reinjury patterns at both individual and system levels is essential for informing targeted prevention strategies. METHODS: We performed a retrospective review (2010-2024) of patients aged 0-18 years presenting with non-self-inflicted firearm injuries to two Level I pediatric and adult trauma centers. Cross-institutional database linkage was used to identify reinjury events. Cumulative incidence functions estimate

Lead Ammunition Use and Post-Shooting Hygiene Practices Among U.S. Firearm Owners.

INTRODUCTION: Lead is a neurotoxicant with no safe exposure level. Firearm use represents an underrecognized source of exposure through aerosolized lead from ammunition and primers, with the potential for secondary and tertiary take-home exposures that may affect children. Few studies have quantified firearm use, and to the authors' knowledge, none have considered ammunition type or post-shooting hygiene behaviors. METHODS: A cross-sectional analysis of the National Firearm Survey, a nationally representative survey conducted in December 2024 and analyzed in 2025-2026, was performed. The samp

Firearm safety counseling is common but inconsistent: A survey of pediatric clinicians in the Southeastern United States.

OBJECTIVE: Firearm injury is the leading cause of death among children. Evidence shows that counseling around firearm safety from trusted clinicians increases safe storage practices. The American Academy of Pediatrics (AAP) has recommended pediatricians provide counseling on the topic to patients and caregivers, yet gaps remain in understanding how counseling is delivered, especially in the Southeastern US. This study aimed to describe patterns of and barriers to firearm anticipatory guidance among pediatric clinicians in the Southeast. METHODS: A cross-sectional survey was developed to asses

Efficacy of Technology-Augmented Behavioral Counseling for Risky Firearm Behaviors: A Randomized Clinical Trial.

IMPORTANCE: Firearm violence is a leading cause of morbidity and mortality for US adolescents and emerging adults (A/EAs). Health care visits are an important opportunity to provide preventive counseling addressing risky firearm behaviors (RFBs), which increase fatal and nonfatal firearm injury risk, as well as associated negative health and social outcomes. OBJECTIVE: To determine the efficacy of a technology-augmented behavioral counseling intervention addressing RFBs among A/EAs seeking emergency department (ED) treatment. DESIGN, SETTING, AND PARTICIPANTS: This prospective, 2-arm randomi

Pediatric healthcare worker perspectives on implementation of a secure firearm storage program: a qualitative study.

OBJECTIVE: Primary care-based secure firearm storage programs are well-positioned to prevent firearm-related injury, the leading cause of death for young people in the United States. While recommended by the American Academy of Pediatrics and US Surgeon General, these programs have yet to become routine practice. Our cluster randomized hybrid effectiveness-implementation trial studied implementation of a universal evidence-based secure firearm storage program, S.A.F.E. Firearm (Suicide and Accident Prevention through Family Education) across 30 clinics in two large health systems. S.A.F.E. Fir

Firearm-Related Upper-Limb Injuries in Children: An 8-Year Single Institution Analysis.

PURPOSE: Pediatric firearm-related injuries are rising, reflecting an ongoing public health concern and burden on the health care system. However, studies examining regional gunshot wound (GSW) trends among children are scarce. We evaluated upper-limb injuries from GSWs at our state's sole level-1 trauma center over 8 years. We hypothesize there would be an increase in upper limb ballistic injuries in our pediatric population and that understanding these trends may lead to improved supportive care and interventions aimed at mitigating future occurrences. METHODS: A retrospective chart review

Firearm homicide and microterritorial concentration of lethal violence in Lima-Callao, Peru, 2018-2025.

BACKGROUND: Homicide is an important injury prevention challenge in Latin American cities, but metropolitan trends can mask substantial local variation. Evidence from Peru remains limited, particularly on whether recent increases in homicide have been driven by firearm violence and concentrated within urban microterritories. METHODS: We conducted a population-based ecological study of homicide mortality in the Lima-Callao metropolitan area using Peru's National Death Information System, covering 2018 to 2025. Evaluating interactive computerized training to teach practitioners to implement firearm safety skills training.

Researchers have shown that behavioral skills training (BST) and in situ training are effective for teaching firearm safety skills to children. Within the safety skills literature, there is evidence that manualized interventions are effective for teaching parents and teachers to conduct BST. An approach that has not been evaluated for teaching safety skills is interactive computerized training (ICT). The purpose of the current study was to evaluate an ICT program with three Board Certified Behavior Analysts (BCBA) who provided services to clients with autism spectrum disorder. In the final pha
