Rapid sequence induction.

Emergency tracheal intubation undertaken by Rapid sequence induction (RSI) is a fundamental component of military anaesthesia. This common emergency procedure has evolved in both civilian and military practice, since it was first described, as new drugs have become available. Current practice now differs significantly from that undertaken by the procedure’s initial advocates. This is particularly the case in the deployed military environment. As military medicine continues to improve injury survivability, RSI will be undertaken in increasingly unstable casualties, requiring a bespoke emergency

Swallowing: A delayed milestone post-surgery in tongue cancers.

Functional rehabilitation remains an important factor in the post-operative period following tongue cancer surgeries. Patients undergoing surgery for tongue cancers require intense rehabilitation in order to restore their swallowing function, and improve their nutritional status and quality of life. Various swallowing scales like the Functional Oral Intake Scale (FOIS), Performance status scale in head and neck cancer (PSSHNC) and 100 ml water swallow test are used to assess functionality in these patients. These aid in timely assessment and early intervention for better rehabilitation, in tur

Randomised prospective study compares efficacy of five different stomach tubes for rumen fluid sampling in dairy cows.

The objective of the study was to compare the performance of five types of stomach tubes for rumen fluid sampling. Rumen fluid was sampled in rumen fistulated cows assigned to a 5×5 Latin square study design. The pH values of samples taken by stomach tubes and via fistulas were measured; the results were compared with indwelling sensor measurements. The practicability of the stomach tubes for regular use was tested in the field. Rumen fluid samples were obtained rapidly. Volumes for transfaunation could be obtained. The pH-values of samples taken with the four out of the five tubes (Dirksen, G

Use of total parenteral nutrition in the refeeding of selected patients with severe anorexia nervosa.

OBJECTIVE: At present there is no consensus on how to refeed patients with severe anorexia nervosa. In these case reports, we describe two patients beset with gastrointestinal comorbidities which impaired their ability to refeed with a staged oral feeding program. The use of total parenteral nutrition (TPN) facilitated their recovery. METHOD: We present two cases of severe anorexia nervosa, complicated by comorbid gastrointestinal disorders, which precluded them from successful refeeding using oral food calories. The treatment with TPN consisted of using a surgically placed, indwelling tunnel

Aortoesophageal fistula due to double aortic arch and prolonged nasogastric intubation: case report and review of the literature.

BACKGROUND: The authors present a patient and describe other cases from the literature which demonstrate that prolonged use of nasogastric tubes can result in life-threatening aortoesophageal fistula formation in patients with a double aortic arch. CONCLUSION: Aortoesophageal fistula (AEF) is an abnormal communication between the esophagus and the aorta. It can cause massive gastrointestinal hemorrhage. We present an infant with an operatively corrected double aortic arch, who developed this complication after prolonged nasogastric intubation.

An Algorithmic Framework for Reducing Near-Fatal Aspiration Events: A Single-Institution Experience.

Aspiration-related cardiopulmonary arrests (CPAs) in hospitalized patients have a survival rate under 10%, representing a serious preventable complication. In October 2023, a multidisciplinary group of physicians, nurses, and safety specialists was formed. Review of aspiration cases from 2020 to 2024 and root cause analysis identified 3 factors: unrecognized risk, refusal of nasogastric (NG) tubes, and inconsistent application of aspiration precautions. Targeted interventions included nurse and resident education, expanded swallow screens, revised NG tubes management, and new signage to flag h

The Impact of Nasogastric Tubes (NGTs) on Swallow Function in Adults with Post-stroke Dysphagia: A Systematic Review.

Nasogastric Tubes (NGTs) are a first-line treatment for delivering short-term nutrition to stroke survivors with dysphagia. However, the impact of NGTs on swallowing function itself is not yet well understood, despite the clinical importance of understanding this relationship to guide NGT treatment decisions. This review examined current evidence on the effects of NGTs on swallowing function in this population, focusing on five outcomes of interest: overall swallowing function, penetration/aspiration, oral and pharyngeal transit times and pharyngeal residue. A systematic search of terms relate

Prevention of microbial colonization of feeding tubes in the intensive care unit.

BACKGROUND: Various microorganisms which increase the mortality rate in the intensive care unit (ICU) cause microbial colonization of the nasogastric tube (NGT) and use the NGT as a reservoir. AIM: To detect the colonization on the NGT and to determine the effect that training regarding hand hygiene, NGT management, and enteral feeding (EF) provided to ICU nurses and auxiliary service staff (ASS) has on the level of NGT colonization. STUDY DESIGN: A quasi-experimental pre-test and post-test control design was used in this study. Microbial samples were taken from the outer and inner parts of

Should nasogastric tube be used routinely in patients undergoing cardiac surgery? A narrative review.

BACKGROUND AND AIM: Nasogastric tube (NGT) use has been common in the immediate postoperative period in surgical patients for decades. Potential advantages include the decompression of gastric contents and the early administration of time-sensitive medications. However, its routine use after cardiac surgery has not been established as a gold standard yet. The NGT use for prevention of postoperative nausea and vomiting has been a matter of debate in literature. Also, NGT use has also been associated with the incidence of some respiratory and gastrointestinal complications and it may be a source

In vitro evaluation of enteral tube administration of lansoprazole orally disintegrating tablets.

Lansoprazole orally disintegrating tablets (ODTs) can be administered orally or through a nasogastric (NG) tube for patients who are unable to swallow. In addition, off-label administration through gastrostomy (G) or jejunal (J) tubes has been reported. The purpose of this study was to develop in vitro methods to assess the risk of clogging during administration of two lansoprazole ODTs through enteral feeding tubes. Feeding tubes of various compositions and geometries were selected for testing. Disintegration, sedimentation, percent recovery, acid phase dissolution testing, and particle size

Immediate chest radiograph interpretation by radiographers improves patient safety related to nasogastric feeding tube placement in children.

BACKGROUND: Despite the publication of a national patient safety alert in 2016, inadvertent feeding through misplaced nasogastric tubes continues to occur, either through failure to review the radiograph, misinterpretation of it, or failure to communicate the results. OBJECTIVE: The objectives were to determine whether training in a new pathway introduced to avoid these "never events" was followed and whether radiographer comments and prompt communication of results could reduce risk and improve patient safety in relation to nasogastric tube placement in children. MATERIALS AND METHODS: Foll

Buried Bumper Syndrome: Early or Late?

Buried bumper syndrome (BBS) is a rare and serious complication of percutaneous endoscopic gastrostomy (PEG) tube placement. In the literature, BBS is considered to be a late complication of PEG procedure, but it may occur in the early period after PEG tube placement. Early diagnosis and proper treatment are important. Different treatment modalities may be used to treat BBS. The aim of this study was to evaluate patients with BBS. During a time frame between January 2015 and February 2020, a hospital medical database was screened for PEG placement and BBS. Buried bumper syndrome was found in 3
